ABSTRACT

Objective To systematic evaluate the effectiveness and safety of driving pressure-guided fixed positive end-expiratory pressure(PEEP)titration in intraoperative mechanical ventilation. Methods PubMed,Web of Science, the Cochrane Library,Embase,CNKI,Wanfang and VIP databases were searched for collect randomized controlled trials(RCTs)of PEEP titration guided by driving pressure in intraoperative mechanical ventilation from inception to November 8,2023.After two researchers independently screened the literature, extracted data, and evaluated the risk of bias of the included studies, the meta-analysis was conducted by Rev-Man 5.4 software. Results Nineteen studies with a total of 2 906 patients were included. There were 1 440 patients in the study group with the lung protective ventilation strategy guided by PEEP titration, and 1 466 patients in the control group with the traditional lung protective ventilation strategy.Compared with the con-trol group, the incidence of postoperative pulmonary complications(PPCs)in the study group was lower in the non-thoracic surgery(RR=0.53,95%CI:0.43-0.65,P<0.001),but there was no statistical difference in the incidence of PPCS in the thoracic surgery(RR=0.89,95%CI:0.78-1.02,P=0.09).Compared with the control group, the intraoperative lung compliance was increased(MD= 6.90 L/cmH2O,95%CI:5.80-7.99,P<0.001),and the length of hospital stay was shortened in the study group(MD=-0.27 d,95%CI:-0.43 to-0.12,P<0.001),while there was no significant difference in intraoperative mean arterial pres-sure(MAP)between the two groups(MD=0.36 mmHg,95%CI:-1.30 to 2.01,P=0.67).Conclusion Com-pared with the traditional lung protective ventilation, driving pressure-guided PEEP titration ventilation can im-prove intraoperative lung compliance, reduce the incidence of PPCs in non-thoracic surgery, shorten the length of hospital stay, and does not increase the risk of hemodynamic disturbances in patients undergoing surgery.

ABSTRACT

Objective To evaluate the effect of acupuncture combined nerve block for surgery using meta-analysis.Methods We searched the data of randomized controlled trial (RCT) in Pubmed,Cochrane library,Embase,CNKI,VIP and Wanfang from the construction of database to May 2017.All RCTs that met the standards of acupuncture combined nerve block for patients undergoing surgery were collected. The meta-analysis was performed by Review Manager 5.3.Results Five trials containing 382 patients were included in this meta-analysis.Compared with nerve block alone, the pooled data showed that acupuncture combined nerve block reduced the fluctuation of intraoperative blood pressure (MD=-13.62,95%CI-15.41--11.84,P<0.001) and heart rate (MD=-6.49,95%CI-8.61--2.36,P<0.001),and VAS scores 8 h (MD=-1.07,95%CI -1.38--0.75,P<0.001),12 h (MD=-1.11,95%CI-1.30--0.93,P<0.001) and 24 h (MD=-0.15,95% CI-0.25--0.04,P =0.006) after operation in the experimental group. Conclusion Acupuncture combined nerve block possibly improves the hemodynamic parameters and postoperative pain of patients undergoing surgery.

ABSTRACT

Objective To explore the relationship between music preferences and mental health in medical students and to propose countermeasures. Methods Music Preferences Questionnaire and Chinese college students mental health scale(CCSMHS) were used to test 538 randomly selected medical students and the data was analyzed by descriptive statistics,T test and variance analysis with SPSS 20.0 and the inspection level was α=0.05.Results ①Statistically significant (P<0.05) differences were found on somatization, anxiety, depression, inferiority, paranoid, force, social withdrawal, reliance, impulse and psychotic tendencies in medical students with different preference of music style; ②Statistically significant (P<0.05) differences were found on anxiety and inferiority in medical students with different preference of music type; ③Statistically significant (P<0.05) differences were found on anxiety, depression, paranoid, force, social withdrawal and reliance in medical students with different participating ways of music; ④Statistically significant (P<0.05) differences were found on somatization, anxiety, inferiority and paranoid in medical students with different attitude about participating music activities; ⑤Medical students with different preferences of music activities preferences did not have significant difference on factor of mental health(P>0.05).Conclusion The relationship between medical students' mental health and music preferences is very close.By training and guiding music preferences of medical students in mental health education,we can improve their mental health effectively.
