ABSTRACT

PURPOSE: This study aimed to investigate the safe medication utilization and the education demands during pregnancy and it further reported the development of educational materials for pregnant women. METHODS: A survey was conducted in two tertiary care university hospitals and one community hospital specialized in obstetrics and gynecology from July 2 to 29 2014. The survey questionnaires included the usage of medications and the unmet needs on medication use during pregnancy. Ad ditionally, pregnant women's requests regarding to medication use were collected through group interviewing of community pharmacists. Based on these results, educational materials were de veloped and implemented. After implementing the education sessions, the satisfaction was evaluated. RESULTS: A total of 152 pregnant women answered completely the questionnaire. Among them, 130 participants (42.8%) were given the medication information from their physicians. Exposure to a teratogenic drug during pregnancy was a major concern for most pregnant women (79.6%). The majority (90.1%) of subjects reported a necessity of medication-related education during their pregnancy. The interview with 48 pharmacists indicated that the most commonly used OTC drugs in pregnancy were vitamins (25.2%), iron supplements (23.7%), and the most frequently used prescription drugs were antiinflammatory-analgesics (25.3%), followed by antibiotics (20.9 %). Based on the results, booklets of drug therapy during pregnancy, leaflet of pregnancy category index were made. Also, on-line flash and presentation materials for instructors were prepared. Through the trial education with the developed materials, it was confirmed that the contents of education materials were well understood and satisfied by the pregnant women. CONCLUSIONS: This study showed the need of the medication-related education for the pregnant women. The developed education materials would be helpful sources to provide accurate and reliable medication-related information to health professionals and pregnant women.

ABSTRACT

PURPOSE: This study was performed to assess how a fetal diagnosis of congenital heart disease affects parents, as regards pregnancy management and care of infants after birth. METHODS: Database search to find out abnormal fetal echocardiography performed at Seoul National University Children's Hospital from July 1988 to June 2003 revealed 370 examinations. After excluding both arrhythmias without structural cardiac disease and multiple pregnancies, 299 pregnancies remained and this data formed the basis of this analysis. We retrospectively reviewed the medical records with special attention to pregnancy outcomes and also tried to find out factors influencing parental decisions on whether to continue or terminate pregnancy. RESULTS: In this study, the mean gestation age at diagnosis was 28+/-6.0 weeks. The mean age of mothers was 30+/-3.9 years old. Younger gestational ages at diagnosis(P=0.000), more severe grades of fetal heart disease(P=0.002) and younger mothers(P=0.014) correlated with terminations of pregnanies. But the grades of fetal status, the grades of associated anomaly, whether in-vitro-fertilization was carried out or not and numbers of previous children were not significant. CONCLUSION: This study found that the earlier gestational ages at diagnosis, younger maternal age and higher grades of fetal heart disease tended to lead parent to select abortions. Fetal echocardiographies were performed too late. Moreover Koreans have a biased view that malformation is a something incurable and a tragedy not only to oneself, but also to a family. So parents select terminations of pregnancy, even in curable cases. This is very unethical.

ABSTRACT

PURPOSE: This study evaluated total hip replacement arthroplasty (THRA) combined with a double-chevron subtrochanteric osteotomy for the treatment of neglected highly dislocated hips in adults. MATERIALS AND METHODS: Six cases treated with THRA combined with a double-chevron subtrochanteric osteotomy under the diagnosis of a neglected highly dislocated hip (Crowe classification IV) from April 1997 to October 2001 were followed up for between 21 months and 75 months. Harris hip scores, leg length discrepancy pre and postoperatively, osteotomy site union and implant loosening were chekced. RESULTS: The average Harris hip score increased from 61 points preoperatively to 94 points postoperatively. The average leg length discrepancy was decreased from 6 cm preoperatively to 1.8 cm postoperatively. Bone union of the osteotomy site was obtained at 4 months after surgery. The radiographs showed no acetabular and femoral loosening sign at the latest follow up. CONCLUSION: According to the clinical and radiological assessments, THRA combined with doublechevron subtrochanteric osteotomy gave satisfactory results in adults with a neglected highly dislocated hip in adults on a midterm follow up.
