ABSTRACT

Objective To compare the efficacy and resistance between S?1 combined with radiotherapy and S?1 alone in the treatment of elderly patients with locally advanced gastric cancer. Methods Fifty?eight elderly patients with unresectable locally advanced ( stage Ⅲ) gastric cancer were randomly and equally divided into S?1 combined with concurrent radiotherapy group ( experimental group ) and S?1 alone group ( control group ) . The experimental group received 4 cycles of S?1 treatment with each cycle containing two?week oral administration of S?1 at a dose of 40 mg/m2 twice a day followed by one?week drug withdrawal. Gastric intensity?modulated radiotherapy was performed concurrently with a dose of 45 Gy ( 1. 8 Gy per fraction) . The control group received the same dose of S?1 alone. Short?term outcomes and adverse reactions were evaluated in the two groups. Comparison was made by chi?square test. Results All patients completed the planning treatment. The experimental group had significantly higher objective response, disease control, and symptom remission rates than the control group ( 52% vs. 24%, P=0. 03;76% vs. 45%, P=0. 016;86% vs. 48%, P=0. 005) . There were no significant differences in the incidence of nausea and vomiting, anorexia, leukopenia, diarrhea, or thrombocytopenia between the two groups ( all P>0. 05) . Conclusions S?1 treatment combined with concurrent radiotherapy improves the short?term outcomes and causes tolerable toxicities in the treatment of elderly patients with locally advanced gastric cancer.

ABSTRACT

Objective To evaluate the efficacy and safety of 125I seed implantation combined with transcatheter arterial chemoembolization (TACE) in the treatment of primary hepatocellular carcinoma (PHC).Methods 156 patients who had unresectable PHC seen from January 2012 to December 2015 in our unit were randomly assigned into the study group (76 patients) and the control group (80 patients).The study group of patients were treated with intra-hepatic implantation of 125I seed + TACE,and the control group of patients were treated with three dimensional conformal radiotherapy (3DCRT) + TACE.After ra diotherapy the two groups of patients underwent 2 to 3 times TACE treatment.The biomarker levels were compared between the two groups before and after treatment,and the short-term efficacy,clinical safety and 1 year survival rates were assessed.Results The success rates of the treatment for the two groups were both 100%.Patients in the study group were implanted with 1 016 125I seeds,with an average of (13.7 ± 2.5) seeds per patient.The two groups had no significant difference on short-term efficacy (P > 0.05).The DCR,ORR and 1 year survival rates in the study group were 63.2%,92.1%,55.5%,and they were 61.3%,90.0%,58.1% in the control group,respectively.There were no significant differences between these 2 groups (P > 0.05).At 1 month after treatment,the two groups had no significant differences onAFP,IGF-Ⅱ and IGFBP-2 [(5.08±0.85) μg/L vs.(5.12 ±0.79) μg/L,(4.77 ±0.58) μg/L vs.(4.86 ±0.53) μg/L,(4.98 ±0.67) μg/L vs.(5.04 ±0.71) μg/L] (P>0.05).There were no signif-icant differences on chemotherapy drug toxicities between the two groups (P > 0.05).The incidence of radi-ation hepatitis in the study group was 1.3%,while the incidences of radiation dermatitis,hepatitis,gastriculcer in the control group were 5.0%,8.8%,2.5%,respectively.The radiation dermatitis incidence inthe study group was significantly lower than that of the control group (P < 0.05).In the study group,2 pa-tients (2.6%) had their 125I seed slightly moved.Conclusion The short-term efficacy of 125I seed implan-tation plus TACE in the treatment of PHC is no less than the 3DCRT + TACE regimen,but with less radia-tion side effects.

Patients with advanced lung cancer should be given proper hospice care .Doctors should not only practice protective medicine on the foundation of laws and regulations , informing the patients about their conditions tactfully , but also try their best to provide the patients with optimal therapeutic strategies , taking the patient's physi-cal tolerance, economy, etc.into account and achieving the goal of humanized and individualized therapy .Mean-while, the construction of professional medical -nursing institution is in urgent need .Only providing general nurs-ing in multiple aspects of illness , mental health , social functions , can we relieve the pain both physically and men-tally, and improve the patient's quality of their terminal times .

ABSTRACT

Objective To investigate the influence of fishbone diagram in the incidence of blood residual after sealing tube indwelling needle in cardiovascular disease patients.Methods Two hundred and ninety-three patients(1,035 times)with sealing tube intravenous indwelling needle were assigned into control group and another 272 ones(1,276 times)with the needle into experiment group according to admission sequence. In the former group the method of conventionally sealing the tube for intravenous indwelling needle was applied and in the latter,fishbone diagram was applied to analyze the key factors for blood residuals and regulate pertinent measures for effective sealing of tube.The two groups were compared in terms of incidence of blood residuals.Result The incidence of blood residuals in the experiment group was significantly lower than that in the control group(P<0.05).Conclusion The fishbone diagram is effective in reducing the incidence of blood residuals after sealing the tube for intravenous indwelling needle by way of taking effective interventional measures after screening out the key influential factors for blood residuals.

AIM: To examine the effects of L-carnitine on apoptosis in cultured neonatal rat cardiomyocytes induced by hypoxia/reoxygenat ion and its possible mechanism. METHODS: A cell culture model of neonatal rat cardiacmyocytes wa s used. The cultured cardiomyocytes were classified into three groups: control g roup, I/R group (anoxia for 120 min, reoxygenation for 240 min) and L-carnitine group (L-carnitine, which was classified into four different concentrations, was added to the cells 2 h before anoxia). The activities of superoxide dismutase ( S OD), the content of malondialdehyde (MDA) and the apoptosis were determined by f low cytometry (FCM). AIM: To examine the effects of L-carnitine on apoptosis and oxidant injury in cultured neonatal rat cardiomyocytes induced by hypoxia/reoxygenation and its possible mechanism. METHODS: The cultured cardiomyocytes were divided into three groups, control, A/R group (anoxia for 120 min, reoxygenation for 240 min) and L-carnitine treatment group, in which cells were exposed to 20 mg/L, 50 mg/L, 100 mg/L, 200 mg/L L-carnitine respectively at 2 h before anoxia. The superoxide dismutase (SOD), succinate dehydrogenase (SDH) activities and malondialdehyde (MDA) content were examined, and the apoptosis was determined by flow of cytometry (FCM). In addition, the ultrastructure was observed by transmission electron microscopy.
