ABSTRACT

Objective: To investigate immunohistochemical patterns of CXorf67 and H3K27me3 proteins in central nervous system germ cell tumors (GCTs) and to assess their values in both diagnosis and differential diagnosis. Methods: A total of 370 cases of central nervous system GCTs were collected from 2013 to 2020 at Huashan Hospital of Fudan University, Shanghai, China. The expression of CXorf67, H3K27me3 and commonly-used GCT markers including OCT4, PLAP, CD117, D2-40, and CD30 by immunohistochemistry (EnVision method) was examined in different subtypes of central nervous system GCTs. The sensitivity and specificity of each marker were compared by contingency table and area under receiver operating characteristic (ROC) curve. Results: Of the 370 cases there were 282 males and 88 females with a mean age of 19 years and a median age of 17 years (range, 2-57 years). Among the GCTs with germinoma, the proportions of male patients and the patients with GCT located in sellar region were both higher than those of GCTs without germinoma (P<0.05), respectively. CXorf67 was present in the nuclei of germinoma and normal germ cells, but not in other subtypes of GCT. H3K27me3 was negative in germinoma, but positive in the nuclei of surrounding normal cells and GCTs other than germinoma. In the 283 GCTs with germinoma components, the expression rate of CXorf67 was 90.5% (256/283), but no cases were positive for H3K27me3. There was also an inverse correlation between them (r2=-0.831, P<0.01). The expression rates of PLAP, OCT4, CD117 and D2-40 were 81.2% (231/283), 89.4% (253/283), 73.9% (209/283) and 88.3% (250/283), respectively. In 63 mixed GCTs with germinoma components, the expression rate of CXorf67 was 84.1% (53/63), while all cases were negative for H3K27me3. The expression rates of PLAP, OCT4, CD117 and D2-40 were 79.4% (50/63), 79.4% (50/63), 66.7% (42/63) and 87.3% (55/63), respectively. The 6 markers with largest area under ROC curve in ranking order were H3K27me3, CXorf67, D2-40, OCT4, PLAP and CD117 (P<0.05). Conclusions: CXorf67 and H3K27me3 have high sensitivity and high specificity in diagnosing germinoma. There is a significant inverse correlation between them. Therefore, they can both be used as new specific immunohistochemical markers for the diagnosis of GCTs.

ABSTRACT

Objective :To study sympathetic activity in aged patients with essential hypertension (EH) complicated left ven-tricular hypertrophy (LVH).Methods : A total of 248 aged EH patients were selected from our hospital from Jun 2015 to Jun 2017- According to blood pressure level ,they were divided into EH stage 1 group (n=78) ,stage 2 group (n=89) and stage 3 group (n=81).According to presence of LVH ,patients were divided into no LVH group (n=122) and complicat-ed LVH group (n=126).According to blood pressure level ,LVH patients were divided into stage 1 group (n=28) ,stage 2 group (n=41) and stage 3 group (n=57).Another 130 healthy aged people undergoing physical examination were regar-ded as normal aged group .Plasma norepinephrine (NE) and epinephrine (E) contents were measured and compared among all groups ,and 24h DCG was performed.Results :Compared with normal aged group ,there were significant rise in plasma contents of NE [ (238-33 ± 18-04) ng/L vs.(312-90 ± 23-20) ng/L ,(279-28 ± 20-85) ng/L] ,E [ (262-86 ± 24-56) ng/L vs.(366-57 ± 25-53) ng/L ,(301-20 ± 23-80) ng/L] in complicated LVH group and no LVH group ( P=0-001 all) ,and plasma NE and E contents gradually and significantly rose along with blood pressure stage rose , P=0-001 all. Compared with normal aged group ,there was significant rise in mean heart rate [ (64-80 ± 5-40) beats/min vs.(69-02 ± 4-56) beats/min ,(66-72 ± 4-98) beats/min] in complicated LVH group and no LVH group ( P=0-001 ,0-017) ;signif-icant reductions in SDNN [ (122-07 ± 7-73) ms vs.(109-51 ± 8-13) ms ,(99-25 ± 7-89) ms] ,SDANN [ (111-72 ± 13-33) ms vs.(103-52 ± 10-41) ms ,(94-78 ± 8-75) ms] ,rMSSD [ (43-98 ± 6-31) ms vs.(37-25 ± 6-42) ms ,(32-05 ± 5-19) ms] and PNN50 [ (7-67 ± 0-71)% vs.(5-41 ± 0-67)%,(3-39 ± 0-63)%] in no LVH group and complicated LVH group , P=0-001 all ,above indexes gradually and significantly reduced along with blood pressure stage rose , P=0-001 all.Conclusion :Sympathetic activity significantly elevates in aged EH + LVH patients.Controlling sympathetic ac-tivity will help to improve prognosis .

ABSTRACT

Portal vein thrombosis (PVT) refers to thrombosis in the main portal vein and portal vein branches and is an important complication of liver cirrhosis.PVT can aggravate portal hypertension,increase the risk of bleeding,and lead to acute ischemic necrosis of the small intestine in case of shedding or retrograde movement.This article focuses on the new points of view on the mechanisms and treatment of PVT in cirrhotic patients and points out that treatment regimen should be selected based on patients'overall conditions.

ABSTRACT

This study is to explore the amelioration of piperine on chronic acute combining stress rat with depression-like behavior, visceral sensitivity, and its effect on the expression of serotonin (5-HT) and synaptophysin. Forty two SD rats were divided into seven groups: blank group, model group, piperine (12.5, 25, 50 and 100 mgkg-1, ig) and imipramine (10 mgkg-1, ip) groups. The rat model of irritable bowel syndrome was established by chronic acute combining stress, and then to evaluate depression-like behavior and visceral sensitivity. The expressions of 5-HT and synaptophysin in the hippocampus and colon were determined by high performance liquid chromatography (HPLC) and Western blotting, respectively. The duration of immobility of IBS rat in the forced swimming test had been significantly increased, the sucrose consumption of IBS rat had been reduced and visceral sensitivity was obviously elevated in the IBS model group as compared with those in the normal control group (P<0.05, P<0.01). As compared with those in the normal control group, the expression of 5-HT significantly decreased, 5-HIAA/5-HT ratio significantly increased in the hippocampus of IBS model group (P<0.05), but opposite presentations were noted in the colon (P<0.05). As compared with that in the normal control group, the synaptophysin expression in the hippocampus decreased significantly but obviously increased in the colon (P<0.05). Piperine improved the behavior of IBS rats, and reversed the levels of 5-HT and 5-HIAA, and 5-HIAA/5-HT proportion in the hippocampus and colon (P<0.05); besides, they significantly reverse the synaptophysin level in the hippocampus and colon (P<0.05). The presence of depression and visceral sensitivity had been changed in IBS rats, with abnormal expression of 5-HT and synaptophysin in the brain-gut system. Piperine can ameliorate the changes of the behavior and regulation of serotonin and synaptophysin expression in IBS rat model.

ABSTRACT

The enzyme-inhibitor model and the sugar tolerance mouse model were used to evaluate the relationship between the inhibition rate of enzyme activity and concentration of Hippophae rhamnoides L. subsp. chinensis Rousi polysaccharide (HRP). The inhibitory patterns of enzyme and dose-dependent effects of HRP's effect on blood glucose using acarbose tablets as control were also examined. The mechanism underlying hypoglycemic effects of HRP was discussed. The results showed: in the enzyme-inhibitor model, the inhibitory activity of different concentrations of HRP (9.80, 19.60, 39.20, 78.40, 156.80 and 312.50 mg x L(-1)) on alpha-glucosaminidase (AG) inhibitory activity were 6.62%, 18.02%, 33.26%, 48.23%, 62.11%, 76.31%, 90.12%, IC50 was 31.59 mg x L(-1). The inhibitory rate of 25.00 x 10(3) mg x L(-1) acarbose tablets was only 64.87%, and IC50 was 10.75 x 10(3) mg x L(-1). In the sugar tolerance mouse model, different doses of HRP (240, 480, 960 mg x kg(-1)) tended to decrease levels of blood glucose compared with control group (acarbose tablets 375 mg x kg(-1)) at 15, 30, 60 and 120 min. It's further confirmed that HRP is a kind of competitive inhibitor of AG activity. Its inhibition rate increases with the increase of concentration in normal mice, and it subsequently improves the sugar tolerance showing the effect of reducing blood sugar.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the therapeutic effect of bone marrow mesenchymal stem cells (BM-MSCs) on injured hepatocytes mediated by paracrine mechanisms and to investigate the potential molecular mechanism of this action.</p><p><b>METHODS</b>A contact-independent model of aberrant hepatic microenvironment was established by co-culturing BM-MSCs with D-galactosamine (D-GalN)-injured human L02 hepatic cells using a transwell assay platform. Secreted levels of insulin-like growth factor-1 (IGF-1) were measured by enzyme-linked immunosorbent assay of the co-culture supernatant. Expression of the IGF-1 receptor (IGF-1R) was assessed by Western blot. The effect of exogenous IGF-1 on proliferation of D-GalN-injured L02 cells was examined by MTT assay.</p><p><b>RESULTS</b>Upon co-culture, BM-MSCs promoted proliferation of D-GalN-injured L02 cells in a contact-independent manner (absorbance values of at 24 h: 0.36+/-0.08, 48 h: 0.52+/-0.06, and 96 h: 0.68+/-0.06; vs. uninjured cells t = 2.493, 3.116, and 2.285, respectively; all P less than 0.05). Robust expression of IGF-1 was identified in the supernatants of co-cultures and was demonstrated to have been secreted mainly from BM-MSCs under the influence of D-GalN-injured L02 cells. Constitutive expression of IGF-1R was found in the D-GalN-injured L02 cells and blocking of IGF-1R by a neutralizing antibody significantly inhibited the paracrine pro-proliferative effect of co-cultured BM-MSCs at 24 h, 48 h, and 72 h (t = 2.909, 2.328, and 2.560, respectively; all P less than 0.05).</p><p><b>CONCLUSION</b>BM-MSC-derived IGF-1 plays an important role in the paracrine pro-proliferative effect on D-GalN-injured L02 hepatocytes by engaging with the constitutively expressed IGF-1R on L02 cells.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of SREBP-1c silencing on lipid metabolism and expression of inflammatory chemokines in a NAFLD model with endoplasmic reticulum stress.</p><p><b>METHOD</b>NAFLD model was established in L02 cells treated with oleic acid. SREBP-1c expression was inhibited using RNA interference with a p Silencer-1.0-U6-4476 vector. After transfection with p Silencer-1.0-U6-4476 or control vector for 0 h, 24 h, 48 h and 72 h, the extent of fatty degeneration was shown by Oil Red O staining. The mRNA and protein expression of inflammatory chemokine CCL2 and basic fibroblast growth factor-21 (FGF21) were determined by real time PCR and Western blot respectively.</p><p><b>RESULTS</b>SREBP-1c silenced L02 cells showed fat droplets with smaller diameter and attenuated fatty deposition, as compared with control cells. The relative CCL2 mRNA levels in SREBP-1c silencing vector transfected L02 cells were 1.03+/-0.11 for 0 h, 1.11+/-0.21 for 24 h, 0.88+/-0.16 for 48 h, and 1.05+/-0.15 for 72 h, which showed no significant difference as compared with control cells (P>0.05, respectively). In addition, no difference was found between the different time points within the same group (P>0.05). However, CCL2 protein levels in SREBP-1c silenced cells were 1.19+/-0.15, 1.07+/-0.18, 0.48+/-0.14, and 0.05+/-0.24 after transfection for 0 h, 24 h, 48 h, and 72 h respectively, which were significantly downregulated as compared to the control group (P<0.01). And CCL2 protein levels between different time points in SREBP-1c silenced cells were also distinct (P<0.01). The relative FGF21 mRNA levels in SREBP-1c silenced L-02 cells were 1.01+/-0.08, 0.91+/-0.22, 0.98+/-0.20, and 1.02+/-0.12 for 0 h, 24 h, 48 h, and 72 h respectively, which were not statistically different as compared with the corresponding control cells. Statistic difference of FGF21 mRNA levels in SREBP-1c knockdown cells of different time points was not found (P>0.05). In striking contrast, robust down regulation of FGF21 protein in SREBP-1c silenced cells was observed, with 0.81+/-0.05, 0.66+/-0.12, 0.58+/-0.08 and 0.19+/-0.13 after transfection for 0 h, 24 h, 48 h and 72 h respectively, as compared to control group (P<0.01). And differences in FGF21 protein level between different time points in SREBP-1c silenced cells were also demonstrated (P<0.01).</p><p><b>CONCLUSION</b>SREBP-1c knockdown attenuated fatty deposition in oleic acid treated L02 cells. In addition, silencing of SREBP-1c expression reduced expressions of CCL2 and FGF21 proteins posttranscriptionally, which may play a role in endoplasmic reticulum stress induced inflammatory response in NAFLD.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the mechanism of action and the therapeutic efficacy of treating patients with perennial allergic rhinitis by Qumin Tongbi Nasal Spraying Agent (QTNSA).</p><p><b>METHODS</b>One hundred and three patients with perennial allergic rhinitis were randomly assigned to two groups. Of them, 57 patients in the treatment group were treated with QTNSA and 46 patients in the control group were treated with Veconase. Seven days were taken as one therapeutic course, two courses in total. The therapeutic efficacy and the serum levels of interleukin-4 (IL-4), IL-8, immunoglobulin E (IgE) and nasal airway resistance (NAR), symptoms and signs integrals before and after treatment were observed in the two groups.</p><p><b>RESULTS</b>There was no statistical significance between the treatment group and the control group in the markedly effective rate (61.40% vs. 63. 04%) and the total effective rate (87.72% vs. 89.13%). After treatment symptoms and signs integrals of the two groups were significantly improved when compared with before treatment (P < 0.01). There was no significant difference between the two groups after treatment (P > 0.05). The serum levels of IL-4, IL-8, IgE, and NAR were significantly higher in the two groups than those of the normal group. They significantly decreased after treatment in the two groups, showing statistical difference (P < 0.01). There was no statistical difference between the treatment group and the control group (P > 0.05).</p><p><b>CONCLUSIONS</b>QTNSA could effectively treat perennial allergic rhinitis. It could obviously lower serum levels of IL-4, IL-8, IgE, and NAR in patients with perennial allergic rhinitis.</p>

ABSTRACT

<p><b>BACKGROUND</b>Our previous study confirmed that oligodendrogliomas had higher frequency of chromosome 1p/19q deletion. In order to improve the diagnostic criteria and to predict the prognosis of oligodendroglioma patients, the status of chromosome 1p/19q deletion, the methylation of O(6)-methylguanine-DNA methyltransferase (MGMT), and the expression of p53 protein were evaluated and investigated in relation to patients' outcomes.</p><p><b>METHODS</b>Methylation of MGMT in 73 cases was analyzed by nested methylation-specific PCR (MSP). The levels of MGMT and p53 protein were tested with immunohistochemistry. Pearson's chi-square test and Fisher's exact test were used. Multivariate and Kaplan-Meier analysis were performed to determine patients' outcomes.</p><p><b>RESULTS</b>Both oligodendrogliomas and astrocytic gliomas exhibited frequent methylation of MGMT. However, the results of MSP did not completely correspond to that of the immunohistochemical staining for MGMT. The expression of p53 protein was more frequently observed in patients without a 1p or 19q deletion in anaplastic oligodendrogliomas (P = 0.032, 0.025). In low-grade oligodendrogliomas, methylation of MGMT was more frequent in patients with 1p/19q deletion than in patients with 1p/19q intact (P = 0.038). Patients with oligodendrogliomas with 1p/19q loss of heterozygosity and p53-negative showed a longer progression-free survival.</p><p><b>CONCLUSION</b>Detection of chromosome 1p/19q status combined with p53 protein immunohistochemistry might be beneficial to improve the pathological diagnosis and to determine the prognosis of patients with oligodendrogliomas.</p>

ABSTRACT

This study was aimed to investigate the effects of peripheral blood Th17 cells, IL-17 and IL-21 in the occurrence and development of acute leukemia. 60 patients with acute leukemia (19 patients with ALL, 41 patients with AML) were divided into non-remission group (group A, n=24), remission group (group B, n=36); 25 healthy volunteers were used as control group (group C). In addition to this, these 60 patients were divided into infection group (n=32) and non-infection group (n=28) on the basis of infection status. The concentration of IL-17 and IL-21 in the peripheral blood mononuclear cell culture supernatant after stimulation with anti-CD3 and anti-CD28 mAb were determined with ELISA. The expression of CD4+ IL-17+ cells was determined by flow cytometry. The results showed that (1) the concentrations of IL-17 and IL-21, and proportion of Th17 cells in group A and group B were much lower than those in group C (p<0.05); (2) the expression levels of IL-17 and IL-21, and the proportion of Th17 cells in group A were lower than those in group B (p<0.05); (3) the expression levels of Th17 and IL-17 in infection group were lower than those in non-infection group (p<0.05). It is concluded that Th17 cells may play important roles in the occurrence and development of acute leukemia through secreting IL-17 and IL-21, and their functional level can partially reflect the status of leukemia and can be used to evaluate the risks of infection in patients with leukemia.

<p><b>OBJECTIVE</b>To study the status of loss of heterozygosity (LOH) of chromosome 1p/19q and p53 protein expression in oligodendroglioma, as compared to astrocytoma.</p><p><b>METHODS</b>One hundred and ninety-one cases of glioma of different histologic types and grades, including 116 cases of low-grade of oligodendroglioma (86 paraffin-embedded and 30 fresh tissues), 45 cases of anaplastic oligodendroglioma (all paraffin-embedded tissues) and 30 cases of astrocytoma of various grades (all paraffin-embedded tissues), were enrolled into the study. The LOH of chromosome 1p/19q was investigated by polymerase chain reaction (PCR)-based microsatellite analysis. The p53 protein expression was demonstrated by immunohistochemical staining.</p><p><b>RESULTS</b>The rates of 1p loss, 19q loss and 1p/19q loss were 69.8%, 64%, and 57.0% respectively in the 86 paraffin-embedded low-grade oligodendroglioma samples, as compared to 71.1%, 60.0% and 55.6% respectively in the 45 paraffin-embedded anaplastic oligodendroglioma samples. There was no difference of LOH of 1p/19q between low-grade oligodendroglioma and anaplastic oligodendroglioma (P>0.05). In the 30 cases of low-grade oligodendroglioma with fresh tissues available, the rates of 1p loss, 19q loss and 1p/19q loss were 70.0%, 63.3% and 60.0% respectively. The LOH of 1p/19q between paraffin-embedded and fresh samples was not statistically significant (P>0.05). In the 30 cases of astrocytoma, the rates of 1p loss, 19q loss and 1p/19q loss were 23.3%, 33.3% and 20.0% respectively, which were significantly less than those in oligodendroglioma (P<0.05). The expression of p53 protein was significantly lower in low-grade oligodendroglioma (8.1%) than in anaplastic oligodendroglioma (31.1%, P=0.007). The expression of p53 protein in oligodendroglioma was also lower than in astrocytoma (P=0.001). Furthermore, p53 protein expression negatively correlated with 1p/19q loss in anaplastic oligodendroglioma (P<0.05).</p><p><b>CONCLUSIONS</b>Both paraffin-embedded and fresh tissues are suitable for analysis of LOH of chromosome 1p/19q. Oligodendroglioma demonstrates a higher frequency of LOH of chromosome 1p/19q and lower expression of p53 protein than astrocytoma. The LOH of chromosome 1p/19q negatively correlates with the expression of p53 protein. These parameters have both diagnostic and prognostic values.</p>

ABSTRACT

OBJECTIVE@#To evaluate the highly-selective regional vascular exclusion in the risk hepatectomy for liver tumor.@*METHODS@#Short hepatic veins were ligated and divided followed by the dissection, and isolation of the inflow and outflow vessels of the tumor-bearing lobe, which were completely devascularized after the occlusion of these vessels. The blood loss volume, postoperative recovering situation of the liver function and the incidence of complication were observed in 68 cases.@*RESULTS@#Main hepatic veins were dissected and isolated exo-hepatically in 65 cases. In the other 3 cases, the main hepatic veins were blocked by Satin skin clamp applied longitudely along the inferior vena cava. Hepatic pedicle was routinely excluded.The amount of blood loss was from 400 to 1200 (600+/-200) mL and 26 (65%) cases didn't receive transfusion.There was no operative mortality and liver function failure. Surgical complications included subphrenic abscess in 2 cases and bile leakage in 2 cases, which were cured conservatively.@*CONCLUSION@#Highly-selective regional exclusion of hepatic blood flow during the risk hepatectomy is safe and effective to prevent massive bleeding and to reduce the incidence of liver failure.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical effect of Tripterygiitotorum (T II) combined with prednisone in treating patients with myasthenia gravis (MG) and the changes of immune function after treatment.</p><p><b>METHODS</b>Sixty-eight patients with MG were randomly divided into two groups, the 36 patients in the treated group were treated with T II plus prednisone and the 32 in the control group were treated with prednisone alone. The therapeutic effect, serum interleukin-6 (IL-6) and peripheral B lymphocyte levels were observed.</p><p><b>RESULTS</b>The therapeutic effect in the treated group was significantly higher than that in the control group (P < 0.05). There were significant decrease in serum IL-6 and peripheral B lymphocyte in both groups after treatment (P < 0.05), with the decrements more significant in the treated group (P < 0.05).</p><p><b>CONCLUSION</b>The therapy of T II plus prednisone is superior to that of prednisone alone in treating MG, it has a more potent effect of immunosuppression.</p>
