ABSTRACT

Objective:To understand the pain experience of discharged patients after thoracoscopic lobectomy, identify the out-of-hospital pain management needs of patients, and help patients improve the level of out-of-hospital pain management.Methods:The phenomenological method in qualitative research was used in the study, and the Colaizzi 7-step data analysis method was used to analyze the interview data.Results:Six topics were summarized: pain cognition, different pain coping styles, misunderstanding of painkillers, negative emotions, postoperative paresthesia, and lack of professional guidance.Conclusion:Medical staff should strengthen the publicity and education of discharge, pay attention to the follow-up, provide professional guidance for patients outside the hospital, relieve pain and improve the quality of life of patients.

ABSTRACT

Objective:To evaluate the inpatient PE incidence and thromboprophylaxis after thoracic surgery according to adjusted Caprini VTE risk assessment model.Methods:The study selected consecutively 500 patients who received thoracic surgery from first of June in consecutive three years of 2015, 2016 and 2017. We retrospectively assessed these 1 500 patients using Caprini VTE risk model and recorded baseline characteristics, postoperative prophylaxis, and PE incidence.Results:Only 19.4% of 2015-year patients received pharmacologic prophylaxis, while the rate reached up to 65.2% in 2016 and 77.2% in 2017. PE occurred on 4 cases in 2015, 1 case in 2016, and 0 case in 2017. The overall incidence of PE was 0.3%(5/1 500). The PE incidence negatively correlated with the implementation of prophylaxis( r=-0.04, P=0.07). The PE risk(odds ratio) increased 4.68 times(95% CI: 0.525-41.800) when patients did not receive prophylaxis in PE cohort. Postoperative pharmacologic prophylaxis with current dosing did not affect the drainage of chest tube after surgery. Conclusion:Implementation of an adjusted Caprini risk assessment protocol can be useful for the thoracic postoperative patients to receive appropriate thromboprophylaxis. Current pharmacologic prophylaxis protocol should be safe and enough to prevent PE after thoracic surgery.

ABSTRACT

Objective To explore the intervention in patients with advanced lung cancer in chemotherapy effect in psychology. Methods According to the routine nursing care of 40 cases of patients with advanced lung cancer chemotherapy, and classified as the control group, the other 40 patients in the control group based on the nursing for patients with psychological intervention, and classified as the observation group, two groups of patients in our hospital from January 2015 to January 2017. Results The intervention of patients before, there were no significant difference between the two groups of patients' quality of life and life ability score, after grouping after intervention in observation group were significantly better than the control group patients were compared between the two groups; for the intervention of the satisfaction rate of visible, The proportion of patients in the observation group 97.5% was significantly higher than the control group of 72.5% patients, showed significant differences between the two groups (P<0.05). Conclusion The psychological intervention analysis find application in patients with advanced lung cancer in chemotherapy, the chemotherapy effect can help to reflect, consolidate the therapeutic effect, improve the quality of life of patients, improve the patients for nursing recognition, therefore it is worthy of reference.

ABSTRACT

Objective To investigate the therapeutic effect of aminophylline on the treatment of patients with thoracic surgery perioperative. Methods 230 cases with thoracic surgery were selected as the research objects and divided into the research group and the control group randomly, and there was 15 cases in each group. the control group were ginven closed drainage of thoracic cavity during the surgery, continued to 3 days after surgery. The control group were given intravenous drip of ciprofloxacin lactate injection at 12 h after operation, and the treatment lasted for 7 days. At this basis, the research group were given aminophylline 0.5 mg/d by intravenous injection at the beginning of anesthesia induction and continued for 3 days. The levels of lung and respiratory inflammation related factor (White blood cells, PCT, hs-CRP, TNF-α, IL-6, IL-8 and IL-10), blood gas index in the two groups pre- and post–surgery were detected, and the complications and adverse drug reactions were observed. Results 24h after surgery, TNF alpha, IL-6, IL-8 were significantly lower than those in the control group (P<0.05), and IL-10 level was significantly higher than that in the control group (P<0.05). And PaO2 in the research groupwas significantly higher than that in the control group patients (P<0.05). After treatment, arterial carbon dioxide partial pressure (PaCO2) in the two groups were significantly lower than preoperative (P<0.05). The complication rate in the research group was significantly lower than that in the control group within ten days(P<0.05). There were no adverse reactions in both groups. Conclusion During the perioperative period in thoracic surgery, aminophylline intervention can effectively improve the prognosis. .

ABSTRACT

Objective To observe the effect and safety of Mailuoning Injection(MLN) in treating patients with type 2 diabetes mellitus accompanying lower extremity atherosclerotic obliterans.Methods The selected patients were assigned by randomized digital table to a treated group (120 cases treated with MLN) and a control group (60 cases treated with alprostadil). The clinical efficacy was assessed after 4 weeks of treatment. Results The total effective rate in the treated group was 95.0%, better than that in the control group,83.3%(P<0.01).Conclusion MLN has the effect of activating blood circulation.dissolving stasis and dredging collaterals, reducing the formation of thrombi. MLN is effective and safe when applied in clinical practice for the treatment of lower extremity diabetic atherosclerotic obliterans.

ABSTRACT

Objective To observe the clinical effects of Xuesaitong Soft Capsule(XSC)on soft plaque in patients with cerebrovascular carotid atherosclerosis.Methods 240 patients with cerebrovascular carotid atherosclerosis were randomly recruited into two groups:120 patients in a conrtol group treated by conventional therapy(aspirin and simvastatin),while the other 120 patients in a treatment group were treated by conventional therapy plus XSC.Both groups received the treatment for 6 months,followed by 1 year's succedent study.Results The nature,number and size of soft plaque were apparently improved after treatment in both groups.Such improvements as subsidence of soft plaques,soft plaque turning into a hard plaque,the number of soft plaque and soft plaque volume in the treatment group were superior to the control group(t=3.525.0.01).Cardiovascular and cerebrovascular events and re-hospitalization in the treatment group were also lower than the control group(t=3.678.0.01).Conclusion XSC is efrective in the treatment of carotid atherosclerosis soft plaque and significantly decrease the incidence of cardiovascular and cercbrovascular disease.

ABSTRACT

Objective To evaluate the feasibility,safety and operation timing of laparoscopic cholecys-tectomy in treatment of gallbladder calculi incarceration. Methods The data of 280 cases of gallbladder stone incarceration performed by LC were retrospectively analyzed. Results Eight out of 280 cases were converted to open cholecystectomy. The successful rate of LC was 97. 1%. There were no complications such as bile and intestinal injury. Conclusion LC for gallbladder calculi incarceration is safe and feasible as long as operators have skilled technique and make right decisions on operation timing.

ABSTRACT

Objective To observe the influence of Xin-ke-shu Capsule (XKSC) on NO and ET in patients with coronary artery disease(CAD) combined with type 2 diabetes mellitus(T2DM), and to investigate its mechanism of treatment. Methods 80 patients with CAD combined with T2DM were randomly recruited into two groups: 40 patients in the treatment group were treated by conventional therapy plus XKSC, and the 40 in the control group treated by conventional therapy exclusively. The plasma level of NO and ET was observed before and after the three-month therapeutic course. Results The plasma level of ET in the treatment group significantly decreased after the treatmaent and was lower than that in the control group(P<0.01), but NO in the treatment group elevated significantly and was higher than that in the control group (P<0.01). Conclusion Xin-ke-shu Capsule displays its effect on protecting vascular endothelia by elevating the level of NO and decreasing the plasma level of ET.
