ABSTRACT

Objective:To analyze the correlation of epidermal growth factor receptor (EGFR) expression in esophageal cancer tissues with sensitivity of radical radiotherapy and prognosis of patients.Methods:A total of 98 esophageal cancer patients admitted to the Affiliated Hospital of Jiangnan University from January 2017 to July 2019 were selected, and all patients received radical radiotherapy. The efficacy was assessed after 4 weeks of radiotherapy, partial remission (PR) + complete remission (CR) was treated as the sensitive group and disease progression (PD) + stable disease (SD) was treated as the tolerated group. The expression of EGFR in cancer tissues and paraneoplastic tissues (more than 5cm from the margin of cancer tissues) was measured by using immunohistochemistry. The expression of EGFR in cancer tissues and paraneoplastic tissues and the clinicopathological characteristics of the sensitive and tolerant groups were compared. Cox proportional risk model was used to analyze the factors influencing the prognosis of esophageal cancer patients; the Kaplan-Meier method was used to analyze the relationship between EGFR expression in cancer tissues and prognosis of patients.Results:The positive expression rate of EGFR in esophageal cancer tissues (66.3%, 65/98) was higher than that in paraneoplastic tissues (29.6%, 29/98), and the difference between the two groups was statistically significant (χ 2=26.49, P < 0.001). All 98 patients successfully completed radical radiotherapy, including 4 cases of CR, 60 cases of PR, 26 cases of SD and 8 cases of PD. The proportion of patients with highly differentiated and tumor length ≤1.5 cm in the sensitive group was higher than that in the tolerant group, and the differences were statistically significant (all P < 0.05). The positive expression rate of EGFR in cancer tissues in the sensitive group (56.3%, 36/64) was lower than that in the tolerant group (85.3%, 29/34), and the difference between the two groups was statistically significant ( χ2=8.39, P < 0.001). Multifactorial Cox regression analysis showed that poor differentiation, tumor long diameter >1.5 cm, and positive EGFR expression were independent risk factors for overall survival in patients with esophageal cancer (all P < 0.05). The difference in overall survival between patients with positive and negative EGFR expression was statistically significant ( χ2=9.70, P = 0.002). Conclusions:Highly-expressed EGFR in esophageal cancer tissues may suggest low sensitivity of radical radiotherapy and poor prognosis of patients.

ABSTRACT

Objective:To explore the efficacy of elective nodal irradiation (ENI) and involved field irradiation (IFI) combined with chemotherapy in treatment of esophageal cancer.Methods:A total of 104 patients with esophageal cancer in Affiliated Hospital of Jiangnan University from May 2018 to May 2020 were selected as subjects for prospective study. All patients were randomly divided into observation group and control group by lottery method with 52 cases in each group. The target volume of observation group was delineated with IFI, and the control group was delineated with ENI. The curative effects, the levels of serum tumor markers [carbohydrate antigen 50 (CA50), squamous cell carcinoma (SCC) and carcinoembryonic antigen (CEA)] before and after treatment, the 1-year overall survival (OS) rate, the incidence of adverse reactions and the scores of various dimensions of health survey summary (SF-36) after treatment were compared between the two groups.Results:The total effective rate in the observation group was 90.38% (47/52), the total effective rate in the control group was 84.62% (44/52), and the difference was not statistically significant ( χ2 = 0.79, P =0.374). There was no statistical difference in CA50, CEA, SCC levels between the two groups before and after treatment (all P > 0.05). After treatment, the CA50, CEA and SCC levels in the two groups were lower than those before treatment, and the differences were statistically significant (all P < 0.05). The 1-year OS rate of the observation group was 94.23%, the control group was 90.38%, and the difference in OS between the two groups was not statistically significant ( χ2 = 0.54, P = 0.462). The incidence of acute radiation esophagitis in the observation group was lower than that in the control group, and the difference was statistically significant ( P < 0.001). There was no statistical difference between the two groups in SF-36 scale scores of physical functioning, role-physical, bodily pain, mental health, vitality, social functioning, role-emotional, and general health after treatment (all P > 0.05). Conclusions:Both ENI and IFI are effective treatments for patients with esophageal cancer. There is no significant difference in the quality of life of patients between the two delineation methods, but the incidence of acute radiation esophagitis is lower in patients with IFI regimen.

ABSTRACT

Objective To compare the efficacy and safety of two concurrent chemoradiotherapy regimens between paclitaxel plus fluorouracil( TF) and cisplatin plus fluorouracil ( PF) in the treatment of locally advanced esophageal squamous carcinoma. Methods 103 patients with locally advanced esophagus carcinoma were treated in Affiliated Hospital of Jiangnan University from December 2014 to February 2016, and randomly assigned to either study group ( TF ) or control group ( PF ) according to random number table, of which 52 patients in the TF group while 51 patients in the PF group. The primary outcome was overall survival(OS), and secondary outcomes include progression-free survival(PFS), local progression-free survival( LPFS) and side effects. Results The 1-year OS for TF group was 76. 9% versus 74. 5% for PF group( P>0. 05 ) , and the 2-year OS for TF group was 59. 6% versus 56. 9% for PF group ( P >0. 05). The 1-year LPFS for TF group and PF group were 71. 2% and 66. 7% respectively(P>0. 05), and the 2-year LPFS for TF group and PF group were 61. 5% and 58. 8% respectively(P>0. 05). The 1-year PFS for TF group was 63. 5% versus 62. 7% for PF group ( P>0. 05 ) , and the 2-year PFS for TF group was 51. 9% versus 39. 2% for PF group ( P>0. 05 ) . The incidence rate of serious ( grade 3- 4 ) leukopenia for TF group was 36. 5% versus 17. 6% for PF group(χ2 =4. 642, P<0. 05). The incidence rate of serious (grade 3-4) acute radiation pneumonitis was 15. 4% in the TF group, higher than that in the PF group with the rate of 3. 9%(χ2 =3. 859, P<0. 05), while the incidence rate of severe nausea and vomiting for PF group was 17. 6% versus 1. 9% for TF group(χ2 =7. 262, P <0. 05). The difference between the two groups was statistically significant. Conclusions Patients who were treated with two concurrent chemoradiotherapy regimens showed no difference in OS, PFS and LPFS. The regimen on the basis of Paclitaxel has higher risk of adverse effects incidence rates of hematological toxicity and acute radiation pneumonitis, while digestive system toxicity must be concerned when concurrent chemoradiotherapy is performed on the basis of cisplatin plus fluorouracil.

ABSTRACT

Objective:To study the relative risk factor of bleeding again of patients with obscure gastrointestinal bleeding (OGIB) of negative result in capsule endoscopy(CE). Methods: 92 patients with OGIB whose extermination results of CE were negative were enrolled in the retrospective analysis, and they were divided into observation group(46 cases, with bleeding again) and control group(46 cases, without bleeding again). The data of casehistory of these patients were compared, respectively, and then these data were analyzed by using Logistic regression analysis.Results: The ratio of 50 years old or older than 50 years old and the number of abnormal blood coagulation of observation group were significantly higher than that of control group(x2=5.386,x2=14.331,P<0.05), respectively. And the number of taking Aspirin of observation group was significantly higher than that of control group, while the number of received special treatment of observation group was lower than that of control group(x2=7.180, x2=23.545,P<0.01). As the results of Logistic regression analysis, 50 years old or older than 50 years old, abnormal blood coagulation, taking Aspirin and non-receiving special treatment were the independent risk factors, respectively, for patients with negative results of CE.Conclusion: In the examination of CE, although the results of patients with OGIB were negative, they may blood again, and all of these factors including 50 years old or older than 50 years old, abnormal blood coagulation, taking Aspirin and non-receiving special treatment are the risk factors which can affect blooding again. These factors should be paid more attention.

ABSTRACT

Objective To study the impacts of berberine on the growth, migration and radiosensitivity in human breast cancer cells.Methods MTT assay was used to evaluate cell growth.In vitro scratch migration assay was used to determine cell migration.Annexin V assay was used to detect cell apoptosis.The distribution of cell cycle was evaluated by flow cytometry assay.Colony formation assay was used to detect the influence of berberine on cell radiosensitivity. Western blot assay was employed to measure protein expression.Results Berberine inhibited cell growth and migration in two human breast cancer cell lines, MCF-7 and MDA-MB-231, in a dose-and time-dependent manner. Furthermore,berberine resulted in a cell cycle G0/G1 arrest.Compared with control,the early apoptosis in MDA-MB-231 and MCF-7 cells treated with 40 pμmol/L of berberine was as high as 86.6% and 66.6% (t =8.79,10.32,P < 0.01 ),respectively. Berberine caused a dose-dependent increase in Bax and Caspase-3 protein expressions,but did not change Cyclin D1 protein expression,while suppressed the expressions of Cyclin B1 and Bcl-2 protein. As analyzed with multi-target click model fitting curves,the SERD0 of berberine-treated cells were 1.12 and 1.22 for MDA-MB-231 and MCF-7cells respectively at the dose D0 of X-rays.Conclusions The berberine inhibited the growth and migration of breast cancer cells via apoptosis induction and cell cycle arrest.Moreover,berberine increases cell sensitivity to X-ray irradiation.

ABSTRACT

<p><b>OBJECTIVE</b>To compare retrospectively the role of (18)F-FDG SPECT-CT and conventional imaging in the detection of recurrence and metastases in postoperative breast cancer patients with elevated level of tumor markers, and to evaluate the impact of (18)F-FDG SPECT-CT on the management of breast cancer patients.</p><p><b>METHODS</b>(18)F-FDG SPECT-CT was performed in 35 breast cancer patients with suspected recurrence based on elevated level of serum tumor markers. Chest, abdomen and pelvic CT were performed in all patients and whole-body bone scan was performed in only 21 patients. The final diagnosis of recurrent breast cancer was confirmed by either pathology or observation by imaging during the follow-up for more than 1 year.</p><p><b>RESULTS</b>Among the 35 patients, the final diagnosis of recurrence or metastasis was established in 19 patients. Of the 114 sites of increased FDG uptake, 93 were interpreted as malignant and 21 as benign. On site-based analysis, the sensitivity, specificity, accuracy, positive and negative predictive values were 93.1%, 55.6%, 84.2%, 87.1% and 71.4%, respectively, for (18)F-FDG SPECT-CT, and 80.5%, 60.5%, 75.6%, 80.2% and 65.1%, respectively, for conventional imaging. On the patient-based analysis, the sensitivity, specificity, accuracy, positive and negative predictive values were 84.2%, 62.5%, 74.3%, 72.7% and 76.9%, respectively, for (18)F-FDG SPECT-CT, and 74.1%, 67.6%, 70.6%, 68.3% and 73.9%, respectively, for conventional imaging. The results of (18)F-FDG SPECT-CT led to changes in the subsequent clinical management of 40.0% of these patients.</p><p><b>CONCLUSION</b>In postoperative breast cancer patients with elevated level of tumor markers during the follow-up, (18)F-FDG SPECT-CT is more sensitive for detecting recurrence and metastases than conventional imaging.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the microstructure of the cell membrane of epileptic neurons using atomic force microscopy (AFM).</p><p><b>METHODS</b>Model of epileptic neurons was established by subjecting the neurons culture for 14 days in vitro to magnesium-free media treatment for 3 h. Patch clamp technique was applied to record the electrophysiological activity of the epileptic neurons. AFM was performed to observe and measure the microstructure of the cell membrane of the epileptic neuron.</p><p><b>RESULTS</b>After a 3-hour treatment with magnesium-free media, the epileptic neurons displayed sustained epileptiform discharge, which continued after the neurons were returned to normal medium culture on day 14. Under AFM scanning size of 80 microm x 80 microm and 2 microm x 2 microm, no obvious difference in the morphology of the cell membrane was noted between epileptic and normal neurons; under the scanning size of 500 nm x 500 nm, small pits occurred in the cell membrane in both groups, but no significant difference was found in the dimension of the pits between the two groups (the diameter and depth of the pits was 114.86-/+9.33 nm and 5.71-/+0.69 nm in epileptic neurons, and 116.4-/+9.13 nm and 5.69-/+0.71 nm in the control neurons, respectively, P>0.05).</p><p><b>CONCLUSION</b>AFM provides a new method for observing neuronal membrane microstructure at nanometer resolutions. No significant alterations occur in the membrane of the neurons after a 3-hour magnesium-free media treatment.</p>

ABSTRACT

Objective To study the clinical value of ~(18)F-fluorodeoxyglucose(~(18)F-FDG) coincidence SPECT imaging in the therapy of malignant lymphoma. Methods Serial ~(18)F-FDG SPECT imaging were performed on 42 patients with malignant lymphoma before and after treatment.The results were compared with other conventional imaging.Patients were divided into two groups. Twenty-seven new-diagnosed patients(group Ⅰ) and 15 operated patients(group Ⅱ) had ~(18)F-FDG imaging pre-and post-chemotherapy. Results(In group Ⅰ,) 15 cases(achieved) clinical remission,five cases relapsed and one case progressed.In group Ⅱ,tumor residues were detected in four patients,and one patient relapsed. Conclusion Serial ~(18)F-FDG imaging during treatment is very useful for therapeutic evaluation in malignant lymphoma.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the relationship between the androgen receptor (AR), oestrogen receptor( ER) and progesterone receptor (PR) by assaying the distributions of these three receptors in the both benign prostate hyperplasia (BPH) and prostatic cancer (PCa) tissues, then to explore the difference of the effects of hormonal treatment between the AR positive and AR negative in PCa patients.</p><p><b>METHODS</b>To observe AR, PR and ER expressions in 30 cases of BPH and 32 cases of PCa tissues by DAKO Envision methods.</p><p><b>RESULTS</b>There were 22 AR positive, 10 ER positive and 6 PR positive cases in 30 BPH patients, and there were 18 AR positive, 14 ER positive and 2 PR positive in 32 PCa patients. There was no significant difference between the two groups. The survival time was 6.41 and 4.28 years for the AR positive and AR negative respectively in 30 PCa patients. The AR positive patients lived longer than the AR negative (P < 0.05).</p><p><b>CONCLUSION</b>These three receptors cant result in PCa and the effects of hormonal treatment on AR negative PCa patients is not certain.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of hyperbaric oxygen (HBO) treatment on the neuronal apoptosis at an earlier stage and the expressions of Cytochrome C (Cyt C), Bcl-2 (B-cell lymphoma-2 family) and Bax (Bcl-2 associated X protein) in rat brain tissues after traumatic brain injury (TBI).</p><p><b>METHODS</b>Forty adult rats were divided into two groups, i.e., Group A (the rats with untreated TBI) and Group B (rats with HBO treatment after TBI). Sections of brain tissues of these two groups were then detected at 3, 6, 12, 24, 72 hours after TBI by immunohistochemistry and electronmicroscope, respectively.</p><p><b>RESULTS</b>HBO treatment could up-regulate the expression of Bcl-2 within 72 hours, reduce the release of Cyt C from mitochondria, attenuate the formation of dimeric Bax and alleviate the mitochondrial edema within 24 hours after TBI.</p><p><b>CONCLUSIONS</b>HBO treatment can alleviate neuronal apoptosis after TBI by reducing the release of Cyt C and the dimers of Bax and up-regulating the expression of Bcl-2.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the efficacy and safety of Jingui Shengqi Pill in treating partial androgen deficiency in aging males (PADAM), and to explore the new approach in improving the quality of life in PADAM patients.</p><p><b>METHODS</b>Forty patients with PADAM were treated with JSP, the efficacy was evaluated with international index of erectile function (IIEF) scoring, PADAM questionnaire scoring, hormone, prostatic specific antigen (PSA), etc., and the data before treatment were compared with those after treatment in the same group.</p><p><b>RESULTS</b>After 3 months of treatment, PADAM scoring and IIEF scoring were all significantly improved. Symptoms regarding physical ability, vasomotion, and psychical and mental condition all got improved more markedly than symptoms regarding sexual hypofunction. The serum level of testosterone was 3.85 +/- 0.36 before treatment and 5.02 +/- 0.83 after treatment (P < 0.05); luteinizing hormone of 7.33 +/- 2.14 and 4.84 +/- 1.43 (P < 0.01), follicle-stimulating hormone of 10.22 +/- 4.48 and 6.47 +/- 3.28 (P < 0.01), respectively. The level of PSA failed to change significantly (1.94 +/- 0.55 and 2.06 +/- 0.47, P > 0.05).</p><p><b>CONCLUSION</b>JSP is effective and safe in treating PADAM, the mechanism of it is different from supplementing extrinsic androgen. It may have produced the effect by means of favorably regulating the condition of sex hormone to improve the balance of pituitary-sex gland axis, so it has more extensive clinical application.</p>
