ABSTRACT

PURPOSE: The etiology of chronic nonbacterial prostatitis is unclear but it appears to be either an infectious disease caused by yet unidentified pathogens or a noninfectious form of inflammation. But, recent data showed that the chronic nonbacterial prostatitis is developed from abnormalities in autoimmunity. We examined the serum CD4, CD8, CD4/CD8 and CD16 levels in patients with chronic nonbacterial prostatitis and normal volunteers, so we tried to reveal their differences in both two groups and the relationship of its pathogenesis. MATERIALS AND METHODS: Between Aug. 1996 and Nov. 1996, we examined the serum level of CD4, CD8 and CD16 in patient with chronic nonbacterial prostatitis(n=20) and in normal patients including volunteers(n=16). RESULTS: The results showed decreased CD4, and increased CD8 which lead to decreased CD4/CD8 ratio and normal CD16 levels. CONCLUSIONS: We believe that our experimental data would show changes of T-cell subclones in chronic nonbacterial prostatitis patients and suggest the basic data in understanding the etiology of chronic nonbacterial prostatitis.

ABSTRACT

PURPOSE: We performed an ileal W-neobladder for total bladder replacement and evaluated this procedure in comparison with previously performed bladder substitution. MATERIALS AND METHODS: Total 21 patients underwent this procedure between September 1996 and December 1997. About 40cm long segment of the distal ileum was isolated and arranged in W-shaped configuration. A spherical pouch, the neobladder, with the ureters are implanted at each serous-lined intestinal trough was fashioned and was anastomosed to the urethra. RESULTS: The mean operation time was 452 minutes. 4 patients were missed on follow-up. Among the 17 remaining patients, there was no perioperative mortality or serious immediate complication but mild ileus, atelectasis or wound disruption. Reoperation was necessary in only 1 case due to wound dehescence. The late complication was 1 case of ureterointestinal stricture which was managed with balloon dilatation and 9/17 cases of nocturnal incontinence at 2 months after operation, which was improved as times goes by. We could follow up more than 6 months in 8 patients and they were all continent at night. Urinary reflux was not observed at 14th postoperative day on cystography. All 17 patients had recognizable sensation of bladder distension closely simulating those of normal bladders. The urodynamic study revealed a capacity approximating that of a normal bladder and showed good compliance during filling by maintaining low pressure. CONCLUSIONS: According to our initial experience, the use of ileal W-neobladder in men after radical cystectomy offers free of a stoma to urinary diversion, resulting in a highly compliant, low pressure, and acceptable residual urine without serious complication.

ABSTRACT

PURPOSE: The chemical sympathectomy is now widely performed procedure for vascular diseases of extremities. But it has been reported that this procedure may produce ejaculatory failure in men. We investigated the changes of the contractility of the vas deferens after chemical sympathectomy by 6-hydroxydopamine, and whether these changes can be recovered by the pretreatment of testosterone. MATERIALS AND METHODS: The rats were divided into 5 groups, which were 1) control 2) 6-hydroxydopamine 100mg group 3) 6-hydroxydopamine 100mg + testosterone100mg group 4) 6-hydroxydopamine 200mg group 5) 6-hydroxydopamine 200mg + testosterone 100mg group. 6-hydroxydopamine was injected intraperitoneally in each group at day 1, 3, 5 and killed at day 6. Testosterone was Injected intraperitoneally 30 minutes prior to 6-hydroxydopamine injection at day 1 . The vas deferns was quickly dissected out and the contractilities by electrical field stimulation, phenylephrine, acetylcholine and KCI were compared with control group. RESULTS: In prostatic vas, the phasic contractions were reduced only in 6-hydroxy- dopamine 200mg group and recovered incompletely by testosterone. Tonic contractions in prostatic vats were reduced in 6-hydroxydopamine looms group and 6-hydroxydopamine 100mg+testosterone 100mg group at 4, 8, 16Hz field stimulation and there was no significant difference between the two groups. The contractilies in 6-hydroxydopamine 200mg group and 6-hydroxydopamine 200mg+testosterone 100mg group were reduced at all frequencies, and testosterone pretreatment group showed slightly recovered contractility at 32, 64Hz, but less than those of control. In epididymal vas, the phasic and tonic contractions were reduced in all groups compared to control except in 6-hydroxydopamine 100mg+testosterone looms group at 32, 64Hz. The effects of 6-hydroxydopamine on phenylephrine or acetylcholine induced contraction showed supersensitivity, however, testosterone pretreatments had no effects on their supersensitivity. In the response to KCI, there was no difference in all the experimental groups. CONCLUSIONS: It seems that chemical sympathectomy results in decreased contraction of the vas deferens, which is the main cause of ejaculatory failure. Although there is some protective effect by testosterone pretreatment, but it is thought that it is not so sufficient to prevent ejaculatory failure. But we suggest that there are possibilities of protective effect of testosterone by altering the dose and time of administration.

ABSTRACT

Wegener`s granulomatosis (WG) is a multisystem disorder of unknown origin characterized by necrotizing granulomatous vasculitis. WG usually affects the upper respiratory tract, lungs, and kidneys with focal glomerulonephritis, but almost any organ can be affected. The ureter is primarily an unusual location for lesions of WG. A 30-year-old woman presented with intermittent right flank pain and hematuria. A renal ultrasound demonstrated unilateral hydronephrosis and a retrograde pyelography revealed a filling defect at right mid ureter and a computed tomography displayed marked concentric thickening of the right ureteral wall which was mimicking ureteral tumor. At nephroureterectomy, the right ureter was found to be obstructed by dense, intramural fibroinflammatory reaction. There was a necrotizing granulomatous vasculitis in the muscle layer of the ureter. Our case represents the rare occurrence of WG presenting ureteral bstruction.

ABSTRACT

Gas-forming infections in the genitourinary tract are usually associated with diabetes mellitus, and emphysematous pyelonephritis is a common disease among them. However, gas forming infection occurring in the scrotum is extremely rare. We experienced a case of gas-forming scrotal wall abscess in a 45-year-old diabetic man, who showed clinical symptoms mimicking acute epididymo-orchitis.
