ABSTRACT

Cubitus varus deformity following elbow fracture in children rarely limits the function of the upper extremity, but the patients and/or their parents often request an operation for the aesthetic purpose. Various surgicsl techniques have been proposed to correct the deformity, however, the complications following operation are still common. We critically reviewed 55 patients with cubitus varus deformity treated by supracondylar closing osteotomy at Seoul National University Hospital between January, 1980 and April, 1989, and report on 25 patients who had at least one year follow-up (average 4.0 years). The results were graded as excellent (13 cases), good (8 cases), and poor (4 cases). The average amount of correction was 29.1 degrees by humero-elbow-wrist angle. There were two cases with ulnar nerve palsy and one case with radial nerve palsy, however, all of which were spontaneously recovered with time. The factors related to poor results were age at operation, degree of deformity, improper wedge size, and nerve palsy. In this article, we also illustrate our new, simple and safe technique of supracondylar closing spring (incomplete) osteotomy based on careful preoperative planning and special attention to surgical details.

ABSTRACT

Six hundred and fifty one cases of bone tumors diagnosed and operated at Department of Orthopedic Surgery, Seoul National University Hospital, from Jan., 1969 to Apr., 1989. were reviewed and analyzed with respect to the types of tumor, the age and sex distribution of the patients, the location of the lesions and the advances of diagnosis and treatment. 378 benign and 273 malignant bone tumors were identified. Osteochondroma(113 cases, 17.4%) was the most common bone tumor, followed by osteosarcoma(72 cases, 11.1%) and giant cell tumor(50 cases, 7.7%) Second decade was the most common age of bone tumors(216 cases, 33.2%) and male(379 cases, 58%) was more common than female. Femur(222 cases) was the most common site of bone tumors, followed by tibia and vertebra. For simple bone cyst, steroid injection was widely used recently. It was a simple and safe procedure and its results were similar to those of curettage and bone graft. Limb salvage operations were performed for malignant bone tumors increasingly. Neoadjuvant chemotyeraphy and limb salvsge operation using tumor prosthesis were performed in 5 patients of osteosarcoma and their results are so far promising. Of 7 cases of chondrosarcoma, the limb was salvaged by resection arthrodesis or resection arthroplasty. 7 cases of malignant fibrous histiocytoma were treated by radical ablative surgery and/or chemotherapy and radiation, but local recurrence or pulmonary metastasis were detected in all of them. So it was thought to be highly malignant bone tumors.

ABSTRACT

Limb salvage operation was introduced to treat, low grade malignant bone tumors about 100 years ago. However, amputation has been the most popular choice for primary malignant bone tumors. Recent improvement of chemotherapy, diagnostic methods and surgical techniques enabled us to treat more malignant bone tumors such as osteogenic sarcoma with limb salvage operation. 6 cases of malignant bone tumors were treated with wide resection and tumor prosthesis from Oct., 1987 to Jan., 1989 at Dept. of Orthopedic Surgery, Seoul National University Hospital. 4 cases of osteosarcoma, 1 case of parosteal osteosarcoma and 1 case of chondrosarcoma were identified. MRI was useful for determining the extent of tumor and surgical planning. All the patients osteosarcoma had neoadjuvant chemotherapy. The mean follow-up was 14.5 months ranging from 8 to 22 months, and theirresults are so far good except one pulmonary metastasis and one local recurrence. We present this as a preliminary report.
