ABSTRACT

Objective: To compare the effects of surgical and conservative therapy in the treatment of orbital blow-out fracture. Methods: 90 cases of obital blow-out fracture were treated by surgical(n = 40) and conservative(n = 50) trerapy respectively, the patients were fllowed up for 12 months. The treatment outcome was retrospectively analysed. Results: Of the 40 patients managed surgically 39 were with complete follow up data, 19 had diplopia in peripheral gaze before surgery, 13 (33%) had at 3-month and 12 (31%) had at 6-month follow-up. 31 had enophthalmus before surgery and 3(8%) had at 3-month and 6-month follow-up. Of the 50 patients managed conservatively 26 were with complete follow-up data, 11 had diplopia in peripheral gaze initially, 9(35%) had at 3-month and 8(31%) had at 6-month follow-up. 15 had enophthalmus initially and 13(50%) had at 3-month and 6-month followup. Conclusion: Surgical therapy is more effective for the treatment of enophthalmus. The effects tend to be stable 3 months after treatment, the ratio of diplopia in peripheral gaze after treatment by the 2 treatments is similar(about 30% of the total cases).

ABSTRACT

Objective: To establish and verify the validity of a finite element(FE) model of fragment injury in swine mandibular composite tissue. Methods: Swine facial composite tissue digital information was obtained by 3D CT,the 3D model and the cylinder fragment with the diameter and height of 5. 5 mm were reconstructed and designed in mimics15. 0. The right mandibular angle region was impacted by the fragment with velocities in finite element analysis software. A two stage light gas gun was used to launch the same shape 30CrMnSi alloy fragment with the speed of 831,1 120 and 1 536 m/s respectively to impact swine mandibular angle area. The actual damage area and acceleration at jaw were measured and compared with the digital simulation results. Results: Compared with the data of digital simulation the fragment with the speed of 831,1 120 and 1 536 m/s resulted in the larger mandibular damage area of entry in the in vivo experiment by 13. 4%,23. 6% and 22. 3%; that of exit by 18. 7%,23. 0% and 26. 5%; the smallar accelaration peak by 16. 7%,15. 3% and 14. 6%,respectively. Conclusion: A digital model of the swine mandible composite tissue fragment injury model is established. The simulation results of the FE model are consistent similar to those of the in vivo test data.

ABSTRACT

Objective:To analyze the curative effects of laparoscopic radical prostatectomy for the prostate cancer and the its effect on the level serum hormones levels.Methods:86 patients with prostate cancer were selected and divided into the control group and the observation group with 43 cases in each group according to the draw method.The control group was treated by open radical prostatectomy,while the observation group was treated by laparoscopic radical prostatectomy.The operation indicators,serum follicle stimulating hormone (FSH),luteinizing hormone (LH),dihydrotestosterone (DHT),free testosterone (FT),total testosterone (T),prostate specific antigen (T-PSA,F-PSA) levels,CD3+,CD4+,CD8+,CD4+/CD8+ before and after the surgery as well as the occurrence of postoperative complications were compared between two groups.Results:The operation time of observation group was significantly longer than that of the control group,and the blood loss,bowel function recovery time,hospital stay,pain scores of observation group was significantly lower or shorter than those of the control group (P<0.05).No significant difference was found in the serum FSH,LH,DHT,FT,T,T-PSA,F-PSA levels before and after the surgery between two group(P>0.05).The CD3+,CD4+,CD4+/CD8+ of observation group was significantly higher than those of the control group (P<0.05),the CD8+ of observation group was lower than that of the control group(P< 0.05).The incidence rate of postoperative complication in the observation group was significantly lower than that of the control group (P<0.05).Conclusion:Laparoscopic radical prostatectomy could play a similar effect with the open surgery on the tumor control,it could improve the level of serum testosterone,immune function with high safety.
