ABSTRACT

Objective:To compare the effects of patellar position on the balance of soft tissue during the operation and the postoperative outcomes in minimally invasive total knee arthroplasty via a midvastus approach.Methods:From December 2018 to February 2019, a total of 55 patients were enrolled for primary total knee arthroplasty via a midvastus approach. All patients were randomly divided into 2 groups by random number table. During the operation, the gap balance technique was used with patella reduced or subluxated to complete the osteotomy and balance of soft tissue. The changes of gap and varus-valgus angle were compared between the patellar reduction group and the patellar subluxation group in both extension and flexion position. These data were also compared before and after reducing patellar in the patellar subluxation group. Furthermore, the differences of femoral prosthesis rotation, mechanical femoral axis to tibial axis angle, Knee Society score (KSS), visual analogue scale (VAS) and range of motion (ROM) were compared between the two groups.Results:All patients finished 6 months follow-up, including 27 patients in the patellar reduction group and 28 patients in the patellar subluxation group. After osteotomy and soft tissue balance during the operation, there was no significant difference in gap and varus-valgus angle between two groups in either extension or flexion position. While in the patellar subluxation group, the flexion gap was 10.5±0.3 mm with patella subluxated, less than 11.0 ± 0.3 mm after reducing the patella. The varus-valgus angle was 1.5±0.3 with patella subluxated, less than 2.3±0.4 degree after reducing the patella. The difference was statistically significant ( t=4.180, P<0.001; t=7.642, P<0.001). There was no significant difference in extension gap and varus-valgus angle before and after patella reduction in the patellar subluxation group ( P>0.05). The rotation angle of the femoral component in the patellar subluxation group was -0.49°±1.2°, and the external rotation angle was smaller than that in the patellar reduction group (0.24°±1.3°). The difference was statistically significant ( t=2.116, P=0.039). At one month after operation, ROM of the patellar reduction group was 109.6°±8.5° which was higher than that of the patellar subluxation group (104.9°±8.6°, t=2.048, P=0.046). There was no significant difference in ROM between the two groups at 3 and 6 months ( P>0.05). Moreover, there was no significant difference in KSS and VAS at 1, 3 and 6 months after operation ( P>0.05). Conclusion:In the minimally invasive total knee arthroplasty, it is suggested to balance the soft tissue as much as possible with the patellar reduced. Otherwise, the consequence of increased flexion space, increased varus and increased internal rotation of femoral prosthesis should be considered. The patients undergoing soft tissue balance with patella reduced have better ROM in the early stage postoperatively.

Objective@#To compare the difference in imaging and clinical effects between gap balance technique and measured resection technique in total knee arthroplasty (TKA).@*Methods@#We recruited 300 patients undertaking TKA from July 2014 to July 2016. TKA were performed by using gap balance technique or measured resection technique randomly. The intraoperative rotation of femoral prosthesis, changes of joint line, osteotomy volume, preoperative and postoperative KSS score, WOMAC score, VAS and ROM were compared between the two groups.@*Results@#A total of 292 patients were included in the present study, and 248 patients were successfully followed up for 2 years. The operation duration of the gap balance group was 65.9±14.1 min, which was significantly less than that of the measured osteotomy group 70.5±16.5 min (t=2.36, P=0.02). The rotation angle α of the femoral prosthesis measured on CT scan postoperative in gap balance group was 0.67°±1.71°, which was significantly smaller than that of the measured resection technique group 1.25°±2.53° (t=2.12, P=0.03). The external rotation angle β between the osteotomy plate and femoral posterior condyle line in the gap balance group was 1.25°±1.26°, which was significantly greater than that of the measured resection technique 0.28°±0.19° (t=6.57, P<0.01). The bone cut volume of femoral medial posterior condyle in gap balance group 9.85±1.37 mm showed a statistically difference (t=2.02, P=0.04) compared with that of the measured osteotomy group 9.52±1.19 mm. The KSS score was 83.8±10.2 in the gap balance group after 1 month. It was superior to the measured resection technique group of 80.5±9.4 with statistical significance (t=2.65, P=0.01). The knee ROM was 101.7°±12.7° in the gap balance group at 1 month after surgery, which was better than that in the measured resection technique group 98.2°±11.8° (t=2.25, P=0.03).@*Conclusion@#The axis of the femoral prosthesis was more consistent with STEA in the gap balance group of TKA. The gap balance group had better ROM and KSS functional scores at the first month after surgery. However, there was no statistical difference in knee function at the 2-year follow-up between the two groups.

ABSTRACT

Objective To investigate the effect of aloe-emodin on Akt/mTOR,apoptosis and migration of autophagy-related pathway in colorectal cancer LOVO cells. Methods The LOVO cells were divided into control group(normal culture),low dose group(LOVO cells+10 μmol/L aloe-emodin culture 30 min), medium dose group (LOVO cells + 30 μmol/L aloe-emodin culture 30 min) and high dose group (LOVO cells + 50 μmol/L aloe veratin culture 30 min). The cell proliferation rate was measured by CCk-8 method at 24,48 and 72 h respectively. The apoptotic rate of each group was detected by flow cytometry. The migration ability of each group was detected by Transwell chamber. Western blot was used to detect the expression of IC3-Ⅰ,IC3-Ⅱ,Beclin-1,mTOR,p-mTOR,Akt and p-Akt.Results Compared with that of the control group,the cell rate,migration ability and apoptosis rate of the low dose group was not significantly changed(P>0.05).The cell rate and migration ability of the medium and high dose groups was significantly decreased and the apoptosis rate was significantly increased:0.43 ± 0.03,0.59 ± 0.04 vs.0.16 ± 0.00;0.57 ± 0.07,1.06 ± 0.17 vs.0.34 ± 0.02;0.37 ± 0.02, 0.49 ± 0.01 vs.0.13 ± 0.00,P<0.05,there was dose dependent(P<0.05).Compared with those of the control group,the expressions of IC3-Ⅰ,IC3-Ⅱ,Beclin-1,mTOR,p-mTOR,Akt and p-Akt in the low dose group showed no significant change(P>0.05),those were decreased in middle and high dose groups which showed significant differences:0.85 ± 0.08,0.37 ± 0.02 vs.2.08 ± 0.07;1.42 ± 0.09,1.19 ± 0.02 vs.1.97 ± 0.11;0.97 ± 0.00,0.84 ± 0.06 vs.1.19 ± 0.02;0.43 ± 0.02,0.31 ± 0.01 vs.0.98 ± 0.08,P<0.05, there were dose dependent (P < 0.05). Conclusions Aloe-emodin can promote autophagy, apoptosis there inhibit the growth,migration of colorectal cancer LOVO cells by reducing the expression of Akt/mTOR signaling pathway and reducing its phosphorylation level.

ABSTRACT

Objective To investigate the distribution of LAP+CD4+T cells in gastric carcinoma microenvironment and the correlation of LAP + CD4 + T cells with the progression of gastric tumor. Methods Forty gastric tumor patients and 20 healthy donors were enrolled in this study. The percentage of LAP+CD4+T cells in the peripheral blood and tumor tissue was detected by flow cytometry. The correlation of LAP+CD4+T cells with the progression of gastric tumor was analyzed. Results The percentage of LAP+CD4+T cells in the peripheral blood of gastric tumor patients was higher than that of health donors:(10.9±3.3)%vs. (4.3 ± 1.2)%;the percentage of LAP+CD4+T cells from tumor tissue was higher than that from non-tumor tissue:(13.5 ± 5.3)%vs. (4.7 ± 1.4)%. The percentage of LAP+CD4+T cells in peripheral blood from metastasis patients was higher than that from non-metastasis patients and health donors: (10.1 ± 6.4)% vs. (4.5 ± 1.3)% and (4.3 ± 1.2)%. Conclusions LAP+ CD4+ T cells accumulates in gastric carcinoma microenvironment and the percentage of LAP+CD4+T cells increases along with the progression of gastric tumor.

ABSTRACT

Objective To investigate the effects of metformin and exaliplatin on proliferation and apoptosis of gastric cancer cell line SGC-7901. The antitumor effects of metformin combined with exaliplatin on gastric cancer cell line SGC-7901 were compared. Methods SGC-7901 cells were cultured in vitro. The proliferation inhibition rate of cancer cell after treatment with various concentrations of metformin(5.0,10.0,15.0 mmol/L)、exaliplatin(2.5,5.0,10.0 μmol/L)or 15.0 mmol/L metformin combined with 10.0 μmol/L exaliplatin was analyzed via MTT colorimetric assay at 24,48,72 h.After 24 h, the SGC-7901 cell apoptosis was detected by flow cytometry, and the relative activity of caspase-3 and bax was detected by western blot. Results The co-administration of metformin and exaliplatin or single agent treatment could increase the proliferation inhibition rates of SGC-7901 cell in a time-and dose-dependent manner, while there was a significant higher proliferation inhibition rate in co-administration versus single-agent treatment (P < 0.05). The apoptosis rate induced by 15.0 mmol/L metformin and 10.0 μmol/L exaliplatin and combined treatment was (16.0 ± 2.1)%, (19.0 ± 2.7)%, (29.0 ± 3.5)%, higher than that in blank control group (10.0 ± 1.1)%, and there was significant difference (P <0.05). Both of metformin and exaliplatin could increase caspase-3 and Bax activity.Caspase-3 and Bax activity in the combination of metformin and exaliplatin was significantly higher than that of metformin group and exaliplatin group (P < 0.05). Conclusions Metformin combined with exaliplatin has synergistic effect in inhibiting the growth and inducing apoptosis of gastric cancer cell line SGC-7901.Enhancing caspase-3 and Bax expression may be the related mechanism

ABSTRACT

Objective To observe the effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes.Methods Sixty diabetes patients were divided into two groups randomly:treatment group were received Sitagliptin Phosphate Tablets and shenmai;the control group were given acarbose.14 weeks later,the indications FBG,2HBG,HbA1 C,TC,TG,LDL-C,HDL-C,hypoglycemia happen rate,BMI,blood rheology,transcranial Doppler ultrasound(TCD)were tested to see if there is any change.Results After treatment,two groups of FBG,2hBG,HbA1 C,TC,TG,LDL-C,HDL-C levels compared with those before treatment were statistically significant(P<0.05 or P<0.01),but the treatment group on lipid control more than those in the control group;treatment group therapy in reducing the risk of hypoglycaemia and weight gain than the control group(P<0.05);the whole blood viscosity,plasma viscosity,low viscosity and hematocrit of treatment group were significantly lower(P<0.05),while the control group showed no significant change;the abnormal rate of treatment group was 52.6%,significantly better than the control group 86.5%(P<0.05).Conclusion The effect of Sitagliptin Phosphate Tablets combined with shenmai on type 2 diabetes was significant,compared with the traditional antidiabetic drugs,the combination is effective hypoglycemic control fat and less adverse reactions.

ABSTRACT

Objective To investigate the chronic complications of diabetes-related conditions and associated risk factors. MethodsClinical data of 738 diabetic patients with chronic complications were retrospectively analyzed.The patients age,duration,glycosylated hemoglobin levels,smoking and family history,body mass index,systolic blood pressure,diastolic blood pressure,triglycerides,cholesterol,high density lipoprotein,low-density lipoprotein-related risk factors etc were analyzed. Results The diabetic patients with chronic complications,was neuropathy,diabetes,eye disease and diabetic nephropathy accounted for the top three,followed by the lower limb vascular disease,cardiovascular disease and cerebrovascular disease.The relevant factors analysis revealed that influential factors had relationship with il1 ness course,age,drinking alcohol,blood pressure and blood-fat Conclusion There were many reasons for diabetic chronic complications,blood sugar.blood pressure,blood lipid should be actively controlled,and regular health education should be requested.

ABSTRACT

Objective To explore the inchoate changes in patients with Graves disease (GD) treated with 131I and appraise the therapeutic effect. Methods According to the thyroid size, patients’ age, thyroid 131I intake rate and course of the disease, 303 patients with GD were given individual 131I doses. Six months later, the symptom, signs, and thyroid hormone levels of every case were compared with those recorded before. Results The symptom and the level of thyroid hormones were significantly meliorated after 131I treatment. Six cases became goggle-eyed after treatment. Among the 303 patients, 249(82.18%) fully recovered, 21 (6.93%) partially remitted, 24(7.92%) developed hypothyroidism, and 9(2.97%) showed ineffective. Conclusion 131I treatment for GD with an individual dose can exert a satisfied therapeutic efficacy. It may as serve the first choice for GD treatment.

ABSTRACT

Objective To evaluate the therapeutic effect and side effects of 89Sr metastron on treating multiple osseous metastasis tumor. Methods The total 83 patients had been treated by intravenously injecting 89Sr metastron at a dose of 2.22 MBq/kg. Results Among 83 cases, 31.3% of them got complete remission, 37.3% moderate remission, and 18.1% slight remission. The total effective rate was 86.7%. The side effects were mild, and only white blood cells and platelets transiently fell by 20%. Conclusion Internal radiotherapy with 89Sr metastron is the good alternative in treating multiple osseous metastasis tumor.
