ABSTRACT
Twenty patients with pure horizontal deviations were selected to evaluate the efficacy of surgical treatment by using the adjustment recession technique to attain ocular alingment with one operation and to minimize the need for staged operation or reoperation. The patients were operated upon using the two stage technique. The first stage is done under general anaesthesia while the second under local topical aneasthesia when the patient is awoke and cooperative four to six hours post-operatively. Ninty five percent of the cases attained the desirable goal with one procedure. Adjustment technique is specially needed whenever the surgical results are unpredictable. One can and should consider it in almost all adult strabismic cases

Forty patients from the University of Alexandria Main Hospital, with large angle deviation varying from 50 to greater than 90 diopters, were selected for this study. A recess resect technique formed was performed in unilateral amblyopia, bilateral recession was done for patients having equal vision. An adjustable suture technique was used on all patients. This permitted alterations in the immediate post operative period up to 25 diopter. The major problem in the surgical correction of large angle deviations has been persistently high rates of under correction. More recent surgical protocols advocates larger amounts of surgery per prism diopter of deviation. Ninty five percent of cases showed perfect motor alignment with two muscles only without much cripling of the movement
