ABSTRACT

Objective:To investigate the clinical efficacy of five-element music therapy in the treatment of bipolar disorder.Methods:Eighty patients with bipolar disorder admitted to Shandong Mental Health Center from January 2018 to December 2019 were included in this study. They were randomly divided into a study group and a control group ( n = 40/group). The control group was treated with conventional drug treatment. The study group was treated with five-element music therapy based on conventional drug treatment. Before, during and after treatment, mania and depression were evaluated. Symptom self-evaluation scale and the Traditional Chinese Medicine Five State Emotion Questionnaire were completed to evaluate the curative effect. Patients were followed up by telephone call. Recurrence at 3, 6 and 12 months was recorded. Results:Before treatment, there were no significant differences in the scores of the Bech-Rafaelsen Mania Rating Scale (BRMs), Hamilton Depression Rating Scale (HAMD), Traditional Chinese Medicine Five State Emotion Questionnaire, Symptom Checklist-90 (SCL-90) between the two groups (all P > 0.05). During and after treatment, BRMs, HAMD, Traditional Chinese Medicine Five State Emotion Questionnaire, and SCL-90 scores were significantly decreased in each group compared with before treatment [Control group: BRMs score during and after treatment: (11.52 ± 1.57) points, (9.43 ± 1.05) points; HAMD score during and after treatment: (16.75 ± 3.05) points, (13.61 ± 2.51) points; Traditional Chinese Medicine Five State Emotion Questionnaire: emotional vocabulary (67.25 ± 5.7) points, (56.38 ± 4.97) points, physical symptoms (29.52 ± 3.94) points, (24.19 ± 3.05) points, SCL-90 score: (100.52 ± 10.26) points, (68.85 ± 7.33) points. Study group: BRMs score during and after treatment: (9.33 ± 1.09) points, (7.85 ± 0.82) points; HAMD score: (13.74 ± 2.54) points, (10.17 ± 1.97) points; Traditional Chinese Medicine Five State Emotion Questionnaire: emotional vocabulary: (58.19 ± 5.06) points, (46.85 ± 4.06) points, physical symptoms (25.14 ± 3.54) points, (20.11 ± 2.57) points; SCL-90 scores: (90.85 ± 8.97) points, (56.87 ± 5.81) points]. During and after treatment, scores of BRMs, HAMD, Traditional Chinese Medicine Five State Emotion Questionnaire and SCL-90 in the study group were significantly lower than those in the control group ( t = 3.77, 7.01, 4.48, 6.99, 8.78, 4.89, 6.05, 4.19, 7.57, all P < 0.05). Total response rate in the study group was significantly higher than that in the control group (90.0% vs. 72.5%, χ2 = 4.02, P < 0.05). At 3, 6 and 12 months after treatment, recurrence rate in the study group was 2.5%, 15.0% and 30.0%, respectively, which were significantly lower than 17.5%, 35.0%, and 52.5% in the control group ( χ2 = 5.00, 4.26, 4.17, all P < 0.05). Conclusion:Five-element music therapy for the treatment of bipolar disorder can markedly alleviate depression and mania, restore patient's emotional state, improve clinical efficacy, and decrease recurrence rate.

ABSTRACT

Objective@#To explore the determinants for health-seeking behavior of the residents after cough for more than 3 weeks in Yiwu, Zhejiang Province, in order to provide reference for prevention and control of respiratory diseases.@*Methods@#A multi-stage cluster random sampling method was used to recruit the community residents aged 5 years and above in Yiwu. A face-to-face questionnaire survey was conducted to collect demographic information, features of cough and health-seeking behaviors in the past month. The multivariate logistic regression model was employed to analyze the associated factors for health-seeking behavior after a cough for more than 3 weeks. @*Results@#Among 6 374 residents investigated, 152 cases had a cough for more than 3 weeks in the past month, accounting for 2.48%. They were( 45.00±21.15 ) years old, including 70 ( 46.05% ) males and 82 ( 53.95% ) females. About 58.55% ( 89 ) of them sought medical treatment. The results of multivariate logistic regression analysis showed that females ( OR=2.100, 95%CI: 1.005-4.391 ), middle school education level ( OR=0.406, 95%CI: 0.168-0.983 ), family annual income of 100 000 to 199 999 yuan ( OR=2.993, 95%CI: 1.215-7.373 ) were associated factors for health-seeking behavior after a cough for more than 3 weeks.@*Conclusion@#The rate of health-seeking behavior after a cough for more than 3 weeks among the residents in Yiwu is 58.55%, which is associated with gender, education level and income.

@#【Objective】To constructset-up errors distribution prediction model for pelvic tumors patients radiotherapy of Varian NovalisTX medical linear accelerator.【Methods】The IGRT set-up errors data of 30 pelvic tumor patients after radiotherapywere analyzed,and Gaussian Mixtures was used to construct the errors distribution prediction model and to solve the model parameters.【Results】The set-up errors is mainly concentrated in four central points of μ1~μ4. The coordinates of each central points shows that the offsets of vertical(-3.88~4.28)mm and Longitudinal(-2.41~1.54)mm were larger than that of lateral(-1.85~0.72)mm. The possibility of offsets clustering near μ2 and μ4(0.301,0.310)is greater than μ1 and μ3(0.190,0.196). And the standard deviation of set- up errors can be up to 5.2 mm.【Conclusions】The Gaussian Mixtures Model made quantitative description and prediction analys is for the set- up errors distribution,which provide references for set-up errors control and planning target expansion of pelvic tumor radiotherapy in the absence of IGRT.

ABSTRACT

We investigated whether hepatitis B spliced protein affect the NF-kappa B activities by interacting with ubiquitously expressed transcript splice variant 1 (UXT-V1).The HBSP-UXT-V1 protein interactions were screened by yeast two hybrid assay,and confirmed by immunoprecipitation,confocal microscopy,mammalian two hybrid assay,and GST-Pulldown assay.The reporter plasmids driven by NF-kappa B promoter were transfected into UXT-knockdown HBSP stably expressed cell lines,and the reporter genes were detected after transfection.Results showed that the interaction between UXT-V1 and HBSP in yeast was demonstrated.Furtherly,HBSP could interact with UXT-V1 in mammalian cells.HBSP could enhance NF-kappa B activities,and this effect was partly achieved by the interaction with UXT-V1.In conclusion,the effect of HBSPUXT-V1 interaction on the NF-kappa B pathway in hepatocytes may have an impact on HBV related liver diseases.

ABSTRACT

·AIM: To observe the pupil diameter in different environment on myopia control effect of orthokeratology. ·METHODS: The optometry, pupil diameter and eye axial length were examined in 70 patients aged 8- to 13-year-old with low and moderate myopia, whom then were given night wear lenses, and we measured the length of eye axis and pupil diameter before wearing lenses,6, 12 and 24mo after wearing. In two different light environment groups, each group took the average pupil diameter as the dividing and divided into two subgroups; the pupil diameter higher than the mean group (19 cases, 17 cases respectively), the pupil diameter lower than the mean group (16 case, 18 cases respectively). The paired t-test, independent sample t-test, repeated measures ANOVA and Pearson correlation analysis were adopted to analyze. ·RESULTS: Whether in darkroom or exposed to natural light, the axial length of pupils whose diameter was lower than the mean diameter increased faster than that above the average diameter of pupils (P<0. 01). The initial pupil diameter was significantly negatively correlated with the growth of the eye axis at the end of the 24-month follow-up (r=-0.4267,P<0.05;r=-0.4925, P<0.01). · CONCLUSION: The myopia controlling effect of orthokeratology under two different illumination conditions showed that the effect of myopia control in children with larger pupils is better than that in children with smaller pupils, which may be that the larger pupil makes the orthokeratology maintain stronger intervention on peripheral retina myopic defocus.

ABSTRACT

Objective@#To investigate the root canal numbers of maxillary second primary molars.@*Methods @#216 max illary second primary molars from 204 children aged 3-8 years old were selected. All the teeth need to do the root canal therapy. After opening the pulp chamber, the root canal were examined and determined with small size K file and 17%ethylene diaminetetraacetic acid (EDTA). Hand instruments were used for root canal preparation and then zinc-oxide iodoform paste was used for filling. @*Results @#216 maxillary second p rimary molars, 122 teeth had three root canals(56.5%), and 94 teeth had four root canals (43.5%). The incidence of second mesiobuccal canal (MB2) was 41.2%. There was no correlation between the incidence of the MB2 canal and gender and tooth position. @*Conclusion@# The root canals of maxillary second primary molars had more variations, and there was a high incidence of the second mesiobuccal canal.

ABSTRACT

<p><b>OBJECTIVE</b>Chinese herbal medicine has been extensively used in the treatment of vascular dementia (VaD), but lacked systematic review on its efficacy and safety. So we conducted a systematic review to assess the efficacy and safety of Chinese herbal medicine in treating VaD.</p><p><b>METHODS</b>CNKI, CBM, PubMed, and Wiley Online Library were retrieved for randomized trials (RCTs) on Chinese herbal medicine treating VaD patients. Randomized parallel control trials by taking Chinese herbal medicine as one treatment method and placebos/cholinesterase inhibitors/Memantine hydrochloride as the control were included. Quality rating and data extraction were performed. RevMan5.2.0 Software was used for meta-analysis. Standardized mean difference (SMD) at 95% confidence interval (CI) was used to indicate effect indicators of results.</p><p><b>RESULTS</b>Seven RCTs met the inclusive criteria. Totally 677 VaD patients were randomly assigned to the treatment group and the control group. Descriptive analyses were performed in inclusive trials. The cognitive function was assessed in all trials. Results showed Mini-Mental state examination (MMSE) score was better in the Chinese herbal medicine group than in the placebo group, but with no significant difference when compared with the donepezil group (P > 0.05). Adverse reactions were mainly manifested as gastrointestinal symptoms such as abdominal pain in the Chinese herbal medicine group. But they occurred more in the donepezil group than in the Chinese herbal medicine group.</p><p><b>CONCLUSIONS</b>The methodological quality of included trials was poor with less samples. Results of different trials were lack of consistency. Present evidence is not sufficient to prove or disapprove the role of Chinese herbal medicine in improving clinical symptoms and outcome indicators of VaD patients. Their clinical efficacy and safety need to be supported by more higher quality RCTs.</p>

ABSTRACT

WU Xiao-ren devoted his whole life into acupuncture practice and education. During his 50 years clinical practice, teaching and researching, he focused on standardization and application of acupuncture manipulations. Through the integration of western and Chinese medicine as well as technique innovation, he developed new therapies for hypertension, stroke and various pain syndromes with the combination of acupuncture and materia medica and various acupoint prescription. He was against parochial prejudice by advocating absorption of others successful experiences and integration of different schools. Moreover, being conscientious and meticulous, WU Xiao-ren was always strict with his followers. He set up examples for his students with both precept and practice, and made great contribution to the inheritance of both acupuncture theory and practice.

ABSTRACT

Professor YU Shu-zhuang is a distinguished acupuncturist in China. He has practiced the TCM acupuncture-moxibustion clinical, educational and scientific research for 60 years in his life. In clinic, he summarized the experiences "five-ming first"; in treatment, he insisted "dredging" and "regulating", protecting the function of spleen and stomach, and needles should be less but specific. In the meanwhile, he made a deep study on the function and clinical effects of specific acupoints, and used the research results of propagated sensation along channel to guide clinical treatment, forming his special academic points. Professor YU has educated a great number of acupuncture-moxibustion talents in China and foreign countries, making great contribution to the popularization of acupuncture-moxibustion in the worldwide.

ABSTRACT

<p><b>BACKGROUND</b>The bacterial composition of periapical lesions in deciduous teeth has not been well documented. This study was designed to explore the bacterial compositions, especially the dominant bacteria in periapical lesions using 16S rRNA sequencing.</p><p><b>METHODS</b>Tissue samples were collected from 11 periapical lesions in deciduous teeth with primary endodontic infections. DNA was extracted from each sample and analyzed using 16S rRNA cloning and sequencing for the identification of bacteria.</p><p><b>RESULTS</b>All DNA samples were positive for 16S rRNA gene PCR. One hundred and fifty-one phylotypes from 810 clones were identified to eight phyla, and each sample contained an average of 25.9 phylotypes. In addition, 59 phylotypes were detected in more than two samples, and Fusobacterium (F.) nucleatum (8/11), Dialister (D.) invisus (8/11), Campylobacter (C.) gracilis (7/11), Escherichia (E.) coli DH1 (6/11), Aggregatibacter (A.) segnis (6/11), and Streptococcus (S.) mitis (6/11) were the most prevalent species. Furthermore, 45 as-yet-uncultivated phylotypes were also identified.</p><p><b>CONCLUSIONS</b>Chronic periapical lesions in deciduous teeth contained polymicrobial infections. F. nucleatum, D. invisus, C. gracilis, E. coli DH1, A. segnis, and S. mitis were the most prevalent species detected by 16S rRNA sequencing.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the chemical compounds from the ethanol extract with inhibitory effects against aldose reductase from Thunbergia.</p><p><b>METHOD</b>Guided by anti-aldose reductase assay, compounds from the bioactive fraction (ethyl acetate extract) were separated and purified by various chromatographic methods including silica gel, Sephadex LH-20, and reversed-phase HPLC. Their structures were indentified based on analysis of the spectroscopic data including 1D and 2D NMR data.</p><p><b>RESULT</b>Eight compounds were obtained and identified as 8-hydroxy-8-methyl-9-methene-cyclopentane [7,11] -1,4, 6-trihydroxy-tetrahydronaphthalene-12-one, named as thunbergia A (1), 3,4-dihydro-4,5,8-trihydroxy-2-(3-methyl-2-butenyl) naphtha[2,3-b] oxiren-1(2H)-one (2), 8-(beta-gluco pyranosyloxy)-3,4-dihydro-2-(3-methyl-2-butenyl)naphtha [2,3-b] oxiren-1(2H)-one (3), galangin (4), quercetin (5), luteolin (6), 5,6,3',4'-tetrahydroxy -3,7-dimethoxy-flavone (7) and upeol (8).</p><p><b>CONCLUSION</b>Thunbergia A was a new derivative of tetrahydronaphthalene, and compounds 2 and 3 were separated from the genus Thunbergia for the first time.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the epdimiology characteristics and the diversity of VP6 gene of GCRV in Lulong, and to provide the basis for GCRV in-depth research.</p><p><b>METHODS</b>793 stool specimens from porcine with diarrhea or not from Lulong in 2007 and 2008. GCRV was detected by nested multiple reverse transcription- polymerase chain reaction (nested RT-PCR) , and analyzed the identity and conducted phylogenetic tree by the seqences.</p><p><b>RESULTS</b>The positive rate of GCRV was 16.65%. Porcine GCRV strains of Lulong had significant homology differences. Phylogenetic analysis indicated porcine GCRVs were with significant diversity. Amino acid analysis showed GCRV strains with the same host shared the nearest kinship.</p><p><b>CONCLUSION</b>The infection rate of GCRV was high from 2007 to 2008 in Lulong. Homology and phylogenetic analysis showed that VP6 gene diversity was widespread. The experimental data provided basis for molecular characteristics of porcine GCRVs.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To obtain information on viral molecular structural and evolutionary characteristics, we conducted the SZ2010422 full-length genomic analysis.</p><p><b>METHODS</b>Primers were designed by New Orleans full sequence, SZ2010422 full genome was amplified by RT-PCR, the whole genome sequence and the capsid domain amino acid sites was analysised after cloned and sequenced.</p><p><b>RESULTS</b>The genome of G II-4 Norovirus SZ2010422 strain was consist of 7559 bp, it revealed three ORFs composites of the whole genome, ORF1 (5100 bp), ORF2 (1623 bp), ORF3 (807 bp) respectively, ORF1 and ORF2 had 19 nucleotide overlap. By evolutionary comparative analysis found SZ2010422 genomic nucleotide sequences with reference strains of G II-4 New Orleans1805 strains the highest homology with a total length of homology was 99.3%, of ORF1 (99.5%), ORF2 (99.2%), ORF3 (98.6%). Phylogenetic analyses showed SZ2010422 belonging to G II-4 New Orleans variant. Date of 541 amino acid analyses showed: New Orleans variant strains of popular sites: aa310N or K, --> S aa341D --> of N, aa359T--> S, aa396H --> P, aa460H --> Y.</p><p><b>CONCLUSION</b>Norovirus SZ2010422 belonged to the G II-4 New Orleans variant. In This study, SZ2010422 full sequence can be used not only as a full-length NoV variant sequence standard for future comparison studies, but also as useful material for the public health field by enabling the diagnosis, vaccine development, and prediction of new emerging variants. ABSTRACT

<p><b>OBJECTIVE</b>To explore which index is more suitable to show the degree of sleep fragment in children with sleep-disordered breathing (SDB).</p><p><b>METHODS</b>Between October 2009 and August 2011, Forty-five children (4 - 8 years) who were diagnosed as obstructive sleep apnea hypopnea syndrome (OSAHS) were enrolled in OSAHS group[obstructive apnea index (OAI) > 1 times/h or apnea hypopnea index (AHI) > 5 times/h, lowest oxygen saturation (LSaO2) < 0.92] and 54 children were enrolled in SDB group (1 ≤ AHI ≤ 5 times/h and OAI ≤ 1 times/h), 18 children with chorditis nodules made up control group (AHI < 1 times/h and LSaO2 ≥ 0.92, without SDB-related history). The difference of respiratory arousal index (RAI), spontaneous arousal index (SAI), total arousal index (ARtotI) and sleep pressure score (SPS) were compared among three groups. The correlation between RAI, SAI, ARtotI, SPS and AHI were also analyzed. Furthermore, RAI, SAI, ARtotI and SPS were compared before and after operation in 14 OSAHS children with detailed pre- and after polysomnography data.</p><p><b>RESULTS</b>The difference of SAI and ARtotI between SDB group and OSAHS group and ARtotI between OSAHS group and control group were not significant (P > 0.017), except this, the difference of other index between any two groups or SAI and ARtotI between otherwise two groups were significant (P < 0.017). RAI and SPS was correlated with AHI (coefficient correlation: 0.751, 0.829, P was 0.000). RAI and SPS decreased after operation and the difference was significant (Z were -3.045 and -2.982, P were 0.002 and 0.003). The difference of sleep structure was not significant.</p><p><b>CONCLUSIONS</b>RAI and SPS were more suitable to show the degree of sleep fragment than other arousal index.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the reasons of multiple operations in children with airway foreign body through analyzing the clinical data of children who received two or more operations.</p><p><b>METHODS</b>From 2003 to 2009, all children with airway foreign body who received two or more operations in hospital were enrolled. The clinical manifestations, image before and after operation and intraoperative conditions were retrospectively analyzed, in order to find the reasons of multiple operations.</p><p><b>RESULTS</b>All children fully recovered, no serious complications or death. The reasons of two or more operations were multiple: 21 cases (42.8%) were related to the factor of apparatus, 20 (40.8%) cases were related to the quality, surrounding conditions and location of the foreign body and experience and surgical skills of operator, 4 (8.2%) cases were due to incarceration of foreign body, another 4 (8.2%) cases were due to unstable intraoperative oxygen saturation.</p><p><b>CONCLUSIONS</b>Both subjective and objective factors (quality, surrounding conditions or location of foreign body, et al) were related to multiple operations. To reduce the chance of multiple operations, careful preoperative assessment and preparation are necessary.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the level of serum uric acid (UA) in children with obstructive sleep apnea/hypopnea syndrome (OSAHS).</p><p><b>METHOD</b>Between Sep. 2008 and Mar. 2010, 138 children with OSAHS were enrolled in study group. Sixty-five children with accessory auricle or ptosis of upper lid were enrolled into the control group. Furthermore, according to apnea/hypopnea index (AHI) or obstructive apnea index (OAI) the study group was further divided into three subgroups (mild, moderate and severe group). At last, the study group and control group were divided into two groups according to the body mass index (BMI), separately. The fasting serum UA level was compared among the different groups. Then the correlation between the serum UA level and AHI, BMI, oxygen desaturation index, least arterial oxygen saturation (LSaO(2)) and the percentage of total sleep time with arterial oxygen saturation < 0.92 was also analyzed in OSAHS children with or without overweight and obesity respectively.</p><p><b>RESULT</b>The difference of serum UA level between the study group and control group (z = -0.443), and the difference among the three groups (χ(2) = 1.241) was not significant(P > 0.05). The serum UA level in overweight and obese children [study group, 273.0 (238.3 - 357.3); control group, 298.0 (253.0 - 336.0)] was significantly higher than that in children with normal BMI [study group, 246.5(215.8 - 300.0); control group, 266.0 (224.0 - 303.3)] (z = -2.084, -2.214, P < 0.05). That serum UA level did not correlate with the above index of OSAHS was observed in children with or without overweight and obesity in study group (P > 0.05).</p><p><b>CONCLUSION</b>Findings of higher serum UA level were not observed in children with OSAHS. There was no correlation between serum UA level and the above indices of OSAHS. The serum UA level in overweight and obese children was significantly higher than that in children with normal BMI.</p>

ABSTRACT

<p><b>BACKGROUND</b>Accurate intraoperative diagnosis of sentinel lymph node (SLN) metastases enables the selection of patients for axillary lymph node dissections during the same operation, reducing the need for a second operation. The present study aimed to prospectively compare the GeneSearch(TM) Breast Lymph Node (BLN) Assay with touch imprint cytology (TIC) for intraoperative evaluation of SLNs.</p><p><b>METHODS</b>SLNs were sectioned in 1.5 - 3.0 mm pieces. TIC was performed on all pieces and the BLN Assay and postoperative histology evaluations were performed on different alternating node pieces. Overall performance of the BLN Assay was compared with that of TIC relative to the postoperative histology results.</p><p><b>RESULTS</b>A total of 90 patients enrolled in the study. Complete intraoperative data for both the BLN Assay and TIC were collected in 86 patients. The sensitivity, specificity, and overall accuracy of the BLN Assay were 82%, 97%, and 92%, respectively on a per patient basis compared with those of TIC which were 67%, 100%, and 90%.</p><p><b>CONCLUSIONS</b>Performance of the BLN Assay was superior to that of TIC and the additional application of TIC did not help improve the total sensitivity and accuracy of the intraoperative assessment. The existence of ectopic breast tissue might be a possible cause of false positive for the BLN assay. In addition, the BLN Assay complements histopathology assessment and can minimize sampling error without increasing pathologists' workload.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the feasibility of D2-40 labeling for detecting lymphatic vessels in breast cancer tissues and the clinicopathological implications of lymphatic vessel invasion (LVI).</p><p><b>METHODS</b>Immunohistochemistry was used to detect the expression of D2-40 and lymphatic invasion in 72 cases of breast cancer and 15 benign breast tumor tissues, and their correlations to the clinicopathological factors were analyzed.</p><p><b>RESULTS</b>The positivity rate of LVI was 69.4% in breast cancer tissue. In patients with lymph node metastasis, the positivity rate of LVI was significantly higher than that in patients without lymph node involvement (85.7% vs 54.1%, P<0.01), and LVI was found to be positively correlated to axillary lymph node metastasis (r=0.382).</p><p><b>CONCLUSION</b>D2-40 can specially and reliably mark the lymphatic vessels in breast cancer tissues. LVI is positively correlated to axillary lymph node metastasis and occurs earlier than the latter, therefore should be included in routine clinicopathological testing.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the expression of D2-40 and CD34 in invasive ductal carcinoma of the breast and the clinical significance.</p><p><b>METHODS</b>D2-40 and CD34 expressions were detected immunohistochemically in 108 cases of invasive ductal carcinoma of the breast and 30 cases of breast fibroadenoma. The lymphatic microvessel density (LMD) and the microvessel density (MD), marked by D2-40 and CD34, respectively, were calculated and their relationship with the clinicopathological factors was analyzed.</p><p><b>RESULTS</b>The LMD and MD for invasive ductal carcinoma of the breast were significantly higher than those of breast fibroadenoma (P<0.01). One-way ANOVA indicated that the LMD differed significantly between tumors of different histological grades, status of lymph node metastasis and TNM stages; the MD also varied significantly with the tumor size, histological grade, lymph node metastasis and TNM stage. The LMD and MD showed a positive correlation to the tumor size (r=0.335, 0.337), histological grade (r=0.580, 0.671), lymph node metastasis (r=0.690, 0.721) and TNM stage (r=0.623, 0.634), but not to ER, PR or Her-2 status (P>0.05).</p><p><b>CONCLUSIONS</b>D2-40 can specifically mark the lymphatic endothelial cells in invasive ductal carcinoma of the breast. The LMD and MD are positively correlated to the clinicopathological factors of the malignancy. D2-40 and CD34 has the potential for use as the predictors to evaluate the tumor progression and metastasis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>to evaluate the application of GeneSearch(TM) breast lymph node assay in intraoperative detection of metastases in sentinel lymph node (SLN) from breast cancer patients.</p><p><b>METHODS</b>a total of 225 SLN from 88 patients was prospectively studied. Each SLN was cut into 2 mm slabs which were examined by intraoperative imprint cytology (IIC) first, followed by GeneSearch assay and post-operative serial sectioning. GeneSearch used real-time fluorescence quantitative RT-PCR technology to detect the expression of CK19 and mammaglobin in SLN. The results of GeneSearch assay were correlated with those of IIC and post-operative serial sectioning.</p><p><b>RESULTS</b>amongst the 88 cases studied, 225 SLNs were found, and obvious metastatic carcinoma cells were identified in 27 SLNs and micrometastasis in 9 SLNs. One hundred and eight-nine SLNs were considered as "negative" (with "isolated tumor cells" present in 5 SLNs). The turn-around time of intraoperative GeneSearch assay ranged from 35 to 45 minutes (mean = 40 minutes). The concordance rate between GeneSearch assay and post-operative serial sectioning was 95.6% (215/225), with a sensitivity of 86.1% (31/36), compared with 94.7% (213/225) and 72.2% (26/36) respectively for IIC. The size of metastatic foci correlated with the Ct value of CK19 and mammaglobin (P < 0.01).</p><p><b>CONCLUSIONS</b>GeneSearch assay for intraoperative detection of metastase in SLN has a satisfactory performance and demonstrates a relatively higher sensitivity than IIC. The potential clinical application still requires further evaluation of larger number of cases.</p>
