ABSTRACT

PURPOSE: Split-thickness skin grafts (STSG), as a treatment of full thickness burn have played a significant role in re-surfacing to date. The major disadvantage of traditional STSG is related to donor site morbidity, including scar formation and cosmetic changes. SureDerm(TM) is acellular human dermis, which is intended for the repair or replacement of damaged soft tissue. Then, we present our experience of using SureDerm(TM) as a tool for the skin graft of full thickness burns. METHODS: We reviewed the medical records of 20 patients treated in our burn center who received SureDerm(TM) graft with thin STSG in full thickness burns since November 2006 to October 2008. RESULTS: SureDerm(TM) was used with thin STSG (range 0.006~0.008 inches) concurrently. Thickness of SureDerm(TM) was 0.2~0.4 mm and the type of SureDerm(TM) was meshed. The average size of SureDerm(TM) used in the burn patients was 329.6 cm2 (32~1,384). All burn areas grafted SureDerm(TM) were full thickness burns and the locations were upper and lower extremities including joints (8 and 6 cases), trunk (3 cases), ankle (2 cases), and axilla (1 case). Each SureDerm(TM) grafted area had more than 95% take-rate. No complications were observed except 1 case of partially infected STSG. The mean follow up period was 8.7 months (1~17), and the assessment of scars, which had more than six months follow up periods was performed by Modified Vancouver Scar Scale and the results were good. CONCLUSION: SureDerm(TM) can be used as a dermal substitute for the treatment of full thickness burns and the result seems to be good cosmetically and functionally while it solves donor site morbidity followed by autograft.

ABSTRACT

The patient, 23 year old female, with the Zosteriform Porokeratosis on right side of trunk and extrimity and a superimposed Disseminated Superficial Actinic Porokeratosis on face, was treated at Dept. of Dermatology, Catholic Medical College. The lesions appeared at the age of 7. Her mother had similar lesions on the face and extrimities. 3% 5-Fluorouracil in Vanishing cream base was applied topically under occlusive dressing for 40 days, and the dressing was changed every 24 hours. On seventh day erythema and papular lesion appeared, and oral administration of steroid and intermittent topical betamethasone dressing was enable to continued the occlusive dressing. One year after the treatment the lesion did not recurred and shallow atropic scar were left with normal skin color.

ABSTRACT

This survey was to elucidate the bacterial clearance time (BCT) among the patient of L-type and B-group leprosy who had been und.er chernotherapy and foll- ow up with bacterial examination 4 times every year more than 3 years at the Chronic Disease Laboratory of Catholic Medical College. The 99 patients, 53 L-type and 46 B-group, were slected for this study. These patients were classified again into 2 groups: 80 patients with negative bacterial index and 19 patient of posit.ive bacterial index. The patient with negative bacterial index was subdivided into 3 groups by BCT. 1. Rapid decrease group (RA group) BCT<(4years) 2. Standard decrease group (ST group) 4BCT<7 R. Slow decrease group (SL group) 7.

ABSTRACT

The 16 out of 19 cases with combined oral administration oral of Descrespet and Tetracycline hydrochloride were considerably more effective in treatment of severe acne than Tetracycline hydrochloride alone or placebo in this study. 10 cases of atopic dermatitis and 6 severe acne with side effect for tetracycline hydrochlorid, such as aMominal discomfortness, also treated with Descresapt: 7 cases of atopic dermatitis and 4 acne cleared definitely faster than in patient not given Descresept Conclusively, it is cnosidered that the Descresept is effective in the tretment of severe acne and atopic dermatitis.

ABSTRACT

The clinical and biological study for Sarcoptes scabiei from 66 scabies patients was performed form Nov. 1974 to April 1975. The diagnosis of these scabies patients were confirmed by method proposed by Muller et al. The results were as follows; 1) Male to female ratio is 5. 6: 1 in total patients and predominant age group is in 16~20 in both sexes(Table 1). 2) 50% of cases had family history of similar troubles and the duration of subjective symptom complaind at their first visit was 1~2 weeks in 20 cases, 3~4 weeks in 25, 2 months in 10, and over 3 months and unknown were 12. 3) Suspected sources of infestation were, 22 cases from one of their family members, 15 at hotel, 7 at boarding house, 2 at camping, 1 at military camp, 19 were unknown. 4) Distribution of burrows was hand(79.4%), wrist(12.4%), foot(3.1%), penis(2.6%) scrotum(2.1%) and axillary area(0.5%). Number of burrows was 1~2 in 61.4%, 3~4 in 19.3%, 5~6 in 5.3%, 7~8 in 1.8%, 9~10 in 5.3% and 10 in 7. 6%. 5) 44 patients (66.7%) were confirmed by finding of female mite and among them 50% had 1 female mite, 36.7% had 2, 6.8% had 3, 2.3% had 4 and 4.5% had 6. 6) 69 among total 79 female mites detected were found at burrow, 6 from papule and 4 from vesicle. Total 7 male mites were found from patients and 4 from papules and 2 from veside and 1 from pustule(Table 5) 7) From total 194 burrows examined 69 female mites (36.1%) were found and non of male mite were found. 8) Mean size of female mite(length*breadth) was 359.3u*266.5u, male mite 21.1u*160.9u, larva 131.8ux94.2u, egg with developing larva 167.3*93.6u, egg with developed larva 159.7u * 92.3u and scybala 18.7u x 12.3u. Sizes of egg with developing larva and egg with developed larva had no significant difference, statistically.

ABSTRACT

A case of familiar benign chronic pemphigus is reported. A 41 years old housewife has recurrent history of developing grouped vesicular eruptions with erythematous erosion on the intertrigenous area since 29 years of her age. The lesions were used to aggrevated in summer and improved in autumn. Familiar history revealed same disease of her grand mother, father, 44 year old elder sister and 39 year old younger sister. All of whom had similar clinical signs and symptoms. Routine physical and laboratoryfindings were within normal limits. The histopathological findings, from vesicular eruption of the lateral aspect of neck, showed multilocular suprabasal clefts with acantholsis, villi formation and hyperkeratosis. The lesions were-improved hy the local application of corticosteroidoid ointment only.

ABSTRACT

A case of hereditary anhidrotic ectodermal dysplasia associated with atopic dermatitis in 16 month old male is reported. He has been suffered from multiple symptoms of anhidrosis, hypotrichosis, anodontia and atopic dermatitis. The skin showed pale, shiny, thin and dry and hairless on whole body except in the scalp. Without sweat gland and flattening of epidermis on skin biopey is compatible with an hereditary anhidrotic ectodermal dysplasia.

ABSTRACT

Two cases of hyperlipoproteinemia is reported, Hyperlipoproteinemia is characterized by various kinds of cutaneous xanthomatosis associated with or without arteriosclerotic changes due to increasing lipoprotein fraction of the serum. Case I . 37-year old housewife had multiple, pea to nut sized, various shaped, cutaneous and subcutaneous nodular masses has been developed last 3 years. Some of her family member have similar symptoms. Serum cholesterol level was 790 mg% and serum triglyceride level was 270 mg% after 14 hour fasting. Case II. 52-year old housewife was suffered from pea sized, multiple, subcutaneous nodular and cutaneous eruptions last 2 years. There was no family histories. Serum cholesterol level was 895 mg% and serum triglyceride level was 350 mg% after 14 hour fasting. Typical Touton giant cells were found in biopsy specimens taken from cutaneous lesons. ECG, X-ray, glucose toleranc test and other laboratory examinations showed within normal limits. Tne agarose electrophoresis pattern of serum lipoprotein fractions are very similiar with that of hyperlipoproteinemia type II. These two patients were treated with nicotinic and 1. 0 gm and clofibrate 2.0 gm daily by mouth and restricted animal fatty foods. Total serum cholesterol level was markedly decreasea 2 months after treatment in case I.
