ABSTRACT

New digital technologies, many involving three-dimensional printing, bring benefits for clinical applications. This article reports on the clinical procedure and fabrication of a skeletally anchored mesialization appliance (Mesialslider) using computer-aided design/computer-aided manufacturing (CAD/CAM) for space closure of a congenitally missing lateral incisor in a 12-year-old female patient. The insertion of the mini-implants and appliance was performed in a single appointment. Bodily movement of the molars was achieved using the Mesialslider. Anchorage loss, such as deviation of the anterior midline or palatal tilting of the anterior teeth, was completely avoided. CAD/CAM facilitates safe and precise insertion of mini-implants. Further, mini-implants can improve patient comfort by reducing the number of office visits and eliminating the need for orthodontic bands and physical impressions.

ABSTRACT Objective: In the present study, five new source compounds isolated from aerial parts of Gymnosporia royleana (G royleana) were screened for antibacterial and antifungal activities. Methods: Extraction from plant material was carried out using cold maceration technique. Isolation of pure compounds was accomplished through repeated column chromatography of different fractions obtained from crude extract and using silica gel as stationary phase. Their structures were established via advanced spectroscopic techniques along with the spectral data previously reported for these compounds. Dilution method was used for the evaluation of antimicrobial potential of the compounds against various microbial strains. Results: Among the tested compounds, Gymnosporin B displayed moderate antimicrobial activity against Escherichia Coli (E coli), Staphylococcus aureus (S aureus), Candida albicans (C albicans) and Aspergillus flavus (A flavus) [minimum inhibitory concentration (MIC) range; 32-64 μg/mL]. Similarly, Gymnosporin C also showed moderate activity against E coli and S aureus (MIC; 32 μg/mL each) as well as weak activity against C albicans and A flavus (MIC; 64 μg/mL each). However, Royaflavone showed moderate antibacterial activity against S aureus only (MIC; 32 μg/mL). Antimicrobial activity of the rest of the compounds was weak and negligible. Conclusion: The present study has provided fascinating results of antimicrobial activities of the isolated compounds. However, the broad antimicrobial spectrum of Gymnosporin B and Gymnosporin C demands for further exploration of these triterpenes, both on the basis of mechanism and quantitative structure-activity relationship.


RESUMEN Objetivo: En el presente estudio, cinco nuevos compuestos de origen aislados de partes aéreas de Gymnosporia royleana (G royleana) fueron tamizados en sus actividades antibacterianas y antifúngicas. Métodos: La extracción de material vegetal se realizó mediante la técnica de maceración en frío. El aislamiento de compuestos puros se logró a través de la cromatografía en columna repetida de diversas fracciones obtenidas del extracto crudo y usando gel de silicona como fase estacionaria. Sus estructuras fueron establecidas mediante técnicas espectroscópicas avanzadas junto con los datos espectrales previamente reportados para estos compuestos. El método de dilución fue usado para evaluar el potencial antimicrobiano de los compuestos contra diversas cepas microbianas. Resultados: Entre los compuestos sometidos a prueba, Gymnosporina B mostró una actividad antimicrobiana moderada contra Escherichia Coli (E coli), Staphylococcus aureus (S aureus), Candida Albicans (C albicans) y Aspergillus flavus (A flavus) [rango de concentración inhibitoria mínima (CIM); 32 - 64 μg/mL]. De manera similar, Gymnosporina C también mostró actividad moderada contra E coli y S aureus (CIM; 32 μg/mL cada uno) así como débil actividad frente a C albicans y A flavus (CIM; 64 μg/mL cada uno). Sin embargo, Royaflavone mostró actividad antibacteriana moderada sólo frente a S aureus (CIM; 32 μg/mL). La actividad antimicrobiana del resto de los compuestos fue débil e insignificante. Conclusión: El presente estudio ha proporcionado resultados interesantes acerca de las actividades antimicrobianas de los compuestos aislados. Sin embargo, el amplio espectro antimicrobiano de la Gymnosporina B y la Gymnosporina C exige una mayor exploración de estos triterpenos, tanto sobre la base del mecanismo como a partir de la relación cuantitativa estructura-actividad.

ABSTRACT Objective: Despite the presence of multitude of synthetic drugs against fever and inflammation, none has been proven entirely safe. In contrast, the accepted safety of plant derived natural products is inspiring the world. Based on this fact as well as in view of the diversified activities reported from the genus Gymnosporia, the present study was designed to evaluate the antipyretic and anti-inflammatory activity of Gymnosporia royleana (G royleana). Methods: The methanolic extract of the aerial parts of G royleana was screened for in-vivo antipyretic activity using the brewer's yeast-induced pyrexia mice model and for anti-inflammatory activity using the carrageenan-induced paw oedema and xylene-induced ear oedema mice model. Results: In the antipyretic assay, G royleana extract showed considerable antipyretic activity in a dose dependent fashion. Statistically significant antipyretic effects (p < 0.05) were observed at the end of the second hour of administration for all doses of extract and remained significant until the end of the experiment. The plant extract also displayed promising anti-inflammatory activity, in a dose dependent fashion, in both models of inflammation ie carrageenan- and xylene-induced oedema models, when compared to the controls. In the carrageenan-induced oedema model, significant effects (p < 0.01) were observed for 300 and 600 mg/kg doses after 60 minutes of xylene administration (ie 55.51% and 65.88% inhibition of oedema, respectively). Conclusion: The study provided evidence supporting the antipyretic and anti-inflammatory activity of the G royleana methanolic extract.


RESUMEN Objetivo: A pesar de la presencia de multitud de fármacos sintéticos en el arsenal contra la fiebre y la inflamación, ninguno ha dado pruebas de ser completamente seguro. En contraste con ello, la seguridad aceptada de los productos naturales derivados de las plantas inspira al mundo. Sobre la base de este hecho, así como en vista de las actividades diversificadas que se reportan con respecto al género Gymnosporia, el presente estudio se diseñó con el objeto de evaluar el potencial antipirético y antiinflamatorio de Gymnosporia royleana (G royleana). Métodos: El extracto de metanol de las partes aéreas de G royleana fue tamizado en busca de actividad antipirética in vivo, utilizando el modelo de pirexia inducida por levadura de cerveza en ratones, y de actividad antiinflamatoria utilizando modelos de ratones con oedema de las patas inducido mediante carragenina, y oedema de las orejas inducido mediante xileno. Resultados: En el ensayo antipirético, el extracto de G royleana mostró una actividad antipirética considerable en forma dependiente de la dosis. Se observó un efecto antipirético estadísticamente significativo (p < 0.05) en el transcurso de la segunda hora de administración para todas las dosis de extracto y se mantuvo significativo hasta el final del experimento. El extracto de la planta también mostró una actividad antiinflamatoria prometedora, de una manera dependiente de la dosis, en ambos modelos de inflamación, es decir, modelos de oedema inducido por carragenina y xileno, en comparación con el control. En el modelo de oedema inducido por carragenina, se observó un efecto significativo (p < 0.01) para dosis de 300 y 600 mg / kg después de 60 minutos de administración de xileno (es decir, 55.51% y 65.88% de inhibición del oedema, respectivamente). Conclusión: El estudio proporcionó pruebas suficientes sobre el potencial antipirético y antiinflamatorio del extracto de G royleana.

ABSTRACT

Background: Multiple breast lesions resembling fibroadenomas are a common imaging finding in patients presenting to the mammography unit at Dr George Mukhari Academic Hospital in the North-West district of Tshwane, South Africa. Patients often present with multiple lesions, up to 20 lesions per breast. These lesions often have atypical features on ultrasound and/or a clinical history of growth is commonly given. Phyllodes tumours may be indistinguishable from fibroadenomas and breast cancers may on occasion present with benign features, which can lead to misdiagnosis. Breast magnetic resonance imaging (bMRI) evaluation of lesions resembling fibroadenomas may improve accurate assessment and identification of lesions requiring biopsy. Objectives: To assess the reliability of bMRI to characterise lesions resembling fibroadenomas on ultrasound, using the Breast Imaging-Reporting and Data System (BI-RADS) and Kaiser scoring systems with histopathological correlation. Method: A quantitative, prospective, investigative study was performed with a sample size of 100 breast lesions among a total of 35 patients at Dr George Mukhari Academic Hospital. Patients were recruited after a breast ultrasound investigation revealed lesions resembling fibroadenomas, but with an indication for ultrasound-guided biopsy, for example, very large size, atypical features on ultrasound or a history of recent growth. The bMRI was performed prior to the ultrasound-guided breast biopsies. Three investigators independently evaluated the bMRI and applied BI-RADS descriptors to each lesion. The Kaiser score was then calculated for each lesion. Statistics were calculated using Pearson's and Spearman's coefficients for inter-reader variability, kappa scores for BI-RADS and Kaiser score correlation with histology. Results: Evaluation with bMRI, BI-RADS and the Kaiser scoring system showed statistically significant correlation with each other and with histopathology results for each lesion. There was statistically significant agreement among the investigators regarding the interpretation of the lesions and allocation of appropriate BI-RADS scores. Conclusion: Multiple lesions resembling fibroadenomas can be evaluated with bMRI when multiple breast biopsies would not be feasible. With a good imaging protocol and technique, adequate interpretation skills by the radiologist and the use of the Kaiser scoring system, an accurate diagnosis can be achieved

Background: The introduction of human immunodeficiency virus (HIV) protease inhibitors (PIs) has led to a dramatic decline in the morbidity and mortality associated with HIV infection. However; the concomitant use of PIs and other antiretrovirals (ARVs) can be complicated by drug-drug interactions (DDIs); adversely affecting levels of PIs. Methods: A quantitative; retrospective drug utilisation study was performed using data obtained from the medicine claims database of a pharmacy benefit management company during 2004; 2005 and 2006. The possible DDIs found among ARVS themselves were identified using the classification by Tatro.Results: The percentage of ARV prescriptions claimed of the total number of medicine items increased from 1.68(n = 43 482) during 2004 to 3.18(n = 51 613) during 2005; then to 4.74(n = 47 085) during 2006. A total of 1 326; 1 863 and 960 possible DDIs were identified among ARVs themselves for 2004; 2005 and 2006 respectively. Of these; ritonavir (unboosted or boosted) presented with the most possible DDIs; accounting for 74.28(n = 985) for 2004; 67.90(n = 1 265) for 2005; and 27.50(n = 264) for 2006. The highest prevalence of DDIs identified was between ritonavir (unboosted) and saquinavir (n = 974; 5) for 2005 and 2006; followed by indinavir (n = 490; 129; 155) for 2004 to 2006; and efavirenz (n = 274) for only 2004; then ritonavir (boosted); co-formulated as lopinavir/ritonavir; and efavirenz (n = 118; 88; 34) for 2004 to 2006; nevirapine (n = 49; 37) for 2004 and 2005; indinavir (n = 9) for 2004; and saquinavir (n = 22) for 2006.Conclusion: These findings indicate that concomitant use of PIs such as ritonavir; a potent cytochrome P450(CYP)3A4 enzyme inhibitor; and other ARVs is complicated by possible DDIs and therefore further studies need to be done on the ARV combinations and management of these DDIs. We investigated long-term changes in the fish fauna of Brokopondo Reservoir, Suriname, the first large reservoir (1560 km²) that was created in tropical rainforest. Before closure of the dam in 1964, the fish fauna of Suriname River had 172 species, high diversity and high evenness. The riverine fauna was dominated by small-sized species, but no single species was dominant in numbers. Large catfishes were dominant in biomass. Species were evenly distributed over riverine habitats: rapids, tributaries and main channel. Four years after closure of the dam, only 62 fish species were collected from Brokopondo Reservoir, but the composition of the fish fauna was still changing. The reservoir fauna in 1978 was very similar to the reservoir fauna in 2005, indicating that a stable equilibrium had been reached 14 years after closure of the dam. The reservoir fauna had 41 species, low diversity and low evenness. Most species of Suriname River and its tributaries with strict habitat requirements did not survive in Brokopondo Reservoir. Fish community structure was different among four habitats of Brokopondo Reservoir. The open-water habitat (10 species) was dominated by the piscivores Serrasalmus rhombeus, Acestrorhynchus microlepis and Cichla ocellaris and their prey Bryconops melanurus and two Hemiodus species. B. melanurus fed on zooplankton, Culicinae pupae and terrestrial invertebrates. Hemiodus fed on fine flocculent detritus, demonstrating that the detritus-based food chain was still important in late stages of reservoir development. Serrasalmus rhombeus also fed on peccaries that drowned when swimming across the large reservoir in rough weather. The shore community (27 species) was dominated by seven cichlids, but early stages and juveniles of the open-water species S. rhombeus and B. melanurus also occurred in the shore habitat. Foram pesquisadas as modificações a longo prazo na ictiofauna do reservatório de Brokopondo, o primeiro de grande porte (1560 km²) construído em floresta tropical úmida. Antes do fechamento da barragem em 1964, a ictiofauna do rio Suriname possuía 172 espécies, com diversidade e equitabilidade altas. A fauna do rio era dominada por espécies de pequeno porte, mas nenhuma delas dominava em número de indivíduos. Em termos de biomassa, os grandes bagres eram dominantes. As espécies estavam homogeneamente distribuídas nos diferentes hábitats: corredeiras, afluentes e canal principal. Quatro anos depois do fechamento da barragem, somente 62 espécies de peixes foram coletadas no reservatório de Brokopondo, mas a composição da ictiofauna ainda estava mudando. Em 1978, a ictiofauna do reservatório era semelhante à de 2005, indicando que um equilíbrio estável foi atingindo 14 anos depois do fechamento da barragem. A ictiofauna do reservatório tinha 41 espécies, diversidade e equitabilitade baixas. A maioria das espécies do rio Suriname que tinham exigências estreitas de hábitat não sobreviveram no reservatório. A estrutura das comunidades de peixes foi diferente nos quatro hábitats do reservatório de Brokopondo. Objetivo: A retençäo de fragmento é um problema comum após a suficiente desintegraçäo de cálculo no trato urinário. Neste trabalho nós revisamos e discutimos os fatores que impedem a excreçäo de fragmentos após LECO. Material e métodos: Nós revisamos a literatura médica utilizando o Knowledge Finder e o MEDLINE. Resultados: Sabe-se que fatores relacionados ao próprio cálculo, anatomia renal, fatores metabólicos, fatores näo associados ao tratamento e fatores näo relacionados aos pacientes influenciam o resultado e a passagem natural de fragmentos após LECO. Conclusöes: A LECO é o tratamento de escolha para a maioria dos casos de cálculo no trato urinário. Fatores particularmente adversos para a desintegraçäo do cálculo säo o peso elevado do cálculo, a localizaçäo do cálculo no pólo inferior, e o impedimento anatômico da passagem devido à anatomia desfavorável, estenoses ou má-formaçöes.
