ABSTRACT

ObjectiveTo determine the influence of COVID-19 prevention and control on the epidemic characteristics and dynamics of notifiable infectious diseases in the first quarters, Zhejiang Province, and to explore more effective countermeasures against infectious diseases. MethodsDescriptive epidemiology was conducted to determine the change in notifiable infectious diseases during the prevention and control of COVID-19 in Zhejiang Province by retrieving the data of notifiable infectious diseases from 2017 to 2022 in the Chinese information system for disease control and prevention. Cumulative reported new cases of notifiable infectious diseases in the first quarters of 2017‒2019 were compared with that of 2020‒2022. ResultsA total of 546 753 cases of notifiable infectious diseases were newly reported in the first quarters of 2017‒2019, with an average incidence of 321.92/105. In contrast, a total of 509 908 cases of notifiable infectious diseases were newly reported in the first quarters of 2020‒2022, during which the COVID-19 epidemic occurred, with an average incidence of 270.39/105. The incidence in 2020‒2022 significantly declined by 51.53/105, compared with that in 2017‒2019 (χ²=8 072.06, P<0.001). In the first quarters of 2020‒2022, the average incidence of zoonotic diseases and vector-borne diseases decreased by more than 50%. In addition, the incidence of respiratory, enteric, blood-borne, and sexually transmitted diseases declined to certain degree. ConclusionThe decline in the newly reported cases of non-COVID-19 notifiable infectious diseases in the first quarters of 2020‒2022 indicates that the countermeasures against COVID-19 epidemic, such as multi-disease co-prevention, multi-sectoral collaboration, societal mobilization and personal hygiene and protection, may also decrease the incidence of multiple infectious diseases. It suggests the countermeasures are effective, which would provide evidence for routine prevention and control of infectious diseases in future.

ABSTRACT

COVID-19 has emerged around the world since December 2019 and rapidly evolves as a major global public health challenge. SARS-CoV-2 is a positive single-stranded RNA virus with a relatively high natural mutation rate. The emergence of SARS-CoV-2 variants is generally considered as the outcome of a “mutation-selection-adaption” process. The enzyme APOBECs that is activated in the inflammatory microenvironment may facilitate viral mutagenesis. Currently, the major SARS-CoV-2 variants that impose public health significance are B.1.1.7, B.1.351, P.1, B.1.6.7.2, B.1.1.529 and so on, with probably increasing transmissibility, decreasing reactivity to the neutralizing antibodies, immune escape, and enhancing disease severity and mortality. Consequently, it warrants up-to-date countermeasures against evolving SARS-CoV-2, including tracing viral mutagenesis, developing vaccines against major variants, and enhancing social distancing.

ABSTRACT

ObjectiveTo investigate the epidemiological characteristics and analyze the incidence trend of SARS-CoV-2 infection in Shanghai, China, and compare with the characteristics of the infection in Jilin Province of China during the same period in 2022 and Wuhan at the beginning of 2020. MethodsInformation of new locally-transmitted confirmed SARS-CoV-2 cases, imported confirmed COVID-19 cases, local asymptomatic SARS-CoV-2 carriers and imported asymptomatic SARS-CoV-2 carriers in Shanghai from March 1 to April 18, 2022 was collected for descriptive analysis. ResultsFrom March 1 to April 18, 2022, a total of 397 933 locally-transmitted SARS-CoV-2 cases were reported in Shanghai. Of those, 27 613 were clinically confirmed cases and 21 were severe cases. Ten deaths were related to COVID-19. The pathogen is Omicron variant BA.2 of SARS-CoV-2. The number of the infected subjects increased rapidly after March 24 and lead to a disease outbreak. Severe and deceased cases had severe comorbidity and were mostly unvaccinated with SARS-CoV-2 vaccines. Asymptomatic SARS-CoV-2 carriers accounted for 93.06%, which is significantly higher than that in Jilin Province during the same period (48.07%, P<0.001). Daily increase in the number of clinically confirmed COVID-19 cases in Shanghai in 2022 was much lower than that in Wuhan, Hubei Province, in 2020. Number of daily newly imported confirmed COVID-19 cases and imported asymptomatic SARS-CoV-2 carriers declined during this period. ConclusionThe Omicron variant in Shanghai 2022 is highly infectious and less pathogenic. Omicron variant BA.2 replicates rapidly in asymptomatic carriers, which makes the carriers the major source of infection. Full-term vaccination of inactivated SARS-CoV-2 vaccine might decrease the pathogenicity and fatality of SARS-CoV-2 variants. SARS-CoV-2 of the Omicron BA2 strain is likely transmitted through aerosols and droplets, which poses a great challenge to the control of the COVID-19 pandemic in large cities with high population density and sophisticated public transportation.

ABSTRACT

Due to the high spatiotemporal resolution , cardiac cine magnetic resonance imaging (CCMRI) has been widely used to evaluate the cardiac function of cardiovascular diseases such as myocardial ischemia and so on. Segmentation-based motion tracking of left myocardium is very important for comprehensive evaluation of cardiac function in the diagnosis and treatment of cardiovascular diseases. However, it is a challenge to track motion of left myocardium, which is homogeneous and cannot provide effective motion information. In this paper, CCMRI imaging techniques for myocardial motion tracking are introduced firstly. Then approaches for motion tracking of left myocardium based on CCMRI image are described in details and are summarized and prospected at the end, which not only helps beginners to have a quick and comprehensive understanding on this topic, but also provides theoretical reference to related researchers for further optimization of approaches for motion tracking of left myocardium. From the current study, motion tracking approaches for left myocardium based on CCMRI image make comprehensive use of the spatiotemporal motion characteristics of CCMRI image, the motion and structures of myocardium of left ventricle and so on, which can make up for the shortcomings of sparse motion information of CCMRI image. However, it still needs improved constraint framework, verification methods and so on.

ABSTRACT

Objective To explore the risk factors in disease progression and the significance of vascular endothelial growth factor ( VEGF ) and vascular intercellular adhesion molecule-1 ( VCAM-1 ) in hand foot and mouth disease ( HFMD) combined with encephalitis .Methods Altogether 92 subjects with HFMD were enrolled in the study and were divided into four groups , including group A ( ordinary group with no complication ) , group B ( severe group with complication ) , group C ( critical group with complication ) , group D ( recovery group with complication ) .Concentrations of VEGF and VCAM-1 in serum and cerebro-spinal fluid were detected by double antibody sandwich ELISA method .Multiple factors logistic regression analysis was performed to analyze main risk factors in disease progression for two combinations , one for ordi-nary group and severe group , the other for severe group and critical group .The results were analyzed by SPSS16.0 statistical software. Results The concentration of VEGF and VCAM-1 in serum and cerebrospi-nal fluid had statistically significant differences among the four groups , but there was no significant difference between group A and group D , and between group B and group C .In addition , the statistically significant factors for prediction of disease progression were duration of fever , limb shaking, cerebrospinal fluid WBC , cerebrospinal fluid protein and EV 71 IgM between ordinary group and severe group , and cerebrospinal fluid WBC, respiratory rate and heart rate between severe group and critical group .The multiple factors logistic regression analysis revealed that limb shaking , cerebrospinal fluid protein , VEGF and VCAM-1 in serum were the main risk factors for disease progression from ordinary to severe (P=0.071, 0.019, 0.020, 0.025 and OR=147.629, 26.572, 5.958, 6.345).And increased heart rate indicated the progression from se-vere to critical with P value of 0.001 and OR value of 2.69.Conclusion (1) Compared with group A, VEGF and VCAM-1 in serum and cerebrospinal fluid were highly expressed in patients with HFMD combined with encephalitis .Therefore , VEGF and VCAM-1 could be used as diagnostic criteria for auxiliary diagnosis of encephalitis in patients with HFMD and reflect the severity and prognosis to a certain extent .( 2 ) Risk factors like limb shaking , cerebrospinal fluid protein , VEGF and VCAM-1 in serum would be helpful to early diagnosis of severe patients .Increased heart rate would be a significant factor for identification of patients with critical disease , according to which a timely treatment would be provided to prevent from worse .

ABSTRACT

Objective To compare the efficacy of the radical resection by laparoscopy versus open approach in perioperative period on the patients with rectal carcinoma, and investigate the feasibility, safety and oncological clearance of the laparoscopy. Methods The clinical data of 44 patients who underwent radical resection of rectal carcinoma by laparoscopy in our hospital were reviewed and compared with another 53patients who underwent an open approach in the same period. The surgery-related data, postoperative recovery status, tumor radical resection index, and postoperative complications by laparoscopy were analyzed by statistics, and compared with those by open approach, and evaluated the deference of too kinds of operation. Results This study showed a longer surgical time (260.45 ± 67.46) min vs ( 179.25 ± 40.92) min,P <0.05,a less intra-operative blood loss( 125.20 ±61.80) mL vs ( 198.02 ± 131.24) mL,P <0.05,in laparoscopic group compared with open approach.Meanwhile,it also showed an earlier recovery of bowel functions for discharge gas from anus, taking in food, and out-of-bed activity (4.34 ± 1.55) d vs(5.45 ± 1.55) d,P <0.05,in the laparoscopic group compared with open approach. There was no statistical difference of incidence of post-operative complications (5 cases vs 11 cases,P >0.05) between the two groups and the laparoscopic approach was also equal to the open approach as regard to post-operative stay (15.34 ±6.62) d vs (16.82±5.73) d,P >0.05,and demand of intra-operative blood transfusion (4 case vs 8 cases,P>0.05 ).Conclusions Compared with open surgery, the radical resection of rectal carcinoma by laparoscopy has shown obvious advantages in smaller incision, less blood loss, less pain, earlier recovery of bowel and bladder functions, and earlier out-of-bed activity. And it is also possible by laparoscopy approach to decrease the post-operative complications and post-operative stay.Meanwhile, there is no significant deference on oncological clearance for laparoscopy compared with open approach during perioperative period, while the long term follow-up data is still needed to support the results.
