ABSTRACT

PURPOSE: This study aims to explore the perceived effectiveness, benefits, and learning experiences of interns with regard to their participation in a communication skills workshop program, which can have implications for the development and implementation of future communication skills training. METHODS: One hundred twenty one interns and 12 senior residents and fellows participated in the workshop program as learners and tutors. The participating interns encountered three difficult communication situations, represented by trained standardized patients. During each encounter, each participant had an individual encounter, group discussion, and feedback on his performance. A self-assessment survey, composed of five itemized questions and seven open-ended questions, was administered after the workshop. A mixed methods approach was used to analyze the quantitative and qualitative data. RESULTS: The participants had positive perceptions of the implementation and effects of the workshop, and they responded higher than 4.0 to all itemized questions on their perception of the effectiveness and benefits of the workshop. The analysis of open-ended questions demonstrated specific learning experiences of the participants, such as the hardship of solving ill-structured communication problems and reflection on their current knowledge, skills, attitudes, and practice as physicians. The participants reported that the workshop provided opportunities of improving diverse communication skills and problem-solving skills and identifying further learning needs. CONCLUSION: The results suggest that communication skills training for interns facilitates their reflection and development of communication skills, as well as their competency of situated problem solving. Consequently, communication skills development should be regarded as an important subject of continuing medical education. Several implications of this study can contribute to the design and development of communication skills-related programs.

ABSTRACT

PURPOSE: Recently, dental implant systems have been widely used for the treatment of the extraction site, but we have been confronted with many limitations in esthetics, phonetics and function. Transitional implants(TI) were developed as a method of providing fixed provisional restorations during conventional implant healing. Until now, little data have been provided on korean transitional implants. The purpose of this study is to evaluate the implant placement site histologically after 4 weeks and 8 weeks. MATERIALS AND METHODS: Test group( IntermetzzoTM MEGAGEN, KOREA) and control group(Mini Drive Lock, Intra Rock, U.S.A.) were immediately placed in interseptal or interproximal bone of beagle dog after mandibular premolars extraction, and had a healing period with non-submerged state but without loading, Both TI surfaces were composed of rough surfaces. RESULTS: In the test group, the average percentage of BIC were respectively 39.40%(SD7.35) after 4 weeks and 44.05%(16.76) after 8 weeks, and In the control group were 50.75%(1.48) and 59.40%(0.00). DISCUSSION: We evaluated the initial ability of the osseointegration of TI through this study. Because TI is placed with a conventional implant simultaneously and loaded immediately, the ability of osseointegration is a very important factor for the success of TI during the initial healing phase. CONCLUSION: The results of the histological evaluation of these two groups were similar to those mentioned in other studies for osseointegration of implant.

ABSTRACT

This study was designed to compare the effects of treatment using chitosan membrane (Nanogide-C(R)) resorbable barrier with control treated by polylactic acid/polylacticglycolic acid membrane(PLA/PLGA membrane, Biomesh(R)). 44 furcation defecs from 44 patients with class 2 furcation degree were used for this study, 22 sites of them were treated by chitosan membrane as experimental group and 22 site were treated by PLA/PLGA membrane as control group. Clinical parameters including probing depth, gingival recession, attachment level and radiographic examination were evlauated at base line, 1 month, 2 month and 3 month. after surgery. Statistical test used to analyze these data included paired t-test, one way ANOVA. The results are as follows : 1. Probing depth was significanlly decreased in the two group and there were significant differences between groups(p<0.05). 2. Gingival recession was not significanlly increased in the two group and there were no significant differences between groups(p<0.05). 3. Loss of attachment was statistically decreased in the two group and there were no significant differences between groups(p<0.05). 4. Horizontal bone level was significanlly increased in the two group and there were significant differences between groups(p<0.05). On the basis of these results, chitoans resorbable membrane has similar potential to PLA/PLGA membrane in GTR for furcation defect.

ABSTRACT

STATEMENT OF PROBLEM: Stress concentration on the neck bone affects the bone resorption, and finally the implant survival. PURPOSE: In order to examine the stress distribution on the neck bone and prosthesis abutment for implants, decreasing abutment sizes were used. MATERIAL AND METHODS: Axisymmetric models were used to obtain the data required. These models were composed of 4mm implants with 3.4mm and 4mm abutments, 5mm implants with 3.4mm and 5mm abutments and 6mm implants with 3.4mm and 6mm abutments. All abutments were designed to received a 10mm high by 10mm diameter gold crown. Functional element analysis was used to obtain these results using data that consisted of 50 N vertical and 45 degree inclination forces. RESULTS: 1. Changing the diameter of the abutment on the implant affects the effect of the inclination forces more than the effect of the vertical forces. 2. Changing the diameter of the abutment on the implant affects the effect of the inclination forces more than the effect of the vertical forces. 3. Experimentation showed that the larger diameter implants provided a decreased neck bone stress, whereas a larger diameter abutment provided a decrease marginal abutment stress. 4. Experimentation showed that the neck bone and abutment received more stress from inclination forces than vertical forces. CONCLUSIONS: By decreasing the size of the abutment on the implant we were able to diminish neck bone stress.

ABSTRACT

Recently, some cases of paradoxical bronchoconsthction with the use of nebulized antiasthmatic respirator solutions have been reported. This study was performed to determine whether benzalkonium chloride, contained in several antiasthmatic respirator solutions as a preservative, may cause paradoxical bronchoconstriction. This was accomplished by comparing the FEV(1) change after Ventolin (benzalkonium containing salbutamol) nebulization with the FEV(1) change after Respolin autohaler (benzalkonium free sa1butamol) inhalation within the same patient. This study consisted thirty patients with stable asthma who regularly attend the allergy clinic at Wallace Memorial Baptist Hospital. Patients were excluded from the study if they had respiratory disease or were taking any other medications. The results are as knows: 1) The baseline FEV(1) in the Ventolin group was 73+/-1O(% predicted) and 74+/-10(% predicted) in the Respolin autohaler group, showing no statistical difference in baseline FEV(1) between the two groups. 2) FEV(1) was decreased in 9 subjects(3096) after Ventolin nebu1ization and in 1 subject(3396) after Respolin autohaler inhalation, showing that the incidence of FEV(1) fall was higher in the Vento1in group than in the Respo1in autohaler group(p<0.OO5, one-tailed t-test). 3) Considering the cases with increased FEV(1) after inhalation, FEVl was increased by 9+/-7 (% predicted) in the Vento1in group and 7+/-5(% predicted) in the Respolin autohaIer group, suggesting that bronchodi1ator effects of both medications are similar. 4) Mild coughing occurred in 4 subjects(13396), and mild chest tightness in 1 subject(33%) in the Ventolin group. These symptoms resolved without any treatment. No side effects were reported by subjects in the Respolin autohaler group. In conclusion, these results strongly suggest that the paradoxical bron-choconstriction occurring with nebulized antiasthmatic respirator solutions is due to the ingredient benzalkonium chloride. Therefore we recommend that a benzalkoniulm free salbutamol product should be used for asthmatic patients, or if a product such as Ventolin which contains benza1konium chloride must be used, we recommend that it should be administered cautiously.
