ABSTRACT

Statement of Problem: Verrucous carcinoma [VC] is a rare type of squamous cell carcinoma [SCC]. These two lesions have many biological and clinical differences and proper diagnosis of these tumors has a great influence on the patient's treatment


Purpose: The purpose of this study was to compare the expression of p53 and Bcl-2 in VC, SCC and pseudoepitheliamatos hyperplasia [PEH]


Methods and Material: In this study, expressions of p53 and Bcl-2 markers in 20 samples of paraffin blocks of VC, 23 samples of SCC, and 22 samples of PEH were examined by immunohistochemistry method. Immunoreactiv-ity was diagnosed for p53 in the nucleus and for Bcl-2 in the cytoplasm of cells. Then, the mean percentage of the expression of p53 and Bcl-2 was compared. The collected data were analyzed, using SPSS Ver. 7.5. For statistical analysis, ANOVA and Tukey tests were used for Bcl-2, and Kruskal-Wallis was used for comparison of the markers


Result: There was a significant difference in the mean of Bcl-2 expression between the lesions which the VC was significantly lower than the others.There was also a significant difference in the mean of p53 expression between the lesions


Conclusion: p53 and Bcl-2 are two immunohistochemical markers which can be helpful in differential diagnosis of VC and SCC

ABSTRACT

Statement of Problems: Sufficient knowledge, experience and careful examination usually lead to appropriate clinical diagnosis. However, lesions with similar clinical features present a diagnostic challenge. Therefore, a confirmatory histopathological examination can be helpful to reach the final diagnosis


Purpose: The present study was conducted to evaluate the agreement between clinical and histopathological diagnoses of oral epithelial and mesenchymal lesions in a period of 30-years [1976-2006] at Mashhad Dental School


Materials and Methods: The percentage of concurrence and discrepancy between clinical and histopathological diagnoses of 625 epithelial and 2480 mesenchymal lesions was compared. Clinical data including the patient's gender and age and the lesion's location were also evaluated in cases with discrepancy. Data were analyzed using Mann-Whitney, T-Test, Chi-Square, Likehood Ratio, Fisher's Exact and Kappa tests


Results: An overall agreement between clinical and histopathological diagnoses was seen in 71/4% of epithelial lesion cases, with maximum parity in squamous cell carcinoma and papilloma. In 57/1% of the mesenchymal cases, the diagnosis was in total agreement. Denture hyperplasia, epulis fissuratum, and pregnancy tumor showed the highest concurrence, respectively


Conclusion: Although histopathological examination leads to final diagnosis, in some cases proper strategies and more attention to clinical features of the lesion are necessary to minimize the discrepancy, especially for those with non-specific clinical presentation

ABSTRACT

Malignant peripheral nerve sheath tumors [MPNSTs] are very rare. Malignant triton tumor is an aggressive variety of MPNSTs with rhabdomyoblastic differentiation. In this study we reported the clinical, radiographic, histopathologic and immunohistochemical features of an intramandibular malignant triton tumor. The patient was a 31 year-old male who was referred to maxillofacial surgery ward of Mashhad dental school with a painful swelling in the anterior part of mandible and facial asymmetry. After histopathological evaluation, surgery was performed and immunohistochemical staining confirmed the diagnosis of Triton tumor. Patient was referred for further necessary treatments. In cases of mandibular tumors with initial histopathologic findings of sarcoma, uncommon differential diagnosis such as peripheral nerve sheath tumors must be considered as well

ABSTRACT

Mucoepidermoid carcinoma is the most common malignancy in salivary glands. This tumor has a variable biologic potential, so it was first divided into two groups: one with malignant behavior and the other with benign behavior and good prognosis. The purpose of this study was evaluation of TP[53] oncoprotein and its relation to different grades of mucoepidermoid carcinoma. This retrospective study included 22 paraffin embedded mucoepidermoid carcinoma samples stained by H and E. The samples were classified into low grade, intermediate grade and high grade. Then, new sections were made and stained by immunohistochemistry method [IHC method] for TP[53] marker. Finally, the relation between the two methods was statistically [ANOVA and Kendall test] analyzed. Seven sections of normal salivary gland tissue were also used as control group. All control cases were negative for TP[53] marker while 68.2% of mucoepidermoid carcinoma samples were positive. A significant relation was revealed between histological grade and nuclear TP[53] staining by IHC method. Parallel to increasing histologic grade of salivary gland mucoepidermoid carcinoma, TP[53] expression is also increased so that immunohistochemistry technique is helpful for determination of the biologic behavior of salivary gland mucoepidermoid corcinoma and prognosis of patients

ABSTRACT

Lipoma is the most common benign tumor which composed 25-50% of soft tissue tumors.This tumor can undergo a variety of changes, one of which is ossification. Osteolipoma is a rare benign tumor which is especially uncommon in the oral cavity. This is a case report of an osteolipoma of the oral cavity in a 68-year-old male who was referred for a painless mass on the mandibular alveolar mucosa since about four years ago. Histopathologically, the lesion was composed of trabeculae of bone and mature adipose tissue confirming to be an osteolipoma.The patient recovered after resection. Osteolipoma can originate from osseous metaplasia within a long-standing lipoma following permanent stress,repeated microtrauma and prolonged ischaemia.Osteolipoma has the same prognosis as simple lipoma.The treatment of choice is excision. There have been no reports of recurrence

ABSTRACT

Introduction: the close relationship between endodontics and periodontics is firmly established. Many authors claimed that pulpal inflammation might occur following periodontal diseases. The purpose of this study was histological evaluation of pulp in teeth with periodontitis


Materials and Methods: 32 single root teeth with periodontal diseases and without any caries and restoration selected for extraction were used in this study. Extractions were done after special clinical and radiographical examination. Samples were fixed in 10% formalin and then decalcified by EDTA for three months. Six micron sections were prepared and stained by H and E. Pulp status was evaluated histologically under light microscope


Results: showed that the amount of inflammatory cells was directly related with mobility and pocked depth. Atrophy in pulpal vessels as well as calcific feature were found in samples with deeper pocket


Conclusion: the findings imply that pulp is significantly affected by periodontal diseases. This should be a carefully considered factor when dental treatment is planned
