ABSTRACT

Lectins are proteins responsible for recognizing the signals of sugar molecules in the body. Sialic acid-binding immunoglobulin-like lectins (Siglecs) regulate the innate and adaptive immune responses in the tumor microenvironment by recognizing the glycan structure containing sialic acid and mediating downstream signals through immune receptor tyrosine inhibitory motifs. In recent years, a variety of tumor treatment strategies targeting the sialic acid-Siglecs axis have been introduced, including sialoglycoprotein-mediated drug delivery and antibody mediated inhibition of Siglecs from recognizing tumor surface ligands. In the future, by combining with glycoprotein nanotherapy, antibody therapy and gene therapy, Siglecs can be used to accurately locate tumor targets and release the anti-tumor immunity, so as to achieve the purpose of effective cure of tumors.

ABSTRACT

Objective:To investigate the relationship between the prevalence of cervical spondylosis and occupational stress among employees in Jing 'an community health service centers in Shanghai. Methods:A concise occupational stress questionnaire was developed based on the pay return imbalance model. The prevalence of cervical spondylosis and the incidence of occupational stress were investigated with the questionnaire survey among employees of all 15 community health service centers in a district of Shanghai from September 2020 to November 2020. Multiple logistic regression was used to analyze the relationship between occupational stress factors and prevalence of cervical spondylosis.Results:Of total 1 816 employees in the community health service centers in the district, 1 515 independently completed the questionnaire (83.4%). The average age of responders was 39 years old. The prevalence of cervical spondylosis was 34.7% (526/1 515); and the incidence of occupational stress was 11.6% (175/1 515). The average score of pay factor in the sick group was higher than that in the non-sick group ( t=-8.04, P<0.001), the average score of return factor was lower than that of the non-sick group ( t=6.06, P<0.001), and the average score of internal input factor was higher than that of the non-sick group ( t=-8.39, P=0.004). Multivariate regression analysis showed that occupational stress ( OR=1.57, 95% CI: 1.05-2.36), medium investment ( OR=1.42, 95% CI: 1.02-2.00), high investment ( OR=1.93, 95% CI: 1.27-2.93) were the risk factors of cervical spondylosis (all P<0.05). Conclusions:Cervical spondylosis is associated with occupational stress among employees of community health service centers. To prevent the prevalence of cervical spondylosis, it is necessary to reduce occupational tension, alleviate the imbalance of pay and return, and prevent individuals from excessive internal investment.

ABSTRACT

Objective To analyze the heterogeneities of hepatitis B virus ( HBV ) reverse transcriptase domain (RT) gene mutations related to nucleos (t)ide analogues (NAs) resistance.Methods Blood samples from 2765 chronic hepatitis B patients with virological breakthrough or poor drug response treated in Ningbo No .2 Hospital and Ningbo Fourth Hospital from April 2011 to March 2018 were collected . According to the medication status , it was divided into LAM monotherapy group ( n =603 ) , LdT monotherapy group (n=147), ADV monotherapy group (n=68), ETV monotherapy group (n=10) and the sequential or combined drug resistance of NAs group (n=365).The resistance mutation sites and drug resistance patterns (pathways) of each group were analyzed .The SPSS 19.0 software was used to analyze the data.Results Among 2765 serum samples, the NAs-related HBV-RT resistance mutations were detected in 1193 cases with an overall mutation rate of 43.15%.The mutation rate of LAM monoclonal resistance group was 62.62% (603/963) with 19 mutation types, the most common single point mutation was rtM204I/V (40.30%, 243/603).The mutation rate of LdT monoclonal resistance group was 45.51%(147/323), and there were 3 mutation types, with the single point mutation rtM204I/V being the most common (59.86%, 88/147).The mutation rate of the ADV monoclonal resistance group was 17.80%(68/382), mainly rtA181T single point mutation (64.71%, 44/68).The mutation rate of the ETV monoclonal resistance group was 4.06%(10/246), and the single point mutation of rtT184A/G/S/I/L/F was the most common one (80.00%, 8/10).The mutation rate of the sequential or combination therapy group was 41.91% (365/871), among which the mutation rate of the LAM/LdT poor response or the resistance with the sequential ADV group was 63.39%(142/224), and the most single mutation point was rtA181V/T ( 35.21%, 50/142 );the mutation rate of LAM/LdT poor response or drug-resistant with combined ADV group was 42.19% (54/128), and the most common mutation point was rtA181V/T (46.30%, 25/54);the mutation rate of LAM/LdT with poor response or resistance after sequential ETV 1.0 mg was 44.66%(117/262), and the most common mutation point was rtL180M+M204I/V+S202G/I (31.62%, 37/117);the LAM/LdT poor response or the drug-resistant ETV combined with ADV group had a mutation rate of 7.14%(5/70), all of which were multi-site mutations;the mutation rate of poor response to ADV or resistant with sequential ETV 0.5 mg group was 28.14%(47/167), all of which were multi-site mutations.Secondary ( compensation ) sites such as rtV173L, rtL180M, and rtV214A, and single-point mutations such as rtV207I/L/G, rtS213Tand rtN238T, which were not fully defined , were detected.The resistance patterns ( pathways ) of NAs monotherapy were relatively simple , and the resistance patterns ( pathways ) of NAs experienced patients ( sequential or combined treatment group ) were complex and diverse, and multiple resistance patterns (pathways) existed, along with NAs increasing in species.Non-first-line NAs-related resistance patterns ( pathways ) showed an overall downward trend sand ETV-related drug-resistant mutation showed an overall upward trend .Conclusion The NAs-related HBV resistance mutation sites ( patterns ) are complex and diverse , especially multi-site mutations , refractory drug resistance mutations, multidrug resistance mutations and cross-resistance mutations.Therefore, the optimization of antiviral treatment strategies and drug resistance management concepts need to be continuously updated .

ABSTRACT

Objective: To observe the efficacy and safety of analgesic drugs in the standardized treatment of cancer pain patients at the pain clinic. Methods: The data of 787 cancer pain patients and their corresponding prescriptions for cancer pain were collected from April, 2012 to April, 2013 at the pain clinic. The obtained information comprise of diseases that lead to cancer pain, cause of pain, pain intensity, and efficacy and side effects of medications. Diseases that caused cancer pain include 658 cases with primary malignant lung cancer. Results: Pain was mainly caused by primary lung cancer in 787 cancer-related patients. An analgesic drug, namely, oxycodone hydrochloride, was administered in 54.6% via single drug therapy. The daily dosage range of this drug was 20 to 90 mg/d in 280 cases. About 35.6% of the studied patients with a daily dosage of 90 mg/d or lower had their pain effectively managed. After the treatment, the number of cases with moderate to severe pain was reduced from 437 (55.5%) to 248 (31.5%). The oral administration of opioid oxycodone hydrochloride tablets ranked first among the prescribed drugs for cancer pain, and single-drug therapy was the choice of medication. The majority of patients had satisfactory pain-relief with a daily dosage of less than 90 mg/d upon the administration of oxycodone hydrochloride sustained-release tablets and morphine sulfate controlled-release tablets. Side effects included mild constipation, nausea, vomiting, dizziness, loss of appetite, urinary retention, somnolence, and so on. Intervention treatment was needed in most of the patients. Conclusion: Pain clinic is critical in the administration of standardized treatment for cancer pain in hospitals. The establishment of pain clinic ensures the standardized treatment of cancer pain.
