ABSTRACT

BACKGROUND: Chronic kidney disease is a severe threat to human health with no ideal treatment strategy. Mature mammalian kidneys have a fixed number of nephrons, and regeneration is difficult once they are damaged. For this reason, developing an efficient approach to achieve kidney regeneration is necessary. The technology of the combination of decellularized kidney scaffolds with stem cells has emerged as a new strategy; however, in previous studies, the differentiation of stem cells in decellularized scaffolds was insufficient for functional kidney regeneration, and many problems remain. METHODS: We used 0.5% sodium dodecyl sulfate (SDS) to produce rat kidney decellularized scaffolds, and induce adipose-derived stem cells (ADSCs) into intermediate mesoderm by adding Wnt agonist CHIR99021 and FGF9 in vitro. The characteristics of decellularized scaffolds and intermediate mesoderm induced from adipose–derived stem cells were identified. The scaffolds were recellularized with ADSCs and intermediate mesoderm cells through the renal artery and ureter. After cocultured for 10 days, cells adhesion and differentiation was evaluated. RESULTS: Intermediate mesoderm cells were successfully induced from ADSCs and identified by immunofluorescence and Western blotting assays (OSR1 + , PAX2 +). Immunofluorescence showed that intermediate mesoderm cells differentiated into tubular-like (E-CAD + , GATA3 +) and podocyte-like (WT1 +) cells with higher differentiation efficiency than ADSCs in the decellularized scaffolds. Comparatively, this phenomenon was not observed in induced intermediate mesoderm cells cultured in vitro. CONCLUSION: In this study, we demonstrated that intermediate mesoderm cells could be induced from ADSCs and that they could differentiate well after cocultured with decellularized scaffolds.

ABSTRACT

Objective To analyze the clinical efficacy and safety of percutaneous microwave ablation guided by multimode ultrasound in the treatment of benign thyroid nodules.Methods The clinical data of 100 cases of benign thyroid nodules admitted from June 2017 to December 2018 were retrospectively analyzed.50 cases of benign thyroid nodules were treated by traditional open surgery as the control group,and 50 cases of benign thyroid nodules were treated by ultrasound-guided microwave ablation as the observation group.The complete clearance rate of lesion tissue and the incidence of comphcations were compared between the two groups.Results (1) The nodule clearance rate of the observation group was higher than that of the control group,with significant statistically difference (P < 0.05).Compared with the control group (transient thyroid dysfunction 16%,recurrent laryngeal nerve injury 10%),the incidence of complications (transient hypothyroidism 2%,recurrent laryngeal nerve injury 0%) was significantly lower in the observation group (P < 0.05).(2) The quality of life scores of the observation group and the control group were significantly higher than those before the treatment (P < 0.05);The quality of life score of the observation group was significantly higher than that of the control group (P < 0.05).Conclusions Compared with traditional open thyroidectomy,multimodal ultrasound-guided percutaneous microwave ablation has better efficacy and safety,which is worthy of wide application and promotion.

ABSTRACT

Objective To explore the value of microvascular angiography in evaluating the neovascularization of internal carotid artery plaque and the prediction of cerebral infarction.Methods A total of 100 patients with suspected cerebral infarction received from March 19th,2016 to December 1 st,2017 in our hospital were enrolled in this study.All patients underwent microvascular imaging and then analyzed the importance of this technique in evaluating cerebral infarction,plaque neovascularization.Results 100 lesions were recorded in 100 patients,among which there were 22 cases with no superb microvascular ultrasound imaging (SMI) and blood flow signal (ie,score 0) in the plaque.SMI imaging could better show neovascularization (ie score 1 or 2 points).In 78 cases,they all grew from the arterial wall to the plaque.There were 61 cases of cerebral infarction in the blood flow group (78.2%),8 cases (36.3%) of cerebral infarction in the group without blood flow,with statistical significance(P =0.023).SMI imaging was highly sensitive to the prediction of cerebral infarction incidence,reaching 88.4%.Conclusions Ultra-microvascular imaging can improve the sensitivity of cerebral infarction prediction and promptly detect neovascularization of internal carotid artery plaque,thus providing a basis for clinical treatment.

ABSTRACT

Objective To investigate the training expectations and training strategies of orthopedic clinical specialist nurses (OCSN).Methods Totally 5 020 orthopedic nurses from 342 hospitals from 30 provinces and cities were selected bv convenience sampling.Participants were investigated hy WeChat platform with self-designed questionnaire.Results A total of 4 982 effective questionnaires were collected.Among investigated nurses,13.7% of them received orthopedic specialist nurses training;91.4% believed that training should be carried out.Qualifications should be:college degree and above,senior nurse and above,at least 3 ~5 years of nursing experience,1~3 years of nursing experience in orthopedic department.For curriculum,public courses expected to be arranged were communieation skills,nursing teaching,nursing management,and nursing research;expected professional courses were functional exercises,pain management,extremity injury nursing,common treatment techniques in orthopedic department,traction nursing,and position nursing.It was suggested that training was divided into subspecialties and off-duty;classroom teaching,teaching rounds,case discussion and experience exchange were expected training methods;"theory-practice-theory-practice" was most preferable traiuing mode.The preferred teachers were orthopedic doctors,orthopedic head nurses,orthopedic specialist nurses,and senior orthopedic nurses.The length of training was expected to be three months,time for clinical practice should be greater than or equal to theoretical teaching,and the practice bases should be tertiary hospitals.Evaluation should be performed before completion,recertification could be later than completion,and the interval time of recertification should be within 5 years.Conclusion Training needs of orthopedic nurses for OCSN are strong.Training expectations(contents,length,modes and teachers) of orthopedic nurses should be considered when designing systematic training program on OCSN.
