ABSTRACT

Pityriasis amiantacea is a distinct scalp disease characterized by asbestos-like, thick, adherent scales that engulf and bind down tufts of hair. Herein, we report two cases of pityriasis amiantacea. One case was a 75-year-old female who showed simultaneous keratotic follicular papules and psoriasiform dermatitis on her body. The other case was a 14 year old female showing seborrheic dermatitis on her face. Previously, only one case of pityriasis amiantacea had been reported in Korea.

Sebaceous trichofolliculoma is a rare benign tumor and represents a variant of trichofolliculoma. It presents as a centrally depressed solitary tumor and occurs in areas rich in sebaceous lobules. Microscopically, it shows keratin-filled cystic cavities and sebaceous lobules. The lobules are connected to the cystic wall. Herein we report a case of sebaceous trichofolliculoma on the right cheek in a 64-year-old man.

ABSTRACT

Ichthyosiform eruption as a specific manifestation of mycosis fungoides is very rare and only a few such cases have currently been reported in the medical literature. A 63- year-old Korean man presented with a 4-year history of a pruritic ichthyotic eruption. There was no personal or family history of ichthyosis or atopy. The ichthyosiform skin changes involved the abdomen, arms, thighs and shins. The face, palms and soles were spared. There was no peripheral lymphadenopathy or organomegaly. The typical lesions of mycosis fungoides were not present. The results of the routine investigations were normal or negative. A skin biopsy specimen revealed the findings of early mycosis fungoides. He was successfully treated with photochemotherapy.

ABSTRACT

Nevus lipomatosus cutaneous superficialis is a fairly uncommon form of connective tissue disorder, and this is characterized by the ectopic deposition of fat lobules in the dermis. There are two clinical variants: the multiple and solitary types. Particularly for the case of the solitary forms, it seems preferable to regard them instead as pedunculated lipofibromas. We report here on a case of a nipple shaped solitary nevus lipomatosus cutaneous superficialis, and we also review the Korean cases. Till now, 22 cases in Koreahave been reported as a solitary nevus lipomatosus cutaneous superficialis and a pedunculated lipofibroma, or both. The review of the Korean cases revealed that solitary nevus lipomatosus cutaneous superficialis almost always exhibits a single skin-colored dome shaped or pedunculated nodule on the thigh and buttock, as well as on the non-girdle area after the 4th decade, and it has no histopathological difference from the multiple forms.

ABSTRACT

We report a case of pancreatic panniculitis associated with acute pancreatitis. The patient was a 63-year-old woman who presented with multiple painful erythematous nodules on both shins and had a mild fever and chill. Histopathologic findings showed fat necrosis with anucleate "ghost-like" fat cells with a thick shadowy wall. Laboratory evaluation revealed an increase in serum blood amylase and lipase. We diagnosed as pancreatic panniculitis associated with acute pancreatitis. Past medical history included rheumatoid arthritis and chronic renal failure treated with hemodialysis. She was taking several medicines due to rheumatoid arthritis and was newly prescribed the mizoribine (Bredinin(R)), 50 mg twice daily for 50 days. We therefore considered the possibility of drug induced pancreatitis due to mizoribine.

ABSTRACT

Sclerotic fibroma of the skin is an uncommon benign tumor characterized histopathologically by a well-circumscribed dermal nodule showing hypocellular hyalinized collagen bundles. The bundles are separated by clefts and arranged in a storiform pattern. The histogenesis of this fibroma is uncertain. Herein, we report a case of sclerotic fibroma presenting on the left ala nasi of a 27-year-old woman without Cowden's disease.

BACKGROUND: Burow's graft is a useful full-thickness skin graft that employs the adjacent lax skin as a donor site and this is especially suitable for the facial region. When reconstructing a facial cutaneous surgical defect, we have to consider the facial cosmetic unit together with various other factors. OBJECTIVE: The aim of this study was to assess the feasibility of using a Burow's graft by dermatologists and the cosmetic outcomes of Burow's graft for reconstructing facial defects, based on different facial cosmetic units. METHODS: From August 2005 to February 2007, 26 patients were treated with the Burow's graft technique for the reconstruction of facial cutaneous defects following Mohs micrographic surgery. According to the facial cosmetic unit, we divided the face into the forehead, periorbital area, nose, cheek and chin, and we redistributed the cases by the cosmetic units. The post-operative result of each case was rated with using serial clinical photographs that were taken at 1, 6 and 12 months, and the patient's charts were reviewed by two independent physicians. RESULTS: The cosmetic results of facial reconstructions by using Burow's graft were satisfactory, regardless of the cosmetic unit. 21 cases out of the 26 cases (81%) showed good to excellent results after 6 months. The early cosmetic consequence of the periorbital region was better than those of the other facial cosmetic units. No side effects were reported other than a trapdoor deformity in three cases. CONCLUSION: Burow's graft can be a good choice for reconstruction of facial cutaneous defects after skin cancer surgery in all the facial cosmetic units.

ABSTRACT

Basal cell carcinoma is the most common malignant tumor of the skin. Most of the lesions are usually small, slow growing and rarely metastasize. However, giant basal cell carcinoma, which is defined as a lesion of more than 5 cm at its greatest diameter, is rare. Most giant basal cell carcinomas are of long duration, and neglect seems to be a major contribution to the development of giant basal cell carcinoma. Herein, we report a rare case of treating a recurrent giant basal cell carcinoma on the face of a Korean elderly woman with surgical excision.

ABSTRACT

Neuromas are hyperplasia of axons and associated nerve sheath cells. Cutaneous neuromas are uncommon. Three kinds of true neuromas exist in the skin and mucous membranes: traumatic neuromas, solitary palisaded encapsulated neuromas, and multiple mucosal neuromas. Traumatic neuromas are usually solitary, skin-colored or pink, firm papules or nodules at the sites of scars following local trauma. A 2-year-old male patient presented a solitary rice grain sized, skin-colored painful and tender polypoid papule on the left 2nd finger. The lesion was superimposed on a small linear scar. The patient had taken a trauma by a broken piece of glass about one month before the development of the neuroma. We report a case of traumatic neuroma with a review of the literature.

ABSTRACT

Tufted angioma is a relatively rare hemangioma which mainly occurs in pediatric age. It is slowly-growing over years, with a clinical picture of papule to plaque. Development with an annular configuration is very rare and only five such cases have been reported worldwide. A 64-year-old woman presented with an egg-sized, red, annular lesion on her abdomen which had started as a papule 8 years previously. The biopsy revealed typical histopathological features of tufted angioma. The lesion was treated three times with surgery or cryotherapy and showed recurrence each time over the seven years. During the course it was apparent that the lesion was showing annular extension. We believe the case is the first report of annular tufted angioma showing central regression in Korea. It was also interesting that the benign lesion showed recurrence twice after complete subsidence.

ABSTRACT

Exposure to sunlight is the major cause of basal cell carcinoma (BCC). The axilla is not a sun-exposed region, and BCC affecting the axilla is rare. To our knowledge, only 4 cases of axillary BCC have been reported in Korea. We report a case of BCC in the axilla of a 67-year-old man. He presented with a 1 year history of a well-demarcated 2.0x0.5 cm pigmented plaque in his right axilla. The skin lesion was histopathologically diagnosed as BCC. He was treated by surgical excision. There was no recurrence after 1 year of follow-up.

ABSTRACT

Although there are many methods for the treatment of actinic cheilitis including cryotherapy, vermilionectomy, and topical fluorouracil, the choice of treatment should be determined after assessment of the lesion and its location. A 67-year old woman developed actinic chelitis, where a thick keratotic plaque occupied a half of her lower lip. Surgical removal of the lesion was difficult because of its size, and repeated cryotherapy treatment also failed. The lesion was then successfully treated by a the topical application of combined fluorouracil and salicylic acid solution.

ABSTRACT

The skin defects made from excision of malignant skin tumors can usually be reconstructed by secondary intention, primary closure, a local flap or skin graft. When reconstructing any skin defect, we must consider the defect site and size, reconstruction method, blood supply, wrinkling, relaxed skin tension line, donor site, patient's age, general health status, maintenance of function and cosmetic aspect. Reconstruction of the periorbital area is challenging to the dermatologist owing to the complex function of the eye, relative lack of adjacent loose tissue, free anatomic margin, and the need to maintain symmetry with the contralateral eye. The Burow's graft is a useful technique for periorbital wound reconstruction, using a full-thickness skin graft where the donor site is the skin adjacent to the wound. The use of adjacent skin as a graft gives a better color, texture, and thickness match than traditional donor sites. It also provides a simple method for reconstruction of wounds, compared to those repaired by a tight primary closure or local flap which may have excessive tension on sutures and distortion of nearby structures.

ABSTRACT

Nicolau syndrome is a rare adverse reaction of the skin at the site of intramuscular drug injection, and has largely unidentified pathogenesis. It consists of the development of an acute, severe pain and a localized erythematous rash during intramuscular injection leading to cutaneous, subcutaneous and even muscular necrosis. We report a case of a 55-year-old woman who presented with a painful skin necrosis in her buttocks following an intramuscular diclofenac-beta-dimethyl-aminoethanol injection.
