ABSTRACT

AIM: To explore Bland-Altman analysis of corneal curvature and astigmatism measured by new swept-source optical coherence tomography(SS-OCT)and Scheimpflug anterior segment analyzer in patients with age-related cataract.METHODS: A total of 177 patients(282 eyes)with age-related cataract admitted to the hospital between January and December 2021 were enrolled. The steep-axis curvature(Ks), flat-axis curvature(Kf), mean corneal curvature(Km), corneal astigmatism and astigmatism axis of anterior corneal surface, posterior surface and the whole cornea were measured by SS-OCT and Scheimpflug anterior segment analyzer respectively. All parameters were detected by paired sample t-test, intra-group repeatability test, Pearson correlation and Bland-Altman consistency analysis.RESULTS: There was no significant difference in Ks, Kf and Km of anterior corneal surface measured by SS-OCT and Scheimpflug anterior segment analyzer(P>0.05). Ks, Kf and Km of posterior corneal surface and whole cornea measured by Scheimpflug anterior segment analyzer were all greater than those measured by SS-OCT(P<0.05). There was no significant difference in astigmatism and axial values of anterior corneal surface, posterior surface and whole cornea measured by SS-OCT and Scheimpflug anterior segment analyzer(P>0.05). The intraclass correlation coefficient(ICC)of all parameters was greater than 0.88, indicating a good intra-group repeatability. Ks, Kf, Km, astigmatism and axis of anterior corneal surface, posterior surface and whole cornea measured by SS-OCT were positively correlated with those measured by Scheimpflug anterior segment analyzer(P<0.05). Bland-Altman consistency analysis showed that Ks, Kf, Km, corneal astigmatism and axis of anterior corneal surface, posterior surface and whole cornea measured by SS-OCT were highly consistent with those measured by Scheimpflug anterior segment analyzer(P<0.05).CONCLUSION: The corneal curvature and astigmatism parameters measured by new SS-OCT are highly consistent with those measured by Scheimpflug anterior segment analyzer, which can be applied in the diagnosis of corneal curvature and astigmatism in patients with age-related cataract.

ABSTRACT

Objective:To explore the surgical techniques and outcomes of closed reduction for geriatric subtrochanteric femoral fractures with certain radiological characteristics.Methods:A retrospective analysis was conducted in the 38 aged patients with subtrochanteric femoral fracture who had been treated at Department of Geriatric Orthopedics, Guangdong Provincial Hospital of Chinese Medicine from January 2015 to December 2019. There were 24 females and 14 males, aged from 62 to 95 years (average, 80.9 years). According to the Seinsheimer classification, there were one case of type ⅡB, 7 cases of type ⅡC, 18 cases of type ⅢA, 9 cases of type Ⅳ, and 3 cases of type Ⅴ. The time from injury to operation averaged 2.3 days (from 0.5 to 8.0 days). The fractures of this cohort were characterized by: ①The starting point of the medial fracture line was at the proximal end and the lesser trochanter separated. ②The fracture end was shortened and overlapped on the medial side and angled outward, showing a varus deformity. ③The alignment returned to normal after traction, with only partial lateral displacement left. Before fixation with proximal femoral nail antirotation (PFNA), all these patients were treated by closed reduction in a supine traction bed. In those whose reduction was unsatisfactory, a main nail was inserted or after the main nail was inserted reduction by leverage was performed through the screw blade incision. Intraoperatively, the quality of fracture reduction was evaluated according to the modified Baumgaertner assessment. Recorded were the patients’ operation time, intraoperative blood loss, fracture healing time, hip function and complications.Results:All the fractures achieved 100% of good to excellent reduction (38/38). The operation time ranged from 25 to 120 min (average, 55.6 min) and the intraoperative blood loss from 10 to 150 mL (average, 42.9 mL). This cohort of 38 patients was followed for 10 to 20 months (average, 15.3 months). All the fractures united after 3 to 8 months (average, 4.8 months). The Harris hip scores at the last follow-up ranged from 70 to 94 (average, 81.5). The follow-ups revealed no such complications as internal fixation failure, fracture re-displacement, malunion or nonunion.Conclusion:Closed reduction and PFNA fixation in a supine traction bed can lead to satisfactory clinical effects for the subtrochanteric femoral fractures with certain radiological characteristics in the aged patients.

BACKGROUND: Three-dimensional (3D) printing technology has been applied to fabricate the personalized metallic biomaterials with low elastic modulus, low cost and precision, aiming at overcoming the defects of the biomaterials fabricated by the traditional technology. OJECTIVE: To summarize the development of metallic biomaterials fabricated by the 3D printing technology. METHODS: The articles were searched by using the databases of PubMed and CNKI. The key words were “metallic biomaterials, metallic 3D printing technology, surgical implants, oral application and cardiovascular devices” in Chinese and in English. As a result, 92 articles were applied after reading and analyzing the title and abstract of the articles published between June 2010 and June 2020. RESULTS AND CONCLUSION: In biomedical applications, metallic 3D printing technology can be divided into two categories: powder bed selective melting and directional energy deposition. Metallic 3D printing enables mass production of metal implants with complex geometric shapes and internal structures, as well as customized medical implant production that meets the needs of specific patients. Faced with many metallic printing technologies, it is necessary to choose a suitable additive manufacturing process according to the complexity and different design of materials. At present, 3D printing of metallic biomaterials such as titanium and cobalt-chromium alloys has made substantial progress, and has been used in clinical orthopedics, dentistry and vascular surgery. Research on 3D printing metal biomaterials based on magnesium and iron is still carried on.

ABSTRACT

Objective: To optimize the extraction conditions of baicalin from Scutellariae Radix by electromagnetic pyrolysis. Method: Based on single factor experiments, taking extraction time, material size and liquid-material ratio as factors, yield of baicalin as index, extraction parameters were optimized by response surface methodology, and compared with ultrasonic method, ethanol refluxing method and decoction method. Result: The optimal extraction conditions were as follows:extraction time of 2.41 min, material size of 100 mesh, liquid-material ratio of 33 mL·g-1. Under these conditions, the yield of baicalin was 12.21%. The yields of baicalin by ethanol refluxing method, decoction method and ultrasonic method were 12.91%, 12.62% and 11.61%, respectively. The yield of baicalin by electromagnetic cracking extraction was close to several other conventional extraction methods, and the extraction time was significantly shortened. Conclusion: As a novel extraction technology of traditional Chinese medicine, electromagnetic cracking extraction has the advantages of high efficiency, energy and time saving, green environmental protection, etc. And it can provide a new method for the industrial extraction of baicalin.

OBJECTIVE@#To classify Right Bundle Branch Block (RBBB),Left Bundle Branch Block (LBBB) and normal ECG signals automatically.@*METHODS@#The MIT-BIH database was used as experimental data sources.The training set and test set were extracted for training and testing network models.Based on convolutional neural network,this paper proposed the core algorithm:sparse connection residual network.Compared the sparse connected residual network with classic network models,then evaluated the recognition effect of the model.@*RESULTS@#The accuracy of the test set the MIT-BIH database was 95.2%,the result is better than classic network models.@*CONCLUSIONS@#The algorithm proposed in this paper can assist doctors in the diagnosis of heart block related disease and place a high value on clinical application.

ABSTRACT

Objective To systematically review the clinical efficacy and safety of early use of recombinant human erythropoietin (rHu-EPO) for neuroprotection in premature infants.Method From establishment of the databases to November 2017,clinical randomized controlled trials (RCTs) of early use of rHu-EPO in premature infants were searched on English databases (PubMed,Embase,Cochrane Collaboration) and Chinese databases (CNKI,SinoMed,Wanfang,and VIP Database).Patients receiving conventional therapy were assigned into control group,and patients receiving both conventional therapy and rHu-EPO were assigned into rHu-EPO group.rHu-EPO group was subdivided into high-dose continuous group and low-dose intermittent group.We retrieved the related literatures,evaluated the quality,and then performed Meta-analysis using RevMan 5.3 software.Result A total of 2 588 patients in 17 studies were included and analyzed.Compared with the control group,Meta-analysis showed that early use of rHu-EPO was more effective in improving NBNA scores at 40 weeks of corrected gestational age (cGA) (MD=2.46,95%CI 1.58~3.33,P<0.001),and high-dose continuous group was better than low-dose intermittent group (MD=3.19,95%CI 0.45~5.93,P=0.002;MD=2.22,95%CI 1.29~3.16,P<0.001).Low-dose intermittent use of rHu-EPO significantly increased mental development index (MDI) (MD=6.66,95%CI 0.80~12.51,P=0.010) and psychomotor development index (PDI) (MD=5.77,95%CI 4.00~7.55,P<0.001),and reduced the incidence of MDI<70 at cGA 18~22 months (OR=0.42,95%CI 0.27~0.67,P<0.001).As to the safety and side effects,treatment with rHu-EPO reduced the risk of necrotizing enterocolitis (OR=0.48,95%CI 0.31 ~0.72,P<0.001) and periventricular leukomalacia (OR=0.52,95%CI 0.35~0.75,P<0.001)without increasing the risk of bronchial dysplasia,patent ductus arteriosus,retinopathy in prematurity and intraventricular hemorrhage.Conclusion Current evidence shows that the early use of rHu-EPO in premature infants is relatively effective and safe,but multi-center RCTs are still needed.

ABSTRACT

We report an important but long-overlooked manifestation of low-resolution power of 16S rRNA sequence analysis at the species level, namely, in 16S rRNA-based phylogenetic trees polyphyletic placements of closely-related species are abundant compared to those in genome-based phylogeny. This phenomenon makes the demarcation of genera within many families ambiguous in the 16S rRNA-based taxonomy. In this study, we reconstructed phylogenetic relationship for more than ten thousand prokaryote genomes using the CVTree method, which is based on whole-genome information. And many such genera, which are polyphyletic in 16S rRNA-based trees, are well resolved as monophyletic clusters by CVTree. We believe that with genome sequencing of prokaryotes becoming a commonplace, genome-based phylogeny is doomed to play a definitive role in the construction of a natural and objective taxonomy.

ABSTRACT

Objective To explore the effect of cervical rotatory manipulation on tensile mechanical properties of carotid artery with atherosclerotic plaque. Methods Thirty healthy male New Zealand rabbits were selected and randomly divided into experimental group, model control group and blank control group. Carotid atherosclerosis model were made in the experimental group and model control group. The experimental group received the intervention of cervical rotatory manipulation, but the model control group did not. The blank control group did not receive any intervention. After 3 groups received different interventions, the differences in tensile mechanical properties of rabbit carotid artery in three groups were observed. Results The ultimate strain, ultimate load and physiological elastic modulus of rabbit carotid arteries in 3 groups were statistically significant (P<0.05); The ultimate strain in the experimental group was 24.87% and 52.09% smaller than that in the model control group and blank control group, respectively; The ultimate load in the experimental group was 2.21 and 3.47 times of the model control group and blank control group, respectively; The physiological elastic modulus in the experimental group was 1.60 and 1.89 times of the model control group and blank control group, respectively. Conclusions Compared with normal carotid artery, the presence of carotid atherosclerotic plaque will reduce tensile mechanical properties of the carotid artery, and cervical rotatory manipulation will further reduce tensile mechanical properties of the carotid artery with carotid atherosclerotic plaque. Therefore, for patients with carotid atherosclerotic plaque, it is better to avoid cervical manipulation.

Objective Double-endobutton technique , as a widely accepted strategy for the treatment of acromioclavicular joint dislocation ( ACD ) , is undergoing constant improvement .This study was to assess the clinical effect of a modified closed-loop double-endobutton technique ( CDT) in the fixation of Rockwood type Ⅲ ACD . Methods This retrospective study included 60 cases of Rockwood type Ⅲ ACD treated between January 2010 and December 2015, 30 by modified CDT (MCDT) and the other 30 by conventional CDT (CCDT).We recorded and compared the operation time, incision length , intraoperative blood loss , pre-and post-opera-tive scores in the Constant-Murley Scale ( CMS ) , UCLA Shoulder Rating Scale ( SRS) , American Shoulder and Elbow Surgeons Rating Scale ( ARS) and Oxford Shoulder Score ( OSS), and the vertical distance from the supraclavian to subcoracoid plane ( CC-Dist ) be-tween the two groups of patients . Results Compared with the CCDT group, the MCDT group showed a significantly shorter operation time ([78.33±11.47] vs [52.33±8.48] min, P<0.05) and less intraoperative blood loss ([103.00±7.38] vs [79.17±9.75] mL, P<0.05).In comparison with the baseline, the patients of MCDT group achieved markedly improved scores at 12 months after surgery in CMS (46.60±2.09 vs 97.67±0.88, P<0.05), SRS (14.70±1.24 vs 32.17±1.21, P<0.05), ARS (44.23±2.40 vs 91.83±1.62, P<0.05), OSS (45.07±3.10 vs 15.80±1.81, P<0.05), and CC-Dist ([16.76±0.88] vs [7.57±0.73] mm, P<0.05). Conclusion MCDT, with the advantages of easier , tighter, and fas-ter reduction and fixation , is obviously superior to CCDT in the treatment of Rockwood type Ⅲ ACD .

ABSTRACT

Objective To evaluate the efficacy and safety of early use of high-dose rHu-EPO in the prevention of anemia in premature infants .Method Available data from clinical randomized controlled trials of early application of high-dose rHu-EPO to prevent premature infant anemia since database building till February 2017 were retrieved, which includes Pubmed, Embase, Cochrane Collaboration and CNKI , SinoMed, Wanfang, VIP.Premature infants less than gestational age of 37 weeks, with routine prevention of anemia as the control group and early combined use of high-dose rHu-EPO≥750 IU/( kg· w) as the treatment group.The relevant data from the literatures obtained were then screened and analyzed by the RevMan 5.3 software.Result A total of 1779 preterm infants from 21 articles including 898 cases of rHu-EPO treatment group and 881 cases in control group.Meta-analysis showed that rHu-EPO treatment group apparently increased the hemoglobin ( MD=27.42, 95%CI 20.01~34.84, P<0.001), hematocrit and reticulocyte count , reduced the blood transfusion. As to the risk of retinopathy of prematurity in rHu-EPO treatment group was significantly higher than that in the control group (RR=1.21, 95%CI 1.01~1.46, P=0.04), and reduced risks of bronchopulmonary dysplasia (RR=0.79, 95%CI 0.46 ~1.35, P =0.39) and patent ductus arteriosus ( RR =0.83, 95% CI 0.68 ~1.01, P=0.07) the difference, however, was not statistically significant, and there was no increasing risks of necrotizing enterocolitis , periventricular leukomalacia , intraventricular hemorrhage .Conclusion Early administration of high-dose of rHu-EPO can effectively prevent anemia in premature infants , but there is an increased risk of retinopathy of prematurity .

Objective:To investigate the changes and significances of serum cystatin C and transforming growth factor-β1 levels for the neonatal asphyxia.Methods:Forty-six asphyxia newborns were chosen as the asphyxia group,and thirty healthy newborns were selected as the control group.The TGF-β1,CysC,BUN,Scr,and GFR levels of both groups were detected on the 1st,3rd,7th day after hospitalization.According to the renal injury,the 46 newborns were divided into normal group and asphyxia group,and the serum indexes were detected and analyzed.Results:On the 1st,3rd,7th day after hospitalization,the TGF-β1,GFR of asphyxia group was obviously increased and was lower than those of the control group (P<0.05);the level of CysC,BUN,Scr in both groups were decreased,and the change degree in asphyxia group were higher than that of the control group (P<0.05);the CysC,BUN,Scr in renal injured group were higher than those of normal group,and TGF-β1,GFR were much lower (P<0.05).Additionally,TGF-β1 level of renal injured group was negatively correlated to the BUN and Scr,and positively correlated with the GFR (P<0.05).The level of serum CysC in renal injured group was positively correlated to BUN and Scr and negatively correlated to GFR (P<0.05).Conclusion:The serum TGF-β1,CysC in asphyxia newborns had significant changes compared with the healthy newborns and was correlated to the renal injured indexes,which had clinical directive significance on the early diagnosis,condition judgment,and prognosis of neonatal asphyxia with renal injury.

Since the definition of sepsis has been first proposed in 1991, the main content of the definition has been developed by infection + systemic inflammatory response syndrome as infection + organ dysfunction,and organ dysfunction is indicated by an increase in sequential organ failure assessment(SOFA) score.These are components of Sepsis-3 which was published in 2016.In Sepsis-3,the SOFA score in the identification and diagnosis of sepsis plays an important role,but Sepsis-3 was mainly used in adults,how to adjust it to children,some questions and details were proposed in this article.

ABSTRACT

BACKGROUND:Anterior cruciate ligament (ACL) injury is a commonly sport-induced knee joint injury that does serious harm to the knee stability. ACL reconstruction is a commonly used treatment method, but researches on 1/2 peroneus longus tendon (PLT) graft are rarely reported. OBJECTIVE: To investigate the clinical outcomes of removing the autologous ipsilateral 1/2 PLT under arthroscopy for ACL reconstruction. METHODS:106 patients with complete ACL rupture in the Affiliated Hospital of Traditional Chinese Medicine, Southwest Medical University from December 2010 to December 2014 were enrolled, and autologous ipsilateral 1/2 PLT was removed under arthroscopy for ACL reconstruction. At baseline, 3, 6 and 12 months postoperatively, the knee stability was evaluated manually through the anterior drawer test, Lachman test, and pivot-shift test, and the knee function was evaluated by Tegner activity scale, Lysholm and International Knee Documentation Committee scores. RESULTS AND CONCLUSION: Postoperative anterior drawer test, Lachman test, and pivot-shift test tests were negative in all patients. In terms of Tegner activity scale, Lysholm and International Knee Documentation Committee scores, there were significant differences at baseline and postoperative 3 months as compared with postoperative 6 months (P 0.05). These results indicate that autologous ipsilateral 1/2 PLT is a good choice for ACL reconstruction under arthroscopy, achieving rapid and satisfactory functional recovery of the knee joint, which is not only minimally invasive and easy to operate, but also exhibits good therapeutic efficacy.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the hemodynamic changes in cynomolgus monkeys with mild carotid atherosclerotic (CAS) plaques after therapy with pushing manipulation on Qiaogong acupoint (MPQ).</p><p><b>METHODS</b>Nine cynomolgus monkeys were equally randomized into MPQ group, mild CAS model group and blank control group. Mild CAS models were established in the monkeys in MPQ and model groups, and the monkeys in MPQ group received treatment with MPQ intervention after the modeling. The conditions of the carotid artery and the hemodynamic changes in the 3 groups were evaluated after the treatment.</p><p><b>RESULTS</b>Formation of CAS plaques was detected in monkeys in both MPQ and model groups. The vascular cross?sectional area, plaque cross?sectional area and stenosis rate of the plaques in the two groups all differed significantly from those in the blank control group (P<0.05), but these parameters were similar between MPQ group and the model group (P>0.05). Compared with those in the blank control group, the hemodynamic parameters showed significant changes in MPQ and the model groups (P<0.05) but remained similar between the latter two groups (P>0.05).</p><p><b>CONCLUSION</b>CAS plaques can cause changes in hemodynamic parameters. Short?term therapy with MPQ does not affect the stability of the plaques or cause adverse effects on hemodynamics in cynomolgus monkeys with mild CAS plaques.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of electroacupuncture on proprioception in cynomolgus monkeys after unilateral anterior cruciate ligament (ACL) injury.</p><p><b>METHODS</b>Twenty-seven cynomolgus monkeys were randomized equally into 3 groups, namely unilateral ACL injury with electroacupuncture group, unilateral ACL injury model group and blank control group. One week after modeling, the monkeys in electroacupuncture group were treated daily for 12 weeks with electroacupuncture at the acupoints Wei Yang, Yin Gu, Xi Yangguan and Qu Quan. At 4, 8, and 12 weeks during the treatment, the changes in the somatosensory evoked potentials (SEPs) and motor nerve conduction velocity (MCV) of the nerves of the ACL were examined with neural electrophysiological method; the changes in the morphology and the total and variable number of the proprioceptors in the ACL were observed with gold chloride staining. RESULTS; In the mokeys in the model group, the incubation period of the SEPs and MCV on the injured side of the knee were significantly extended and the amplitudes were decreased with the passage of time (P<0.05). Intervention with electroacupuncture significantly reduced the incubation period and increased the amplitudes of the SEPs and MCV (P<0.05). The total number of the proprioceptors in the ACL was significantly decreased and the variable number of the proprioceptors in ACL was increased with time in the monkeys in the model group (P<0.05); electroacupuncture intervention obviously slowed the reduction rate of total number of the proprioceptors (P<0.05) but without affecting the variable number of the proprioceptors (P>0.05). CoONCLUSION: ACL injury causes attenuation of the proprioception on the injury side, and electroacupuncture intervention can produce a positive effect on the proprioception in cynomolgus monkeys.</p>

ABSTRACT

The effects of stocking density and exchanging water frequency on growth, digestive enzyme activity, anti-oxidative enzyme and inner quality of Whitmania pigra Whitman were evaluated with corresponding measures. The results showed that the eventual biomass, specific growth rate, gained weight rate, activities of amylase, lipase, protease, SOD, CAT, and ALP correlated positively with stocking density and negatively with exchanging water frequency (P<0.05). Exchanging water frequency had negative correlation with ammonia nitrogen, nitrite, and hydrogen sulfide while revealed positive correlation with dissolved oxygen in the water. Stocking density and exchanging water frequency showed no significant effects on the contents of moisture, total ash, and acid-insoluble ash. It suggested that the optimum stocking density was 7.5 million per hectare and the appropriate exchanging water interval was 72 h.
