ABSTRACT

Objective@#To observe the clinical effect of self-made Jiuxin decoction in the treatment of end-stage hypertrophic non-obstructive cardiomyopathy.@*Methods@#From July 2016 to October 2017, 60 patients with end-stage hypertrophic non-obstructive cardiomyopathy in the First People′s Hospital of Yuhang District were randomly divided into two groups according to digital table, with 30 cases in each group.The control group was treated with conventional western medicine, and the observation group was treated with self-made Jiuxin soup for a week.The changes of body weight, degree of edema, NYHA class, Lee score, B-type natriuretic peptide, blood angiotensin Ⅱ, blood potassium, sodium, chlorine content and other indicators before and after treatment were observed in the two groups.@*Results@#After one-week treatment, the cardiac function and heart failure indicators of the two groups were all significantly improved(the observation group: t=10.527, 15.358, all P<0.01; the control group: t=7.504, 12.417, all P<0.01), and the indicators of the observation group were better than those of the control group, and the differences between the two groups were statistically significant(t=3.772, 4.446, all P<0.01). After treatment, the levels of B-type natriuretic peptide of the two groups were significantly lower than those before treatment (the observation group: t=5.628, the control group: t=11.423, all P<0.05). The angiotensin and renin levels in the observation group were all significantly lower than those before treatment(t=4.355, 6.418, all P<0.05), and the indicators of the observation group were all significantly lower than those of the control group(t=3.521, 4.275, all P<0.05). The incidence rate of adverse reaction of the observation group was significantly lower than that of the control group(3.3% vs.23.3%, χ2=5.192, P=0.023).@*Conclusion@#Self-made Jiuxin decoction can significantly improve the cardiac function in patients with end-stage hypertrophic non-obstructive cardiomyopathy and it is a safe way of treatment.

ABSTRACT

Objective@#To observe the effect of ticagrelor for non-ST-segment elevation acute coronary syndrome after percutaneous coronary intervention (PCI).@*Methods@#A total of 200 patients with non-ST-segment elevation acute coronary syndrome after PCI were enrolled in this study.And the patients were randomly divided into the observation group (18 months treatment group, 100 cases) and control group (12 months treatment group, 100 cases) according to the digital table.The control group was given dual antiplatelet therapy (DAPT) for 12 months, and then, suspended the usage of ticagrelor.The observation group was treated by DAPT for 18 months.The major adverse cardiovascular events (MACCE) and the secondary end point events were observed.@*Results@#The incidence rate of MACCE between the two groups had no statistically significant difference (χ2=0.298, P=0.862). And there was no statisticallysignificant difference in the incidence of the secondary end point events between the two groups (χ2=1.963, P=0.375).@*Conclusion@#The usage of DAPT beyond or equal 12 months doesn't reduce the incidence of MACC and it will not increase the blooding rate.The use of DAPT (aspirin and ticagrelor) should be used less than 12 months.

ABSTRACT

Objective To explore the therapeutic efficacy of irbesartan combined amlodipine for the treatment of hypertension and diabetic nephropathy.Methods 120 patients with diabetic nephropathy and hypertension received from March 2011 to March 2014 were randomly divided into three groups according to its administration circumstances ( irbesartan group and amlodipine group, irbesartan and amlodipine groups) , with 40 patients in each group.Blood pressure, 24h urine albumin, 24h urinary protein excretion and urine β2-microglobulin and other indicators of the three groups of patients were observed and compared after 10 weeks of treatment.Results After treatment, the total efficiency of A, B two groups was no significant difference.The total effective rate of group C (92.5%) was significantly higher than that in A group (χ2 =6.65, P<0.05), the total effective rate of group C was significantly higher than that of group B (χ2 =7.81, P<0.05).The blood pressure in patients of three groups were all dropped to normal after treatment, but the one in combination group was significantly lower than the irbesartan group and amlodipine group, the difference was statistically significant (P<0.05).The urine tests related indicators of patients in the three groups were also significantly reduced after treatment , but 24 h urine albumin, 24 h urinary protein excretion and urine β2-microglobulin in combination group were significantly lower than the irbesartan group and amlodipine group, the difference was statistically significant (P<0.05).Three groups were no obvious adverse reactions. Conclusion Irbesartan combined amlodipine for the treatment of hypertension and diabetic nephropathy has significant efficacy compared to single drug.
