ABSTRACT

Objective: to investigate the incidence of cage loosening in patients operated for tuberculosis of spine in different teaching hospitals


Material and Methods: this retrospective study was done on 50 individuals who had tuberculosis of dorsal spine with paraplegia and were operated. These patients underwent anterior decompression and cage fixation between 2000 and 2004. Those who had completed 4 years of follow up were included in the study. The diagnosis of loosening of cage was made on x rays which were available at the time of study


Results: out of 123 cases operated between 2000 and 2004 for tuberculosis of dorsal spine, 50 were available for follow up. These patients were examined clinically and radiographically. Loosening was defined as a zone of osteolytic more than 2 mm around the cage on either view or cage displacement from its operative position more than 4mm.There were 31males and 19 females with age ranging from 21 to 70 years. Cage loosening was seen only in one patient [2%]. 49 patients showed good results and were disease free


Conclusion: cage loose11ing has not been noticed in our patients who were operated for T.B Spine where anterior decomposition and cage fixation was done for treatment purposes

ABSTRACT

Purpose: To see the advantages of mini muscle splitting incision for dynamic hip screw [DHS] fixation


Methods: Forty Eight patients with intertrochanteric fractures were operated by open reduction and internal fixation with DHS through a new mini muscle splitting incision. The patients were reviewed for operating time, duration of hospital stay and drain output. This is a new approach for DHS and is being practiced in the department of Orthopedic and Traumatology, SIMS since Jan, 2009


Results: The patients operated through mini muscle splitting approach had shorter operating time [average 39 minutes], shorter hospital stay [one day] and less than 90 cc blood loss post operatively in the drain


Conclusion: Short term study to see the advantages of mini incision was encouraging

ABSTRACT

Background: most of the traumatic injuries to the tibia are caused by motor cycle or automobile accidents. Motor cycle related tibial injuries have high incidence of skin and soft tissues loss. To decrease the ensuing burden, a spectrum of activities is needed, including injury surveillance, injury prevention and improvement in care of the wound e.g. by use of fasciocutaneous flaps


Objective: to determine the efficacy of using fasciocutaneous flaps in covering soft tissue defects in open tibial fractures


Patients and Methods: this study was a descriptive study, conducted in the orthopaedic unit of Sheikh Zayed Medical College, Rahim Yar Khan, from July 2008 to June 2009. All those patients who had skin loss over the tibia with intact distal neurovascular status along with ample normal surrounding skin were included in the study. Data was collected regarding age, sex of patients, size of the wound, site of the wound, level of fracture, mode of injury and use of any preventive/ protective measures


Results: twenty cases had open tibial fractures that needed fasciocutaneous flaps; there were 19 [95%] males and one [5%] female. Twelve [60%] flaps were proximally based, two [10%] transversally based and six [30%] distally based. Eleven [55%] flaps were between 6cm to 8cm. All [20] the flaps survived [100%]. Complications were noted in 9 [45%] patients. Majority [95%] did not use any protective/ preventive measures for accidents


Conclusion: this study concluded the worthwhile efficacy of the fasciocutaneous flaps. The incidence of the injury can be reduced by observing traffic rules and improving road safety
