ABSTRACT

Interventional embolization therapy is widely used for procedures such as targeted tumour therapy, anti-organ hyperactivity and haemostasis. During embolic agent injection, doctors need to work under X-ray irradiation environment. Moreover, embolic agent injection is largely dependent on doctors' experience and feelings, and over-injection of embolic agent can lead to reflux, causing ectopic embolism and serious complications. As an effective way to reduce radiation exposure and improve the success rate of interventional embolization therapy, embolic agent injection robot is highly anticipated, but how to decide the injection flow velocity of embolic agent is a problem that remains to be solved. On the basis of fluid dynamics simulation and experiment, we established an arterial pressure-injection flow velocity boundary curve model that can avoid reflux, which provides a design basis for the control of embolic agent injection system. An in vitro experimental platform for injection system was built and validation experiments were conducted. The results showed that the embolic agent injection flow speed curve designed under the guidance of the critical flow speed curve model of reflux could effectively avoid the embolic agent reflux and shorten the embolic agent injection time. Exceeding the flow speed limit of the model would lead to the risk of embolization of normal blood vessels. This paper confirms the validity of designing the embolic agent injection flow speed based on the critical flow speed curve model of reflux, which can achieve rapid injection of embolic agent while avoiding reflux, and provide a basis for the design of the embolic agent injection robot.

ABSTRACT

OBJECTIVE:To screen the main effective components of Compound Agrimonia pilosula enteritis capsules and targets of enteritis. METHODS :UHPLC-MS/MS,MWDB database and relevant literature analysis were used to identify main chemical components in methanol extract of Compound A. pilosula enteritis capsules. TCMSP ,PubChem and UniProt database were adopted to predict and screen the active ingredients and their potential targets. GeneCards and OMIM database were used to predict and screen enteritis related targets ;common targets were screened by R language 4.0.2. The chemical components corresponding to the common targets were matched with the chemical components in the methanol extract of the preparation to obtain the main effective components of the preparation. With the help of STRING database and Cytoscape 3.7.1 software,the protein-protein interaction network was constructed ,and the key targets of the preparation were screened by degree. RESULTS : A total of 48 compounds were identified ,including 13 phenolic acids ,10 alkaloids,8 flavonoids,6 terpenoids,6 other compounds,3 lipids,1 tannin and 1 organic acid. Compared with the network pharmacology data ,apigenin,luteolin,quercetin (3,7-di-O-methylquercetin,Quercetin-3-O-β-D-galactoside,Quercetin-7-O-glucoside,Quercetin-3-O-β-D-glucoside),palmatine, protocatechuic acid- 4-glucoside and 3,4-dimethoxycinnamic acid were the main effective components. The key targets for the treatment of enteritis included RELA ,APP,CCND1,EGFR,INS,ESR1,IL6,NCOA1,CASP8,FOS. CONCLUSIONS :A total of 9 main effective components (including apigenin ,luteolin,quercetin,etc.)and 10 key targets for enteritis (including RELA,APP,CCND1,etc.)of Compound A. pilosula enteritis capsules are found.

ABSTRACT

Objective To systematically evaluate the performance of the Sysmex CS2000i Automatic Blood Coagulation Analysis System .Methods According to the standards of the Clinical and Laboratory Standards Institute (CLSI) ,the precision ,accuracy ,lin‐earity ,biological reference interval and carry‐over rates of the Sysmex CS2000i Automatic Blood Coagulation Analysis System were detected ,meanwhile the detections of 5 clinical specimens were compared between this system and the Sysmex CA 1500 Automatic Blood Coagulation Analysis System .Results The intra‐assay precision coefficient of variation (CV) ,inter‐day precisions CV carry‐over rate ,accuracy and linearity of plasma prothrombin time ,activated partial thromboplastin time ,fibrinogen and D‐dimmer detec‐ted by the Sysmex CS2000i Automatic Blood Coagulation Analysis System were all consistent with the quality target requirements of our laboratory .Conclusion The Sysmex CS2000i Automatic Blood Coagulation Analysis System has better performance in vari‐ous aspects ,the detection results could be used for the clinical diagnosis and treatment of related diseases .

ABSTRACT

Objective To evaluate the efficacy and safety of retrograde flexible ureteroscopic lithotripsy ( RFUL) with holmium:YAG laser for the treatment of renal stones with a large series.Methods The data of 466 patients who underwent RFUL with holmium:YAG laser for the treatment of kidney stones between January 2013 and December 2013 were collected.The maximum diameter of stone was 23 ±16 mm.The stone free rate, complications, retreatment rate were evaluated.Results Out of the 466 patients, the mean operative time and postoperative hospital stay were 33.5 ±18.8 min and 2.3 ±2.0 d.The stone free rate was 67.6% ( 315/466 ) after single procedure, which increased to 69.5% ( 324/466 ) via re-treatments after 3 months.The retreatment rate was 4.3%(20/466), with a total of 493 RFUL procedures performed and 1.06 times per patient.There were 67 (14.4%) cases undergoing complications.Five cases had false passage of ureter orifice causing slight ureteral wall injuries.Steinstrasse occurred in 9 cases, ureteral perforation in 3 cases and perirenal hematoma in 1 case.Overall postoperative fever rate was 10.7%(50/466) with urosepsis in 16 cases (3.4%).When the stone size was≤10, 11-20, 21-30, 31-40, and >40 mm, the stone free rates after a single procedure were 97.0% ( 65/67 ) , 84.2% ( 160/190 ) , 63.1%(70/111), 29.2%(14/48), 12.0%(6/50) (P<0.001), and the postoperative fever rates were 1.5%(1/67), 9.5%(18/190), 13.5% (15/111), 14.6% (7/48), 18.0% (9/50), respectively. The postoperative fever rates were 19.3%(27/140) and 7.1%(23/326) (P<0.001) in the patient with positive and negative preoperative urine leukocyte, and 22.0% ( 22/100 ) and 7.7% ( 28/366 ) ( P <0.001) in patients with positive and negative preoperative urine culture.Conclusions RFUL with holmium:YAG laser is a safe and effective treatment for kidney stones.The postoperative fever rate would increase and stone free rate would reduce with the increased stone size.

ABSTRACT

Objective To investigate the characteristics of clinical application of skin flap supplied by sural nerve nutrition blood vessel. Methods The flaps were used in 17 cases to repair the soft tissue defect in heel,mallelous,anterior tibial and achilles tendon. Results 14 cases of 17 survived satisfactory. 2 cases presented edema and partially necrosis,they finally survived after open decompression and change of dressing. 1 case presented chronic sinus because of osteomyelitis of calcaneus. 6 months later, when the fracture healed, we performed debridement, removal of internal fixation, curettage and irrigation. It finally healed. Conclusion The sural nerve nutrition blood vessel flap was easy to elevate and safe, which was an ideal flap for repairing the soft tissue defect around the heel, malleolus, achilles tendon and the anterior tibial.
