ABSTRACT

ObjectiveTo investigate the effect of Mahonia bealei leaf extract on depression of rats and the underlying mechanism. MethodThe chemical constituents of the extract were analyzed by HPLC-MS/MS. Forced swimming test and tail suspension test were carried out to estimate the antidepressant effect. The mice were randomly assigned into the following groups: blank group, positive control group (fluoxetine, 10 mg·kg-1), and Mahonia bealei leaf extract groups (10, 2.5 g·kg-1). The gavage lasted for 12 days and the immobility time of the mice in the tests was recorded 1 h after the last administration. Furthermore, to explore the underlying mechanism of the antidepressant effect, we established the rat depression model by intraperitoneal injection with reserpine (0.5 mg·kg-1). Rats were grouped as follows: blank group, model group, positive control group (fluoxetine, 1.8 mg·kg-1), and Mahonia bealei leaf extract groups (10, 2.5 g·kg-1). The gavage, once a day, lasted for 10 consecutive days. The depression of rats was detected by behavioral tests 1 h after the last administration. The levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in serum of rats were detected by enzyme-linked immunosorbent assay (ELISA). The protein expression of IL-6 and interleukin-1β (IL-1β) in brain tissue was detected by immunohistochemical staining. The protein levels of nuclear transcription factor-κB (NF-κB) and NOD-like receptor protein 3 (NLRP3) in hippocampus of rats were detected by Western blot. ResultSeven chemical constituents, mainly alkaloids, were identified from the extract. Compared with the blank group, Mahonia bealei leaf extract shortened the immobility time of mice in tail suspension and forced swimming tests. Compared with the blank group, the modeling of rat depression increased the blepharoptosis incidence and retention time in circles (P<0.05, P<0.01), elevated the levels of IL-6 and TNF-α in serum (P<0.05), and up-regulated the protein levels of IL-6, IL-1β, NF-κB, and NLRP3 in brain tissues (P<0.01). Compared with the model group, high dose of Mahonia bealei leaf extract shortened the retention time in circles (P<0.05), lowered the levels of IL-6 and TNF-α in serum (P<0.05, P<0.01), and down-regulated the protein levels of IL-6, IL-1β, NF-κB, and NLRP3 (P<0.01) in brain tissues. ConclusionMahonia bealei leaf extract had significant antidepressant effect and alleviated the inflammatory response in reserpine-induced rat model of depression, the mechanism of which may be related to the inhibition of NF-κB/NLRP3 signaling pathway.

ABSTRACT

Angiomyfibroblastoma (AMF)is a kind of rare benign tumour. The clinical and imaging findings of AMF are nonspecific. In October 2019, 1 case of Angiomyfibroblastoma of perineum was admitted to Heze Municipal Hospital for surgical treatment. Final pathological diagnosis was AMF. So far, neither recurrence nor metastasis has been detected for 5 months.

ABSTRACT

Objective To investigate the effect of ultrasound-guided quadratus lumborum block (QLB) on postoperative analgesia after caesarean section.Methods Sixty parturientsscheduled for cesarean section, aged 20-40 years, ASA physical status Ⅰ orⅡ, were randomly divided into 2 groups (n = 30 each) using a random number table:QLB group (group Q) and control group (group C).Parturients in group Q received bilateral QLB with the use of 20 ml 0.33% ropivacaine per side after surgery, while QLB was not done in group C.Both groups received patient-controlled intravenous analgesia (PCIA) after surgery which contains 800 mg tramadol, 40 mg nefopam and 80 ml normal saline.The accumulative consumption of tramadol, the score of numerical rating scale (NRS) for pain at rest and on movement and the bruggrmann comfort scale (BCS) score were recorded at 4, 8, 12, 24, 48 hafter operation.The cutaneous sensory block area was determined in group Q at 4, 8, 12, 24, 48 hafter operation.The patient′s satisfaction with postoperative analgesia and adverse reactions were also recorded.Results The consumption of tramadol in group Q was significantly decreased compared with that in group C (P<0.05) at 4, 8, 12, 24, 48 hafter surgery.NRS for pain at rest at all times, NRS for pain when cough at 12, 24 hafter surgery and NRS for pain when turning over at 4, 48 hafter surgery were significantly lower, while the BCS score was higher in group Q than that in group C (P<0.05).Quadratus lumborum block affected T7-L1 dermatomes at 4, 8, 12 hafter surgery and T8-L1 dermatomes at 24 hafter surgery.The analgesic plane of quadratus lumborum block disappeared at 48 hours after operation.The patient's satisfaction with postoperative analgesia was higher in group Q compared with that in group C (P<0.05).The incidence of postoperative nausea, vomiting and dizziness was similar between the two groups.Conclusion Ultrasound-guided quadratus lumborum block can remarkably reduce the consumption of tramadol after caesarean section, lower the postoperative pain score, improve the patient′s comfort and satisfaction.

ABSTRACT

Objective To investigate risk factors for postoperative complications after percutaneous nephrolithotomy (PCNL) according to the modified Clavien scoring system. Methods The clinical data of 523 patients who underwent PCNL from 2008 to 2015 were retrospectively analyzed, and the risk factors of the complications of PCNL were analyzed. Results There were 417 patients without postoperative complications and 106 patients with postoperative complications, among whom 25 patients had major complications. After multivariate Logistic regression analysis, elder (P = 0.002), previous PCNL (P=0.035), two or more comorbidities (P=0.001), staghorn calculi (P=0.031), longer operative time (P = 0.000) had association with the postoperative complications. Two or more comorbidities (P =0.013), multiple tracts (P=0.008), and longer operative time (P=0.003) had association with the major postoperative complications. Conclusions Elder, previous PCNL, two or more comorbidities, staghorn calculi, multiple tracts, longer operative time have association with the major postoperative complications.

ABSTRACT

Sulfur compounds in the diesel were selectively derived into methylsulfonium salts by reacting with iodomethane in the presence of silver tetrafluoroborate, and characterized by positive-ion electrospray ionization (ESI) fourier transform ion cyclotron resonance (FT-ICR MS). The conversion ratios and react selectivities of the methylation for various sulfur compounds were investigated by gas chromatograph coupled with pulse flame photometric detector (GC-PFPD). Result shows that the sulfur compounds in the diesel can react with iodomethane easily at room temperature, the most of sulfur compound derived into methylsulfonium salts;the homologue of benzothiophene get the higher conversion ratio and react selectivity than the homologue of dibenzothiophene (DBT). It is found that primarily sterically hindered alkylated DBT, for example, 4- or 4-, 6- DBT, is recalcitrant to be methylated. Other than benzothiophenes and dibenzothiophenes, one- and two-ring sulfides, as well as other sulfur compounds with a double bond equivalent (DBE) value ranged from 1 to 12 are identified in the diesel.
