ABSTRACT

Purpose: The purpose of the study was to evaluate the knowledge and perception of health-care professionals (HCPs), such as doctors/surgeons, pharmacists, nurses, optometrists, and lab technicians, on clinical trials (CTs) in India. Methods: The study was a pan-Indian cross-sectional survey initiated by the Indian Ophthalmology Clinical Trial Network (IOCTN) by using a previously validated questionnaire for three months of data collection. An online survey was used to record information regarding demographics, CT knowledge, and CT perception among HCPs. Results: A total of 630 responses were recorded from HCPs: 207 doctors and surgeons, 159 pharmacists, and 264 laboratory technicians, nurses, and optometrists across India. Over 90% of HCPs had a clear knowledge on the purpose of CTs, the informed consent (IC) process, ethical approval by the Drugs Controller General of India (DCGI). About 80% and 90% were aware of confidentiality of patients, voluntariness of participation, and good clinical practice. Surprisingly, less than 50% had lesser knowledge regarding monetary incentives of CT participants (CTPs). A slightly positive perception was observed regarding the potential benefits of CTPs, compensation related to injury, and importance of obtaining IC. Less than 50% had a negative perception that monetary compensation to CTPs led to bias and deprivation of standard treatments. However, no significant difference was observed between other aspects of demographics and perception regarding CTs. Conclusion: We observed doctors and surgeons to be having the highest regarding CTs, followed by pharmacists. The survey highlighted the necessity of scheduling awareness programs among the HCPs, which would improve their misconceptions and perception of CTs while interacting with patients for CT enrollment.

ABSTRACT

Background and Aim: The appropriate size of endotracheal tube for children are traditionally selected based on their age, height and or weight by using formulas. The ultrasound guided measurement of subglottic laryngeal diameter can be used to appropriate selection of endotracheal tube. The endotracheal tube is selected based on the nearest external diameter corresponding to the measured subglottic region. The aim of the study is to compare the appropriateness of uncuffed endotracheal tube selection based on age based formula with that of ultrasound assessment of subglottic diameter in children. A prospective randomised parallel group study involving 60 children Materials and Methods: of age 2-6years undergoing elective surgeries under general anaesthesia were selected for study. They are divided into two groups as Group A – Endotracheal tube selection based on ultrasound assessment of diameter of subglottic region and Group B – Endotracheal tube selection based on age based formula which is (Age /3)+ 3.5mm ID. The noted parameters were internal diameter (ID) and external diameter (OD) of the predicted ETT by the two methods, ID and OD of the appropriate size ETT used and the ultrasound assessed subglottic diameter. The incidence of ETT size predicted by ultrasound assessment of Results: subglottic diameter was 83.33% while with age based formula it is 53.34%. The linear regression equation model in Group A showed R²= 0.694, P = 0.001 which was statistically signicant. While in Group B it was R²=0.258., p=0.001. There Conclusion: was a strong correlation between the outer diameter of clinically used uncuffed ETT and the subglottic diameter assessed with ultrasound examination in paediatric patients. Hence USG assessment of the subglottic diameter is a better tool in predicting the appropriate uncuffed ETT in paediatric patients undergoing elective surgeries under general anaesthesia than the age- based formula

Background: The clinical features and epidemiological data about patients with phebolymphedema or lymphedema related to Chronic Venous Disease in Mexico is limited and has been understudied; at the same time, the relationships between its clinical features, physical activity level and disability remained unknown. Methods: This is a longitudinal cohort study based on the analysis of clinical data of 90 patients gathered between 2021 and 2022. The statistical analysis was carried out using the software SPSS version 25 and GrandhPad Prism 8; a descriptive analysis was developed using measures of central tendency for the variables of a quantitative nature and frequency distribution for those categorical variables. The behavior of the variables was revealed through the Shapiro-Wilk statistic. The mean difference analysis was carried out with the Student’s T for independent samples. To identify the effect of gender, age, and severity of the disease on the study variables, a three-way analysis of variance was obtained with a Sidak comparison analysis. For the associations between qualitative and dichotomous nature variables, the Chi-Square statistic was obtained along with the odds ratio to determine the intensity of the relationships found. Results: A total of 90 patients were included in the analysis; 71% (64) were female and 29% (26) were male; with a mean age of 62.7 years old (± 30.5). A mean BMI of 33.2, 79.9% (77) of patients reached overweight and obesity ranges. 50% (45) of patients reported disability to perform one or more daily life activities related to the disease’s condition (signs, symptoms, volume). Only 12% (10) of all patients performed at or above the minimum physical activity recommended for their population group, and 88% (80) of patients had no physical activity or performed under the proper population group’s recommendation of minimal physical activity. It was found that the practice of physical activity, the number of compromised segments, the stage of the pathology, and the presence of cardiological and metabolic antecedents provide a statistically significant association with disability. Notable statistical difference among sex as a risk factor was not found. Regarding the BMI, the only differences were observed in the level of severity of the disease, regardless of the presence of any disability (p=0.006), evidencing that the greater the severity, the higher the BMI in both men and women. About physical activity, it is observed that inactivity or minimal practice of physical activity is a risk factor for referred disability (p<0.05), since it is prolonged that the non-practice of physical activity increases by 230% the chances of referring a disability associated to the disease compared to those people who referred to practice minimal or above minimal physical activity; the presence of a cardiological and/or metabolic history is related to an increase of about 150% to 180% of presenting disability compared to patients who do not have these pathologies. Conclusion: This study shows clinical and epidemiological features of phlebolymphedema and their relationship with the level of physical activity and reported disability of 90 patients. Further studies are needed to improve and broaden the understanding of the clinical characteristics of phlebolymphedema and its correlations.

ABSTRACT Purpose: Evaluate the patients in two neonatal intensive care units in Paraná/Brazil and identify the risk factors for the development of retinopathy of prematurity. Methods: We performed a prospective cohort study on premature infants with gestational age ≤32 wk and/or with birth weight ≤1500 g who were admitted to the neonatal intensive care unit of Hospital do Trabalhador and Hospital Infantil Waldemar Monastier. These hospitals admit patients referred from other maternity hospitals in the state of Paraná. The study duration was 12 mon. Results: The incidence of retinopathy of prematurity was higher in the Hospital Infantil Waldemar Monastier than in the Hospital do Trabalhador for premature infants who needed to be transported from their birthplace to the intensive care unit (52.2% vs. 29.6%). The following risk factors were associated with the development of the disease: longer hospitalization, low gestational age at birth, longer oxygen use, vasoactive drugs use, no antenatal corticosteroids use, intracranial hemorrhage, and any glycemic disorder. Low birth weight was an independent risk factor for the development of retinopathy of prematurity. Conclusion: Early neonatal care and transportation of premature infants may influence the occurrence and prognosis of retinopathy of prematurity.


RESUMO Objetivos: Avaliar duas unidades de terapia intensiva neonatais do Paraná e identificar os fatores de risco que levam ao desenvolvimento da retinopatia da prematuridade nestas unidades neonatais. Metodos: Foi realizado um estudo de coorte, prospectivo, com avaliação dos bebês prematuros examinados no período de 12 meses com idade gestacional ≤32 semanas e/ou com peso de nascimento ≤1500 gramas, internados na unidade de cuidados intensivos neonatais do Hospital do Trabalhador e do Hospital infantil Waldemar Monastier, que recebe neonatos transportados das maternidades de todo o estado do Paraná. Resultados: A incidência de retinopatia da prematuridade foi maior no Hospital Infantil Waldemar Monastier, entre os prematuros que necessitaram de transporte do local de nascimento para a unidade de cuidados intensivos (52,2% vs 29,6%), Os fatores de risco associados ao desenvolvimento da doença foram; Maior número de dias de internamento, baixa idade gestacional ao nascimento, maior tempo de uso de oxigênio, uso de drogas vasoativas, ausência de uso de corticoide pré-natal, presença de hemorragia intracraniana e qualquer tipo de alteração da glicemia. Conclusão: Os cuidados neonatais precoces e o transporte do recém-nascido pré- termo podem influenciar a ocorrência e o prognostico da retinopatia da prematuridade.

ABSTRACT

Background:The objective of this study was to assess the demographical characteristic, laboratory and radiological findings associated with COVID-19 mortality in hospitalizedpatients and also to co-relate neutrophil-to-lymphocyte ratio (NLR)and chest x-ray (CXR)score with severity of the disease.Methods:This is a retrospective study done in Bowring and Lady Curzon hospital between the periodof May 2021 to July 2021. 100 patients who were tested positive for SARS-CoV2 with RT-PCR were taken for the study after fulfilling the inclusion criteria. On day 1 of admission, routine blood investigations including CBC with differential count and chestX ray is taken. From the above said data, NLR and CXR score is calculated and a comparison is made to determine severity and in-hospital mortality between mild, moderate and severe COVID pneumonia patients. This study is being carried out after obtaining institutional ethical committee approval clearance. All analysis were performed using SPSS software version 10.Results: The sample size studied was 100. The mean age of patients was 28.3 in mild, 49.9 in moderate and 62.6 in severe COVID patients. Among these 67% were males and 33% were females. It was noted that, leukocytosis(mean-13245), neutrophilia (mean-83.05%), lymphocytopenia (mean-10.45%) and chest X-ray score (mean-4.98) was seen among severe group with p value being significant.Conclusions: TLC, NLR and CXR score were significantly different between severe and non-severe patients, so assessment of these simple parameters may help identify high risk COVID-19 patients at an early stage in a resource limited setting from the data retrieved from our hospital, NLR and CXR Score showed an acceptable efficiency to separate COVID-19 patients among severe and non-severe patients with a significant p value thereby helping in triaging the patients and need for early ICU needs.

ABSTRACT

Background? There is a lack of clinical and epidemiological knowledge about cancer-related lymphedema in Mexico. The objective of this study is to present a broad and deep report of cancer-related lymphedema’s clinical characteristics and epidemiological features based on the analysis of CRL patients’ data, collected from a specialized rehabilitation clinic in Mexico. Methods? This is a cohort study based on the analysis of clinical data gathered between 2015 and 2022. The study was developed in a private clinic that specializes in oncological patients’ rehabilitation. Clinical assessments and interviews were performed to collect each case’s clinical history, considering diverse clinical characteristics and demographic information, classified in a matrix, and statistically evaluated. Results? Among 307 lymphedema patients, gender distribution was represented by 87.9% (270) female and 12.1% (37) male; mean age of 54.5 years (± 32.5); 79.8% of the patients corresponded to breast cancer diagnosis, the rest of them were associated with other 19 different types of cancer. 90.2% of patients underwent some lymph node removal procedure, 68.1% of patients received fractioned radiotherapy sessions, and 87.7% received some chemotherapy scheme treatment. 64% of patients had a diagnosis of lymphedema and had onset symptoms of lymphedema in the first 5 years after oncological interventions and/or cancer diagnosis, most of them during the first 2 years. The general distribution of clinical staging was: 23.7% in stage 0; 20.1% in stage I; 44.6% in stage II, and in stage III 11.4%. Regarding body mass index, it was found a mean of 26.7 BMI. 61.2% of patients ranged between overweight and obesity. 43.3% of patients with CRL reported a disability to perform one or more activities due to lymphedema. 80% of patients had no physical activity or performed under the minimal physical activity recommended for the population group they belong. Conclusion? This study reports broad epidemiological and clinical features of a group of cancer-related lymphedema patients in Mexico, it was possible to identify diverse potential factors and relations that influence the clinical features of CRL; it is necessary to explore all factors and the interactions among them for a better understanding of CRL patients in the country.

ABSTRACT

BioProtein® (BP) is a trademark for single cell (bacteria) protein, based on conversion of methane, with the addition of ammonia and oxygen, to a protein source. BP is produced by Norferm AS in Norway, and has been authorized by the EU as a protein source in animal feeds since 1995, for fattening pigs (8%), calves (8%) and salmon (19-33%). Significant immune effects were revealed in a toxicity study performed in rats fed a nucleic acid reduced BP product (NABP) and thereafter, similar, but less severe effects were also found after feeding with untreated BP. Additional studies confirmed increased mesenteric lymph node weights in cats and foxes. Due to the undesirable effects and also due to applications for extended use, BP has been assessed by the Scientific Committee on Animal Nutrition (SCAN) and EU’s Scientific Committee on Food (SCF) in 1995, by SCAN in 2001 and 2003 and by the European Food Safety Authority (EFSA) in 2005. The EU memberstates United Kingdom, France and Finland have also conducted assessments. The EU approval from 1995 remains unchanged. The Norwegian Food Safety Authority requested the Norwegian Scientific Committee for Food Safety (VKM) to assess the risk of using BP as a protein source in feedingstuffs, both for the animal categories already authorized and for extended use to pet animals, chickens and pigs from weaning to slaughter. The Norwegian Scientific Committee for Food Safety was asked to consider all existing documentation. Based on all documentation enclosed with the request from the Norwegian Food Safety Authority and published scientific articels, an opinion on the safety of BP assessed by the VKM panel on Animal Feed was published on 20 March 2006 (05/608-final-rev1). The Producer made a complaint regarding this opinion and claimed that not all documents on BP had been evaluated. The Norwegian Food Safety Authority then requested VKM to revise its opinion on the risk of using BP as a protein source in feedingstuffs, based on 17 documents previously not available to VKM, in addition to the 20 documents included in the opinion published on 20 March 2006. BP is composed of a protein with a somewhat different amino acid compostion compared with fish meal, but BP and fish meal have more similarities in amino acid content than soybean meal. BP has relatively high level of nucleic acids, phospholipids, lipopolysaccarides and minerals. Effect studies with BP have been conducted in rats/mice, pigs, chicken, cats, foxes, and salmon. Most of the concern regarding the side effects of BP in feed is related to the immune response. The main findings include changes in weight and morphology of mesenteric lymph nodes, followed by induction of specific antibodies. Histopathological examination after feeding with NABP also revealed changes in the intestines and several internal organs indicating systemic effects. The Producer claims that the immune response seen in BP-fed mice/rats is most likely a normal response to ingestion of large doses of a foreign antigen, and further, that oral tolerance towards this protein is induced over time. However, these interpretations are not adequately supported by the supplied documentation. A tendency towards adaption might be indicated in some of the studies, other results argue against tolerance induction. It is unclear whether the content of phospholipids, lipopolysaccarides, nucleic acids or the protein structure, or the combination of these compounds is responsible for the immunological changes observed. However, the particulate structure of BP has been shown to influence the observed immune response as the systemic immune response was avoided by ingestion of BP free of whole cells. The studies conducted in target species have not included adequate examinations of the immune effects from ingestion of BP. Concerning terrestrial species, no histopathological effects were revealed in the pig, chicken, cat or fox studies. However, increased mesenteric lymph nodes were reported in cats and foxes fed BP. In the remaining studies the main focus has been on production parameters; weight gain, feed intake, feed efficiency, metabolism of nutrients, observation of clinical health, and product quality. When the contents of amino acids were balanced, the inclusion of low levels of BP (9%) tended to stimulate growth in pigs and the same tendency was found in chicken with 6% BP. Higher feed levels of BP tended to cause a reduction in growth. In salmon, a dose dependent improvement of growth was reported in a short term experiment (8 weeks). However, in longer term experiments with salmon, depressed growth and increased liver weight were observed in freshwater at 19% BP with no-effect-level at 10%. In seawater studies, a tendency of reduced growth was found in salmon fed with 20% BP in the diet, and BP levels of 27% and higher levels resulted in significantly reduced body weight. Furthermore, levels of 27% BP and above in fish feed reduced specific immune responses, but increased lymphocyte response, and also tended to improve the survival after bacterial and viral infections. At 37% BP in the diet histopathological changes in the distal intestine, and reduced storage of glycogen and increased lipid deposition and liver weight were observed. No negative effects were seen in salmon in seawater at a feed level of 13.5% BP. The results indicate negative effects in salmon at BP levels in fish feed considerably lower than those currently approved (19 and 33%, in feed for salmon in fresh and sea water, respectively). To conclude, in terrestrial target species documented effects of BP include reduced weight gain and increased weight of mesenteric lymph nodes. In the more thoroughly studied species the rat, findings incluse histopathologic effects in mesenteric lymph nodes, changed humoral immune responses, increased weight of other lymphoid tissue (spleen), increased level of neutrophile granulocytes, and slight leakage of hepatic and renal tubuli enzymes. In terrestrial target species, 6% BP in the feed is considered to be the highest inclusion level not to cause such effects. The results from the rat studies show a similar no-effect-level. In salmon, reduced weight gain, liver storage effects, changed humoral and celluar immune responses and histopathological effects in the intestine are documented. A 10% level of BP in fish feed is the highest level tested without causing such effects. There are relatively few valid studies for the risk assessment of BP in target species, and the designs of the assessed studies are very variable. Thus, there are qualitative and quantitative uncertainties concerning the effects of BP in target species. The Panel on Animal Feed is of the opinion that an inclusion level of BP of 6% in the diets to terrestrial target animals and a 10% maximum inclusion level in salmon feed (both for fresh- and seawater stages) would reduce the risk of potentially adverse effects in the animals. The risk associated with the human consumption of products from animals fed on BP is considered negligible. However, the production of single cell protein for feed production represents a relatively new scientific approach which implies precautionary handling.

Background: Syncope is usually a benign condition preventable by simple measures. Its diagnosis by history and inexpensive tests like head-up tilt table (HUTT) test save the patient unnecessary expenditure and stress. Objectives: To determine the outcome of head-up tilt test in older adult patients aged 60 years and more who present with typical versus atypical history of syncope, along with a study of the prodromal symptoms, association with comorbid burden, medications, grip strength, and cognitive function. Materials and Methods: An observational study of the patients presenting to the department of geriatrics with a history of neurocardiogenic syncope from May 2017 to October 2017 undergoing HUTT test looked at the association of the type of syncope based on the history and outcome of their HUTT test and association with prodromal symptoms, comorbidities, medications, triggering factors, electrocardiography, and Holter. Conclusions: The HUTT positivity rate for patients with a history of syncope was 31.8% with no association between typical or atypical syncope and HUTT positivity. Dizziness, lightheadedness, blurred vision, and sweating were significantly associated with typical syncope. No comorbidity or medication had an association with syncope or HUTT response but the comorbid burden had a significant association with positive HUTT response.

ABSTRACT

Dysfunctional uterine bleeding (Raktapradara) is the commonest gynaecological disorder affecting the quality women’s life of reproductive age group. A clinical study was conducted with KVGAP’S Femicare Syrup on 28 patients of DUB. KVGAP’S Femicare syrup is a polyherbal Ayurvedic formulation prepared from ingredients like Ashoka, Ashwagandha, Gokshura, Guduchi, Shatavari, Manjhistha, Usheera, Kumari, Sariva, Pippali, and Shilajathu. The parameters assessed were number of bleeding episodes, excessive bleeding, investigators global assessment scale, VAS for pain, participants overall assessment scale, clot and interval (number of days of the cycle). Patients were assessed at the beginning of the trail, after 1 month and after 2 months. 56.52%, 73.08%, 77.6%, 85%, 81.93%, 100% improvement were observed in the clinical parameters assessed (number of bleeding episodes, excessive bleeding, investigators global assessment scale, VAS for pain, participants overall assessment scale, clot) respectively. Results were statistically significant. Interval didn’t show any changes. Out of 28 patients in this study, 1 patient shown mild improvement, 11 patients (39%) were shown moderate improvement and 16 patients (57%) had marked improvement. Overall effect of the treatment was 77.31%. This study showed that KVGAP’S Femicare syrup showed promising results in treating DUB. Hence it can be concluded that KVGAP’S Femicare Syrup is effective and safe in the management of dysfunctional uterine bleeding.

Introduction: Hypertension and Diabetes are two of the most common chronic non-communicable diseases and multifactorial disorders affecting both developed and developing countries. Method: A cross sectional study was conducted for a period of 3 months among patients’ attendees above the age of 30 years in a tertiary care hospital of Mangalore, between July - September 2018. Data was collected from 150 patient attendees by personal interviews followed by anthropometry, blood pressure measurement and random blood sugar and entered into Microsoft excel. Analysis was done using Statistical package for social sciences (SPSS) software version 21. Results: It was seen that 72(48%) were Normotensive, 44 (29.3%) were Pre- hypertensive and 34 (22.7%) were Hypertensive while 65(43.3%) had a random blood sugar of > 140 mg/dl. There was an association between hypertension and age, gender, type of family, occupation, religion, alcohol intake, smoking habits and physical activity which was statistically significant (p< 0.05). While age more than 45 years, illiteracy, belonging to Christian faith, having a family history of diabetes mellitus and being obese/ overweight were associated with a random blood sugar of >140 mg/dl which was statistically significant (p< 0.05). It was also seen that 23(67.6%) of hypertensive attendees and 18(40.9%) of pre- hypertensive attendees also had diabetes as a comorbidity. Conclusion: Opportunistic screening of individuals for hypertension and diabetes will help in early detection of these diseases. This will help in early prevention and timely treatment

Background: Increasing use of drugs has led to drug-drug interactions (DDIs) which necessitate their awareness among the health-care providers to reduce the hospital admissions due to their adverse drug reactions. Aim and Objectives: This study aims to assess and compare the knowledge, attitude, and practice (KAP) of DDIs among interns and nurses and to evaluate the impact of an educational program with a pre-and post-test questionnaire. Materials and Methods: An educational program about DDIs was conducted for the interns and nurses to evaluate their KAP by a pre-test and post-test pre-verified 20-point questionnaire about DDI. Results: There was a statistical significant difference (P < 0.05) in gender (females: Males = 77:16) among the interns and nurses and their mean age was 23 ± 0.87 years and 21.3 ± 0.83 years, respectively. Both the groups fared well in post-test compared to pre-test in the knowledge (questions=7), attitude (questions=5), and practice (questions=8) domain which was statistically significant. On comparing the post-test scores, both the groups showed no difference statistically (P > 0.05) in the knowledge and attitude domain, while the nurses group showed more improved mean score than interns in terms of practice of DDI. Conclusion: The educational program about DDIs was effective among the interns and nurses with regard to their KAP assessment and they equally performed well. This implies that the awareness program about DDIs was successful among the interns and nurses who form the lower strata in delivering the health-care needs to the society.

Objectif : déterminer les étiologies des fièvres prolongées inexpliquées au service de médecine interne du CHU de Bouaké. Matériel et méthodes : Il s'agissait d'une étude rétrospective à visée descriptive mené de janvier 2019 à décembre 2020. Nous avons inclus les patients hospitalisés pour fièvre prolongée inexpliquée ou ayant présentés une fièvre prolongée durant l'hospitalisation. Les données anamnestiques, cliniques et paracliniques ont été analysées. Résultats : Nous avons retenus 204 patients sur 1650 patients hospitalisés (12,6%). L'âge moyen des patients était de 32±16,1 ans avec des extrêmes de 16 et 87 ans. Le sexe ratio était de 1,1. La fièvre (32,4%) et AEG (22%) constituaient les motifs d'hospitalisation les plus fréquents. La fièvre était le plus fréquent des symptômes physiques (94,1%) suivi de l'amaigrissement (50%), de la déshydratation (18,1%), de l'hépatomégalie (12,7%) et de l'obnubilation (12%). Les principales étiologies étaient les infections (82,3%), les tumeurs (10,3%), et les maladies inflammatoires (1,5%). Dans 5,9% des cas l'étiologie n'avait pas été retrouvée. Les principales infections étaient le VIH et ses complications (38,1%), la tuberculose (21,4%), et les infections urogénitales (19,9%). Les tumeurs étaient dominées par le cancer primitif du foie (33,4%) et le cancer de la prostate (19,4%). La polyarthrite rhumatoïde, le lupus érythémateux disséminé et la sclérodermie étaient maladies inflammatoires retrouvées. La létalité était de 36%. Conclusion : Les étiologies des fièvres prolongées inexpliquées sont variées. Elles sont dominées dans notre contexte par la tuberculose, l'infection à VIH et ses complications infectieuses.

Introduction Les maladies auto-immunes systémiques (MAIS) sont peu connues malgré les progrès diagnostiques et thérapeutiques réalisés ces dernières années. L'objectif de ce travail était de décrire le profil épidémiologique, diagnostique et thérapeutique des MAIS dans les services de Médecine Interne et de Dermatologie du Centre Hospitalier Universitaire de Bouaké (CHU). Méthodes Il s'agissait d'une étude transversale descriptive réalisée dans les services de Médecine Interne et de Dermatologie du CHU de Bouaké, sur une période de 10ans (janvier 2009- décembre 2018). Résultats : Sur 30906 patients, 50 présentaient une MAIS soit une prévalence hospitalière de 0,16%. Les MAIS les plus fréquentes étaient le lupus érythémateux systémique (50%) et la sclérodermie systémique (42%). L'âge moyen était de 39,5ans ±15ans et le sex-ratio de 0,19. Le délai moyen de consultation était de 26,2 mois. Le tableau clinique était dominé par les signes généraux (98%), les manifestations cutanéomuqueuses (96%) et les manifestations articulaires (90%). Chez 37 patients ayant réalisé l'hémogramme, l'anémie représentait 51,3% des cas. Le syndrome inflammatoire était objectivé chez 67% des 12 patients possédant un bilan inflammatoire. Les auto-anticorps réalisés chez 05 patients étaient contributifs chez 03 patients. Les corticoïdes par voie générale étaient prescrits dans 56% des cas et les perdus de vue étaient observés dans 90% des cas. Conclusion : Les MAIS étaient rares dans notre étude, dominées par le lupus érythémateux systémique et la sclérodermie systémique. L'amélioration du plateau technique et l'accessibilité du bilan immunologique et la sensibilisation paraissent indispensables afin d'améliorer la prise en charge des patients.


Introduction: Systemic autoimmune diseases (SAID) are little known despite the diagnostic and therapeutic progress made in recent years. The objective of this work was to describe the epidemiological, diagnostic and therapeutic profile of SAID in Internal Medicine and Dermatology departments of the university hospital of Bouake. Methods: This was a cross-sectional study carried out in the Internal Medicine and Dermatology departments of the university hospital of Bouake, over a period of 10 years (January 2009-December 2018). Results: Of 30,906 patients, 50 presented SAID with a hospital prevalence of 0.16%. The most common SAID were systemic lupus erythematosus (50%) and systemic sclerosis (42%). The mean age was 39.5 ± 15 years, and the sex ratio was 0.19. The average consultation time was 26.2 months. The clinical picture was dominated by general signs (98%), mucocutaneous manifestations (96%) and articular manifestations (90%). In 25 patients who performed the blood count, anemia represented 76% of cases. The inflammatory syndrome was objectified in 67% of the 12 patients with an inflammatory profile. The auto-antibodies made in 05 patients were contributory in 03 patients. Systemic corticosteroids were prescribed in 56% of cases and patients were lost to follow-up in 90% of cases. Conclusion: SAID were rare in our study, dominated by systemic lupus erythematosus and systemic scleroderma. Improvement of the technical platform and accessibility of the immunological assessment appears essential in order to improve patient's care.

ABSTRACT

Clinical oncology has shown outstanding progress improving patient survival due to the incorporation of new drugs. However, treatment success may be reduced by the emergency of dose-limiting side effects, such as intestinal mucositis and diarrhea. Mucositis and diarrhea management is symptomatic, and there is no preventive therapy. Bacterial and fungal-based compounds have been suggested as an alternative for preventing the development of diarrhea in cancer patients. Using probiotics is safe and effective in immunocompetent individuals, but concerns remain during immunosuppressive conditions. Paraprobiotics, formulations composed of non-viable microorganisms, have been proposed to overcome such limitation. The present literature review discusses current evidence regarding the possible use of paraprobiotics as an alternative to probiotics to prevent gastrointestinal toxicity of cancer chemotherapy.

ABSTRACT

The objective of this study was to quantify the number and frequency of monocyte (MnOF) and multi-oocyte (MtOF) follicles in ovaries of bitches subjected to ovary salpingohysterectomy (OSH). Right and left ovaries of 38 bitches were collected after OSH, prepared, and a histological analysis was carried out. The ovaries were subjected to surface and deep histological cuts; the follicles were classified, and the number of follicles and cumulus oophorus complexes (COC) per follicle were quantified for each histological cut. MnOF and MtOF were found in all ovaries, at different developmental stages; primary follicles were grouped in the ovarian cortex, and follicles at other follicular stages presented a random distribution. MtOF containing two, three, four, or more COC were found in the ovaries of bitches, with a decreasing frequency trend, according to the number of COC in the MtOF. The effect of the age, number of estrus, estrus interval, and number of progenies per delivery was not significant for the number and frequency of MtOF in the ovaries of the bitches, whereas the size, number of pregnancies, use and number of contraceptive applications had some effect on the number and frequency of MtOF in the ovaries of the bitches.(AU)


Objetivou-se, com este estudo, quantificar o número e a frequência de folículos monocitários (MOF) e polioocitários (POF) provenientes de ovários de cadelas submetidas à ovariossalpingo-histerectomia (OSH). Para tanto, coletaram-se os ovários (direito e esquerdo) de 38 cadelas após OSH, com posterior preparação e análise histológica. Cada ovário foi submetido a dois cortes histológicos (superficial e profundo) onde se quantificou o número e a classificação dos folículos, bem como o número de complexos cumulus oophorus (COCs) por folículo em cada corte histológico. Observaram-se MOF e POF em todos os ovários estudados, em diferentes estádios de desenvolvimento, sendo os folículos primários agrupados no córtex ovariano, frente a uma distribuição aleatória dos outros estádios foliculares. FOPs contendo dois, três, quatro ou mais COCs foram observados nos ovários de todas as fêmeas estudadas, e sua frequência tendeu a diminuir de acordo com o número de COC presente no POF. Não se observou influência da idade, do número e do intervalo de estros, assim como do número de filhotes por gestação sobre o número/frequência de FOP nos ovários das cadelas estudadas, enquanto o porte, o número de gestações, o uso e o número de contraceptivo apresentaram algum grau de influência sobre o número/frequência de FOP nos ovários das cadelas estudadas.(AU)

ABSTRACT The paradigm of biological systems provides a framework to quantify the behavior of biological processes. Mathematical modeling is one of the analytical tools of biological systems used to reproduce the variables of a system for prediction. This article presents the analysis of muscular contraction, the physiological process responsible of generating force in skeletal muscle, from the point of view of mathematical modeling. The aim is to provide numerical evidences about the force generated by the sarcomere, and the energy required to produce such a force. The proposed scheme includes a model to activate the contractile cycle, based on the action potential that reaches the neuromuscular junction, the calcium release into the sarcoplasm, the contraction response, and the quantification of the energy that the sarcomere requires to perform mechanical work. The results shows that the proposed scheme is acceptable because it reproduces experimental data of force, velocity, and energy reported in the literature. The results of the proposed scheme are encouraging to scale the model at the muscle or muscle group level, in such a way that the quantification of energy can be an alternative to the indirect estimation methods of energy consumption that currently exist.

ABSTRACT

The objective of this research was to study the population structure of the Cattle Conservation Nucleos Curraleiro Pé Duro of the Instituto Nacional do Semiárido (NCP_INSA) based on pedigree data. Genealogical information from 338 animals registered in the period from 1991 to 2019 was used. The number of founding animals (Nf), the effective number of founders (fe), effective number of ancestors (fa), inbreeding coefficient (F), and average relatedness coefficient (AR), in addition to Fis, Fit and Fst were estimated. It was possible to identify ancestors up to the third generation, with an increase in information over the generations. Of the total pedigree information evaluated, 90.53% had the identification of the father and mother. The effective size of the population was smaller than those proposed by FAO, suggesting the need to redefine the herd management and genetic management plan strategies, promoting gene flow and breed expansion.(AU)


O objetivo com essa pesquisa foi estudar a estrutura populacional do Núcleo de Conservação de Bovinos Curraleiro Pé-Duro (NCP) do Instituto Nacional do Semiárido (INSA), por meio de dados de pedigree. Utilizaram-se informações genealógicas de 338 animais registrados no período de 1991 a 2019. Foi estimado o número de animais fundadores (Nf), o número efetivo de fundadores (fe), o número efetivo de ancestrais (fa), o coeficiente de endogamia (F) e o coeficiente de parentesco médio (AR), além do Fis, Fit e Fst. Foi possível identificar ancestrais até a terceira geração, com aumento crescente das informações ao longo das gerações. Do total de informações avaliadas, 90,53% possuíam identificação do pai e da mãe. O tamanho efetivo da população foi inferior ao mínimo proposto pela FAO, o que sugere a necessidade de redefinir as estratégias do plano de gestão e de manejo genético do rebanho, de modo a promover fluxo gênico e expansão da raça.(AU)

Background@#With the enactment of the Brazilian Law Arouca 11,794/2008 and Decree 6.899/2009, there has been an urgent need for changes in the processes related to animal experimentation in Brazil; in particular, there is a need for improvements in enhancements of the lab animal management. To improve the management capacity of the Lab animal facility of the Carlos Chagas Institute’s Laboratory Animals Science (LAS), BioterC software was developed and implemented in 2014 for tracking mouse laboratory colonies. Five years after the implementation of this software, we sought to analyze the information in the database originated from BioterC using the Exploratory Analysis Data methodology (EDA). This article aims to identify animal breeding patterns using a data mining tool (Data Science) with Python programming language. @*Results@#The results show that from September 2014 to June 2019, under the license IACUC number LW- 6/17, 15.106 animals were produced. The C57BL/6, BALB/c and Swiss strains were the most frequently produced strains.The distribution of births due to crosses between these strains showed a median of 6 to 10 animals, depending on the genetic homozygosis and heterozygosis of the animal. The median number of days of mating was 35 days. In the sexing period, the records reported a median of 19 days. A total of 393 requests for animals from internal and external laboratories were registered. It was noted that approximately half of the animals produced to meet the demand for orders were discarded. Of the 15,106 animals, 38% were requested for animal experimentation, 58% were discarded and 4% did not have an outcome recorded in the data. @*Conclusions@#This volume of data provides an initial view of the information retrieval capabilities contained in BioterC, allowing for unique breeding knowledge by installing laboratory animals.

ABSTRACT

@#PURPOSE: This study was done to explore the experiences of young adult males with below-knee amputation at surgical wards and clinics in Teaching Hospital, Kurunegala, Sri Lanka. DESIGN: The study used the phenomenology design of qualitative research. METHODS: Face-to-face interviews were conducted for data collection using a theme list with a saturation point reached at 16 participants. The duration of an interview was 30-45 minutes and was recorded on audiotape and transcribed later. Phases of familiarization with the data, coding, searching for themes, reviewing themes, defining and naming themes, and writing up were conducted accordingly to thematic analysis. FINDINGS: The young adult male below-knee amputees showed changes in lifestyle with dependence on others, supporting aids, and experiencing discomfort. They showed mental distress with dissatisfaction with life and suffering of life. Additionally, they lacked power with identity changes and encounter economic problems in their lives. CONCLUSIONS AND RECOMMENDATIONS: It is essential to give special education to the staff caring for the below-knee amputees to minimize their unpleasant and challenging experiences. Health staff can motivate family members and friends for individuals' mental adjustment to their new life. Additionally, the social attitude of the community should be changed positively regarding amputated patients.
