ABSTRACT

Myxomas are the most frequent,cardiac benign cardiac tumors which often present with stroke caused by tumorous orthrombotic emboli.The treatment of embolic stroke due to cardiac myxoma is still a clinical and technical challenge.A 61-year-old man who had an embolic stroke in the left middle cerebral artery was admitted to the Third Poeple's Hospital of Huizhou.The initial National Institutes of Health Stroke Scale (NIHSS) score was 16.He received endovascular thrombectomy after intravenous recombinant tissue plasminogen activator (rt-PA) one hour after stroke onset.No intracranial hemorrhage developed.Pathological study of embolus showed a myxoma.A cardiac mass was found in the left atrium and removed surgically three weeks after stroke.Pathological study of the tumor showed a myxoma.At the one-month follow-up after excision of myxoma,the NIHSS score was 1 and the modified Rankin scale score was 0.No recurrence of embolism occurred after surgical resection.Endovascular thrombectomy after intravenous rt-PA (bridging therapy) for embolic stroke due to cardiac myxoma is safe and effective,and early resection of atrial myxoma can effectively avoid recurrence of cerebral infarction.

ABSTRACT

Objective To study the prevalence of hyperuricemia and its risk factors in elderly people of Yiyang District,Hunan Province,China.Methods A total of 1363 elderly people undergoing physical examination in our hospital were divided into hyperuricemia group (n=245) and control group (n=1118).Their general clinical data were recorded and compared.The risk factors for hyperuricemia were analyzed.Results The age was significantly older,number of males was significantly larger,the serum TG,LDL-C,urea,UA levels and body height,BMI,SBP were significantly higher while the serum TC and HDL-C levels and Cr clearance rate were significantly lower in hyperuricemia group than in control group (P<0.05,P<0.01).Pearson correlation analysis showed that serum UA level was positively related with age,serum TG and urea level,BMI,SBP and DBP,but negatively related with Cr clearance rate and serum TC,LDL-C,HDL-C levels (P< 0.01).Binary logistic regression analysis showed that age,male gender,hyperlipidemia,and obesity were the independent risk factors for hyperuricemia after adjustment of age (OR=1.857,95 % CI:1.302-2.649,P=0.001;OR=1.866,95%CI:1.363-2.555,P=0.000;OR=2.214,95% CI:1.716-2.856,P=0.000).However,serum HDL-C level and Cr clearance rate were the protective factors for hyperuricemia (OR =0.388,95 % CI:0.242-0.623,P =0.000;OR =0.948,95%CI:0.937-0.959,P=0.000).Conclusion The prevalence of hyperuricemia is rather high in elderly people of Yiyang Distric,Hunan Province,China,suggesting that the lifestyle of elderly people should be improved and hyperuricemia should be treated with appropriate drugs.

ABSTRACT

Objective The microbiology specimen types were various and complex ,the system of main specimen types in micro-biological detection was established under the laboratory information system (LIS) for realizing the management target of quality control before microbiological analysis .Methods A total of 3304 submitted microbiological samples were collected from January 1 to 31 in 2015 .After setting the microbiological item application procedure of main specimen types in LIS ,1532 submitted microbio-logical specimens from June 20 to 24 were performed the statistics .The error rates of specimen types were compared before and af-ter setting .Then 1635 and 1340 submitted microbiological specimens were re-collected form July 9 to 13 and August 10 to 15 ;the change of error rates was continuously observed for comparing whether the statistical difference of error rates existing between be-fore and after setting .Results The error rate of submitted microbiological specimens before setting was 4 .6% (152/3304) ,which after setting was 1 .3% (20/1532)(χ2 =31 .224 ,P<0 .001) ,which during the continuous observation period maintained the lower level of 1 .04% (17/1635) ,χ2 =39 .658 ,P<0 .001) and 0 .9% (13/1340 ,χ2 =34 .673 ,P<0 .001) .Conclusion Re-setting the LIS reduces the error rate of microbiological specimen type ,effectively increase the working efficiency and reaches the quality control in-dex before microbiological analysis .
