ABSTRACT

As fístulas das artérias coronárias são anomalias congênitas raras e caracterizam uma comunicação anormal entre as artérias coronárias e as câmaras cardíacas ou grandes vasos. Muitos dispositivos que incluem vários coils, plugues vasculares, oclusores e stents recobertos têm sido usados no fechamento transcateter de fístulas de artérias coronárias. Neste relato, realizamos com sucesso o fechamento de uma fístula de artéria coronária, entre a artéria descendente anterior e o ápice do ventrículo direito, usando o dispositivo Amplatzer PiccoloTM, anteriormente chamado de tamanho adicional do AmplatzerTM Duct Occluder. Em nossa opinião, o dispositivo Amplatzer PiccoloTM é adequado para oclusão transcateter de fístula da artéria coronária, podendo ser usado no lugar de plugues vasculares.


Coronary artery fistulas are rare congenital anomalies and characterize an abnormal communication between coronary arteries and cardiac chambers or major arteries. Many devices, including various coils, vascular plugs, occluders, and cover stent, have been used in transcatheter closure of coronary artery fistulas. In this report we successfully closed coronary artery fistula, between left anterior descending artery and right ventricle apex, by using Amplatzer PiccoloTM device, formerly called as AmplatzerTM Duct Occluder additional size. In our opinion, Amplatzer PiccoloTM device is suitable for transcatheter occlusion of coronary artery fistula and can be used instead of vascular plugs.

ABSTRACT

We present a case of successful repair of the mitral valve for active infective endocarditis. Mitral valve repair was performed through debridement of vegetation and abscess, resection and repair of the posterior mitral leaflet and posterior repair with autologous pericardium. Postoperative period was uneventfully, with no evidence of recurrent infection, and echocardiogram showed mitral valve competence with mild mitral regurgitation. We demonstrate that valve repair is a feasible choice in cases of active endocarditis in children.


Apresentamos um caso de reparo bem-sucedido da valva mitral decorrente de endocardite infecciosa ativa. Reparo da valva mitral foi acompanhado de desbridamento da vegetação e do abscesso, ressecção e plastia do folheto mitral posterior, e anuloplastia posterior com pericárdio autólogo. Pós-operatório sem evidências de infecção recorrente, e ecocardiograma demonstrou competência da valva mitral com regurgitação mitral trivial. Concluímos que o reparo valvar é uma escolha viável em caso de endocardite ativa nas crianças.

ABSTRACT

OBJECTIVE: Intoxication is one of the most common causes of admissions to emergency department in pediatric age group. Incidence of iron poisoning gradually increased because of wide spread use of iron containing drugs. METHOD: In this report, we present five cases of iron ingestion who were admitted to our emergency department within a year. RESULT: Whole bowel irrigation in addition to gastric lavage with an iron dose of over 50 mg/kg as well as deferoxamine treatment for patients in whom clinical and laboratory indications are present. CONCLUSION: The prompt recognition and treatment of children with acute iron poisoning is the single and the most critical point for decreasing the morbidity and mortality associated with iron containing products.
