ABSTRACT

Objective:This study aims to explore the expression of miR-143 in gallbladder cancer tissues and its correlation with tumor angiogenesis, clinicopathologic features and clinical prognosis.Methods:The clinical and pathological data of 78 patients with gallbladder cancer who were surgically resected and pathologically confirmed from January 2008 to June 2016 in the Department of General Surgery, Xuancheng Hospital Affiliated to Wannan Medical College were collected, including 20 males and 58 females, with an average age of (68.0±1.4) years. 78 cases of gallbladder cancer tissue were used as the tumor group, and 32 cases of normal gallbladder tissue adjacent to cancer were randomly selected as the control group. The expression level of miR-143 in the tumor group and the control group was detected by in situ hybridization. Microvessel density (MVD) were evaluated with immunohistochemistry to detect the expression of CD34 in tumor group. The expression of miR-143 and its correlation with tumor MVD, pathological characteristics and prognosis were analyzed by Cox regression analysis.Results:The results of in situ hybridization showed that the positive rate of miR-143 in gallbladder cancer tissues and average expression score were significantly lower than those in adjacent normal gallbladder tissues [29.49% (23/78) vs 62.5% (20/32), χ 2=10.39, P=0.001; (2.705±0.172) vs (4.688±0.405), t=5.33, P<0.001]. Correlation analysis showed that the expression of miR-143 was associated with lymph node metastasis of gallbladder cancer ( P=0.036). The MVD counts in the miR-143 positive group were significantly lower than those in the miR-143 negative group [(126.2±8.483) vs (157.4±6.412), t=2.75, P=0.007)]. The median survival time of gallbladder cancer patients with miR-143 positive and negative was 16 and 9 months, respectively. Cox regression analysis showed that the low expression of miR-143 was an independent risk factor affecting the clinical prognosis of gallbladder cancer. Conclusion:The expression of miR-143 is down-regulated in gallbladder cancer and participates in angiogenesis and clinical prognosis of gallbladder cancer.

ABSTRACT

Objective To explore the clinical effect of minocycline hydrochloride ointment and iodine glycerol in the treatment of chronic periodontitis and the effect on the inflammatory cytokines in gingival crevicular fluid (GCF).Methods A total of 90 patients with chronic periodontitis (115 affected teeth) in Stomatology Hospital of Zhangjiakou from December 2015 to December 2016 were included and randomly divided into observation group(57 affected teeth) and control group (58 effected teeth),with 45 patients in each group.All patients were given routine periodental non-surgical treatment,based on this,the patients in the control group were smeared iodine glycerol (20 g · L-1) in the periodontal pocket with a probe;the patients in the observation group were injected minocycline hydrochloride ointment in the periodontal pocket until the ointment overflowed the gingival margin.The patients in the two groups were treated one time a week for 4 weeks.After 4-week treatment,the gingival index (GI),gingival sulcus bleeding index (SBI),probing depth (PD),plaque index (PLI),periodontal attachment level (AL) and the clinical efficacy were recorded.The levels of hypersensitive C-reactive protein(hs-CRP),interleukin8 (IL-8),interleukin-10 (IL-10),tumor necrosis factor-α (TNF-α),matrix metalloproteinases-8 (MMP-8) and soluble intercellular adhesion molecule-1 (sICAM-1) were detected by enzyme linked immunosorbent assay before and after treatment and compared between the two groups.Results There was no statistic difference in GI,SBI,PD,PLI,and AL between the two groups before treatment (P > 0.05);the GI,SBI,PD,PLI,and AL after treatment were significantly lower than those before treatment (P < 0.05);the GI,SBI,PD,PLI and AL in observation group were significantly lower than those in the control group after treatment (P < 0.05).There was no statistic difference in the levels of hs-CRP,IL-8,IL-10,TNF-α,MMP-8 and sICAM-1 in GCF between the two groups before treatment(P > 0.05);the levels of hs-CRP,IL-8,TNF-α,MMP-8 and sICAM-1 in GCF after treatment were significantly lower than those before treatment (P < 0.05),but the IL-10 level was significantly higher than that before treatment in the two groups(P < 0.05);the levels of hs-CRP,IL-8,MMP-8,TNF-α and sICAM-1 in GCF in observation group were significantly lower than those in the control group after treatment (P < 0.05);the level of IL-10 was significantly higher than that in the control group after treatment (P < 0.05).The total effective rate of affected teeth in observation group(96.49%,55/57) was significantly higher than that in the control group (74.14%,43/58) (x2 =11.40,P < 0.05).Conclusion Minocycline hydrochloride ointment can effectively improve the inflammatory reaction status in patients with chronic periodontitis,and it can control the inflammatory damage of periodontal tissues by inhibiting the inflammatory reaction,and effectively alleviate the clinical symptoms.

ABSTRACT

PURPOSE: This study aimed to identify factors influencing intra-operative core body temperature (CBT), and to develop a predictive model for intra-operative CBT in laparoscopic abdominal surgery. METHODS: The prospective observational study involved 161 subjects, whose age, weight, and height were collected. The basal pre-operative CBT, pre-operative blood pressure, and heartbeat were measured. CBT was measured 1 hour and 2 hours after pneumoperitoneum. RESULTS: Explanatory factors of intra-operative hypothermia (<36℃) were weight (β=.361, p<.001) and pre-operative CBT (β=.280, p=.001) 1 hour after pneumoperitoneum (Adjusted R2=.198, F=7.56, p<.001). Weight was (β=.423, p<.001) and pre-operative CBT was (β=.206, p=.011) 2 hours after pneumoperitoneum (Adjusted R2=.177, F=5.93, p<.001). The researchers developed a predictive model for intra-operative CBT (℃) by observing intra-operative CBT, body weight, and pre-operative CBT. The predictive model revealed that intra-operative CBT was positively correlated with body weight and pre-operative CBT. CONCLUSION: Influence of weight on intra-operative hypothermia increased over time from 1 hour to 2 hours after pneumoperitoneum, whereas influence of pre-operative CBT on intraoperative hypothermia decreased over time from 1 hour to 2 hours after pneumoperitoneum. The research recommends pre-warming for laparoscopic surgical patients to guard against intra-operative hypothermia.

ABSTRACT

PURPOSE: This study investigated the surgical outcomes of canalicular trephination combined with endoscopic dacryocystorhinostomy (DCR) in patients with a distal or common canalicular obstruction. It also identified the factors affecting surgical success rates associated with this technique. METHODS: We retrospectively reviewed the medical records of 57 patients (59 eyes) in whom a canalicular obstruction was encountered during endoscopic DCR. All patients were treated with endoscopic DCR, followed by canalicular trephination and silicone tube placement. The surgical outcome was categorized as a functional success according to the patient's subjective assessment of symptoms, including epiphora, and as an anatomical success according to a postoperative nasolacrimal duct irrigation test. Surgical success rates were compared based on age, sex, location of the obstruction, number of silicone tubes, and experience of the surgeon. RESULTS: Functional success was achieved in 55 of 59 eyes (93%) at one month, 50 eyes (84%) at three months, and 46 eyes (78%) at six months. Anatomical success was achieved in 58 of 59 eyes (98%) at one month, 52 eyes (88%) at three months, and 50 eyes (84%) at six months. There was a statistically significant difference in surgical outcome according to the experience of the surgeon. The anatomical success rate at the six-month follow-up exam was 95.4% in the >5 years of experience group, and 53.3% in the <5 years of experience group (p = 0.008, Pearson chi-square test). CONCLUSIONS: The success rate of canalicular trephination combined with endoscopic DCR in patients with a distal or common canalicular obstruction decreased gradually during the six-month follow-up period. In particular, patients undergoing procedures with experienced surgeons tended to show excellent surgical outcomes at the six-month follow-up exam.

ABSTRACT

PURPOSE: To compare electroretinogram (ERG) waveforms acquired using a ganzfeld stimulator and a Fresnel ganzfeld stimulator. METHODS: ERGs were recorded with a ganzfeld stimulator and a Fresnel ganzfeld stimulator from both eyes of 25 volunteers. Peak-to-trough amplitudes and peak implicit times were compared between both eyes and between a ganzfeld stimulator and a Fresnel ganzfeld stimulator. ERGs taken from the sitting and supine positions were also compared using the Fresnel ganzfeld stimulator. RESULTS: There was no statistically significant difference between the Fresnel ganzfeld stimulator and dome-shaped ganzfeld stimulator in amplitude and implicit time of dark adapted 0.01, 3.0 ERG, photopic 3.0 ERG, photopic 3.0 flicker. The differences in amplitude and implicit time between the right and left eyes were not influenced by the Fresnel ganzfeld stimulator. Additionally, no differences were observed in ERGs obtained from the sitting and supine positions using the Fresnel ganzfeld stimulator. CONCLUSIONS: The newly developed ganzfeld stimulator with a sealed Fresnel lens can be considered as a reliable alternative method for measuring ERGs. Fresnel ganzfeld stimulator is useful for patients having problems with mobility or uncooperative children.

<p><b>OBJECTIVE</b>To discuss the methods of implantable medical device supervision reform.</p><p><b>METHODS</b>Methods of literature review, comparative study, the research of situation, developmental study and case study were adopted.</p><p><b>RESULTS</b>It is the untrue and asymmetric information of implantable medical device that results in improper behavior in the implantable medical device industry.</p><p><b>CONCLUSIONS</b>A implantable medical device electronic monitoring system, namely establishing the independent but interrelated real-time dynamic updating of the databases, which would guarantee the information of implantable medical device is open and transparent, eliminate false food information transmission, vindicate the good order of domestic implantable medical device industry and protect the vital interests of the public.</p>

ABSTRACT

PURPOSE: The length of a patient's stay in a hospital is an indication of the patient's recovery rate. The length of the hospital stay has an important economic factor for hospitals. This study was conducted to determine the relationship of the nutritional status at the time of admission to mortality and the length of the hospital stay (LOHS). METHODS: The study subjects were 1,619 patients who suffered with cancer, were admitted to Kosin University Gospel Hospital during 2005 and they met the study criteria. The patients were classified to the not at risk group, the patients having one risk factor for malnutrition were the I group, the patients having two risk factors for malnutrition were the II group, the patients having three risk factors for malnutrition were the III group and the patients having 4 risk factors for malnutrition were the IV group, based on the established criteria of serum albumin or =240 mg/dl or or =120% or <90% ideal body weight. RESULTS: 24.3% of the patients were classified into the not at risk group, 37.6% were classified into the at risk group I, 24.2% were classified into the at risk group II, 10.3% were classified into the at risk group III and 3.6% were classified into the at risk group IV. The at risk group (at risk III, at risk IV) had a significantly higher prevalence of liver disease. The relationship between liver disease and low serum albu-min levels may have confounded the data. Although the estimated LOHS was similar in all the groups, the average length of stay was 14.4+/-16.38 days in the malnourished group (at risk group IV) compared to approximately 2.8 days in the not at risk group. The more nutritional risk factors the patients had, the longer was the LOHS and the mortality rates were higher. Correlation was not observed between the risk factors and the length of the hospital stay, as well as the lack of correlation with the mortality rate. CONCLUSION: These results suggest that a patient's nutritional status upon admission has an effect on the length of the hospital stay for patients with carcinoma.

ABSTRACT

OBJECTIVE: This paper aims at investigating the fitness of available computer mouse interfaces to each level of cervical spinal cord injury (SCI) patients ranging from C4 to C7. METHOD: Several commercially available computer mouse interfaces were selected for performance evaluation. They were head pointer, touch screen, button mouse, big-ball mouse, and mouse pad. Usual mouse with rubber pad was also tested for the C6-C7 SCI patients. The performance of each interface was evaluated in terms of average click rate of moving target box and patient's satisfaction. RESULTS: The results that the touch screen and the head pointer were superior to other interfaces in terms of click rate. However, in terms of the satisfaction ratio, C4-C5 SCI patients preferred the head pointer and C6-C7 SCI patients preferred the big-ball mouse. The difference in click rate and satisfaction ratio was attributed to the convenience of the interface, i.e., the necessity and type of orthosis when using the mouse interface. CONCLUSION: The results showed that the patients' preference was determined mainly by the convenience. Therefore, the convenience of the interface must be fully considered in the design and selection of computer interface for the SCI patients.

ABSTRACT

Acute hepatitis E occur commonly as outbreaks in endemic areas, but can occur sporadically in other part of the world. Acute hepatitis E has been reported rarely in Korea. A case of concurrent acute hepatitis E virus (HEV) infection and hyperthyroidism was reported in an inactive hepatitis B surface antigen carrier. We experienced two cases of concomitant acute HEV infection in patients with hyperthyroidism. The first case had acute HEV infection with subclinial hyperthyroidism while taking propylthiouracil. The second case suffered from acute HEV infection in a patient with Graves' disease intractable to propylthiouracil. Herein, we suggest the possible association between HEV infection and hyperthyroidism.

ABSTRACT

BACKGROUND: Tsutsugamushi disease is an acute febrile disease in Korea. Some patients with tsutsugamushi disease have severe illness, which needs care in intensive care unit, and may die due to the disease. We analyzed the cases with tsutsugamushi disease who lived in Chuncheon and neighboring communities to find out clinical manifestations, laboratory findings especially in severe cases. METHODS: We reviewed the clinical records of the 81 cases who were diagnosed as tsutsugamushi in Chuncheon Sacred Heart Hospital from April, 1997 to December, 2002. We analyzed the clinical and laboratory findings of the 81 cases, and the treatment results of 79 cases who recieved the treatment in this hospital. We also compared the characteristics of the patients between admission to general ward and intensive care unit (ICU). RESULTS: Seventy three cases developed the disease in autumn. Seventy cases had fever. 58 cases had eschar in their bodies. With the doxycycline treatment, forty five cases subsided the fever within 48 hours, but in 13 cases, fever continued after 5 days of treatment. Twenty five cases were admitted to ICU, 8 cases among them were treated with ventilator, 3 cases died due to the multi-organ failure. When we compared the clinical characteristics of the patients admitted to ICU with those to general wards, leukocyte count, the level of BUN and creatinine were higher, the albumin level was lower in ICU patients than those of general ward patients. CONCLUSION: Tsutsugamushi disease can progress to severe disease, and be fatal to the patients. So it is necessary to diagnose early and treat carefully. We suggest that leukocyte count, BUN, creatinine, or albumin can be used to predict the prognosis.

ABSTRACT

BACKGROUND/AIMS: Rupture of gastric varices was one of the most dreadful complications of cirrhosis. Recently, a new interventional procedure, balloon-occluded retrograde transvenous obliteration (B-RTO) was introduced for the treatment of gastric variceal bleeding. This study was performed to evaluate the therapeutic efficacy of B-RTO in the treatment of gastric varices with gastro-renal shunts. METHODS: From March 2000 to June 2003, we performed B-RTO in 17 patients with gastric varices and gastrorenal shunts. All patients had history or high risk factors of gastric variceal bleeding. For the evaluation of therapeutic efficacy, we performed esophagogastroduodenoscopy (EGD) and computed tomography (CT) at 1, 6 and 12 months after B-RTO. Successful B-RTO was judged by combined CT findings and EGD findings (disappearance of gastric varices or markedly reduced gastric variceal size or bleeding risk) during follow-up periods (1-14 months, mean:6.18). We analyzed the clinical factors related to clinical success of B-RTO. RESULTS: Technical success were achieved in all patients except one (94.1%). Gastric varices were disappeared or decreased after B-RTO in 13 patients (81.2%). Complications related to procedure included transient hematuria (n=5), puncture site oozing (n=1) and partial splenic infarction (n=1), and all were conservatively managed. During the follow up periods, neither significant hepatic nor renal functional damages occurred. Statistically, no significant factors related with B-RTO success. CONCLUSIONS: B-RTO is effective and safe in the management of gastric varices in cirrhotic patients with gastrorenal shunt.

ABSTRACT

By finding the documents of Korean physical anthropology in one place easily, everyone can use them fruitfully. We developed internet site to search them, which published from early 20th century to present days. We also analyzed documents of physical anthropology and databased the tables which is considered to be kernel of results. The advantage of this internet site is followings. We classified the documents of physical anthropology by body regions and we also classified them in detail to reach them easily. The documents are also divided into materials and its methods to search conveniently. Everyone can search a data by classification and also by using a keyword they can find document or data. There are 3 steps to search. First step is searching the title of document and author, second step is searching the name of table including the first step and the last step is searching items in table. If they don't know the contents they can search data by index. We added some pictures and explanations to restore the document or table, which are difficult and hard to understand the point. By these additions, we could understand them briefly. We could quote from past data and also we could easily compare previous data with ours by using this search engine.

ABSTRACT

PURPOSE: To evaluate the charateristic CT and MR findings of primary ovarian fibromas. MATERIALS AND METHODS: We retrospectively reviewed 11 cases which had undergone precontrast and postcontrast scanning, and two in which cases T1-weighted (WI) and postcontrast T1WI and T2WI images had been done. All cases were pathologically confirmed after surgical resection. These masses were analysed on the basis of clinical symptom, age, size (longest diameter), laterality, margin, attenuation (unenhanced and enhanced), signal intensity (SI), calcification, and amount of the ascites. RESULTS: The patients' mean age was 46.6 (range, 22-81) years, and the longest diameter was 14.8 (range, 8-28)cm. All tumors were unilateral, and eight were located in the left ovary and five in the right ovary. In all cases, the tumor margin was well-defined; seven were lobulated, four were oval, one was round, and one was nodularly marginated. On CT scan, the masses showed mildl to moderate heterogenous enhancement with irregular lower density portions. The amount of the ascites was marked in three cases (23%) mild in two (15%), and minimal in three cases. Calcifications were seen in 3 of 11 CT cases (27%), and in one, this was extensive. On MR scans, signal intensity (SI) of the masses on T1WI was isoSI, relativetive to the uterine myometrium, and heterogeneously enhanced after infusion of contrast media. On T2WI, SI was slightly lower that of the uterine myometrium with internal high SI portions. CONCLUSION: The characteristic finding of ovarian fibroma is a unilateral, well-defined, oval or lobulated, solid mass with or without ascites and calcification. On CT scan, tumor has mild to moderate heterogeneous enhancement. On MR scan, SI of mass is isoSI on T1WI with heterogeneous enhancement, and low SI on T2WI due to fibrous component.

ABSTRACT

PURPOSE: To determine the frequency of complications and diagnostic accuracy of CT-guided percutaneous automated gun biopsy, and to compare the results with those reported for fine needle aspiration. MATERIALS AND METHODS: Using automated biopsy devices, 118 CT-guided percutaneous biopsies of pulmonary lesions were performed. An 18-gauge needle was used. Final diagnosis was made with operation or other methods. We retrospectively analyzed the frequency of complications and diagnostic yields of 118 biopsies. RESULT: Four of 118(3.3%) patients developed pneumothorax and two of these required chest tube insertion. Other complications were resolved spontaneously. 106 biopsies (89.8%) yielded sufficient tissue for pathologic evaluation. For cases of malignant and of benign disease, sensitivity was 91.8% and 87.7% respectively ; the corresponding figures for diagnostic accuracy were 88.5% and 78.9%. CONCLUSION: CT-guided automated gun biopsy of the pulmonary lesions is safe, witha pneumothorax rate comparable to that of fine needle aspiration. In the absence of a trained cytologist at the time of biopsy, the diagnostic accuracy of automated gun biopsy of pulmonary lesions compared favorably with the reported accuracy of fine needle aspiration.
