ABSTRACT

OBJECTIVE To analyze the dose-adjusted concentrations of Posaconazole oral suspension in patients undergoing hematopoietic stem cell transplantation (HSCT) and their influential factors. METHODS Data were collected from hospitalized HSCT patients admitted to the First Affiliated Hospital of Shandong First Medical University (Shandong Provincial Qianfoshan Hospital) from January 2021 to April whtwhm@yeah.net 2023 who took Posaconazole oral suspension for the prevention of invasive fungal disease (IFD) and received blood concentration of posaconazole. The rate of concentration attainment and clinical failure rate of posaconazole for the prevention of IFD were evaluated, and one-way and multiple linear regression analyses were performed for the influential factors of dose-adjusted concentrations (C0/D) of posaconazole. RESULTS A total of 44 patients were enrolled; the mean C0 of posaconazole in patients was (0.99±0.94) µg/mL, and 20 patients had a C0≥0.7 μg/mL, with a concentration attainment rate of 45.45% for the prevention of IFD; 13 cases were clinical failures, with a clinical failure rate of 29.55%. Of 24 patients who did not achieve C0/D of posaconazole for IFD prophylaxis, one patient was a clinical failure despite timely dose adjustment of posaconazole in seven patients; seven of the thirteen patients who did not undergo dose adjustment were clinical failures; and the remaining four patients were switched to other antifungal agents. The results of univariate analysis showed that gender, body mass index (BMI), renal function, combined use of sodium phenytoin, omeprazole and metoclopramide had a significant effect on the C0/D of posaconazole (P<0.05); the results of multivariate linear regression analysis showed that gender, BMI and combined use of sodium phenytoin were the independent factors affecting the C0/D of posaconazole (P<0.05). CONCLUSIONS Significant individual differences are reflected in the blood concentration of Posaconazole oral suspension; gender, BMI and combined use of sodium phenytoin are independent factors affecting the C0/D of posaconazole.

ABSTRACT

OBJECTIVE@#To study the biomechanical characteristics of InterTan for the treatment of femoral intertrochanteric fracture of Evans-Jensen IV.@*METHODS@#Scanning the femur and internal plant of volunteers with spiral CT to obtain DICOM format data. Three-dimensional models of left femur and InterTan were reconstructed by Mimics software. On this basis, a three-dimensional finite element model of internal fixation for Evans-Jensen IV intertrochanteric fracture of femur was established. The stress and microstrain distribution of Von Mses in different models were studied. The biomechanical stability after internal fixation of the Evans Jensen IV femoral intertrochanteric fracture was analyzed.@*RESULTS@#The stress pattern of the femur of InterTan model was the same as that of the normal femur, which was mainly located on the medial side of the proximal femur and the lower third of the femur. However, the stress of femur in InterTan model was lower than that in the same part of normal femur. The peak stress of the femur in the model was 13.92 MPa, located at the end of the inner plant in contact with the femur. The stress peak of the plant in the model was 146.5 MPa at the lower contact point between the tension nail and the main nail.@*CONCLUSIONS@#InterTan fixation has obvious biomechanical advantages and is not easy to cause stress fractures in the middle femur in patients with osteopenic Evans-Jensen IV intertrochanteric fractures. In particular, for patients with greater activity in the intertrochanteric fracture of the Evans-Jensen IV femur, InterTan fixation has better stability and provides a theoretical basis for the choice of internal fixation.

ABSTRACT

Objective To investigate the distribution of peripheral arterial stiffness,endothelial function and their correlations with cardiovascular risk factors in the 7-17 year-olds.Methods Normal weight and obese subjects aged 7-17 years with completed data on questionnaires,anthropometric and blood biochemical tests,were recruited from a cross-sectional population-based study on childhood hypertension in Minhang district of Shanghai.Automatic waveform analyzer (BP-203RPE-I) and Endopat 2000 were used to measure the arterial stiffness.Endothelial function with brachial-ankle pulse wave velocity (baPWV) and reactive hyperemia index (RHI) were recorded.BaPWV and RHI were standardized by age.Skewed biochemical variables were log transformed.Linear correlation analysis was performed to observe association between baPWV,RHI and other measured variables.Results A total of 452 normal-weight and 94 obese subjects were recruited,including 299 males.Data showed that baPWV and RHI increased with age in normal weight subjects (r=0.33,P<0.01;r=0.36,P<0.01).Results from Linear correlation analysis revealed that baPWV was positively correlated with BMI (r=0.13,P=0.002),systolic blood pressure (SBP) (r=0.20,P<0.01),diastolic blood pressure (DBP) (r=0.27,P<0.01),triglycerides (TG) (r=0.11,P=0.010),insulin (r=0.21,P=0.004) and the HOMA insulin resistance index (r=0.21,P=0.005),but negatively correlated with high-density lipoprotein cholesterol (r=-0.09,P=0.039).RH1 was positively correlated with BMI (r=0.10,P=0.018) but negatively correlated with DBP (r=-0.10,P=0.016).Males had higher baPWVs than females (P=0.04).However,RHI did not differ between genders.Conclusions The fact that baPWV and RHI increased along with age,indicated that the arterial stiffness and endothelial function continued to develop in normal weight childhood and adolescence.Arterial stiffness was correlated with cardiovascular risk-related parameters whereas endothelial function was not.BaPWV might be more sensitive in evaluating the cardiovascular risk in children and adolescents than RHI did.
