ABSTRACT

The hereditary multiple osteochondromatosis is a hereditary disorder characterized by gradual development of numerous osteocartilagenous masses from the metaphyseal region of long bones. The abnormality is transmitted as an autosomal dominant trait and its etiology is unknown but many theories of pathogenesis have been advanced. Four members of a family with hereditary multiple osteochondromatosis who are much shorter in height are presented with a brief review of literatures.

ABSTRACT

The extensive traumatic wound has posed a serious problem to the surgeon. Various drugs and dressing methods have been used for this kind of wound but there has been no ideal method. The pigskin xenograft was used initially for the treatment of burn wound. The salutary effect of porcine xenograft are the stimulation of growth of healthy granulation tissue, as well as epithelial borders, inhibition of bacterial growth and pain relief. We have performed porcine xenograft in 20 cases having extensive traumatic wound with skin defect from January 1981 to February 1982. The results obtained are as follows: 1. Protect large open wound until autograft are available. 2. Alleviate pain in the wound. 3. Promote granulation tissue growth which is needed for early autograft. 4. Protect the exposed wound areas (bone and tendon) from infection. 5. The healing time of the wound depended upon the severity and the state of underlying tissue in the wound rather than wound size.

ABSTRACT

Twenty-six cases of fracture of the femur and tibia on the same leg in twenty-five patients were treated at the Dept. of Orthopedic Surgery, Kyungpook National University Hospital during the period of 1973 to 1979. Twenty-two patients were sustained by the motor vehicle accident. Open fractures of the femur and tibia were ten cases, and closed femur fracture and open tibia fracture were twelve cases. Concomitant Injuries were brain Injury in eight patients and hemorrhagic shock In seven patients. Eight patients were treated by internal fixation (Kuntschernall or Compression plate) on femur and by conservative treatment on tibia. Conservative treatment was done in eleven patients on both femur and tibia fracture. Five patients were amputated. Average healing time of fracture was: twenty-two weeks in femur and twenty-seven weeks in tibia. Functional end results were assessed and rated with satisfactory results In six patients and fair and poor in elght patients respectively.

ABSTRACT

In the past 13 years period, authors experienced 14 cases of giant cell tumor, 3 of which were treated by bone cementing after curettage. The results of various methods of treatment applied were compared and literatures surveyed. The followings were noted: 1. The most efficient method of treatment for giant cell tumor is en bloc resection. 2. The method of bone graft after curettage has such disadvantages as high recurrence rate and sequelae due to long periods of immoblization, especially for the large lesion with severe bone destruction. 3. Bone cementing after curettage seemed to be a choice in the methods of primary treatment of long bone giant cell tumor, especially useful for the tumors near the knee joint; the main advantages over other methods of treatment were considered to be technical simplicity and lack of complications.

ABSTRACT

Authors have treated a case of glomus tumor which was occurred in the tip of the right thumb, 34 years old female. The patient has complained of pain, tenderness, and cold sensitivity on the lateral side of the tip of the right thumb for 10 years. Reddish purple discolorarion was noticed under the nail bed. On X-ray, erosion of the lateral cortex of the distal phalanx and increased soft tissue density around the erosion was noted. Grossly, the tumor was purple in color, more deeply colored than the surrounding tissue and tumor tissue will shell out of the surrounding bone and soft tissue. Glomus tumor was confirmed by the excisional biopsy.

ABSTRACT

Authors had analyzed 35 patients of traumatic posterior dislocation of the hip which was treated at Kyungpook National University Hospital during the period of 7 years from 1972 to 1978. 21 of the 35 dislocation of the hip had sustained the injury by automobile accidents. 33 patients were reduced by closed reduction under general anesthesia. Within one week following closed reduction, intra-articular bony fragments were removed in 4 patients and posterior fragments of acetabular fractures were reduced and fixed with screws in 8 patients. 17 of the 35 patients were followed for eight months to 91 months, average 38 months. The results were rated according to criteris of the Stewart and Milford. Satisfactory results were obtained in 11 patients and those of 9 patients were reduced within 12 hours after injury. As the complication, avascular necrosis, myositis ossificans and osteroarthritis were developed in two patients, four patients, and seven patients respectively.

ABSTRACT

Belt injury is an increasingly serious injury and one that frequently results in undesirable disabilities. We reviewed 20 cases of belt injury which had been treated in Kyungpook National University Hospital during the five and a half year period from Auguat, 1972 to January, 1978. 1. This injury is more prevalent in males (M:F = 4:1) with the peak incidence between the second and fourth decades (85%). 2. Upper extremities are involved in almost all cases (95%). 3. ABSTRACT

Tuberculous tenosynovitis affecting flexor tendon sheaths of the hand is a rare condition and quite disabling to the patient like tuberculosis elsewhere. It should be attacked by operation and antituberculous chemotherapy, but has a tendency to recur after surgery. The authors have encountered a case of tuberculous tenosynovitis affecting flexor tendon sheaths in the palmar area of the left hand, and treated the case with surgery and anti-tuberculous chemotherapy.

ABSTRACT

One hundred and fifty two open wounds of extremities including open fractures were treated at the Kyoungpook Nstional Univ. Hospital from March 1977 to August 1977. Of 152 open wounds there were 32 cases (21.5%) of wound infection even though the initial wound culture revealed bacterial growth in 44 cases(28.9%). However, there were only 10 cases of infection among the cases with positive bacterial growth in the original culture and the same organisms were found in the 6 cases between the initial wound culture and post infection culture. Sensitivity studies of the organisms, the Cephalosqorines appeared to be most effective as a prophylactic antibiotics. It was sensitive to all gram positive organisms and majority of gram negative organisms including Escherichia coli. In bacterial culture and sensitivity studies of the 82 open fracture wounds 18 of them showed gram positive and gramnegative bacteria. The prominant organism were Staphylococcus, Escherichia coli, Streptococcus and Pseudomonas. The infection rate was lower in the cases whar the wound was managed within six hours from the time of injury than after six hours.

ABSTRACT

A lymphangioma, which is analogous in some ways to hemangioma due to a common origin, develop and grow in identical fashion and show certain common features in response to treatment, is a benign tumor composed of lymphatic tissue. But lymphangiomas are much less common than hemangiomas in frequency and most of these lesions are usually present at birth. Most of these lesions are found in the neck, few are located in the axilla and the remainders are scattered throughout the body. Soft tissue hyertrophy of the thenar eminance and proximal portion of index finger of left hand due to congenital lymphangioma, 7-months male baby, treated by surgical resection is presented with a review of the literature.

ABSTRACT

The purpose of this study was to analyse th results of closed, and open reductions of displaced proximal humeral fractures, and to evaluate the technical aspects of thtreatment of the ese fractures. Forty six cases of these fractures, followed for from six months to five years, were presented. 1) The age of the patients ranged from thirty to forty years, and the distribution was almost equal. 2) The majority of these fractures were caused by traffic accidents. 3) The fractures were classified according to Neers method. The majority of the cases were 2-part fratsures, rating 52%, while 4-part fractures rated 2. 3%.4) The cases with 1-part and 2-part fractures were generally treated by conservative methods and open reductions were performed only in cases wilh 3-part or 4-part fractures and in a few unsuccessful closed reduction cases of 2-part fracture. 5) The results of these patients were rated by the Neers numerical system. Of forty six cases, thirty eight had exellent results, five, good results, and three, poor results.

ABSTRACT

Overgrowth and hypertrophy of one or more extremities is an infrequentIy encountered congenital or acguired anomaly, It is referred to in the literature by different authors as local gigantism, hypertrophy etc. The etiology remains unexplained and the condition is only seldom found to be familiar. Hypertrophy of left lower extremity associated with neurofibromatosis of the sural nerve in the calf, in a 8 years old school boy, is presented with a review of the literature. Chief complaints were enlargement of left lower leg and limping gait since 5 years old. There was no change in sensory and motor function of the involved leg. X-ray films showed enlargement of tibia and fibula and diffuse hypertrophied soft tissue shadow in the left leg. One case of local excision of hypertrophied sural nerve and subcutaneous fat tissues was presented.

ABSTRACT

The steadily increasing frequency of car accidents and industrial injuries have contributed to increasing numbers of injuries to the femoral shaft. Fractures of the femoral shaft have always created camplicated problems associated with long periods of immobilization, resultant functional disability and economic loss. The methods of treatment are variable, therefore it is important to choose the correct form of treatment. The authors have reviewed 67 fractures of the femoral shaft in 66 patients for the 5 years period from Jan., 1972 to Dec., 1976, who were treated in the department of Orthopedic Surgery, Kyoungpook National University Hospital. The results were as follows; 1. Most of fractures occured in person who are in their third and fourth decades. Male are predominant. 2, Eliciting causes were mainly traumatic and most of them were car accidents. 3. Middle third of the femur was most prevalent location of the fracture site, and comminuted type was most frequent in the fracture type. 4. The period of immobilization for femoral shaft fracture was 7 weeks for those treated with compression plate and Kuntscher nailing; 11. 7 weeks with conventional plate and screw fixation and 17. 8 weeks with skeletal traction. 5. Postoperative infection rate was 3% and infection was treated by conservative methods.

ABSTRACT

Flexor tendon injuries have long been considered one of the Problem areas for hand surgeons because of occasional unsatisfactory results of treatment. The authors reviewed 111 cases of flexor tendon injuries which were treated by primary repair in the Department of Orthopedic Surgery, Kyungpook National University Hospital for about 6 and a half years from Jan. 1977 to Aug. 1977. The results obtained were as follows; 1. Sex ratio between male and female was 4. 3: 1. 2. Age distribution showed the most frequent occurrence between 21 and 30 (38.7%). 3. Most of the patients were injured by glass (67.5%) and at the wrist level or proximal to it. 4. Average follow up period was 6 months and over-all end result was satisfactory in 83.9% by the Klienert criteria. 5. 9 cases of the 10 flexor pollicis longus only injured showed satisfactory result by the arbitrary criteria based on active flexion range of thumb i-p joint and functional difficulties in the daily living.

ABSTRACT

In 1941, Jaffe, Lichtenstein and Sutro collected a group of similar but variously named synovial lesion of joints, bursae and tendon sheaths and gave them the descriptive title “pigmented villonodular synovitis, bursitis and tenosynovitis”. Since then, numerous reports have appeared which further defined the entity or added series of cases in sufficient number to indicate that the lesion is not uncommon. We have experienced a rare case of pigmented villonodular bursitis on bursa anserina. The patient, who were 36 year old, female, has been complained of painful soft swelling on bursa anserina area of right knee for one year. The excisional biopsy on the affected bursa confirmed the diagnosis.

ABSTRACT

The Knee joint is one of most susceptible joints to ligamentous injuries and its incidence has tendency to increase in frequency year by year. In 1938, Palmers monograph pointed out the essential pathological features of acute ligamentous injuries to the knee, and, since then, treatment of these conditions has undergone evolution from an almost completely conservative approach to one in which it is considered essential to undertake primary repair of major ruptures within 10 to 14 days at least. The aim of surgical apporoach is to restore the ligaments to their previous anatomic position and tension. Many authors have made significant contributions to anatomy, mechanism of injury, a method of diagnosis and surgical repair and rehabilitation. The authors have reported a clinical study in 33 patients with ligamentous injuries of medial collateral ligament of the knee, who were treated by surgical repair during the past 5 years from Jan. 1971 to Dec. 1975. and results are as follows. 1. Peak of age incidence was in 3rd decade, and male was more predominant. 2. Traffic accident was most common cause. 3. Among 33 cases of ligamentous injuries of medial side, rupture of medial collateral lig. alone was 12 cases, and combined injury with cruciate ligament or meniscus were 21 cases. 4. As to site of rupture of medial collateral lig. femoral attchment was most common as 14 cases. 5. The results of those cases of which surgical repair have been performed within 2 weeks after injury were better than those cases of delayed surgical repair.

ABSTRACT

Osteopetrosis is a very rare bone dysplasia characterized by failure of resorption and persistence of calcified chondroid and primitive bone. We have experienced: a familial osteopetrosis, father and three siblings out of seven, which was thought to be a autosonal dominant inheritance. One of those family, 18 years old body, has been complained of low back pain without radiating pain to the lower extremities. No other clinical symptoms has manifested. The other 3 patients has not been complained of any clinical symptom, but X-Ray examination of the entire skeletal survey of those members of the family have revealed the uniform opacity of long bones and flat bones. The normal architecture was altered, with no distinction between cortical and cancellous bone in all long bones. Histologic picture have noted the irregular patches of immature chondro-osseous tissue embedded in the matrix of coarse fiber bone with a wide and prominent cement line.
