ABSTRACT

Objective@#To assess the effectiveness of seasonal influenza vaccine among children aged 6 to 72 months.@*Methods@#The test-negative case control study was conducted based on available surveillance data which was from China Influenza Surveillance Information system (CSIS). From October 2016 to April 2017 and from October 2017 to April 2018,1 161 cases aged 6-72 months with influenza-like illness in Yongkang and Yiwu city, were selected as the study subjects, and the cases with influenza test-positive were selected as the case group (403 cases). Test-negative subjects were selected as control group (758 cases). The etiology and immunization data of the subjects were obtained from CSIS and Immune Information and Management System (IIMS) respectively. Vaccine effectiveness was estimated using multivariate logistic regression model,and the mixed effects of non-randomized control in TNCC study were equalized by using the propensity score (PS) method in the statistical analysis.@*Results@#The age of the subjects was (2.44±1.60) years,and there were 681 boys (58.66%). The age of case group was (2.62±1.58) years, and there were 246 boys (61.04%). The case group was including of 237 cases (58.81%) of influenza A (H3N2), 92 cases (22.83%) of influenza A (H1N1) pmd09, 62 cases (15.38%) of influenza B(Victoria) lineage, 11 cases (2.73%) of influenza B (Yamagata) lineage and one case (0.25%) co-infection of influenza [A(H3N2)+B (Victoria)]. The mean age of the control group was (2.35±1.61) years,and there were 435 boys (57.39%). Overall vaccine effectiveness (VE) against all type influenza for two seasons combined was 58% (95%CI: 31%-74%). An analysis by age groups showed 68% (95%CI:41%-82%) of the VE estimate among children aged 36-72 months while it was 28%(95%CI:-80%-71%)of the VE estimate among children aged 6-35 months. The VE estimate value was 54% (95%CI:16%-75%) against all type influenza and 65% (95%CI:24%-83%) against influenza A (H3N2) during the 2016-2017 season. During the 2017-2018 season, the VE estimate value was 69% (95%CI:18%-88%) against all type influenza.@*Conclusion@#Influenza vaccine is effective in preventing influenza virus infection during the flu season,especially the effect among children aged 36-72 months is higher compared to that among children aged 6-35 months.

ABSTRACT

Objective:To forecast Urban Employees' Basic Medical Insurance ( UEBMI) in the next 85 years based on population size, age and gender distribution, and to calculate its sustainability gap. Methods:According to the population policy of Beijing and economic development of the region;based on the data of the sixth census in Bei-jing, the estimation model was constructed, and its parameters were defined and selected. Based on the period of 2010, the future of UEBMI in the next 85 year was predicted based on the population size, age and gender distribu-tion. Results:The population of UEBMI in Beijing will reach the maximum value in 2020, and then the downward trend will increase. The aging rate of the insured population will accelerate year after year and will reach the peak in the middle of this 21st century. The sustainability gap is predicted to reach at least 9541. 35 Billion dollars, and the Beijing municipal government is expected to level an average of at least 44 . 693 billion yuan per year to set aside the financial funds in order to fill the gap, so sustainable development of the fund is ensured. Conclusion:From the point of view of the insured population, there is a huge financial sustainability risk in the future of UEBMI in Beijing.

This paper analyzed the moral hazard of public hospitals 'investment stakeholders in the investment process and how to affect the hospital medical equipment over-investment from the perspective of institutional arrange-ments.The result showed that each agent has a severe moral .The impulse of over-investment from patients to doc-tors, managers, government regulators is strong .This paper also put forward the suggestions on the prevention and governance of over-investment from institutional innovation .

ABSTRACT

Objective To explore the relationship between autism spectrum disorder (ASD) and gastrointestinal problems. Methods 172 ASD children were investigated with self-made questionnaire from July, 2014 to Augest, 2015. The frequencies of motor behaviors, vocal behaviors and behavioral problems, and the intensity of the behavioral problems in the period with or without gastrointestinal symptoms were compared. Results The frequencies of motor behaviors (F=67.4, P<0.001), vocal behaviors (F=43.6, P<0.001) and behavioral prob-lems (F=79.4, P<0.001), and the intensity of the behavioral problems (F=65.7, P<0.001) were all more in the period with gastrointestinal symptoms than without gastrointestinal symptoms. Conclusion There are more behavioral problems in the ASD children with the presence of gastrointestinal symptoms;while the behavioral symptoms may be a indicator of gastrointestinal problems in children with ASD.

ABSTRACT

Objective Learn about prevalence of depressive symptoms and to determine the relationship of depressive symptoms with social capital and social networks among migrant female adolescents.Methods A cross-sectional survey was conducted among 329 single females selected by convenience sampling who were aged from 16 to 28 years old,having worked for from one month to ten years.Center for Epidemiologic Studies Depression Scale(CES-D) was used to evaluate the status of depressive symptoms that were sentenced by total score ≥20.Self-compiled items were applied to assess social capital with six aspects involving organizational characteristics,exclusion,etc.and social networks included size and density with score ranging from 1 to 15 and from 0 to 1 respectively.Results The prevalence of depressive symptom was 52.9% in participants.Working in restaurants(OR=0.554),being younger(OR=0.506),high scores of solidarity,trust and cooperation of neighborhood(OR=0.964,0.758) were associated with low risk of depressive symptoms.Conclusions Female migrant adolescents from rural to urban should be paid more attention to in depression prevention.Improving solidarity,trust and cooperation in neighborhood in city could probably decrease risk of their depressive symptoms.
