ABSTRACT

ObjectiveTo explore the association between daily executive function and core symptoms, the symptoms of attention deficit hyperactivity disorder (ADHD) in children with autism spectrum disorder (ASD), and the moderating effect of theory of mind and other cognitive abilities on this association. MethodsChildren aged 6-12 years with ASD were recruited, and 86 children were identified according to the inclusion and exclusion criteria. Wechsler Intelligence Test for Children, Fourth Edition (WISC-Ⅳ), Strange Story Test (SST) and Behavior Rating Inventory of Executive Function (BRIEF) were used to evaluate children's cognitive ability. Swanson Nolan and Pelham-Version Ⅳ Scale (SNAP-Ⅳ), Social Responsiveness Scale (SRS), and Repetitive Behavior Scale-Revise (RBS-R) were used to assess the severity of ADHD symptoms, social impairment, and repetitive stereotyped behavior. Multiple linear regression was used to explore the association between daily executive function and ADHD symptoms, social impairment, repetitive stereotyped behaviors. ResultsAfter controlling for the score of strange stories, verbal comprehension index (VCI) and other factors, the full scale score and each index of BRIEF were positively correlated with full scale score of SNAP (b = 0.619-0.741, b’ = 0.637-0.755), SRS (b = 0.928-1.200, b’ = 0.417-0.513) and RBS-R (b = 0.326-0.525, b’ = 0.339-0.520) in children with ASD (P< 0.05), and the SNAP total score was more strongly correlated with the full scale BRIEF score and each index score (b’ = 0.637-0.755,P< 0.01). In addition to daily executive function, strange stories score (b = -2.218- -1.839) and age (b = 3.181-4.037) were also the important factors affecting the social function of children with ASD (P< 0.01). There were no moderating effects of strange stories score and age on the association between BRIEF score and full scale score of SNAP, SRS, and RBS-R(P> 0.05). ConclusionThe deficits of daily executive function in school-aged ASD children are significantly associated with core symptoms and ADHD symptoms, and the association is independent of other cognitive domains, such as theory of mind and verbal comprehension intelligence quotient.

A total of 1 034 micro - small and medium sized manufacturing enterprises in Jiangxi Province were selected as the research subjects using a stratified cluster sampling method. The exposure of occupational hazards and the implementation of occupational hazards prevention and Results , control measures were investigated. Among the 1 034 enterprises the small and micro enterprises accounted for , 91.5%. The enterprises with serious occupational hazards were mainly concentrated in metallurgy building materials industry , and machinery equipment and electrical appliance manufacturing industry. The exposure rate of occupational hazard factors in , , ( vs the categories of occupational hazard factors from high to low was physical factors dust and chemical factors 78.9% 52.8% vs ,P ) ( ), , 25.0% <0.01 . The exposure rate from high to low was metallurgy and building materials industry 60.8% machinery ( ), , equipment and electrical equipment manufacturing industry 42.9% light industry textile and tobacco processing industry ( ), , ( )(P ) 32.0% chemical petrochemical and pharmaceutical industry 21.0% <0.01 . Noise exposure accounted for 98.3% in the workers exposed to physical factors. The implementation rate of prevention and control measures for occupational hazards , in enterprises from high to low was the staffing of occupational health management personnel the establishment of , , occupational health management institutions the establishment of occupational health management systems the detection , , of occupational hazards the evaluation of the control effect of occupational hazards of construction projects and the - ( vs vs vs vs vs ,P ) pre evaluation of occupational hazards of construction projects 32.5% 25.7% 23.7% 16.2% 6.9% 4.2% <0.01 . Most of the micro small and medium sized manufacturing enterprises have not carried out the detection of occupational hazards and evaluation of occupational hazards in accordance with the law. The situation of occupational disease prevention and control is still challenging.

ABSTRACT

ObjectiveTo observe the clinical efficacy of Jiawei Xiaochaihutang combined with microwave ablation (MWA) in the treatment of primary hepatocellular carcinoma (HCC) and its influence on tumor microenvironment. MethodA total of 128 patients were randomly divided into control group (64 cases: 2 cases of dropout,2 cases of elimination,and 60 cases of completion) and observation group (64 cases: 3 cases of dropout,2 cases of elimination,and 59 cases of completion). Both groups were given comprehensive treatment after MWA surgery. Patients in control group took Biejiajian Wan orally (3 g/time,3 times/d), and those in observation group took Jiawei Xiaochaihutang (1 dose/d). The treatment lasted for 3 consecutive months. The size of solid tumor before and after treatment was evaluated to record the progression-free survival (PFS). The alpha-fetoprotein-L13 (AFP-L3),des-γ-carboxy prothrombin (DCP),Golgi protein 73 (GP73),tumor necrosis factor-α (TNF-α),transforming growth factor-β (TGF-β),vascular endothelial growth factor (VEGF) and matrix metalloproteinase-2 (MMP-2) levels,as well as performance status (PS),liver function and syndrome of liver depression and Qi stagnation scores were also detected before and after treatment. In addition, the incidence of side effects of grade Ⅲ and above was compared. ResultThe total effective rate of solid tumor in observation group was 91.53% (54/59),higher than that (76.67%, 46/60) in control group(χ2=4.895,P<0.05). The PFS in observation group was (7.16±0.95) months, longer than that (6.24±0.89 months) in control group (P<0.01). The effective rate of traditional Chinese medicine (TCM) syndrome in observation and control groups were 88.14% (52/59)and 70.00% (42/60), respectively (χ2=5.897,P<0.05). The observation group (57.63%,34/59) had higher marked effective rate of TCM syndrome than control group (31.67%,19/60) (χ2=8.116,P<0.01). The AFP-13,DCP,GP73,TNF-α,TGF-β,VEGF and MMP-2 levels and the PS,liver function and syndrome of liver depression and Qi stagnation scores in observation group were lower than those in control group (both P<0.01). The cumulative incidence of side effects of grade Ⅲ and above in observation and control groups was 16.95% and 33.33%, respectively(χ2=4.261,P<0.05). ConclusionConsolidation treatment of HCC after MWA surgery with Jiawei Xiaochaihutang relieved symptoms and side effects,improved PS and liver function,regulated tumor microenvironment,inhibited tumor markers and prolonged survival time. The clinical effect was better than that of Biejia decoction pill, and thus it was worthy of clinical use.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the value of serum procalcitonin (PCT) for the guidance of antibiotic therapy in children with lower respiratory tract infection (LRTI).</p><p><b>METHODS</b>A prospective randomized controlled study was conducted in 396 children with LRTI who visited Weifang Maternity and Child Care Hospital. The participants were randomly assigned into a PCT group in which the antibiotic therapy was guided by serum PCT level and a control group in which the standard therapy was given according to clinical guidance. Afterwards, a subgroup analysis was performed according to whether the patient was diagnosed with community-acquired pneumonia (CAP). After 14-day treatment, antibiotic prescription rate, duration of antibiotic treatment, and side events were compared between the groups.</p><p><b>RESULTS</b>A total of 396 cases were recruited and equally assigned into the PCT group and the control group, among whom the numbers of the children with CAP were 125 and 123, respectively. The mean duration of antibiotic treatment was significantly shorter in the PCT group than in the control group (P<0.05). The subgroup analysis showed that the duration of antibiotic treatment in both CAP and non-CAP PCT subgroups was significantly shorter than in the control subgroups (P<0.05), however, the antibiotic prescription rate in the non-CAP PCT subgroup was significantly higher than that in the non-CAP control subgroup (P<0.05). There were no differences in the rate and duration of side events from antibiotic therapy, hospitalization rate, the length of hospital stay, and safety between the PCT and control groups.</p><p><b>CONCLISOPNS</b>Serum PCT-based guidelines on antibiotic use can shorten the duration of antibiotic therapy in children with LRTI.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of whole lung lavage (WLL) on the pulmonary function and exercise capacity in patients with pneumoconiosis.</p><p><b>METHODS</b>Forty-one patients with pneumoconiosis who quit dust-exposed work not more than 6 months before underwent WLL. Clinical symptom assessment, pulmonary function test, and cardiopulmonary exercise test were performed before and one week after WLL, and the results were compared.</p><p><b>RESULTS</b>The patients with pneumoconiosis showed no significant changes in clinical symptoms after WLL. At one week after WLL, the patients with pneumoconiosis showed nonsignificant increases in forced vital capacity, forced expiratory volume in one second (FEV1.0), and percent predicted FEV1 (P > 0.05); peak oxygen uptake (peak VO₂) increased from 2140.6 ± 353.2 ml/min before WLL to 2374.6 ± 362.4 ml/min after WLL, percent predicted peak VO₂ increased from 82.2 ± 13.7% before WLL to 91.0 ± 14.0% after WLL, peak VO₂/kg increased from 30.6 ± 3.5 ml/min×kg before WLL to 34.2 ± 3.7 ml/min×kg after WLL, and ventilatory equivalent for carbon dioxide decreased from 30.6 ± 3.1 before WLL to 26.1 ± 2.7 after WLL (P < 0.05).</p><p><b>CONCLUSION</b>WLL can remarkably improve the oxygen uptake and ventilatory efficiency in patients with pneumoconiosis during exercise, so it can improve the exercise capacity of these patients.</p>

ABSTRACT

Wnt proteins together with their downstream effectors forms a set of important signal pathways. The Wnt signal pathway is important in a wide variety of development processes including cell growth, cell differentiation, cell polarity and apoptosis. Wnt4 is a key regulator of gonadal differentiation in humans and mice, playing a pivotal role in early embryogenesis. With RACE technique based on a EST identified in our lab, a novel gene including a complete open reading frame was cloned and named Sjwnt4 (GenBank accession No. DQ643829). Sequence analyses showed that SjWnt4 had a typical characteristics of Wnt family proteins, sharing 43% similarity to Dugesia japonica and 37% to human Wnt4. The ORF of Sjwnt4 contains 1311 nucleotides, encoding 436 amino acid with 49.6 kD molecular weight. Real-time PCR analysis from the worms of various stages of S. japonicum revealed that the mRNA level of Sjwnt4 is highest in the 19 days schistosomula, followed by 44 days female worms, 14 days schistosomula, 31 days adult worms and 44 days male worms, suggesting a stage-and-gender differential express. The Sjwnt4 cDNA fragment was subcloned into a modified expression vector pGEX-4T-2 and transformed into E. coli BL21 (DE3) cells, and the production of recombinant Sjwnt4 protein fused to a GST tag was analysed. In the presence of IPTG, the 76kD fusion protein was expressed in included bodies. Western-blotting revealed that the fusion protein could be recognized by the rabbit serum specific to Schistosoma japonicum adult worm antigen preparation. The study provides important basis for investigating the regulation mechanism of the Wnt signaling pathway during the development especially gonadal differentiation processes of Schistosoma japonicum.

ABSTRACT

<p><b>BACKGROUND</b>Now lung volume reduction surgery (LVRS) has become one of the most effective methods for the management of some cases of severe chronic obstructive pulmonary disease (COPD). We evaluated the mid-term effects of LVRS on pulmonary function in patients with severe COPD.</p><p><b>METHODS</b>Ten male patients with severe COPD aged 38 - 70 years underwent LVRS and their pulmonary function was assessed before, 3 months and 3 years after surgery. The spirometric and gas exchange parameters included residual volume, total lung capacity, inspiratory capacity, forced vital capacity, forced expiratory volume in one second, diffusion capacity for CO, and arterial blood gas. A 6-minute walk distance (6MWD) test was performed.</p><p><b>RESULTS</b>As to preoperative assessment, most spirometric parameters and 6MWD were significantly improved after 3 months and slightly 3 years after LVRS. Gas exchange parameters were significantly improved 3 months after surgery, but returned to the preoperative levels after 3 years.</p><p><b>CONCLUSIONS</b>LVRS may significantly improve pulmonary function in patients with severe COPD indicating for LVRS. Mid-term pulmonary function 3 years after surgery can be decreased to the level at 3 months after surgery. Three years after LVRS, lung volume and pulmonary ventilation function can be significantly improved, but the improvement in gas exchange function was not significant.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinicopathologic features and differential diagnosis of cryptogenic organizing pneumonia (COP).</p><p><b>METHODS</b>The clinical, radiologic and pathologic features of 11 patients with COP confirmed by open or video-assisted thoracoscopic (VATS) lung biopsy were analyzed. Treatment information and follow-up data were also obtained.</p><p><b>RESULTS</b>COP usually affected female patients over 40 years of age. Clinical presentations included cough, sputum and exertional dyspnea. High-resolution computerized tomography showed scattered speckled, patchy and trabecular shadows over both lung fields. Honeycomb changes were not found. Histologically, polypoid growths of granulation tissue were noted within respiratory bronchioles, small airways and alveolar spaces. These lesions had a patchy and peribronchiolar distribution and were uniform in appearance. The overall response rate to glucocorticoid was 81.8% (P < 0.01). The duration of follow up ranged from 6 to 134 months. Apart from one patient who developed aggravation of symptoms, the disease pursued a relatively stable clinical course.</p><p><b>CONCLUSIONS</b>In general, COP responds well to glucocorticoid therapy. Open or VATS lung biopsy is important for arriving at a definitive diagnosis, especially for those cases presenting with atypical clinical and radiographic manifestations. Multiple biopsies with larger samples are preferred in order to avoid misdiagnosis.</p>

ABSTRACT

Objective To study the clinical significance of pulmonary membrane diffusing capacity(Dm) and pulmonary capillary blood volume(Vc) in patients with stable chronic obstructive pulmonary disease(COPD). Methods Spirometry was performed in 38 patients with stable COPD and 35 healthy individuals in resting condition.The changes of pulmonary parameters were obtained and compared between groups. Results Spirometry test revealed that the percent predicted forced expired volume in one second(FEV1),FEV1/forced volume capacity(FVC)and the percent predicted maximal ventilatory volume(MVV) were declined from stage Ⅰin patients with COPD in comparison with healthy individuals,while diffusing capacity for carbon monoxide of lung(DLCO),carbon monoxide diffusing capacity per liter of alveolar(DLCO/VA),Dm and Vc were declined from stage Ⅱ.Dm in patients with COPD of stageⅠwas sig-nificantly decreased compared with the controls,while Vc was increased compared with the controls(both P
