Objective To analyze and minimize iatrogenic hospital acquired conditions (HACs). Methods " Admission conditions" in homepage of medical records was studied to collect iatrogenic problems between 2014 and 2015. Self-inspection of physicians was made to identify causes of iatrogenic HACs for improvement measures. The post-improvement HACs data ( 2015 ) were compared with that of before (2014). Results Physicians came to learn following the self-examination the importance of pre-admission communications, doctor-patient communications and cooperation between the departments in less HACs. Occurrence of HACs in 2015 was significantly lower than 2014. Conclusions Enhanced analysis and management of HACs, especially for iatrogenic problems, significantly contribute to less HACs.

As introduced in the paper, the average days of stay of clinical department were calculated as a management target, in view of the complexity of disease and case mix index. This method could avoid the deficiency of traditional methods and make the management of average length of stay of clinical department more conforming to the actual situation, and also more scientific and reasonable.

BACKGROUND: Alveolar bone grafting is known to reduce nasal asymmetry by supporting a defective alar baseand a sunken nostril. However, there are no studies on details of changes in the upper lip with appropriatemeasurements.OBJECTIVE: To measure the change in the upper lip height and nostril sill after alveolar bone grafting.METHODS: Forty-six congenital unilateral cleft lip and alveolus (UCLA) patients (mean age 9.85 years). The patients underwent alveolar bone grafting with autogenous iliac bone grafts. The average bone graft volume was 2.98 cm3.Photographs of the frontal, lateral and basal views were obtained using standardized photographic techniques. The fixedposition points on the patients' faces were measured. Preoperative and postoperative upper lip height and projection, theproportion indexes of the nostril sill were compared.RESULTS AND CONCLUSION: The 6-month follow-up results showed that the height of the upper lip was increased,but no significant difference was found. The projection of the upper lip was more prominent, but there was not statisticallysignificant difference bbetween pre- and post-operation. The nostril sill was significantly elevated in three of four distanceitems. In conclusion, the height of the upper lip is elongated and the nostril sill is elevated after alveolar bone grafting inUCLA patients.

Hospital-acquired conditions (HACs)are key to patient safety.This paper introduced a new classification system of HACs,the CHADx model,and described the principle and method of establishing the model as well as the problems found during its use.The model can provide a basis for studying the problem of incidence,causes and influence factors of hospital acquired conditions,so as to help medical institutions to explore the causes of HACs and to ensure patient safety and improve the quality of health care.

Objective To learn the impacts of major diagnosis, other diagnoses, major surgery and other surgeries on the grouping of DRGs, and to optimize the DRGs data grouping quality by analyzing the main influencing factors of DRGs.Methods Based on regrouping results of the 1 940 questionable cases which have been corrected, using SPSS 13.0 to study the impacts of the questions found on the grouping of DRGs.Results 438 Patient records affecting DRGs grouping were regrouped according to major diagnosis, other diagnoses, major surgery and other surgeries.Influences of the above four groups on the grouping vary in general For comparison between two groups, P＞0.007 1 between main surgery and other surgeries;while P＜0.007 1 for comparison between other groups.Conclusion The highest influence of grouping was found in questionable major diagnosis, much higher than such other factors as other diagnoses, major surgery and other surgeries.This conclusion conforms to the steps of basic DRGs grouping logic-sorting the eases according to major diagnosis first of all Correct naming of surgery influences DRGs grouping, but the influence extent of major surgery and other surgeries is close.

Hospital acquired conditions(HACs)are a common headache for hospitals,which often give rise to waste of medical resources and additional payment of medical insurance funds,as well as deterioration of patient safety and doctor-patient relations.These aftermaths have made HACs a high priority in hospitals management worldwide,as they are tightening control of HACs,reducing medical resource waste,improving quality of care,and enhancing patient safety.That is why HACs are so important for us.
