ABSTRACT

Four cases of acrokeratoelastoidosis are reported. All four patients had symmetrically situated asymptomatic keratotic plaques along the margin of first interdigital space of the hand, The patients were all elderly korean farmers. Elastic stain showed aggregated thickened black elastotic materials occupying mainly the upper dermis. Epidermis showed marked hyperkeratosis and acanthosis. Although this cutaneous condition is not well known to physicians in the country, we think it may be not an uncommon condition in the rural part of Korea.

ABSTRACT

Eosinophilic cellutitis occurred in an otherwise healthy 38-year-old woman. The patient had 4 months history of extremely pruritic erythematous wheal-like lesions on the extremities with blood eosinophilia. The individual lesions persisted for up to 12 weeks and responded well to systemic corticosteroid therapy. Elistopathologically the lesion showed diffuse tissue eosinophilia, histiocytic granuloma and characteristic flame figures in the dermis.

ABSTRACT

Primary cutaneous cryptococcosis of ulcers and plaques developed on the face of an otherwise healthy 7-year-old boy. Lesions were successfully treated with 10% 5-fluorocytosine ointment under occlusive dressing, Cultures for the organism from the involved area became negative after 10 weeks of treatment but the organism was observed under microscope until 10 weeks after treatment. The extensively ulcerated granulomatous lesions healed with scar formation in 15 weeks. The patient had completed 20 weeks of treatment. Follow-up examination for 1 year showed no evidence of recurrence.

ABSTRACT

Acral fibrokeratoma, more often called acquired digital fibrokeratoma, is a firm hyperkeratotic horn-like projection of skin surrounded by a slightly elevated epidermial collarette. Most frequently they appear on the finger of adult man. Authors report two cases, one developed on the palm and the other on the ankle. Because of the rarity of these lesions and their even more unusual location, these case were deemed worthy of report.

ABSTRACT

A 25-year-old man had been intermittently treated with topical steroid ointment for his eczematous lesions on the scrotum. One year later, a few hard nontender nodules were found with surrounding thickened lichenified skin on the previous lesions. Histopathologic examination of the skin revealed moderate acanthosis of the epidermis and multiple scattered calcified materials in the upper demis and within the hair follicle. We present this case, being considered a dystrophic calcinosis cutis due to local injury.

ABSTRACT

A 24 year old man presented a large keloidal patch on the posterior thigh with persistent purulent fistulae and comedones on it. Histopathological findings showed folliculoperifolliculitis with polymorphs, lymphocytes, plasma cells, multinucleated giant cells and hypertrophic sclerotic connective tiasue. Clinically and histopathologically the diasgnoais of folliculitis keloidalis was made, which is known to develop mostly on the posterior neck. The lesion was treated by surgical excision and skin graft.

ABSTRACT

Polymorphic pemphigoid is a polymorphic variant of bullous pemphigoid. Tbe authors had observed a case of polymorphic pemphigoid in a 54-year-ald woman, in whom generalized chronic blistering lesions and aevere pruritus were the main symptoms. The diagnosis of polymorphic pemphigoid was based on clinical, histopa.thological, immunopathological and therapeutic features.

ABSTRACT

Pemphigus vulgaris is serious mucocutaneous bullous disease and is rarely reported in this country. The authors had observed a patient with pemphigus vulgaris from its early stage to the autopsy, including a total of five admissions. The patient had satisfactorily responded to low doses of prednisolone (60mg/day) initially and high doses (up to 180mg/day) later, but the patient died unexpectedly during admission. Autopsy finding revealed extensive soft tissue abscesses in both gluteofemoral regions from which Pseudomonas aeruginosa was cultured.

ABSTRACT

In order to evaluate the the effectiveness of low-dose longterm Vibramycin (Doxycycline) treatment on Acne Vulgaris, a double-blind study was performed on fifty two patients with Acne Vulgaris. The patients were classified as to the severi.ty of their disease before treatment according to Wand. et al. The degree of the effcctiveness of the treatment was evaluated every other week for 12 weeks on the grounds of clinical improvement. The therapeutic results obtaincd were as follovs: araong the 19 patients who received 50 mg per day for 12 weeks, 13 cases (72.2%) showed excellent improverrent after 6 weeks; whereas in thc placebo group only 4 out of 15 patients (26. 7%) showed good response, Statistical analysis of the data revealed that there was a significant difference (p<0.05) in effectiveness between the Vibrarzycin 50mg per day group and the placebo group. Among the 19 patient who received a 100mg capsile orally per day for 4 weeks (subsequently this group received 50mg per day for 8 weeks) 13 cases.(68.4%) showed excellent improvement in 4 weeks; whereas in the placebo group only 4 cases(26, 7%) showed good respnnse Statistical analysis of this data revealed that there was a significant difference (p<0. 05) in efectivencess the Vibramycin 100mg per day group and the placebo group, It was noted that 44 out of 52 patients (83%) had a distinct family history of the disease, suggesting that there might be a predisposing hereditary factor involvcd in the manifestation of Acne Vulgaris It is concluded f rom this study that Vibramycin, when administrated in a dose of 50mg per day for more than 6 weeks is effective in the treatment of Acne Vulgaris.
