ABSTRACT

Objective To compare the serum levels of interleukin(IL)-6,IL-18 and tumor necrosis factor alpha(TNF-α)in patients with DPD,PD,depression disorder and healthy controls and to analyze the correlations of serum inflammatory factors in DPD patients. Methods Serum levels of IL-6,IL-18 and TNF-αwere measured using enzyme linked immunosorbent assay(ELISA) kits. Several scales were performedin DPD patients. Results DPD,PD and depression disorder patients had significant lower baseline levels of IL-6 and TNF-αwhen compared to healthy controls(P < 0.05). The levels of IL-6 and TNF-α in DPD patients were significantly increased after 4 weeks of anti-depression treatment(P < 0.05). No difference of cytokines levels in gender and in severity of DPD patients was detected Serum levels of these inflammatory cytokines were not significantly correlated with the UP-DRS Ⅲ,H & Y,MMSE and HAMD scores in DPD patients. Conclusions Serum inflammatory factors(IL-6, TNF-α)were altered in patients with DPD in the earlier course of disease. However ,the role of IL-18 remained unknownin the occurrence of DPD disease.

ABSTRACT

Objective To investigate the incidence and the related factors of the depression in Parkinson's disease. Methods 52 cases with idiopathic Parkinson's disease were included in this study. Such scales as Hamilton depression scale-24 item (HAMD-24), Non Motor Symptoms Scale for Parkinson's disease (NMSS), 39-item Parkinson's Disease Questionnaire (PDQ-39), unified Parkinson's disease rating scale (UPDRS), revised Hoehn-Yahr rating scale (H-Y), revised Schwab and English daily life event scale (S-E), Minimum Mental State Examination (MMSE), etc. were applied and underwent Spearman correlation analysis. Results Thirty-six patients with Parkinson's disease (69.23%) met the criteria for depression. The spearman rank correlation analysis showed that MMSE (r=-0.294, P=0.036) and S-E (r=-0.506, P<0.001) had negative correlation with depression in Parkinson's disease, and the course of the disease (r=0.274, P=0.049), UPDRS (r=0.485, P<0.001), H-Y (r = 0.459, P = 0.001), NMSE (r = 0.729, P < 0.001) and PDQ-39 (r = 0.619, P < 0.001) had positive correlation with depression in Parkinson's disease. Conclusions The patients with Parkinson's disease had high incidence of depression. The depression in Parkinson's disease is associated with many factors and affected the quality of life of the patients.

ABSTRACT

<p><b>BACKGROUND</b>Based on the excellent medical care and management system for Chinese veterans, as well as the detailed medical documentation available, we aim to construct a Chinese Veteran Clinical Research (CVCR) platform on non-communicable diseases (NCDs) and carry out studies of the primary disabling NCDs.</p><p><b>METHODS</b>The Geriatric Neurology Department of Chinese People's Liberation Army General Hospital and veterans' hospitals serve as the leading and participating units in the platform construction. The fundamental constituents of the platform are veteran communities. Stratified typical cluster sampling is adopted to recruit veteran communities. A cross-sectional study of mental, neurological, and substance use (MNS) disorders are performed in two stages using screening scale such as the Mini-Mental State Examination and Montreal cognitive assessment, followed by systematic neuropsychological assessments to make clinical diagnoses, evaluated disease awareness and care situation.</p><p><b>RESULTS</b>A total of 9 676 among 277 veteran communities from 18 cities are recruited into this platform, yielding a response rate of 83.86%. 8 812 subjects complete the MNS subproject screening and total response rate is 91.70%. The average participant age is (82.01±4.61) years, 69.47% of veterans are 80 years or older. Most participants are male (94.01%), 83.36% of subjects have at least a junior high school degree. The overall health status of veterans is good and stable. The most common NCD are cardiovascular disorders (86.44%), urinary and genital diseases (73.14%), eye and ear problems (66.25%), endocrine (56.56%) and neuro-psychiatric disturbances (50.78%).</p><p><b>CONCLUSION</b>We first construct a veterans' comprehensive clinical research platform for the study of NCDs that is primarily composed of highly educated Chinese males of advanced age and utilize this platform to complete a cross-sectional national investigation of MNS disorders among veterans. The good and stable health condition of the veterans could facilitate the long-term follow-up studies of NCDs and provide prospective data to the prevention and management of NCDs.</p>

ABSTRACT

Objective:To observe the influence of valsartan combined with Bailing capsules on urinary albumin excretion rate in early stage of type 2 diabetic nephropathy ( DN) , and explore its protection in early DN. Methods:Sixty patients with early DN were randomly divided into two groups. On the basis of diet control and blood glucose regulation, the control group (n=30) was given valsartan 160 mg, qd, while the prevention group (n=30) was treated by valsartan (160 mg·d-1) combined with Bailing capsules (2. 0g, po, tid), and the treatment course was 12 weeks. The urinary albumin excretion rate ( UAER) , mean arterial blood pressure ( MAP) , serum creat-inine ( Scr) and hemoglobin A1c ( HbA1c) were measured and compared before and after the treatment in the two groups. Results:UAER in the two groups was significantly reduced after the treatment compared with that before the treatment (P0. 05). Conclusion:Valsartan combined with Bailing capsules shows certain effects in the treatment of early stage of type 2 diabetic nephropathy by decreasing the urinary albumin excretion.

ABSTRACT

Objective To evaluate the curative effect of selective window decompression surgery for over 80-year-old patients with mul-tiple segment lumbar spinal stenosis. Methods The patients received the operation through posterior median approach. The location of le-sions were determined according to patients' symptom,physical examination and imaging examination,the selective window decompression sur-gery was performed in patients,JOA scoring method was used to evaluating the effect of surgery. Results In the recent follow-up of first 6 months after surgery,the response rate of 52 patients with follow-up was 100%. During the mid-term follow-up after 24~36 months,the re-sponse rate of 47 patients with follow-up is 90. 4%,the average follow-up time was 30. 4 months. The preoperative JOA score and postopera-tive score were (6. 93 +0. 52) and (11. 98 +0. 72) respectively,of which 40 cases achieved optimal standard (76. 9%),10 cases (19. 2%) achieved a good standard,2 cases reached the standard (3. 9%),the excellent rate was 96. 1%. The JOA score of mid-term fol-low-up was (12. 06 +0. 46),of which 36 cases reached optimal standard (76. 6%),10 cases achieved a good standard (21. 3%), but standard was in 1 case (2. 1%),the excellent rate was 97. 9%. The difference of JOA scores was statistically significant (P<0. 01). Con-clusion Selective windowing decompression has the advantages of small trauma,short recovery time and satisfied curative effect,which is suitable for elderly patients with lumbar spinal stenosis.

ABSTRACT

Objective To analyze the advantages of intracrani al hematoma smash puncture (IHSP) in the treatment of cranial hemorrhage for the elderly. Methods A total of 104 cases of cerebral hemorrhage a dmitted in the recent 3 years were collected and divided into 4 grou ps, i.e . group A, elderly with IHSP treatment (n=36), group B, elderly with regular medical treatment (n=33), group C, non-elderly with IHSP treatment (n= 21) and group D, non-elderly with routine therapy (n=14). The clinical and follow-up outcomes were studied and analyzed. Results There w as significant difference in rehemorrhage rate after puncture between grou p A (0/36) and group C (2/21). Significant difference was found in early mortali ty rate in group A (16.67%) and group B (39.40%). The rate of activity of daily living (ADL) 1-2 was significantly higher in group A (58.62%) than group B (5.88 %)(P<0.005) and that of ADL 4-5 was obvious lower in group A (13.79%) than g roup B (47.06%)(P<0.025). Conclusion Intracranial hematoma smash hemorrage is safe and efficient in the treatment of elderly cerebr al hemorrhage, especially in reducing the disability rate.
