ABSTRACT

Abstract A novel, simple and sensitive high-performance liquid chromatography with fluorescence detection method was developed and validated for the characterization of the preclinical pharmacokinetics of melatonin under pregnant conditions. Plasma samples (25 µL) were treated with 30 µL of ethanol absolute (containing the internal standard, IS). After a centrifugation process, aliquots of supernant (5 µL) were injected into the chromatographic system. Compounds were eluted on a Xbridge C18 (150 mm x 4.6 mm i.d., 5 µm particle size) maintained at 30°C. The mobile phase consisted in a mixture of aqueous solution of 0.4% phosphoric acid and acetonitrile (70:30 v/v). The wavelengths were set at 305 nm (excitation) and 408 nm (emission) and the total analysis time was 8 min/sample. All validation tests were obtained with accuracy and precision, according to FDA guidelines, over the concentration range of 0.005-20 µg/mL. Pharmacokinetic study showed that melatonin systemic exposure increased from day 14, with a significant difference at 19 days of gestation compared to the control group. Our findings suggest a decreased metabolism of melatonin as result of temporary physiological changes that occur throughout pregnancy. However, other maternal physiological changes cannot be ruled out.

Biomarcadores de inflamação derivados do hemograma como a razão neutrófilos/linfócitos (NLR), a razão plaquetas/linfócitos (PLR), a razão neutrófilos/plaquetas (NPR) e o índice de inflamação imune sistêmico (SII) já foram investigados como preditores de prognóstico de doenças inflamatórias sistêmicas, cardiovasculares, malignas, etc., e com a ocorrência da pandemia de SARS-CoV-2, também passaram a ser estudadas como biomarcadores de interesse nessa doença. Objetivo: Comparar retrospectivamente o valor dessas 4 razões hematimétricas durante os dois anos da pandemia (2020-2021), com o período anterior (os anos de 2018-2019), em uma população de pacientes doentes renais em hemodiálise. Métodos: Esta pesquisa foi submetida ao Comitê de Ética e Pesquisa da Universidade Estadual de Ponta Grossa (CEP-UEPG) e foi aprovada sob o número do parecer 5.024.864. Foram incluídos pacientes tratados no setor de Terapia Renal Substitutiva do Hospital Santa Casa de Ponta Grossa durante os anos 2018 ­ 2021, com n amostral de 155 pacientes. A coleta dos dados se baseou na consulta de prontuários eletrônicos. O valor das razões NLR, NPR, PLR e SII foram calculados a partir da contagem dos neutrófilos, linfócitos e plaquetas dos hemogramas, considerou-se um hemograma a cada trimestre nesse intervalo de 4 anos. Como a normalidade dos dados não foi atestada, seguiu-se a metodologia não-paramétrica, o valor de α foi fixado em 0,05. Resultados: O teste de Skillings-Mack apresentou um valor de p-simulado significativo (< 2.2e-16) na comparação de cada uma das quatro razões. Na análise post-hoc, as comparações trimestrais das medianas do SII, não apresentaram diferença significativa, as razões PLR, NPR e NLR apresentaram respectivamente, duas, três e seis, comparações com diferenças significativas. Conclusão: Houve comparações com diferenças significativas pontuais, não houve um aumento geral no valor das razões SII, PLR, NPR e NLR durante o período de pandemia (2020 - 2021) entre os pacientes em hemodiálise.


Complete blood count derived inflammation biomarkers such as neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), neutrophil/platelet ratio (NPR) and systemic immune inflammation index (SII) have already been investigated as predictors of prognosis of systemic inflammatory, cardiovascular, malignant diseases, etc., and with the imminent SARS-CoV-2 pandemic, they also began to be studied as biomarkers of interest in this disease. Objective: To retrospectively compare the value of these 4 hematimetric ratios during the two years of the pandemic (2020-2021), with the previous period (2018-2019), in a population of kidney disease patients undergoing hemodialysis. Methods: This research was submitted to the Ethics and Research Committee of the State University of Ponta Grossa (CEP-UEPG) and was approved under protocol number 5.024.864. Patients treated in the Renal Replacement Therapy sector of Hospital Santa Casa de Ponta Grossa during the years 2018 ­ 2021 were included, with a sample size of 155 patients. Data collection was based on consultation of electronic medical records. The value of the NLR, NPR, PLR and SII ratios were calculated from the count of neutrophils, lymphocytes and platelets in the blood counts, considering one blood count every quarter in this 4-year interval. As the normality of the data was not attested, the non-parametric methodology was followed, the value of α was set at 0.05. Results: The Skillings-Mack test showed a significant simulated p-value (< 2.2e-16) when comparing each of the four ratios. In the post-hoc analysis, the quarterly comparisons of the SII medians did not show a significant difference, the PLR, NPR and NLR ratios showed two, three and six comparisons with significant differences, respectively. Conclusion: There were comparisons with significant specific differences, there was no general increase in the value of the SII, PLR, NPR and NLR ratios during the pandemic period (2020 - 2021) among hemodialysis patients.


Los biomarcadores de inflamación derivados del hemograma, como el índice de neutrófilos/linfocitos (NLR), el índice de plaquetas/linfocitos (PLR), el índice de neutrófilos/plaquetas (NPR) y el índice de inflamación inmune sistémica (SII), ya se han investigado como predictores del pronóstico de enfermedades sistémicas. enfermedades inflamatorias, cardiovasculares, malignas, etc., y con la inminente pandemia del SARS-CoV-2 también comenzaron a estudiarse como biomarcadores de interés en esta enfermedad. Objetivo: Comparar retrospectivamente el valor de estos 4 ratios hematimétricos durante los dos años de la pandemia (2020-2021), con el período anterior (2018-2019), en una población de pacientes con enfermedad renal en hemodiálisis. Métodos: Esta investigación fue presentada al Comité de Ética e Investigación de la Universidad Estadual de Ponta Grossa (CEP-UEPG) y fue aprobada con el dictamen número 5.024.864. Se incluyeron pacientes atendidos en el sector de Terapia de Reemplazo Renal del Hospital Santa Casa de Ponta Grossa durante los años 2018 ­ 2021, con un tamaño de muestra de 155 pacientes. La recolección de datos se basó en la consulta de historias clínicas electrónicas. El valor de los índices NLR, NPR, PLR y SII se calcularon a partir del recuento de neutrófilos, linfocitos y plaquetas en los hemogramas, considerando un hemograma cada trimestre en este intervalo de 4 años. Como no se comprobó la normalidad de los datos, se siguió la metodología no paramétrica, fijándose el valor de α en 0,05. Resultados: La prueba de Skillings-Mack mostró un valor p-simulado significativo (< 2,2e-16) al comparar cada una de las cuatro proporciones. En el análisis post-hoc, las comparaciones trimestrales de las medianas del SII no mostraron diferencia significativa, los ratios PLR, NPR y NLR presentaron, respectivamente, dos, tres y seis comparaciones con diferencias significativas. Conclusión: Hubo comparaciones con diferencias específicas significativas, no hubo un aumento general en el valor de los índices SII, PLR, NPR y NLR durante el período pandémico (2020 - 2021) entre los pacientes en hemodiálisis.

ABSTRACT

Abstract Objective Grazing is a disturbed eating pattern that has been associated with eating disorders and obesity. One of the new measures to investigate this eating behavior is the Short Inventory of Grazing (SIG), a two-item questionnaire that assesses grazing in general and grazing associated with the feeling of loss of control over eating (LOC grazing). However, the psychometric properties of the SIG have not been assessed in the Brazilian population. The present study aimed to cross-culturally adapt and validate a Brazilian version of the SIG. Methods The SIG was adapted to the Brazilian context following international guidelines. Then, 90 undergraduate students completed an online survey including questions from the SIG, the Binge Eating Scale (BES), the Patient Health Questionnaire-9 (PHQ9), the Generalized Anxiety Disorder-7 (GAD7), and a question related to self-reported health status. The internal consistency, test-retest reliability, and convergent validity of the questionnaire were assessed. Results The prevalence rates of at least one weekly episode of grazing in general and LOC grazing were 71.1 and 54.5%, respectively. The internal consistence of the SIG was acceptable (0.81). In addition, SIG scores on both items were positively and significantly associated with BES, GAD7, and PHQ9 scores, and with poorer self-rated health. However, SIG test and retest scores differed significantly. Conclusion Overall, the Brazilian version of the SIG demonstrated adequate psychometric properties. The instrument had adequate internal consistency, with both items exhibiting good convergent validity with related measures.

ABSTRACT Objective: To identify barriers to adherence to home oral maintenance chemotherapy in children with leukemia treated at a specialized cancer center. Methods: We used the Brief Medication Questionnaire (BMQ) as a tool for screening barriers to adherence. The level of adherence was calculated considering at least one positive response in each BMQ domain, defined as Regimen Screen, Belief Screen, and Recall Screen. A positive screening for belief barriers (PSB) indicates that the caregiver reports not understanding the medication's mechanism of action and adverse effects. Results: Three important barriers to adherence were identified: beliefs, number of children of the caregiver, and age of the caregiver. The primary caregivers included 32 mothers (80%), four fathers (10%), three grandmothers (7.5%), and one unrelated caregiver (2.5 %). Most caregivers with a PSB were mothers. A PSB indicates that the caregiver reports not understanding the medication's mechanism of action and adverse effects. Caregivers with two or more children (median, three) had more barriers to adherence. Caregivers with potential non-adherence tended to be older than those with potential adherence, although without statistical significance (p=0.079, Mann-Whitney U test). Conclusions: The main barriers to adherence to home oral maintenance chemotherapy in children with leukemia identified through interviews with their caregivers, most often mothers, were lack of understanding of the treatment regimen, a greater number of children, and older age.


RESUMO Objetivo: Identificar barreiras de adesão ao tratamento de manutenção da quimioterapia via oral domiciliar, em uma amostra de crianças diagnosticadas com leucemia atendidas em um serviço especializado em oncologia. Métodos: O Brief Medication Questionnaire (BMQ) foi utilizado como instrumento de coleta para a identificação de barreiras de adesão. O nível de adesão foi calculado considerando-se pelo menos uma resposta positiva no domínio do BMQ, definido como regime, crença e recordação. Uma crença positiva mostra que o cuidador reporta não entender o mecanismo de ação e os efeitos adversos. Resultados: Três importantes barreiras de adesão foram identificadas, incluindo crença, o número de filhos do casal e a idade dos cuidadores. A mãe como principal responsável pelo tratamento da criança apresentou frequência maior entre as pessoas com rastreamento positivo para barreiras de crenças (BPC). Crença positiva significa que o cuidador relata não entender o mecanismo de ação dos medicamentos e os efeitos adversos. Quanto ao número de filhos, o estudo mostrou que quanto mais filhos (dois filhos ou mais, mediana=três) maior a barreira de adesão. Houve tendência de responsáveis com potencial não adesão serem mais velhos que os responsáveis com potencial adesão, embora sem significância estatística ao nível de significância de 5% (p=0,079, teste U de Mann-Whitney). Conclusões: As principais barreiras de adesão dos cuidadores de crianças com leucemia ao tratamento medicamentoso de manutenção foram dificuldades relatadas pelos cuidadores, na maioria das vezes as mães, que não entenderam como o medicamento funcionava, o número de filhos — quanto mais filhos menor a adesão — e a idade dos cuidadores. Cuidadores mais velhos aderiram menos ao tratamento prescrito.

ABSTRACT Introduction: Physical exercise can be an alternative for preventing and treating the harmful effects of obesity, mainly inflammatory effects on skeletal muscle and liver tissues. However, no consensus exists regarding this purpose's best physical training model. Objective: Evaluate morphological, metabolic, and inflammatory alterations in rats' skeletal and hepatic muscle tissues caused by aerobic and resistance training. Methods: 24 Wistar rats were divided into sedentary (S), aerobic (AE), and resistance training (R) groups. Blood glucose, total cholesterol, and serum triglycerides were measured periodically. After euthanasia, body mass was measured to calculate the total mass gain during the experiment. High-density lipoprotein (HDL) was measured. Adipose tissue was extracted to calculate its percentage relative to body mass and the liver, soleus, and gastrocnemius muscles for morphological analyses and concentrations of glycogen, lipids, and Tumor Necrosis Factor α (TNF-α). The Kruskall-Wallis test and Dunn's post-test were performed for statistical analysis, adopting p<0.05. Results: Both training models reduced the percentage of adipose tissue, body mass gain, and hepatic TNF-α concentration (p<0.05). AE increased serum HDL, gastrocnemius fiber diameter and reduced the fractal dimension in the soleus (p<0.05). R reduced blood glucose and serum and liver lipids, increased liver and soleus glycogen concentrations, increased gastrocnemius fiber diameter, and decreased TNF-α (p<0.05). Conclusion: Both training models reduced body mass, relative visceral adipose tissue, serum total cholesterol concentration, and liver inflammation. However, resistance training was more effective in promoting metabolic effects in the liver and skeletal muscle and reducing muscle inflammation in rats. Level of Evidence V; Expert Opinion.


RESUMEN Introducción: El ejercicio físico puede ser una alternativa para prevenir y tratar los efectos nocivos de la obesidad, principalmente los efectos inflamatorios sobre los tejidos del músculo esquelético y del hígado. Sin embargo, no existe consenso sobre cuál es el mejor modelo de entrenamiento físico para este fin. Objetivo: Evaluar las alteraciones morfológicas, metabólicas e inflamatorias del entrenamiento aeróbico y de resistencia en sobre los tejidos músculo esqueléticos y hepáticos de ratas. Métodos: 24 ratas Wistar se dividieron en grupos sedentarios (S), aeróbicos (AE) y de entrenamiento de resistencia (R). Se midieron periódicamente glucosa en sangre, colesterol total y triglicéridos. Después de la eutanasia, se midió la masa corporal para calcular la ganancia de masa total durante el experimento. Se midió la lipoproteína de alta densidad (HDL). Se extrajo tejido adiposo para calcular su porcentaje relativo a la masa corporal, así como hígado, músculos sóleo y gastrocnemio para análisis morfológicos y concentraciones de glucógeno, lípidos y Factor de Necrosis Tumoral α (TNF-α). Para el análisis estadístico fueron utilizados Kruskall-Wallis y el post-test de Dunn, adoptando p<0,05. Resultados: Ambos entrenamientos redujeron el porcentaje de tejido adiposo, masa corporal y la concentración de TNF-α hepático (p<0,05). AE aumentó el HDL sérico, el diámetro de la fibra del gastrocnemio y redujo la dimensión fractal en el sóleo (p<0,05). R redujo la glucosa en sangre y los lípidos séricos y hepáticos, aumentó las concentraciones de glucógeno hepático y sóleo, aumentó el diámetro de la fibra del gastrocnemio y disminuyó el TNF-α (p<0,05). Conclusión: Ambos modelos de entrenamiento redujeron la masa corporal, el tejido adiposo visceral relativo, la concentración sérica de colesterol total y la inflamación hepática. El entrenamiento de resistencia demostró ser más eficaz para promover los efectos metabólicos en el hígado y el músculo esquelético, además de reducir la inflamación muscular en ratas. RESUMO Introdução: O exercício físico pode se apresentar como uma alternativa para prevenção e tratamento de efeitos deletérios da obesidade, principalmente efeitos inflamatórios sobre os tecidos muscular esquelético e hepático. No entanto, não há consenso quanto ao melhor modelo de treinamento físico para tal finalidade. Objetivos: Avaliar alterações morfológicas, metabólicas e inflamatórias dos treinamentos aeróbico e resistido sobre os tecidos muscular esquelético e hepático de ratos. Métodos: 24 ratos Wistar foram divididos nos grupos sedentário (S), treinamento aeróbico (AE) e resistido (R). Glicemia, colesterol total e triglicerídeos séricos foram mensurados periodicamente. Após a eutanásia, a massa corporal foi mensurada para calcular o ganho total de massa durante o experimento. A lipoproteína de alta densidade (HDL) foi dosada. O tecido adiposo foi extraído para cálculo de sua porcentagem relativa à massa corporal assim como o fígado e os músculos sóleo e gastrocnêmio para as análises morfológicas e das concentrações de glicogênio, lipídios e Fator de Necrose Tumoral α (TNF-α). Para análise estatística, foram utilizados o teste de Kruskall-Wallis e o pós-teste de Dunn, adotando-se p<0,05. Resultados: Ambos os modelos de treinamento reduziram o percentual de tecido adiposo, ganho de massa corporal e concentração hepática de TNF-α (p<0,05). AE aumentou o HDL sérico, o diâmetro das fibras do gastrocnêmio e reduziu a dimensão fractal no sóleo (p<0,05). R reduziu a glicemia e os lipídios séricos e hepáticos, aumentou a concentração de glicogênio hepático e sóleo, aumentou o diâmetro das fibras gastrocnêmicas e diminuiu o TNF-α (p<0,05). ABSTRACT BACKGROUND: Among the complications related to chronic kidney disease (CKD), those of a neurological nature stand out, and for a better quality of life for patients, the diagnosis and treatment of these complications is fundamental. OBJECTIVES: This study aimed to assess the effect of hemodialysis on intracranial pressure waveform (ICPw) in patients with chronic kidney disease undergoing hemodialysis and those who are not yet undergoing substitutive therapy. DESIGN AND SETTING: An observational study was conducted in two stages at a kidney replacement therapy center in Brazil. The first was a longitudinal study and the second was a cross-sectional study. METHODS: Forty-two patients on hemodialysis were included in the first stage of the study. In the second stage, 226 participants were included. Of these, 186 were individuals with chronic kidney disease (who were not undergoing substitutive therapy), and 40 did not have the disease (control group). The participants' intracranial compliance was assessed using the non-invasive Brain4care method, and the results were compared between the groups. RESULTS: There was a significant difference between the hemodialysis and non-hemodialysis groups, with the former having better ICPw conditions. CONCLUSIONS: Hemodialysis influenced the improvement in ICPw, probably due to the decrease in the patients' extra-and intracellular volumes. Furthermore, ICPw monitoring can be a new parameter to consider when defining the moment to start substitutive therapy.

ABSTRACT Introduction: Pre-apheresis peripheral blood CD34+ cell count (PBCD34+) is the most important predictor of good cell mobilization before hematopoietic stem cell transplantation, albeit flow cytometry is not always immediately available. Identification of surrogate markers can be useful. The CD34+ cells proliferate after mobilization, resulting in elevated lactate dehydrogenase (LDH) activity and correlating with the PBCD34+ count. Objective: To determine the LDH cut-off value at which adequate CD34+ cell mobilization is achieved and its diagnostic yield. Materials and methods: A total of 103 patients who received an autologous stem cell transplantation (ASCT) between January 2015 and January 2020 were included. Demographic and laboratory characteristics were obtained, including complete blood count, pre-apheresis PBCD34+ and LDH levels. Receiver operating characteristic (ROC) curves were performed to identify the optimal serum LDH activity cut-off points for ≥ 2 and ≥ 4 × 106 cells/kg post-mobilization CD34+ count and their diagnostic yield. Results: A post-mobilization serum LDH cut-off value of 462 U/L yielded a sensitivity (Se) = 86.8% (positive predictive value [PPV] = 72.7%), a pre- and post-mobilization serum LDH difference cut-off value of 387 U/L, an Se = 45.7% (PPV = 97%) and an LDH ratio of 2.46, with an Se = 47.1% (PPV = 97%) for an optimal mobilization count (CD34+ ≥ 4 × 106). Conclusion: The LDH measurement represents a fast and affordable way to predict PBCD34+ mobilization in cases where flow cytometry is not immediately available. According to the LDH diagnostic yield, it could be used as a surrogate marker in transplant centers, supporting the CD34+ count, which remains the gold standard.

Abstract Introduction: Obesity is thought to play a role in the disruption of cardiac rhythmicity in obese children, but this is mostly an unexplored field of investigation. We aimed to evaluate the impact of overweight and obesity on circadian and ultradian cardiovascular rhythmicity of prepubertal children, in comparison with normal weight counterparts. Methods: We performed a cross sectional study of 316 children, followed in the birth cohort Generation XXI (Portugal). Anthropometrics and 24-hour ambulatory blood pressure were measured and profiles were examined with Fourier analysis for circadian and ultradian blood pressure (BP) and heart rate (HR) rhythms. Results: Overweight/obese children presented more frequently a non-dipping BP pattern than normal weight counterparts (31.5% vs. 21.6%, p = 0.047). The prevalence of 24-hour mean arterial pressure (MAP) and 8-hour HR rhythmicity was significantly lower in obese children (79.3% vs. 88.0%, p = 0.038 and 33.3% vs. 45.2%, p = 0.031, respectively). The prevalence of the remaining MAP and HR rhythmicity was similar in both groups. No differences were found in the median values of amplitudes and acrophases of MAP and HR rhythms. Discussion: The alterations found in rhythmicity suggest that circadian and ultradian rhythmicity analysis might be sensitive in detecting early cardiovascular dysregulations, but future studies are needed to reinforce our findings and to better understand their long-term implications.


Resumo Introdução: Acredita-se que a obesidade desempenhe um papel na desregulação da ritmicidade cardíaca em crianças obesas, mas esse é um campo de investigação ainda pouco explorado. O objetivo deste trabalho foi avaliar o impacto do sobrepeso e da obesidade na ritmicidade cardiovascular circadiana e ultradiana de crianças pré-púberes, em comparação com crianças com peso normal. Métodos: Realizamos um estudo transversal com 316 crianças, acompanhadas na coorte de nascimentos Geração XXI (Portugal). Foram medidos dados antropométricos e a pressão arterial ambulatorial de 24 horas, e os perfis foram examinados com uma análise de Fourier para ritmos circadianos e ultradianos de pressão arterial (PA) e frequência cardíaca (FC). Resultados: Crianças com sobrepeso/obesidade apresentaram mais frequentemente um padrão de PA não-dipper em comparação com crianças com peso normal (31,5% vs. 21,6%; p = 0,047). A prevalência da pressão arterial média (PAM) de 24 horas e da ritmicidade da FC de 8 horas foi significativamente menor em crianças obesas (79,3% vs. 88,0%; p = 0,038 e 33,3% vs. 45,2%; p = 0,031, respectivamente). A prevalência das restantes ritmicidades da PAM e da FC foi semelhante em ambos os grupos. Não foram encontradas diferenças nos valores medianos das amplitudes e acrofases dos ritmos de PAM e FC. Discussão: As alterações encontradas na ritmicidade sugerem que a análise da ritmicidade circadiana e ultradiana pode ser sensível na detecção de desregulações cardiovasculares precoces, mas são necessários novos estudos para reforçar nossos achados e entender melhor suas implicações a longo prazo.

Objetivo: Analisar as práticas assistenciais de saúde no processo do abortamento tipificadas como violência obstétrica. Metodologia: Estudo qualitativo e descritivo, realizado em um hospital público no Brasil com 15 profissionais de saúde de nível médio e superior. Os dados foram coletados presencialmente, através de um roteiro de entrevista semiestruturada. O perfil foi analisado pela estatística descritiva simples e as questões abertas pela técnica de conteúdo temática proposta por Bardin. Resultados: Evidenciou-se que as práticas que denotam violência obstétrica relacionam-se a alocação das mulheres em processo de abortamento no mesmo ambiente que gestantes e puérperas e ao emitirem juízos de valor no momento da assistência às mulheres. Conclusão: Há necessidade de educação permanente para que as/os profissionais possam assistir de maneira humanizada e qualificada.

Las prácticas que denotan violencia obstétrica se relacionan a la asignación de las mujeres en proceso de aborto en el mismo ambiente que gestantes y puérperas, y al emitir juicios de valor en el momento de la asistencia. Conclusión: Hay necesidad de educación permanente para que los profesionales puedan asistir de manera humanizada y cualificada.


Objective: To analyze health care practices in the abortion process typified as obstetric violence. Methodology: Qualitative and descriptive study, conducted in a public hospital in Brazil with 15 health professionals of medium and higher education. The data were collected in person, through a semi-structured interview script. The profile was analyzed by simple descriptive statistics and the questions opened by the thematic content technique proposed by Bardin. Results: It was evidenced that health professionals had insufficient knowledge about obstetric violence at the time they restricted it to childbirth or abortion, physical/verbal typing and identified only doctors and nurses as the main perpetrators. The practices that denote obstetric violence are related to the allocation of women in the process of abortion in the same environment as pregnant women and puerperal women and when issuing value judgments at the time of care. Conclusion: There is a need for permanent education so that the professionals can assist in a humanized and qualified way.

Objetivo: Relatar um caso clínico de um paciente com fratura zigomático orbitária vítima de tiro de bala de borracha e mostrar a importância de um planejamento adequado. Relato de caso: Paciente do sexo masculino, 19 anos de idade, deu entrada no Hospital de Urgências de Teresina, relatando ter sofrido uma agressão por bala de borracha durante uma manifestação. Foi observado no exame clínico: aumento de volume, equimose palpebral superior e dificuldade de abertura bucal. Na imagem foi observado, projétil de borracha na região zigomática esquerda, fraturas de zigoma com comunicação na região de pilar zigomático, fratura com deslocamento na região de margem Infraorbital e fratura na sutura fronto-zigomática. Foi proposto para o caso clínico, cirurgia sob anestesia geral com acesso intrabucal para fixação do pilar zigomático com placa do sistema 2.0mm, acesso superciliar para fixação da sutura fronto-zigomática com placa 1.5mm, e reconstrução do assoalho orbitário com malha de titânio. Conclusão: No pós-operatório o paciente não apresenta relato de enoftalmia, distopia ou diplopia, ausência de dor e déficits visuais.


Objective: To report a clinical case of a patient with zygomatic-orbital fracture, victim of rubber bullet shooting and show the importance of an adequate planning. Case Report: A 19-year-old male patient was admitted to the Teresina Emergency Hospital, reporting having suffered an aggression by rubber bullet during a demonstration. On clinical examination was observed: increased volume, upper eyelid ecchymosis and difficulty opening the mouth. In the image it was observed, rubber bullet in the left zygomatic region, zygoma fractures with communicationin the region of the zygomatic pillar, fracture with displacement in the region of infraorbital margin and fracture in the fronto-zygomatic suture. It was proposed for the clinical case, surgery under general anesthesia with intraoral access for fixation of the zygomatic pillar with a 2.0 mm plate, superciliary access for fixation of the fronto-zygomatic suture with a 1.5 mm plate, and reconstruction of the orbital floor with titanium mesh. Conclusion: Postoperatively, the patient did not report enophthalmia, dystopia or diplopia, absence of pain and visual deficits.


Objetivo: Informar de un caso clínico de un paciente confractura zigomática-orbital víctima de un disparo de bala de goma y mostrar la importancia de una planificación adecuada. Reporte de caso: Paciente masculino, de 19 años, fue admitido enel Hospital de Urgencias de Teresina, informando haber sufrido una agresión por bala de goma durante una manifestación. Se observó en elexamen clínico: aumento de volumen, equimosis del párpado superior y dificultad para abrir la boca. En la imagen se observó, bala de goma en la región cigomática izquierda, fractura del cigoma con comunicación en la región del pilar cigomático, fractura con desplazamiento en la región del margen infraorbitario y fractura en la sutura fronto-cigomática. Se propuso para el caso clínico, cirugía bajo anestesia general conacceso intraoral para fijacióndel pilar cigomáticocon sistema de placas de 2,0 mm, acceso superciliar para fijación de la sutura fronto-cigomática de placas de 1,5 mm, y reconstrucción del suelo orbitario con malla de titanio. Conclusión: En el post operatorio, la paciente no presentó informes de enoftalmia, distopía o diplopía, ausencia de dolor y déficit visual.

ABSTRACT

Se presenta el caso de una paciente de 35 años, femenina, con antecedentes de presentar un soplo desde la infancia y que debía ser intervenida quirúrgicamente por fibroma uterino. El anestesiólogo, durante la consulta preoperatoria, constató el soplo ya mencionado por lo que se decidió posponer la cirugía e interconsultar con un cardiólogo. El examen ecocardiográfico demostró que la paciente era portadora de una estenosis aórtica severa con fracción de eyección conservada, prueba ergométrica: clase funcional I, sin isquemia. Mediante anestesia general orotraqueal, se realizó histerectomía total abdominal sin complicaciones. Es de vital importancia para el anestesiólogo el manejo perioperatorio de la estenosis aórtica.


The case of a 35-years-old female patient with a history of presenting a murmur since childhood and who had to undergo surgery for uterine fibroid is presented. The anesthesiologist, during the preoperative consultation, verified the aforementioned murmur, so it was decided to postpone surgery and consult with a cardiologist. The echocardiographic examination showed that the patient had severe aortic stenosis with preserved ejection fraction, stress test: functional class I, without ischemia. Using orotracheal general anesthesia, a total abdominal hysterectomy was performed without complications. The perioperative management of aortic stenosis is of vital importance for the anesthesiologist.

El síndrome de compresión medular es una urgencia neuroquirúrgica debido a que un diagnóstico precoz y un tratamiento temprano podría revertir las incapacitantes secuelas ocasionadas por esta enfermedad. Las causas de este síndrome pueden ser traumática, metastásica, infecciosa y vascular (hematomas). La etiología infecciosa no es frecuente y el principal germen involucrado suele ser Staphylococcus aureus. A continuación presentamos el caso de una paciente de 58 años con síndrome de compresión medular de etiología infecciosa quien fue ingresada en el Servicio de Clínica Médica del Centro Médico Nacional.


Spinal cord compression syndrome is a neurosurgical emergency because early diagnosis and early treatment could reverse the disabling consequences caused by this disease. The causes of this syndrome can be traumatic, metastatic, infectious, and vascular (hematomas). Infectious etiology is not frequent and the main germ involved is usually Staphylococcus aureus. Below we present the case of a 58-year-old patient with spinal cord compression syndrome of infectious etiology who was admitted to the Medical Clinic Service of the National Medical Center.

O objetivo deste estudo é descrever o caso de mulher com síndrome de Meigs e apresentar a revisão narrativa sobre o tema. Paciente do sexo feminino, 30 anos, nulípara, encaminhada ao hospital por massa anexial e história prévia de drenagem de derrame pleural. Evoluiu com instabilidade hemodinâmica por derrame pleural hipertensivo à direita, sendo submetida a drenagem torácica, com citologia do líquido negativa. Após, foi submetida a laparotomia: realizada salpingo-ooforectomia esquerda. A congelação e a análise histopatológica diagnosticaram fibroma ovariano. A citologia ascítica foi negativa. CA-125 elevado, presença de derrames cavitários e exame de imagem suspeito podem mimetizar um cenário de neoplasia maligna de ovário em estágio avançado. Entretanto, na síndrome de Meigs clássica, o tratamento é cirúrgico, sendo o diagnóstico obtido por meio da análise histopatológica do tumor ovariano. O manejo da síndrome de Meigs clássica é cirúrgico e, após a remoção do tumor, o derrame pleural e a ascite desaparecem.


To describe a case of Meigs syndrome and present a narrative review of the condition. Female patient, 30 years old, nulliparous, referred to the hospital due to an adnexal mass and a previous drainage of pleural effusion. She developed hemodynamic instability due to a hypertensive right pleural effusion being submitted to chest drainage, with negative cytology of the fluid. She underwent laparotomy: Left salpingo-oophorectomy was performed and frozen section and histopathological analysis diagnosed an ovarian fibroma. Ascites cytology was negative. Elevated CA-125, presence of cavitary effusions, suspicious imaging exam can mimic a scenario of ovarian cancer at an advanced stage. However, in classical Meigs syndrome, treatment is surgical, and the diagnosis is obtained through histopathological analysis of the ovarian tumor. Classical Meigs syndrome' management is surgical. After tumor removal, pleural effusion and ascites resolve.

ABSTRACT Introduction: Acute myeloid leukemia (AML) is most commonly presented in older adults; however, it appears 10 years earlier in Latin American countries. Clinical evolution in older adults from this populations has not been characterized. We analyzed outcomes and survival predictors. Methods: Patients ≥ 55 years old diagnosed with AML at a hematology referral center from 2005 to 2020 receiving intensive chemotherapy (IC), low-dose cytarabine (LDAC) and best supportive care (BSC) were included. Survival analysis included the Kaplan-Meier and Cox models and the cumulative incidence of relapse (CIR). Results: Seventy-five adults were included and the overall survival (OS) was 4.87, 1.67 and 1.16 months, using IC, LDAC and BSC, respectively. The IC led to a higher OS (p < 0.001) and was a protective factor for early death, at a cost of more days spent hospitalized and more non-fatal treatment complications; non-significant differences were found between the LDAC and BSC. Eight (10.7%) patients underwent hematopoietic cell transplantation, with a higher OS (p = 0.013). Twenty (26.7%) patients achieved complete remission; 12 (60%) relapsed with a 6-month CIR of 57.9% in those < 70 years old vs. 86.5% in those ≥ 70 years old, p = 0.034. Multivariate analysis showed the white blood cell count (WBC) and IC had a significant impact on the patient survival, whereas chronological age and the Charlson comorbidity index (CCI) did not. Conclusion: AML in low-middle income countries demands a different approach; the IC improves survival, even with a high incidence of relapse, and should be offered as first-line treatment. Eligibility criteria should include WBC and a multidimensional evaluation. The age per se and the CCI should not be exclusion criteria to consider IC.

ABSTRACT Introduction: The albumin-to-creatinine ratio and total protein-to-creatinine ratio in spot urine samples have already been validated as surrogates for 24-hour albuminuria and proteinuria measurements. Thus, we hypothesized that the type of proteinuria, detected by the electrophoretic pattern of 24-hour urine, could be predicted by the simple proportion of albumin in the total urine protein content, using the albumin-to-protein ratio (APR). Our study sought to validate the use of APR as a cheaper substitute for urinary protein electrophoresis (UPE). Methods: Using different mathematical models, we compared, the albumin fraction in 24-hour urine samples by electrophoresis and the APR ratio in spot samples from 42 outpatients with chronic kidney disease (CKD). Results: A strong log-order correlation r = 0.84 (0.75-0.92; 95% CI, p = 0.001) was observed between APR and the albumin fraction in the UPE. Conclusion: The APR can substitute electrophoresis in CKD outpatients.


Resumo Introdução: A utilização da razão albumina/creatinina e da razão proteína total/creatinina em amostras isoladas de urina já foram validadas como substitutos para a albuminúria e proteinúria em 24 horas. Assim, nossa hipótese é que o tipo de proteinúria, dado pelo padrão eletroforético da urina de 24 horas, poderia ser previsto pela simples proporção de albumina no conteúdo total de proteínas na urina, utilizando a razão albumina/proteína (RAP). O presente estudo procurou validar o uso da RAP como um substituto mais prático e de menor custo da eletroforese de proteínas urinárias (EPU). Métodos: Foram utilizados diferentes modelos matemáticos a fim de comparar a fração de albumina pela eletroforese em amostras de urina de 24 horas e a RAP em amostras isoladas em 42 pacientes ambulatoriais com doença renal crônica. Resultados: Foi observada uma forte correlação logarítmica r = 0,84 (0,75-0,92; 95% CI, p = 0,001) entre a RAP e a fração de albumina pela EPU. Conclusão: A RAP pode substituir a eletroforese urinária em pacientes renais crônicos ambulatoriais.

Introdução: A eletromiografia (EMG) tem sido utilizada para avaliar alterações no sistema mastigatório proporcionada pela disfunção temporomandibular (DTM). Objetivos: Verificar a confiabilidade teste-reteste de um protocolo de coleta do sinal EMG do músculo masseter em indivíduos com DTM. Métodos: Estudo transversal composto por 20 indivíduos com DTM. Os testes foram realizados em duas sessões com 7 dias de intervalo. O sinal EMG foi avaliado pelas variáveis integral (iEMG) e a raiz quadrada da média do sinal (RMS). Resultados: Na análise intradia a confiabilidade para o RMS (ICC2,k: 0.85 a 0.96 ) e iEMG ICC2,k: 0.85 a 0.95) foi excelente, enquanto que a confiabilidade teste/reteste foi pobre para ambas as variáveis RMS (ICC2,1: 0.03 a 0.29) e iEMG (ICC2,1: 0.08 a 0.39). Conclusões: O protocolo de coleta do sinal EMG do músculo masseter em indivíduos com DTM demonstrou que os dados são reprodutíveis somente na mesma sessão de coletas enquanto que, para coletas em dias alternados a confiabilidade foi pobre.


Introduction: Electromyography (EMG) has been used to assess alterations in the masticatory system caused by temporomandybular disorders (TMD). Objectives: The aim of this study was to verify the test-retest reliability of a protocol for recording the EMG signal from masseter muscle in TMD patients. Methods: This is a cross-sectional study comprising 20 individuals with TMD. Tests were performed in two sessions 7 days apart. The EMG signal was evaluated by the variables integral (iEMG) and the root mean square of the signal (RMS). Results: In the intraday analysis the reliability for the RMS (ICC2,k: 0.85 to 0.96) and iEMG (ICC2,k: 0.85 to 0.95) was excellent, while the test/retest reliability was poor for both RMS (ICC2,1: 0.03 to 0.29) and iEMG (ICC2,1: 0.08 to 0.39) variables. Conclusions: The protocol for collecting the EMG signal from the masseter muscle in an individual with TMD demonstrated that the data are reproducible only in the same recording session, whereas for c recording on alternate days, the reliability was poor.

Objetivos: Describir las características clínicas y el tratamiento del embarazo ectópico implantado en la cicatriz de cesárea, así como las complicaciones y el pronóstico obstétrico. Materiales y métodos: Estudio de cohorte retrospectivo de gestantes con diagnóstico de embarazo ectópico implantado en la cicatriz de cesárea según los criterios de la Sociedad de Medicina Materno-Fetal, atendidas entre enero de 2018 y marzo de 2022 en dos instituciones de alta complejidad, pertenecientes a la seguridad social, ubicadas en Lima, Perú. Se hizo un muestreo consecutivo. Se midieron variables sociodemográficas y clínicas de ingreso, diagnóstico, tipo de tratamiento, complicaciones y pronóstico obstétrico. Se hace un análisis descriptivo. Resultados: Se incluyeron 17 pacientes, de 29.919 partos. De estas, el 41,2 % recibió tratamiento médico y el resto recibió tratamiento quirúrgico. Se realizó un manejo local exitoso con metotrexato en el saco gestacional en dos pacientes con ectópico tipo 2. Cuatro de las pacientes requirieron histerectomía total. Seis pacientes experimentaron una gestación después del tratamiento, y 4 de ellas culminaron el embarazo con una madre y un neonato saludables. Conclusiones: El embarazo ectópico implantado en la cicatriz de una cesárea es una entidad poco frecuente, para la cual se cuenta con alternativas de manejo médico y quirúrgico con aparentes buenos resultados. Se requieren más estudios con mayor calidad metodológica de asignación aleatoria que ayuden a caracterizar la seguridad y la efectividad de las diferentes alternativas terapéuticas para las mujeres con sospecha de esta patología.


Objectives: To describe the clinical characteristics and treatment of ectopic pregnancy arising in the cesarean section scar, as well as its complications and obstetric prognosis. Material and methods: Retrospective cohort study of pregnant women with the diagnosis of a scar pregnancy in accordance with Maternal-Fetal Medicine Society criteria, seen between January 2018 and March 2022 in two high complexity institutions of the social security system, located in Lima, Peru. Consecutive sampling was used. Baseline sociodemographic and clinical variables were measured, including diagnosis, type of treatment, complications and obstetric prognosis. A descriptive analysis was performed. Results: Out of 29,919 deliveries, 17 patients were included. Of these, 41.2 % received medical management and the rest were treated surgically. Successful management with intra-gestational sac methotrexate was performed in two patients with ectopic pregnancy type 2. Four patients required total hysterectomy. Six patients became pregnant after the treatment and 4 completed their pregnancy with healthy mother and neonate pairs. Conclusions: Ectopic pregnancy implanted in a cesarean section scar is an infrequent occurrence for which medical and surgical management options are available with apparently good outcomes. Further studies of better methodological quality and random assignment are needed in order to help characterize the safety and effectiveness of the various therapeutic options for women with suspected scar pregnancy.

Resumen Un grupo creciente de investigaciones ha sugerido la existencia de diferentes patrones de violencia en las relaciones de pareja durante la adolescencia. El objetivo de este estudio fue revisar y sintetizar la evidencia científica respecto a patrones de violencia en las relaciones de pareja en adolescentes, a través de una revisión sistemática de la literatura. La búsqueda se realizó en las bases de datos Pubmed, Scopus, SCIELO, EBSCO, Web of Science, Sage Journals, Taylor and Francis Journals, Wiley, y el motor de búsqueda Google Académico; no hubo restricciones en el año de publicación. Se incluyeron investigaciones que analizaron la violencia desde una perspectiva centrada en la persona (e.g., uso de análisis de clases latentes). Para evaluar la calidad de los estudios transversales se usó la herramienta Axis de 20 ítems, y para los estudios longitudinales, la lista de verificación de Tooth et al. (2005) de 33 ítems. Se identificaron 212 estudios y después de elegir por criterios de elegibilidad, se incluyeron 27. Los patrones de violencia con mayor frecuencia fueron: violencia multiforme (34%), victimización o perpetración por violencia psicológica y física (26%), violencia bidireccional psicológica/verbal (26 °/o), baja violencia (26 °%) y alta violencia (23 °/o). Los patrones de violencia se relacionaron con diferentes variables tales como experiencias adversas en la infancia, variables sociodemográficas, relación familiar e indicadores de salud mental. Estos patrones evidencian que la violencia en las relaciones de pareja durante la adolescencia son un fenómeno complejo y multifactorial.


Abstract A growing body of research has suggested different patterns of teen dating violence. The aim of this study was to review and synthesize the scientific evidence on patterns of teen dating violence, through a systematic review of the literature. The search was conducted in the databases Pubmed, Scopus, SCIELO, EBSCO, Web of Science, Sage Journals, Taylor and Francis Journals, Wiley, and the Google Scholar search engine; there were no restrictions on the year of publication. Studies that indicate analyzing violence from a person-centered perspective (e.g., use of latent class analysis) were included. The 20-item Axis tool was used to assess the quality of the cross-sectional studies, and the 33-itemTooth et al. (2005) Checklist was used for longitudinal studies. Two hundred and twelve studies were identified and after screening for eligibility criteria, 27 studies were included. The most frequent patterns of violence were multiform violence (34%), victimization or perpetration by psychological and physical violence (26%), bidirectional psychological/verbal violence (26%), low violence (26%) and high violence (23%). Violence patterns were related to different variables such as adverse childhood experiences, sociodemographic variables, family relationship, and mental health indicators. These patterns show that teen dating violence is a complex and multifactorial phenomenon.

Fundamento: A avaliação da área valvar mitral por meio da reconstrução multiplano na ecocardiografia tridimensional é restrita a softwares específicos e à experiência dos ecocardiografistas. Eles precisam selecionar manualmente o frame do vídeo que contenha a área de abertura máxima da valva mitral, dimensão fundamental para a identificação de estenose mitral. Objetivo: Automatizar o processo de determinação da área de abertura máxima da valva mitral, por meio da aplicação de Processamento Digital de Imagens (PDI) em exames de ecocardiograma, desenvolvendo um algoritmo aberto com leitura de vídeo no formato avi. Método: Este estudo piloto observacional transversal foi realizado com vinte e cinco exames diferentes de ecocardiograma, sendo quinze com abertura normal e dez com estenose mitral reumática. Todos os exames foram realizados e disponibilizados por dois especialistas, com autorização do Comitê de Ética em Pesquisa, que utilizaram dois modelos de aparelhos ecocardiográficos: Vivid E95 (GE Healthcare) e Epiq 7 (Philips), com sondas multiplanares transesofágicas. Todos os vídeos em formato avi foram submetidos ao PDI através da técnica de segmentação de imagens. Resultados: As medidas obtidas manualmente por ecocardiografistas experientes e os valores calculados pelo sistema desenvolvido foram comparados utilizando o diagrama de Bland-Altman. Observou-se maior concordância entre valores no intervalo de 0,4 a 2,7 cm². Conclusão: Foi possível determinar automaticamente a área de máxima abertura das valvas mitrais, tanto para os casos advindos da GE quanto da Philips, utilizando apenas um vídeo como dado de entrada. Background: The evaluation of mitral valve area through multiplanar reconstruction in 3-dimensional echocardiography is restricted to specific software and to the experience of echocardiographers. They need to manually select the video frame that contains the maximum mitral valve opening area, as this dimension is fundamental to identification of mitral stenosis. Objective: To automate the process of determining the maximum mitral valve opening area, through the application of digital image processing (DIP) in echocardiography tests, developing an open algorithm with video reading in avi format. Method: This cross-sectional observational pilot study was conducted with 25 different echocardiography exams, 15 with normal aperture and 10 with rheumatic mitral stenosis. With the authorization of the Research Ethics Committee, all exams were performed and made available by 2 specialists who used 2 models of echocardiographic devices: Vivid E95 (GE Healthcare) and Epiq 7 (Philips), with multiplanar transesophageal probes. All videos in avi format were submitted to DIP using the image segmentation technique. Results: The measurements obtained manually by experienced echocardiographers and the values calculated by the developed system were compared using a Bland-Altman diagram. There was greater agreement between values in the range from 0.4 to 2.7 cm². Conclusion: It was possible to automatically determine the maximum mitral valve opening area, for cases from both GE and Philips, using only 1 video as input data. The algorithm has been demonstrated to save time on measurements when compared to the usual method. (AU)
