ABSTRACT

BACKGROUNDNasopharyngeal carcinoma (NPC) is a squamous-cell carcinoma especially prevailing among the natives of southern China. The regimen of concurrent chemoradiotherapy (CCRT) that include platinum and 5-fluorouracil (5-FU) is considered to be the standard treatment for NPC. However, its clinical use is limited by its toxicity. Our purpose was to evaluate the efficacy and safety of the regimen of CCRT with taxanes and platinum versus the regimen of CCRT with 5-FU and platinum in NPC treatment.

METHODSMedline, the Cochrane library, and the Chinese medical literature database were searched for eligible studies. Meta-analysis was performed using Review Manager (Version 5.2).

RESULTSSix random controlled trials (RCTs) including 514 patients met our criteria. Meta-analysis showed that the regimen of CCRT with taxanes and platinum had an improved significant difference in complete remission (CR) and less incidence rate in adverse reactions such as gastrointestinal impairment grades III-IV, liver and kidney impairment grades I-II, and radiodermatitis grades III-IV versus the conventional regimen of CCRT with 5-FU and platinum, while the longterm effectiveness rate of overall survival, locoregional failure-free survival, or distant metastasis failure-free survival between the two groups was therapeutic equivalence.

CONCLUSIONSThe regimen of CCRT with taxanes and platinum in NPC therapy may be more efficient and safe compared to the conventional modality of 5-FU and platinum in CCRT. However, we need more high-quality studies of multi-center and randomized double-blind clinical trials to further compare, analyze, and confirm the findings.

ABSTRACT

Objective Effective intervention on the setbacks and safety loopholes of clinical medication, in an effort to explore, establish and improve the ADR watchfulness system in the hospital step by step. MethodsCollection and compilation of ADRs and drug compatibility cases found in 2010 in the hospital. Intervention on drugs of safety risk exposure or medication factors by means of the ADR prewarning mechanism, medical rounds by hospital's directors regarding drug issues, communication between pharmacists and clinicians, ADR monitoring leadership team, and pharmaceutical affairs council. Results 143 cases of ADR were interfered with among all the 393 ADR cases reported by clinical departments in 2010, accounting for 36.4% of all the reported cases. ConclusionHospitals need to pay more attention to the monitoring, analyzing and evaluation of ADRs, and take efficient intervention means to make drug use more rational and ensure safe medication for the people.

ABSTRACT

OBJECTIVE:To explore the new model and feasibility of the combination of ward round conducted by business president and pharmaceutical administration.METHODS:The organization form, procedures, content and experience of ward round conducted by business president were introduced based on the practice of our hospital.RESULTS & CONCLUSIONS:The pharmaceutical ward rounds conducted by business president are carried out once a month in our hospital to intervene irrational medication by means of medication survey, administration intervention and education with guidance and supervision of rational use of drug as core.Multi-disciplinary cooperation is effective measure for improving rational use of drug.It can strengthen public perception on rational use of drug in all areas by enhancing experience at key point which is worth of spreading.

ABSTRACT

Objective To change the two-tiered C/S architecture of traditional PACS.Methods Based on the deeply research of PACS architecture, combination with DICOM and distributed object technology, propose distributed PACS system architecture, the three-tire client/server framework of distributed object technology introduced into the architecture of PACS to design the new architecture. Results In comparison with traditional PACS that were based on DICOM networks, the distributed PACS were improved themselves in system architecture and performance, and PACS were more open and interoperable. Conclusion The new architecture not only enhances the performance of PACS,but also meets the need of open and high interconnectivity and interoperability.
