ABSTRACT

ObjectiveTo determine the trend in diabetes-related death and probability of premature mortality among residents in Yangpu District of Shanghai and provide evidence for the formulation of relevant intervention strategies. MethodsMortality and demographic data were collected among residents in Yangpu District of Shanghai from 2002 to 2020. Statistical analysis was conducted using chi-square test with SPSS 21.0 and Excel 2010. Joinpoint regression was used to determine annual percent change (APC). The mortality was standardized by utilizing the world standard population in 2000. ResultsA total of 25 091 cases of diabetes-related deaths were reported in Yangpu District, Shanghai from 2002 to 2020. The average annual crude mortality of diabetes-related diseases was 122.10/105, which was 116.13/105 in males and 128.23/105 in females. The difference between males and females in crude mortality was statistically significant (P<0.05). Moreover, primary causes of diabetes-related deaths were diabetes, cardiovascular and cerebrovascular diseases, and tumors. Among diabetic deaths, peripheral circulatory complications accounted for 50.79%, followed by renal complications (16.05%). The crude mortality in males, females, and total of diabetes-related diseases showed an upward trend, while the standardized mortality remained stable with an upward trend in male and a downward trend in female. Furthermore, the crude mortality in males, females, and total of diabetes complicated with cardiovascular and cerebrovascular diseases showed an increasing trend. In contrast, both the standardized mortality in males and in total showed an increasing trend, while that in females remained stable. The overall crude mortality of diabetes was on the rise, which was increasing in males and stable in females. The overall standard mortality of diabetes was on the decline, which was increasing in males while declining in females. In addition, the probability of premature mortality caused by diabetes-related diseases, cardiovascular and cerebrovascular diseases complicated with diabetes, and diabetes decreased from 2002 to 2020 with no statistical significance. Males showed an upward trend while females showed a downward trend. ConclusionThe mortality of diabetes-related diseases, cardiovascular and cerebrovascular diseases complicated with diabetes, and diabetes among residents in Yangpu District of Shanghai is on the rise. Similarly, standardized mortality and probability of premature mortality in males for all three diseases are also on the rise. It warrants more attention to the health of male diabetes patients and targeted measures to reduce the disease burden.

ABSTRACT

ObjectiveTo assess the effects of countermeasures against coronavirus disease 2019 (COVID-19) in Shanghai from March to May 2022 in comparison with epidemiological trend of COVID-19 in New York City. MethodsDaily confirmed cases, asymptomatic SARS-CoV-2 carriers, and daily deaths were obtained in the Shanghai Municipal Health Commission and the Center for Disease Control and Prevention (CDC) of the United States. Descriptive study was conducted by using these data. ResultsFrom March 1 to May 17, the number of daily asymptomatic SARS-CoV-2 infections in Shanghai was up to 58 times as large as that of daily confirmed cases; however, the number of daily confirmed cases in Shanghai was generally less than that in New York in the same time period. At the peak of the COVID-19 epidemic, the growth of daily attack rate in Shanghai was significantly lower than that in New York (P<0.05). Moreover, the number of daily death was evidently less than that in New York. In addition, the vaccination rate in the elderly (aged ≥60 years) in Shanghai was evidently lower than that in New York (aged ≥65 years). ConclusionThe COVID-19 epidemics in Shanghai from March to May 2022 and in New York after December 2021 were both caused by the Omicron variant. Compared with the Delta variant, the Omicron variant has stronger replication ability and infectivity, resulting in challenges to the containment of the epidemic in metropolis such as Shanghai and New York City. The epidemic in New York City remained crucial due to absence of effective countermeasures, while that in Shanghai has been effectively contained with strict countermeasures. The prevention and control strategies may be adjusted along with the continual evolution of SARS-CoV-2 and increasing trend of imported COVID-19 cases.

Objective To investigate the effects of hepatitis B virus (HBV) genotype and mutations on the development of hepatocellular carcinoma (HCC) and to establish a new qualified HCC risk scores.Methods A cohort study enrolling patients with chronic HBV infection was conducted.HBV genotypes were identified by nested multiplex PCR.HBV mutations in the basic core promoter region and PreS region were sequenced after PCR amplification.Scores on risk factors were set based on nomogram.Results Totally,1 525 patients were followed-up in this research.A total of 1 110 patients infected with genotype C were followed-up for 8.52 (QR:5.36-11.68) years on average,of whom the incidence of HCC was 11.93/1 000 person-years.In genotype C HBV infected patients,male gender,aged 40 years and over,and four DNA mutations (T 1674CG,A 1762T/G 1764A,A3120T,and A2962G) can increase the risk of HCC (P<0.05);interferon therapy can reduce the risk of HCC (P<0.05).A new HCC predicting model was established according to the results.After validation,the predicted disease-free survival rate was consistent with the real one.Conclusions Hepatitis B virus genotypes and mutations were closely associated with HCC.The new risk scoring system can well predict HCC occurrence in genotype C HBV infected patients.

ABSTRACT

Beta-lactams are the most widely used antibiotics. One of the principle mechanisms for Gram-negative bacteria to resist β-lactams is by producing β-lactamases that degrade β-lactams. This review highlights two regulatory mechanisms for inducing β-lactamase in Gram-negative bacteria. In the ampR-ampC paradigm, the induction of β-lactamase is intimately linked to peptidoglycan recycling. AmpR, a LysR-type transcriptional regulator, plays a central role in regulating expression of β-lactamase. Recent studies found that two-component signal transduction pathway is activated by β-lactams, which in turn induces the expression of β-lactamase. Finally, we discussed the future research directions in β-lactam resistance in Gram-negative bacteria.

BACKGROUND@#We previously showed that the expression of follistatin-like protein 1 (FSTL1) was significantly down-regulated in metastatic clear-cell renal cell carcinoma (ccRCC). In this study, we aimed to characterize the role of FSTL1 in the development of ccRCC.@*METHODS@#The effects of FSTL1 on cell activity and cell cycle were investigated in ccRCC cell lines with altered FSTL1 expression. Gene expression microarray assays were performed to identify the major signaling pathways affected by FSTL1 knockdown. The expression of FSTL1 in ccRCC and its effect on postoperative prognosis were estimated in a cohort with 89 patients.@*RESULTS@#FSTL1 knockdown promoted anchorage-independent growth, migration, invasion, and cell cycle of ccRCC cell lines, whereas FSTL1 overexpression attenuated cell migration. FSTL1 knockdown up-regulated nuclear factor-κB (NF-κB) and hypoxia-inducible factor (HIF) signaling pathways, increased epithelial-to-mesenchymal transition, up-regulated interleukin-6 expression, and promoted tumor necrosis factor-α-induced degradation of NF-κB inhibitor (IκBα) in ccRCC cell lines. FSTL1 immunostaining was selectively positive in epithelial cytoplasm in the loop of Henle, and positive rate of FSTL1 was significantly lower in ccRCC tissues than in adjacent renal tissues (P < 0.001). The multivariate Cox regression analysis showed that the intratumoral FSTL1 expression conferred a favorable independent prognosis with a hazard ratio of 0.325 (95% confidence interval 0.118-0.894). HIF-2α expression was negatively correlated with FSTL1 expression in ccRCC specimens (r = - 0.229, P = 0.044). Intratumoral expression of HIF-2α, rather than HIF-1α, significantly predicted an unfavorable prognosis in ccRCC (log-rank, P = 0.038).@*CONCLUSIONS@#FSTL1 plays a tumor suppression role possibly via repressing the NF-κB and HIF-2α signaling pathways. To increase FSTL1 expression might be a candidate therapeutic strategy for metastatic ccRCC.

ABSTRACT

Objective To investigate the applicative value of enhanced 3D multi-echo GRE T2*-weighted angiography(ESWAN) sequence phase values in evaluating brain gray nuclei iron content in idiopathic restless legs syndrome(RLS) patients, providing imaging basis in diagnosis and treatment of idiopathic RLS. Methods In our institute from June 2012 to September 2016,forty-five RLS patients were selected as the RLS group, and 45 healthy volunteers as the control group. ESWAN sequence was performed and serum ferritin values were obtained in all patients and volunteers. The raw data of ESWAN was postprocessed , where the phase maps were obtained. Phase analysis was performed on localized brain gray nuclei regions of interest (substantia nigra, red nucleus, dentate nucleus, thalamus, pallidum, putamen and caudate nucleus ) selected on phase maps. Differences between the 2 subject groups were evaluated using ANCOVA including age as a covariate. Results The phase values of the substantia nigra, thalamus, pallidum and putamen in the RLS group were (-0.087 ± 0.021), (-0.053 ± 0.012), (-0.161 ± 0.008), (-0.125 ± 0.019) radians , respectively. The phase values of the substantia nigra, thalamus, pallidum and putamen in the control group were (-0.127 ± 0.007), (-0.066 ± 0.007), (-0.166 ± 0.007), (-0.150 ± 0.010) radians, respectively. There were significant differences between the two groups (F=142.492, 37.988, 10.558, 60.725;P0.05). Conclusions Phase values can make a quantitative assessment of brain gray nuclei iron content in RLS patients, our results supported the hypothesis of reduced brain iron content in RLS patents , which may have an important role in the pathogenesis of the disorder. However, iron content change in some brain regions was not correlated with serum ferritin concentration changes.

ABSTRACT

Objective To study the correlation between the diffusion-weighted imaging (DWI) measurements and glomerular filtration rate (GFR), Katafuchi scores in IgA nephropathy. Methods Thirty-five patients with IgA nephropathy (IgAN group) and twenty healthy volunteers (control group) were enrolled in this study. All of the subjects underwent bilateral renal DWI measurements with 3.0T MRI scanner. The values of apparent diffusion coefficient (ADC) of renal cortex and medulla were measured. GFR of IgAN group was detected with 99Tcm-DTPA scintigraphy. Based on the Lee classification and the Katafuchi score system, the pathological grading was carried out in patients of IgAN group. The ADC values were compared between control group and different grades of IgAN group. The correlations between ADC and GFR values were analysed in defferent groups. The correlations between ADC values and Katafuchi scores were analysed in IgAN group. Results The renal cortical ADC values were significantly higher than medulla ADC values in both control group and IgAN group (P 0.05). There was a positive correlation between the renal cortical and medulla ADC values and the GFR values in IgAN group (P medulla ADC values and the Katafuchi scores in IgAN group (P<0.05). Conclusion The diffusion-weighted imaging can reflect the physiological functions of kidney. It was feasible for application DWI in IgA nephropathy, which can be used for assessing the renal filtration function and the pathological damage. However, DWI measurement is not sensitive to early renal disease.

ABSTRACT

Objective To investigate the relationship of the serum levels of high sensitivity C-reactive protein(hs-CRP) and homocysteine with middle cerebral artery(MCA) intraplaque hemorrhage.Methods A total of 63 patients who met the inclusion criteria showed high-grade(≥ 70%)MCA stenosis.The levels of serum hs-CRP and Hcy were detected.All patients were divided into symptomatic group and asymptomatic group,intraplaque hemorrhage group and non-intraplaque hemorrhage group.All patients performed conventional MRI and high-resolution MRI(HR-MRI).The correlation of the serum levels of hs-CRP and homocysteine with middle cerebral artery intraplaque hemorrhage was analyzed.Results Totally 37 symptomatic and 26 asymptomatic MCA stenoses were analyzed.The occurrence rate of intraplaque hemorrhage was significantly higher in symptomatic MCA stenosis group than in asymptomatic MCA stenosis group (40.5 % vs.11.5 %,x2 =6.29,P< 0.05).Symptomatic group displayed a higher hs-CRP and Hey levels (8.97 ± 3.36 mg/L and 20.00 ± 3.16 μmol/L,respectively) than did asymptomatic group [(5.26 ± 3.12) mg/L and (12.22 ± 1.88) μmol/L,t =4.43 and 11.23,respectively,each P < 0.001].The hs-CRP and Hcy levels were higher in intraplaque hemorrhage group [(10.53 ± 3.59) mg/L and (21.70 ± 2.40) μmol/L,respectively] than in non-intraplaque hemorrhage group[(6.20 ± 3.02) mg/L and(11.77±1.69) μmol/L,t=4.87 and 18.58,respectively,each P<0.001].The hs-CRP and Hcy levels were positively correlated with the risk for middle cerebral artery intraplaque hemorrhage(r=0.461 and 0.519,each P<0.001).Conclusions The serum levels of hs-CRP and Hcy are closely associated with middle cerebral artery intraplaque hemorrhage,which can be used to evaluate the stability of plaque.The MCA intraplaque hemorrhage is associated with ipsilateral stroke.

ABSTRACT

Objective To study the value of MR diffusion tensor imaging(DTI) in the staging of chronic kidney disease(CKD).Method MR diffusion tensor imaging was used on 26 CKD patients.All patients were diagnosed as CKD according to the criteria of clinical diagnostic.All MR images achieved diagnostic requirements.Diethylenetriamine pentaacetic acid(99mTc-DTPA) renal dynamic imaging was used to determine the unilateral renal function.CKD patients were determined as mildly renal impairment group and moderately to severely renal impairment group based on the glomerular filtration rate(GFR).Twenty healthy volunteers underwent DTI at the same time.The cortical and medullary ADC value and FA value were measured in all subjects who underwent conventional MRI and DTI.The paired Student's t test was used to compare the cortico-medullary difference of the mean ADC and FA values in all subjects.The oneway analysis of variance(ANOVA) was carried out to assess the difference among the medullary and cortical diffusion parameters(ADC,FA) of all the three groups,and LSD was used to assess multiple comparisons.The correlation of GFR and ADC value of the patients,and FA value of the patients were tested by Pearson correlation analysis.Results The cortical and medullary ADC values of 46 subjects were(2.25±0.25) × 10-3 and(2.10±0.20) × 10-3 mm2/s respectively,FA of them were(3.48±0.61) and(4.27±0.75) respectively(t =6.754,10.043 respectively; P<0.01).The cortical ADC values of volunteers,mild renal impairment group,severely renal impairment group were(2.25±0.22) ×10-3,(2.31±0.19) ×103,(2.18±0.32) ×10-3mm2/s respectively.The medullary ADC value of the three groups were(2.09±0.19) × 10-3,(2.17±0.17) × 10-3,(2.06±0.24) × 10-3mm2/s respectively(F=0.968,0.882 respectively; P>0.05).The cortical FA values of the three groups were 3.85± 0.47,3.18±0.62,3.20±0.52 respectively.The medullary FA of the three groups were 4.92±0.38,3.93±0.57,3.62 ± 0.53(F=8.725,33.705 respectively; P<0.01).There was no correlation between cortical ADC,medullary ADC,cortical FA of the patients and GFR [(30.14±15.79) ml· min-1 · 1.73 m-2](r =0.243,0.197,0.196 respectively; P>0.05).There was a positive correlation between medullary FA of the patients and GFR (r=0.556,P=0.003).Conclusion FA value has certain value in evaluating CKD early diagnosis and renal function.

ABSTRACT

Objecyive To investigate the perioperative nursing of type Ⅱ diabetes mellitud treated by laparoscopic gastric bypass surgery.Methods Pefioperative nursing and follow up were performed for twenty five patients with type Ⅱ diabetes who were treated by laparoscopic gastric bypass surgery.Results The conditions of these patients were improved significantly by prompts of fasting blood glucose and 2h post-prandial blood glucose after glucose tolerance test was performed for every patient.Conclusions Pertinent perioperative nursing has proactive effect on recovery of patients with type diabetes mellitus and treated by laparoscopic gastric bypass.

ABSTRACT

During the installation of a pipe pile, the soil around the pile will be squeezed out. This paper deals with this squeezing effect of open-ended pipe piles using the cylindrical cavity expansion theory. The characteristics of soil with different tension and compression modnli and dilation are involved by applying the elastic theory with different moduli and logarithmic strain. The closed-form solutions of the radius of the plastic region, the displacement of the boundary between the plastic region and the elastic region and the expansion pressure on the external surface of the pipe piles are obtained. When obtaining these solutions, the soil ping in the open-ended pipe pile is considered by employing an incremental filling ratio to quantify the degree of soil plugging. Moreover, the effects of the ratio of tension and compression moduli, angle of dilation and incremental filling ratio on the radius of the plastic region and the expansion pressure on the external surface of the pipe pile are investigated. The parametric analyses show that it is necessary and important to consider the difference between the tension modulus and compression modulus, dilation angle and incremental filling ratio for studying the squeezing effect of open-ended pipe pile installation. It is concluded that the analytical solutions presented in this paper are suitable for studying the squeezing effect of open-ended pipe piles.

ABSTRACT

<p><b>OBJECTIVE</b>To prepare effervescent osmotic pump tablet (EOPTs) according to the rhythm of coronary heart disease based on efficacy material and the mechanism of compound Danshen and to study the mechanism of drug released of that tablets.</p><p><b>METHOD</b>Since compound Danshen consist of compounds with polyphenolic groups or carboxyl groups, such as phenolic acids, flavonoids, and triterpenoids that they were acidic. EOPTs were prepared from tablet cores which containing NaHCO3 as effervescent, NaCL and manitol as osmotic agents, HPMC as retarding agents coating with CA membrane. And study the mechanism of drug released according to the change of tablet osmotic pressure.</p><p><b>RESULT</b>The results of in vitro experiments showed that no difference was observed among the profiles of Danshensu, protocatechuic aldehyde, ginsenoside Rg1, Rb1, notoginsenoside R1 release EOPTs. The drug was completely released from the device with a zero-order release rate over 12 h.</p><p><b>CONCLUSION</b>EOPTs are Successfully obtained EOPT which the drug is released from the device over 12 h and the release mechanism of EOPTs is explained.</p>

ABSTRACT

Objective To discuss the characteristics and clinical results of rotating platform high flexion(RDHF)total knee replacement TKR. Methods A total of 61 patients(78 knees)including 13males(15 knees)and 48 females(63 knees)at age range of45-79 years(mean 64.3 years)were treated with Sigma(RP-HF)rotating platform high flexion total knee replacement.Osteoarthritis was found in 56patients and rheumatoid arthritis in 5.By medial parapatella approach,denervation and facet repair of the patella were done,without replacing them. Results All 78 knees were followed up for 3-16 months (mean 10.5 months).the X-ray film demonstrated no prosthesis loosening,patella dislocation or subluxation.According to the knee society scoring system,the preoperative score was 48 points(33-72 points)and the knee functional score was 41 points(28-63 points)on average.After surgery,the knee score was 97points(88-100 points),the functional score 96 points(84-99 points),and the range of motion(ROM)130.(110.-150.)on average.No spin-out or infection were detected. Conclusions The Sigma RP-HF knee system provides sufficient contact area.full range of patella locus and sufficient axial rotation,which helps reach a stable and durable high flexion performance.Our short-term clinical follow up result is good but the advantages of low wear and longevity of the Sigma RP-HF prosthesis still need further study.

Objective To evaluate the availability and sensitivity of MR whole body diffusion weighted imaging (DWI) in searching primary tumors in patients with metastases.Methods Thirty-four patients with symptoms of metastases and unknown primary leasions were scanned by whole body DWI.All the metastases cases were diagnosed by CT,single photon emission computed tomography(SPECT) or MRL For the cases of suspected primary tumors,apparent diffusion coefficient (ADC) values of the primary and metastatic lesions were measured respectively.For the cases with primary lung cancers and metastases proved by pathology of biopsy or surgical specimens,statistics analysis of ADC values of primary and metastatic lesions was performed with paired samples t-test.The detection rate and sensitivity of this technique for screening primary tumors were evaluated.Results By whole body DWI,24 cases were found with suspected primary lesions,in which 23 cases were proved primary tumors,and 1 case was proved benign lesion.Ten cases were not identified with primary lesions on whole body DWI,but in which 2 cases were proved primary malignant tumor by biopsy later,and the other 8 cases remained unknown within follow-up of half a year to one year.For the 14 definite cases of lung cancers,the average ADC values of primary and metastatic lesions was (1.24±0.18)×10-3 and(1.16±0.27)×10-3mm2/s,respectively (t=1.28,P>0.05).The detection rate and sensitivity of whole body DWI for searching primary tumors was 67.6% (23/34) and 92.0% (23/25),respectively.Conclusion The sensitivity of whole body DWI for searching primary tumors is relatively high,and it can be used to search primary tumors in patients with metastases.

ABSTRACT

[Objective]To introduce the surgical technique of ceramic-on-ceramic total hip replacement and discuss the difference between two implant design.[Method]From November 2001 to June 2006,123 hip in 101 patients were treated by total hip arthroplasty with Osteonics ABC and Option Hip System.The prosthesis was fixed in strict position following some special technique for ceramic components.The theoretic range of motion of two hip systems were measured from experiment in vitro.The different cup positions which impingement happened were record for each prosthesises.[Result]The clinical score and the radiographic position of implants was good without evidence of wear,loosening,osteolysis and ceramic fracture.One chipping of ceramic insert happened during operation,and two patients had temporary irritation of iliopsoas.No other sever complication were found so far such as dislocation and infection.Experiment result:The deepened cup design obviously decreased the hip range of motion and "safe zone" was narrow than the flat cup design.[Conclusion]Ceramic-on-ceramic hip implant highly demands for surgical technique especially accurate cdp position.The deepened cup design cup should be posited with large anteversion.

ABSTRACT

Objective Studying the experience of total hip revision in the femur side with AML prosthesis retrospectively,and to analyze its value.Methods Thirty-five cases were revised with AML prosthesis after failed total hip arthroplasty in the femur side,the bone loss was I-Ⅲb according to Paprosky classification.Results Mean follow-up periods was 17 months,no screenage of prosthesis subside,migrating and more bone loss were showed in all cases postoperatively.Mean Harris score was increased from 37 to 92.Conclusion AML prosthesis is a good choice for the total hip revision in the femur side with I-Ⅲb by Paprosky classification.
