ABSTRACT
OBJECTIVE：To explore the mec hanism of baicalein plat elet aggregation inhibitiory effect and lung tissue protective effect of baicalein in model rats with acute pulmonary embolism. METHODS ：Totally 36 rats were randomly divided into normal control group （n＝6）and modeling group （n＝30）. The acute pulmonary embolism model was established by autologous thrombus replication in modeling group ，and the sham operation of rats in normal control group was carried out. After modeling ， 30 model rats were randomly divided into model control group ，positive drug group （low molecular weight heparin calcium 0.01 mL/kg，subcutaneous injection ），baicalein low-dose ，middle-dose and high-dose groups （25，50，100 mg/kg，intraperitoneal injection），with 6 rats in each group. Normal control group and model control group were intraperitoneally injected constant volume of normal saline ；administration groups were given relevant medicine ，once a day ，for consecutive 7 d. After medication ， platelet aggregation rates of rats after activated with adenosine diphosphate （ADP） and arachidonic acid （AA） and platelet activation index （RPI）were detected ；lung histopathology was observed by HE staining ；serum platelet activation markers granule membrane（CD62P）and lysosomal membrane glycoprotein （CD63），growth differentiation factor- 15（GDF-15）and N-terminal B-type natriuretic peptide （NT-proBNP）were measured by ELISA. The mRNA expression levels of Notch 2，Notch3 and Notch signaling ligand PLL 1，JAG2 were detected by RT-PCR method. The protein expression levels of Notch 2，Notch3，DLL1 and JAG2 in lung tissue were detected by immunohistochemistry and Western blotting assay. RESULTS ：Compared with normal control group，plasma ADP-activated platelet aggregation rate ，AA-activated platelet aggregation rate ，RPI，serum levels of CD 62P， CD63，GDF-15 and NT-proBNP were increased significantly （P＜0.05）. The lung tissue of rats was in a state of severe inflammatory infiltration. mRNA and protein expression levels of Notch 2，Notch3，DLL1 and JAG 2 in lung tissue decreased significantly（P＜0.05）. Compared with model control group ，changes of above indexes of rats were improved significantly in baicalein groups （P＜0.05）. CONCLUSIONS ：Baicalein can reduce platelet aggregation and improve the pathological state of lung tissue in rats with acute pulmonary embolism. Its mechanism 0270） may be related to activating Notch signal pathway.

Among coronavirus disease 2019 (COVID-19) patients who infected with 2019 novel coronavirus (2019-nCoV), compared with the symptomatic infection patients, 2019-nCoV carried by asymptomatic infection patients are more likely to be widely spread due to secrecy and neglect, thus brings severe challenges to the current prevention and treatment of COVID-19. The therapies of asymptomatic 2019-nCoV infection are still in research. Through excavating the Chinese medical classics, it was found that the theory of "pathogen hiding in the pleurodiaphragmatic interspace", with its unique syndrome differentiation system and curative effect of treating symptoms and causes, can provide clinical reference for the treatment of asymptomatic 2019-nCoV infections and provide theoretical basis traditional Chinese medicine in the battle against infectious diseases.
ABSTRACT
Objective To investigate the efficacy and safety of different schemes of methylprednisolone sodium succinate in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods A hundred patients with AECOPD were selected and divided into the control group and the observation group by random number table ,with 50 cases in each group .The control group received intravenous injec-tion of methylprednisolone sodium succinate ,40mg/d,the treatment course was 15d.The observation group received intravenous injection of methylprednisolone sodium succinate ,40 mg/d,the treatment course was 5d.The lung function indicators(RV/TLC,FEV1),blood gas indicators (PaCO2,PaO2,PaO2/FiO2),quality of life index(QLI) score and incidence of adverse reactions were detected and compared between the two groups before and after treatment .Results Compared with before treatment,the RV/TLC of the observation group[(44.2 ±7.4)% vs.(56.7 ±11.1)%]and control group[(43.2 ±7.1)%vs.(55.2 ±10.2)%]after treatment decreased significantly ,the FEV1 of the observa-tion group and control group [(69.7 ±8.7)L vs.(52.4 ±11.6)L,(67.2 ±8.1)L vs.(54.8 ±11.5)L]significantly increased,the differences were statistically significant (t=3.656,3.985,3.364,3.875,all P<0.05),but the differ-ences between the two groups were not statistically significant (t=2.869,1.969,all P>0.05).Compared with before treatment,the PaCO2 of the observation group and control group [(35.7 ±4.5) mmHg vs.(49.6 ±5.2) mmHg,(36.2 ±5.4) mmHg vs.(48.6 ±5.2) mmHg] decreased significantly,and the PaO2 [(84.9 ±13.2) mmHg vs .(50.2 ±4.6)mmHg,(81.1 ±12.3)mmHg vs.(49.6 ±4.9)mmHg]and PaO2/FiO2[(222.5 ±30.7)mmHg vs .(119.3 ±14.6)mmHg,(217.4 ±25.6)mmHg vs.(117.2 ±15.5)mmHg]increased significantly,the differences were statistically significant (t =4.656,3.876,4.985,4.364,3.989,4.875,all P <0.05),but the differences between the two groups were not statistically significant (t=2.526,1.234,1.986,all P>0.05).After treatment,the improvement degree of QLI in the observation group was significantly better than that in the control group [(8.7 ± 2.2)points vs.(7.1 ±1.9)points],the difference was statistically significant (t=4.675,P<0.05).In the observa-tion group,there were 1 case of gastrointestinal discomfort ,2 cases of elevated blood sugar ,and in the control group , there were 4 cases of gastrointestinal discomfort ,2 cases of elevated blood sugar , and 2 cases of hypokalemia .The incidence rate of adverse reactions in the observation group was significantly lower than that in the control group (6.0%vs.20.0%),the difference was statistically significant (χ2 =4.332,P<0.05).Conclusion The short -term methylprednisolone in the treatment of elderly patients with AECOPD has similar effect with conventional dose , but it has minor adverse reactions ,and can improve the quality of life of patients .

Objective To compare the outcomes of percutaneous Kirschner wire assisted reduction with minimally invasive plate osteosynthesis and open reduction with locking plate in treatment of Robinson type 2B midshaft clavicular fractures.Methods A retrospective case control study was conducted to analyze the clinical data of 96 patients with Robinson type 2B midshaft clavicular fracture from June 2009 to October 2016.There were 70 males and 26 females with an average age of 34.5 years.The patients were divided into two groups according to the different surgical methods:49 patients were treated with percutaneous Kirschner wire assisted reduction and minimally invasive locking plate osteosynthesis (minimally invasive group);47 patients were treated with open reduction with locking plate internal fixation (open plating group).According to the Robinson fracture typing,there were 28 patients with type 2B1 and 21 with type 2B2 in the minimally invasive group;there were 23 patients with type 2B1 and 24 with type 2B2 in open plating group.The length of incision,operation time,visual analog scale (VAS) 2 days after surgery,length of hospital stay,time of fracture healing,Constant score results of postoperative 6 months,1 year,and at the last follow-up,feedbacks on incision and functional satisfaction,and complications were compared between the two groups.Results The average follow-up was (19.6 ± 5.5)months in minimally invasive group and (20.3 ± 6.2)months in open plating group.The incision lengths of the minimally invasive group and the open plating group were (4.8 ±0.7)cm and (8.3 ± 1.6) cm,respectively.The minimally invasive group had significantly longer operation time [(75.5 ±21.6)minutes] compared with open plating group [(60.2 ± 19.1)minutes] (P ＜0.0l).In the minimally invasive group and open plating group,the VAS 2 days after surgery was (2.9 ± 1.5) points and (3.9 ± 1.7) points;the hospitalization time was (7.1 ± 2.6) days and (9.5 ± 2.9) days;the fracture healing time was (11.7 ± 2.2) weeks and (13.4 ± 2.9) weeks;patients' satisfaction with the appearance of the incision was 84％ and 60％,respectively.There were no significant differences in the Constant score and functional satisfaction between the two groups at postoperative 6 months,l year,and the last follow-up (P ＞ 0.05).The incidence of postoperative incisional hypertrophic scar was 8％ and 30％,and the incidence of local skin numbness or sensory loss was 10％ and 32％ in minimally invasive group and open plating group,respectively (P ＜ 0.0l).No significant differences in nonunion,internal fixation failure,incision infection,subcutaneous prominent plate or revision rate were found between the two groups (P ＞ 0.05).Conclusion Percutaneous Kirschner wire assisted reduction with minimally invasive locking plate and open reduction with locking plate internal fixation can both achieve good clinical outcomes in the treatment of Robinson 2B midshaft clavicular fracture;but the former shows more advantages in terms of incision length,incision appearance,operation time,pain 2 days after surgery,fracture healing time,and local skin sensory disturbances.

<p><b>OBJECTIVE</b>To assess the effect of balance acupuncture for migraine without aura.</p><p><b>METHODS</b>Blind evaluation was conducted. Forty patients with migraine without aura were randomized into an observation group and a control group, 19 cases in each one with 1 patient dropped out respectively. In the observation group,, the middle point was used in the hollow before the 1, 2 metatarsal combination, and in the control group, a sham point was applied in the hollow before the 3, 4 metatarsal combination. The manipulation in the two groups was the same. The treatment was given for 4 weeks, once a day, 5 times a week. The comprehensive score and visual analogue scale (VAS) were used before and after treatment, as well as 4 weeks after treatment.</p><p><b>RESULTS</b>The comprehensive score and VAS score after treatment in the observation group decreased after treatment (both<0.05), of which the total headache time and attack number for one month were lower than those before treatment (both<0.05), but at follow-up the VAS score was higher than that after treatment (<0.05). The above indices in the control group were not significantly different from those before treatment (all>0.05). All the indices in the observation group after treatment and at follow-up were lower than those in the control group (all<0.05). The different values for the comprehensive score and VAS score before and after treatment, before treatment and at follow-up in the observation group were better than those in the control group (all<0.05). The different values after treatment and at follow-up had no significant difference between the two groups (both>0.05).</p><p><b>CONCLUSIONS</b>Balance acupuncture atachieves obvious effect for migraine without aura, which can relieve pain.</p>

Objective To explore the treatment of neuropathic pain,the combined use of parecoxib sodium after treatment based ontheuseoffentanyl on transparent paste.Methods In the period from August 2014 to August 2015 by the neuropathic pain patients in our hospital were selected in 38,they were divided intotwogroups,1groupsusingonlyfentanyltransparent paste treatment,2 group based on the use of fentanyl transparent paste on the combined use ofparecoxibsodium,comparingtwotreatmentsoftreatment effect.Results After five days,fifteen days and twenty-five days after treatment,the vas of the patients in the two groups was lower than that before the study,and there was a significant difference between the two groups(P<0.05).There is a big gap between the two groups(P<0.05).Conclusion Fentanyl transparent dressing in treating neuropathic pain can play an ideal therapeutic,for the use of a drug when the poor efficacy of the patient alone combined with parecoxib sodium can significantly improve the therapeutic effect.

This therapy emphasizes infusing qi and extract silt. The treatment continues when pathological product scatters and disappears, so the short-term and long-term efficacy is significant, and especially suitable for the treatment of TCM arthralgia. This article introduced the clinical therapy in treating two rheumatism patients by Yang-palm, explained process and study method of Yang-palm therapy, and shared treatment experience and explored the basic principles of treatment, with a purpose to provide a new treatment for rheumatism.

OBJECTIVE:To compare the efficacy and safety of tamsulosin or nifedipine or racanisodamine in the treatment of lower ureteral calculi. METHODS:160 patients with lower ureteral calculi were randomly divided into group A(40 cases),group B(40 cases),group C(40 cases)and group D(40 cases). All patients drank plenty of water to make daily urine output more than 2 000 ml. Based on it,group A orally received 0.4 mg Tamsulosin hydrochloride sustained release capsule,once a day. Group B orally received 10 mg Nifedipine tablet,3 times a day. Group C orally received 10 mg racanisodamine tablet,3 times a day. Group D received no other drugs except for increasing drinking. The treatment course for all groups was 2 weeks. Clinical efficacy,litha-gogue time,lithagogue size,residual calculi size and the incidence of adverse reactions in all groups were observed. RESULTS:The total effective rate in group A was higher than group C,which was higher than group B and group D,the differences were sta-tistically significant(P0.05). Lithagogue size in group A was more than group C,which was more than group B and group D,the differences were statistically significant(P0.05). Residual cal-culi size in group A was less than group B,which was less than group C and group D,the differences were statistically significant (P<0.05). And there were no severe adverse reactions in all groups. CONCLUSIONS:The efficacy of tamsulosin is superior to nifedipine and racanisodamine in the treatment of lower ureteral calculi,while nifedipine and racanisodamine show similar effica-cy,with better safety.

Objective To investigate curative efficacy of interferon in combination with ganciclovir in treatment of herpes zoster and its effects on post herpetic neuralgia and IL-6, IL-10.Methods 106 patients of herpes zoster who received therapy in Heilongjiang electric power hospital from January 2014 to February 2015 were selected as research objects.Those patients were divided into control group(n=52) and observation group(n=54) by random number table.All patients were given symptomatic treatments, such as analgesia.Besides that, control group was treated with ganciclovir, while observation group was treated with interferon in combination with ganciclovir.Then, the curative efficacy of herpes zoster, incidence of post herpetic neuralgia, pain score of patients with post herpetic neuralgia and levels of inflammatory factors were compared.And during the treatment, adverse reactions were observed.Results After treatment, the total therapeutic efficacy ratio of herpes zoster in observation group and control group was 92.6% and 78.8% respectively with statistical difference(P<0.05).And incidence of post herpetic neuralgia in observation group and control group was 44.4% and 50.0% respectively with no statistical difference.Score of VAS which reflects pain in observation group after the treatment was statistically lower than that in control group in patients with post herpetic neuralgia(P<0.05).As to inflammatory factors, in comparison with control group, level of IL-6 in observation group was statistically lower [(137.6 ±32.4) pg/mL vs(340.5 ±30.6) pg/mL, P<0.05] and level of IL-10 was statistically higher [(179.3 ±22.5)μg/mL vs(150.6 ±24.1)μg/mL, P<0.05].During the treatment, there was no case of severe adverse reaction. Conclusion Interferon in combination with ganciclovir is effective for herpes zoster, which can significantly increase curative efficacy, alleviate pain degree of post herpetic neuralgia with minor adverse reactions .

ObjectiveTo comprehend the similarities and differences of postoperative quality of life between mastectomie avec dissection axillaire and modified radical mastectomy among patients with breast cancer,and in order to supply references for establishing corresponding nursing measures. Methods160patients with early breast cancer were selected,40 patients accepted the mastectomie avec dissection axillaire (the conservative group)and 120 patients accepted modified radical mastectomy (the radical group).The quality of life of patients was surveyed with questionnaires. ResultsThe EORTC QLQ-C30 score of the conservative group was not significant than the radical group.The EORTC QLQ-BR 23 score showed that the conservative group had a better self-evaluation of their configuration. ConclusionsThe patients who accepted the mastectomie avec dissection axillaire will live a better life than those who accepted modified radical mastectomy.
