ABSTRACT

BackgroundGender and age differences in the clinical manifestations of patients with bipolar disorder can affect the clinical diagnosis and treatment process. The current treatment effect of bipolar disorder in adolescents is not ideal, which has become the main reason for disability during the learning period. ObjectiveTo analyze the clinical features and medication therapy status of bipolar disorder in adolescents, and to provide references to support for personalized diagnosis and treatment. MethodsOn January 16, 2023, 1 169 patients with bipolar disorder who were hospitalized at Beijing Anding Hospital Affiliated Capital Medical University from January 1, 2014 to December 31, 2017 were retrospectively enrolled. Medical records were collected and analyzed to compare the clinical features among patients of different gender and age groups, and to explore the differences in medication use among patients of different genders in different types of seizures. ResultsMale patients reported a larger proportion of manic episodes, and a smaller proportion of depressive episodes than female patients (P<0.05).Female patients reported a larger proportion of non-suicidal self-harm behaviors than male patients (χ2=7.761, P<0.01).And patients in low-age group featured a larger proportion of mixed seizures, impulsive behaviors and family history of bipolar disorders along with a smaller proportion of manic episodes than those in high-age group (P<0.05 or 0.01). High-age group had a longer average length of hospital stay than low-age group (t=-2.930, P<0.01). In manic episode patients, males were found to have a larger proportion of valproate and atypical antipsychotic drug administration than females (P<0.01). Among atypical antipsychotic drugs, males accounted for a larger proportion of administration of risperidone and olanzapine (χ2=26.957) than females (P<0.05 or 0.01), while females constituted a larger proportion of administration of quetiapine (χ2=14.865) and aripiprazole than males (P<0.01). In depressive episode patients, females had a larger proportion of administration of olanzapine than males (P<0.01). In patients with mixed seizures, females occupied a larger proportion of administration of lithium carbonate than males (χ2=9.253, P<0.01), and males exhibited a larger proportion of administration of valproate than females (P<0.05). ConclusionDifferences have been shown in diagnostic classification and concomitant symptoms among adolescent bipolar disorder of different genders and ages. Furthermore, medications of lithium carbonate, valproate, atypical antipsychotic and other drugs differ by gender among adolescents of different subtypes of bipolar disorder. ABSTRACT

Objective:To analyze the intervention effect of rehabilitation platform-based online psycho-education on patients with bipolar disorder (BD) in remission stage.Methods:In this randomized controlled study, 91 patients with BD in remission stage who attended the community health center in Xicheng District, Beijing from July to August 2021 were randomly divided into a test group (46 cases) and a control group (45 cases) according to a 1∶1 ratio using the random number table. Baseline data were collected from both groups, and the control group received conventional medication and community telephone follow-up, while the test group was given online mental health education in the form of a WeChat subscription number on this basis, including BD mental health education course push (twice a week) and disease self-management (daily recording of mood, sleep, medication, exercise and gratitude diary), and the intervention period was 6 months in both groups. During the intervention, one patient in the test group was admitted to hospital due to exacerbation of mental illness and the trial was terminated. A total of 90 cases were included in the study. The scores of Medication Adherence Rating Scale (MARS), Hamilton Depression Scale (HAMD), Young Mania Rating Scale (YMRS) and Perceived Devaluation-Discrimination Scale (PDD) were assessed at baseline, after 3 months and 6 months of intervention in both groups, respectively. And the differences in baseline data between the two groups were compared using two independent samples t test and χ2 test, and the repeated-measures ANOVA was used to compare the differences in MARS, HAMD, YMRS, and PDD scores between the two groups before and after the intervention, and to analyze the intervention effects of network mental health education based on the rehabilitation platform on patients in remission stage of BD. Results:After 6 months of intervention, MARS scores in the test group was significantly higher than that in the control group [(8.47±1.75) vs (7.47±1.85)], and was significantly higher than that at baseline (7.36±2.13) and after 3 months of intervention (8.04±1.68) (all P<0.05). YMRS and PDD scores in the test group were significantly lower than those at baseline after 3 and 6 months of intervention [YMRS, 2.0(1.0,4.0),2.0(0,3.0) vs 3.0(1.0,5.5); PDD, (31.18±4.65), (30.13±4.76) vs (32.51±4.51)] (all P<0.05); the differences in YMRS and PDD scores in the control group were not statistically significant (all P>0.05). There was no statistically significant difference in HAMD scores between the two groups before and after the intervention (all P>0.05). Conclusion:Combining mental health education based on rehabilitation platform with conventional medication and community management can significantly improve the medication compliance of patients with BD in remission stage, and improve their manic symptoms and reduce the stigma of the disease.

ABSTRACT

Objective:To compare the biomechanical characteristics and cognitive task performance of the young and the old adults as ascending and descending staircase. Methods:From March to April, 2018, 25 volunteers, including twelve old persons aged (71.54±3.25) and 13 young persons aged (22.24±1.56), were tested with continuous minus seven as sitting (pre-test), walking up and down stairs, walking up and down stairs with continuous minus seven and continuous minus seven as sitting again (post-test). Results:The performance of continuous minus seven was better as walking up stairs and post-test than pre-test in the old persons, and it was better as both up and down stairs, as well as post-test than pre-test in the young persons. As walking up stairs, the main effects of both age (F > 5.037,P < 0.05) and task (F > 6.122,P < 0.05) were significant in walking time, step length, frequency and speed; while walking down stairs, the main effects of both age (F > 17.252,P < 0.01) and task (F > 6.274,P < 0.05) were significant in walking time, step frequency and speed. Conclusion:People would alter the focus on cognitive tasks according to the difficulty of the action. The old adults would be more conservative in the action as upstair during the dual task, and would pay less attention to cognitive task additionally as downstair.

ABSTRACT

@#Objective To explore the biomechanics of different methods of taping on knee during sudden stop action, to prevent knee from joint injury. Methods From March to May, 2017, Eight male college students without lower extremity disorders participated in this study. Their relevant biomechanical parameters were collected when sudden stopping with traditional taping, kinesio taping or no taping of knee, using kinematics, kinetics and electromyogram (EMG). Results Peak horizontal force (F = 3.131, P < 0.01) and horizontal loading rate (F = 2.912, P < 0.05) were the largest, center of pressure moved rearward the most (F = 4.291, P < 0.01), and time of peak horizontal force appearance was the latest (F = 5.144, P < 0.001) with no taping. When peak horizontal force appeared, the flex of knee was the most with no taping (F = 8.141, P < 0.01). The angle of of knee changed the most with kinesio taping during landing (F = 5.491, P < 0.05), while the EMG of rectus femoris and gastrocnemius was the most with no taping (F > 5.322, P < 0.01). The active reset error on 45° and 60° of joint angle was the least with traditional taping (F > 5.631, P < 0.001). Conclusion Traditional and kinesio taping can restrict the forward movement of anterior cruciate ligament, while the kinesio taping is more effective on the maximal knee joint flex, the traditional taping works well for supporting knee, but limiting the range of movement.

ABSTRACT

BACKGROUND: Increasing evidence has shown that high-intensity warming up, including dynamic and static kinetotherapies, can improve the performance of muscle explosive power. However, the training scheme to achieve the best effect remains unclear. OBJECTIVE: To discuss whether the high-intensity dynamic and static kinetotherapies can enhance post-activation potentiation of the upper and lower limbs, so as to fully understand the high-intensity warming up, and thus promote the training efficacy. METHODS: Ovid Medline, Elsevier SDOL, PubMed, and CBJ databases were retrieved for the articles addressing the post-activation potentiation of high-intensity dynamic and static kinetotherapies published before 2016. The articles were classified in accordance with Oxford Centre for Evidence-based Medicine Levels of Evidence. RESULTS AND CONCLUSION: The possible mechanisms of the post-activation potentiation include the phosphorylation of myosin regulation light chain, the increase in motor unit recruitment as well as the change of pinnate angle. The resistance exercise at an appropriate mode, including dynamic, static, and dynamic combined with static, significantly induces post-activation potentiation, and improves the explosive force. Post-activation potentiation is impacted by muscle contraction type, amount of exercise, exercise intensity, induction time, and history of the subjects (training experience, absolute muscle force, and muscle fiber composition). High-intensity stimulus of post-activation potentiation may be accompanied by the generation of fatigue, and the inappropriate recovery time may result in the imperfect performance of post-activation potentiation.

ABSTRACT

BACKGROUND: Downhill running has been found to cause a significant change in the stiffness of the support leg, thus affecting the magnitude and speed of the center of pressure travel. If the stiffness cannot effectively adjust the impact of exercise, sport performance will be weakened and even induce sport injuries. OBJECTIVE: To understand the stiffness to be as the mechanical parameters of human movement, and to assess the mechanisms underlying musculoskeletal system regulating stiffness. METHODS: Medline and CNKI databases were retrieved for relevant articles using the keywords of "landing, spring-mass model, stiffness, human body movement" in English and Chinese, respectively. The stiffness model, stiffness assessment, as well as the role of stiffness during human movement and its influencing factors were reviewed, thereby revealing the significance of stiffness during human movement. RESULTS AND CONCLUSION: When the human body interacts with the ground, the visual information is different, and the neuromuscular system generates coordinated control to arrange appropriate muscle activity of the lower extremities, such as stiffness regulation. The movement settings, working condition, and individual factors all make effects on the stiffness during human movement, and the stiffness efficacy can be controlled by changing the dynamic and kinematic parameters during movement. Appropriate adjustment of the stiffness can promote the human movement performance, and avoid or reduce sport injuries. Running and jumping are the indispensable movements, and taking off is the key step during jumping action, and the stiffness control during taking off action combined with sports skills can improve the sport performance. These results indicate that the vertical stiffness and leg stiffness will be changed with the human actions, accompanied with changes in the stiffness of hip, knee, and ankle joints. In addition, the spring-mass model is helpful for understanding the movement performance and preventing exercise injuries.

BACKGROUND:Kinesio taping is commonly used,but there is a lack of rigorous and scientific research,especially in biomechanics and exercise.OBJECTIVE:To overview the biomechanics characteristics of the ankle joint after treatment with kinesio taping,and to summarize the biomechanics difference in kinematics,kinetics,and electromyography of the ankle joint between non-kinesio and kinesio taping.METHODS:Ovid Medline,Elsevier SDOL,PubMed and CNKI databases were retrieved for the Chinese and English literatures concerning the biomechanics and exercise performance of kinesio taping published before December 2015.Subsequently,all literatures were classified according to the Oxford Centre for Evidence-based Medicine Levels of Evidence.RESULTS AND CONCLUSION:Most of literatures support the viewpoint that kinesio taping can improve ankle joint stability,increase the arch height and decrease the external load of key muscles of the ankles and feet.Some studies have suggested that during landing,kinesio taping shortened the duration of the peak of support reaction forces,reduced the flexion force,and increased the loading rate of lower extremities,thereby resulting in an increased risk for injury.Other studies have shown that kinesio taping increases the recruitment of motor units and changes the order of neuromuscular contraction,but it cannot improve the athletic performance,and even decrease the athletic performance in most cases.Whether kinesio taping improves muscular force,increases the muscular performance,and increases the power of lower extremities is under discussion;hence,in-depth studies are needed in the future.

ABSTRACT

Objective To study the differences in retinoic acid receptor γ(RARγ) mRNA expression levels in blood leukocytes between antipsychotic-free and antipsychotic-naive schizophrenic patients and healthy control,especially in different genders. Methods Forty-three acute schizophrenic patients who were antipsychotic-naive or antipsychotic-free for at least three months (male = 34, female = 9) and 39 age- and sex-matched healthy subjects (male =25 ,female = 14) were included for blood leukocytes expression of RAR γ mRNA ,using real-time PCR technique. Results Kolmogorov-Smirnov Z analysis showed a significant increase of RARγ mRNA (P =0.041) level in blood leukocytes of pooled schizophrenic patients(0. 027 ± 0. 003) than in the healthy subjects (0. 020 ± 0.002). The elevation was mainly found in the female patients (0. 030 ± 0.003). Within-sex analysis showed that the female schizophrenic patients showed a trend increase (P = 0. 166) of RAR γmRNA expression compared with the male patients (0. 026 ± 0. 001) and exhibited greater RARγ mRNA expression (P = 0. 014)when compared with the female healthy subjects(0. 018 ±0.004). Conclusions The present findings showed an abnormal expression of leukocyte RARγ mRNA level in antipsychotic-free and antipsychotic-naive schizophrenia especially in the female patients. Blood RARγ markers could add to the diagnosis and individualized pharmacotherapy of schizophrenic patients ,especially the female patients.
