ABSTRACT

ObjectiveTo investigate the clinical efficacy of Niaoxue No.1 Prescription in treating Henoch-Schönlein purpura (HSP) nephritis with blood heat and stasis syndrome and its effect on urine erythrocyte, urine protein, blood neutrophils, and blood routine-derived indicators. MethodA multicenter, randomized controlled trial (RCT) was conducted involving 108 HSP nephritis patients from three hospitals. The patients were randomly divided into a control group (54 cases) and a treatment group (54 cases). The treatment group received Niaoxue No.1 prescription once daily, while the control group was treated with captopril and ferulic acid tablets. Both groups underwent a 4-week course of treatment. The urine erythrocyte, urine microalbumin (mAlb), urine sediment red blood cell count, traditional Chinese medicine (TCM) syndrome score, 24-hour urine protein, blood neutrophil count, neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), lymphocyte to monocyte ratio (LMR), D-dimer, and immunoglobulin A were detected. The recurrence rate of HSP nephritis was followed up for 6 months. ResultThe total effective rates were 88.9% (48/54) in the treatment group and 70.4% (38/54) in the control group, and the treatment group was superior to the control group (χ2=5.708, P<0.05). Compared with the results before treatment, after 14 days of treatment, the TCM syndrome total score, urine erythrocyte, urine mAlb, and 24-hour urine protein in both groups significantly decreased (P<0.05,P<0.01), and the improvement was more significant in the treatment group than the control group (P<0.05). After 28 days of treatment, compared with the results before treatment, the TCM syndrome total score, urine erythrocyte, urine mAlb, urine sediment red blood cell count, D-dimer, and 24-hour urine protein in both groups significantly decreased (P<0.05,P<0.01), with the treatment group showing a more significant reduction in urine mAlb than the control group (P<0.05). On the 14th and 28th days of treatment, the neutrophil percentage and NLR were lower in the treatment group than in the control group (P<0.05), while there was no statistically significant difference in PLR and LMR. The recurrence rate of nephritis in both groups showed no statistically significant difference after a 6-month follow-up. ConclusionNiaoxue No.1 Prescription in the treatment of HSP nephritis with blood heat and stasis syndrome can significantly improve clinical symptoms, shorten the course of the disease, and reduce urine erythrocyte, urine mAlb, 24-hour urine protein, blood neutrophils, and NLR, thereby effectively alleviating the inflammatory state and reducing kidney damage in children with HSP nephritis.

ABSTRACT

With the increasing detection rate of early upper gastric cancer and adenocarcinoma of esophagogastric junction, the safety of proximal gastrectomy with clear indications has been verified, and function-preserving proximal gastrectomy has been widely used. However, proximal gastrectomy destructs the normal anatomical structure of esophagogastric junction, resulting in severe postoperative gastroesophageal reflux symptoms and seriously affecting the quality of life. Among various anti-reflux surgery methods, reconstruction of "cardiac valve" has always been the focus of relevant scholars because its similarity with the mechanism of normal anti-reflux. After years of development, evolution and optimization, the designed seromuscular flap anastomosis includes tunnel muscle flap anastomosis, Hatafuku valvuloplasty, single muscle flap anastomosis and double muscle flap anastomosis. The double muscle flap anastomosis has become a research hotspot because it shows good anti-reflux effect in clinical application. This paper reviews the history, research status and hot issues of seromuscular flap anastomosis of esophageal remnant stomach at home and abroad.

ABSTRACT

The quality control and standardization of procedures in radical gastrectomy for gastric cancer, especially the standardized processing of specimens after radical gastrectomy for gastric cancer, is very important. It is not only the basis of accurate pathological staging, but also the evidence of surgical quality and the original data of clinical research, which plays a pivotal role. The examination and classification of lymph nodes, specimens processing records, and data uploading and archiving after radical gastrectomy for gastric cancer are indispensable. It is necessary for surgeons to participate in the processing of surgical specimens. This article will combine the current research status and progress at home and abroad to review the standardized processing of specimens after radical gastrectomy for gastric cancer.

ABSTRACT

@#AIM: To study the effect of preoperative meibomian gland physiotherapy on ocular surface after cataract surgery in patients with meibomian gland dysfunction(MGD). <p>METHODS: Totally 76 eyes of 76 cataract patients with MGD were selected and divided into two groups by random grouping method in our hospital. Patients in the experimental group was given eyelid hot compress and meibomian gland massage before operation(after a full set of treatment in the hospital, patients were instructed to have family hot compress and meibomian gland massage every night), and patients in the control group was not given any treatment. Cataract phacoemulsification and intraocular lens implantation were performed in both groups. The non-invasive tear break-up time(NITBUT)and the tear meniscus height(TMH)were examined by ocular keratograph, the thickness of lipid layer(LLT)were examined by LipiView, and the Ocular Surface Disease Index(OSDI)questionnaire was conducted at 1wk before operation and 1wk, 1mo after operation.<p>RESULTS: The NITBUT, TMH and OSDI were significantly different between the two groups before and after operation(<i>P</i><0.05). The NITBUT were significantly lower than in both groups 1wk and 1mo after operation than those before operation(<i>P</i><0.05). The NITBUT in the control group were significantly lower than those in the experimental group 1wk and 1mo after operation(<i>P</i><0.05). The TMH was significantly lower one week after operation in the experimental group than those before operation(<i>P</i><0.05), and returned to the preoperative level 1mo after operation(<i>P</i>>0.05). The OSDI were significantly improved after operation compared with those before surgery in both groups 1wk and 1mo(<i>P</i><0.05). The OSDI in the control group were significantly higher than those in the experimental group 1wk and 1mo after operation(<i>P</i><0.05).<p>CONCLUSION: Phacoemulsification of cataract can destroy the function of meibomian gland and the ocular surface condition in patients with meibomian gland dysfunction. However, preoperative meibomian gland physiotherapy can significantly improve the ocular surface condition of patients with meibomian gland dysfunction after cataract surgery, and improve the satisfaction of patients.

ABSTRACT

Objectivc To compare the accuracy and practicability of OPD-Scan Ⅲ marker methods and slit light narrow band marker methods in phacoemulsification and Toric IOL implantation.Methods A prospective randomized controlled study was conducted in 70 eyes of 70 patients with regular astigmatism (1.00-3.00 D).All patients underwent phacoemulsification and Toric IOL implantation and were randomly divided into two groups according to preoperative marker methods:observant group,in which OPD-Scan Ⅲ marker methods were used in 35 eyes of 35 patients,and control group,with application of slit light narrow band marker methods in 35 eyes of 35 patients.The postoperative visual acuity,residual astigmatism and deviation of lens axis (LAD) were observed and compared between the two groups.Results The uncorrected visual acuity (UCVA) between preoperative data and 3 months after surgery was statistically significant [(0.11 ± 0.09) vs.(0.59 ± 0.25)] (P =0.000).The difference in the postoperative 1 day and 3 months LAD was no statistically significant (P > 0.05).The astigmatism between preoperation and 3 months after treatment was significant [(1.88 ± 0.49) D vs.(0.54 ± 0.30) D] (P =0.000).There was no significant difference in LAD between the two groups neither the first day nor 3 months after operation (both P =0.621).Conclusion OPD-Scan Ⅲ marker methods and slit light narrow band marker methods are both highly accurate and practical,but the former requires less for operator and is more easily accepted by patients.

ABSTRACT

<p><b>OBJECTIVE</b>To study the CT axial manifestations of iliolumbar ligament(ILL) and discusses its clinical effects on locating lumbosacral vertebral segments.</p><p><b>METHODS</b>From May 2008 to March 2010, 706 adult patients diagnosed lumbar disc disease were performed with axial scans by single slice helical CT. Among the patients, 436 patients were male and 270 patients were female, ranging in age from 25 to 82 years, the median age was 44 years, 78 cases with lumbosacral transitional vertebrae (LSTV) were verified by X-radiography or fluoroscopy. The morphology, origin and insertion, courses of ILL and the relationship of ligament and spinal segments on axial plane images were used to study. The location method of spinal segments by ILL was compared with the other four location methods on CT.</p><p><b>RESULTS</b>Of the 628 cases with normal lumbosacral segmentations sides of ligament, the main part of ILL originated from L5 transverse processes and terminated at the iliac crest, the morphological characters were divided into two types: double band (71.8%, 451/628) and single band (28.2%, 177/628). The tiny branches from posterior and outside edge of L4, lumbar disc were seen simultaneity in 3 cases. The ILL of 78 cases with LSTV all also originated from L5 transverse processes. Using ILL as a marker of the L5 vertebral level, 78 cases with LSTV were correctly numbered, the accuracy rate was higher than the other location methods, there was statistical significance between the location method by ILL and the location method by iliac crest (P < 0.05).</p><p><b>CONCLUSION</b>The main part of ILL originates from L5 transverse processes, the anatomic location is relatively steady and can be clearly displayed on axial CT, which can be used as a measure in the idenlification of LSTV in clinical practice, it is worthy to be applied widely in basic-level hospitals.</p>

ABSTRACT

Using naproxen as model drug, the formulation of microemulsion vehicle for transdermal delivery was optimized by genetic algorithm. The ranges of microemulsion composed of Tween 80, IPM, alcohol and water, were defined through construction of the pseudo-ternary phase diagrams. Systematic model microemulsions containing naproxen 1.12% were prepared by a three-factor, three-level center design method, and their diffusion studies via excised rabbit skin were performed. Using the quadratic regression model of steady-state permeation rate (Js) of naproxen as objective function, the consequence of center design experiment was optimized by genetic algorithm, and the formulation of microemulsion with highest permeation rate for naproxen was screened. The optimum formulation was composed of 21.41% Tween 80, 15.17% alcohol, 4.14% IPM, and 59.28% water, and the anticipated Js was 183.57 microg x cm(-2) x h(-1). The results of back substitution test showed the steady-state permeation rate of naproxen microemulsion prepared according to optimum formulation was 189.43 microg x cm(-2) x h(-1), which was higher than anticipated value. This result demonstrated optimizing formulation of microemulsion for transdermal delivery by genetic algorithm is feasible, reliable and reasonable.

ABSTRACT

The progress of Chinese medicine modernization is slow in china, and one of the reason is the dated process method. The fermentation technology of pharmaceutical fungi is more and more perfect. To process Chinese medicine by pharmaceutical fungi could enhance the effective density of active ingredient in Chinese medicine, and the fungi could produce a variety of active secondary metabolite, moreover, some compounds in Chinese medicine could be bio-transformed to produce new compound by pharmaceutical fungi in the fermentation. So process Chinese medicine by pharmaceutical fungi could produce de novo Chinese medicine preparations which include a variety of bioactive compounds, and it is suitable to produce in a large scale and convenient to control the quality. To process Chinese medicine by submerged fermentation of pharmaceutical fungi provides a new process method for Chinese medicine.

ABSTRACT

<p><b>OBJECTIVE</b>To study the radiobiological effects of fast neutron/photon mixed irradiation on human cancer cell in vitro and to discuss the mechanism in relation with cell cycle and apoptosis, thus to provide experimental support for the further application of fast neutron radiotherapy of cancer.</p><p><b>METHODS</b>Exponentially growing human nasopharyngeal cancer cell line CNE-1 was irradiated in vitro with 35 MeV p-->Be fast neutron and 6 MV-X ray in grading doses (0 cGy, 40 cGy, 80 cGy, 120 cGy, 160 cGy, 240 cGy, 320 cGy and 400 cGy for neutron, and 0 cGy, 100 cGy, 200 cGy, 300 cGy, 400 cGy, 600 cGy, 800 cGy and 1000 cGy for X ray). Clonogenic assay was performed, and relative biological effectiveness (RBE) of fast neutron was determined with D(10) by means of cell survival curves. Isoeffective doses of 35 MeV p-->Be fast neutron and 6 MV-X ray were obtained according to the RBE. The cells were assigned into two irradiation regimens, (1) the one-week-fractionation regimen, which adopted the radiation pattern of X x 5, N x 2 and X-N-X-X-N. After irradiation the clonogenic assay was performed to compare their survival fractions; (2) the two-dose regimen, with the radiation pattern of X + N, N + X and X + X. Flow cytometry was done at different time points after irradiation to analyze cell cycle distribution and apoptosis. Fast neutron dose was delivered on Tuesday and Friday, and all the other irradiation intervals were 24 h.</p><p><b>RESULTS</b>The RBE of fast neutron to X ray in CNE-1 cells according to the D(10) ratio was 2.40. The neutron isoeffective dose for a single dose of 200 cGy of 6 MV-X ray was approximately 80 cGy. In clonogenic assay, the cell survival fractions were significantly lower in X-N-X-X-N group (0.0079) than those in X x 5 (0.018) and N x 2 (0.017) groups. The flow cytometry suggested a higher percentage of apoptotic cells after mixed irradiation, and different sequence of X ray and neutron irradiations caused varying changes in cell cycle arrest.</p><p><b>CONCLUSION</b>Mixed irradiation of fast neutron and X ray showed a synergic effect in vitro on CNE-1 cell killing. Cell cycle arrest and apoptosis may play some role in the radiation damage repair mechanisms of mixed beam irradiation.</p>

ABSTRACT

Enzymatic synthesis of monoglycerides by glycerolysis of oil and fats in microaqueous solvent-free media was investigated by using lipase from pseudomonus fluorescens (PFL). Initial eutectic point(IEP) was substituted for melt point of oil and fats in Critical Temperature Theory. By investigating the glycerolysis under different IEP, it is showed that there is a relationship between composition of the oils and the yield of monoglycerides: Y = -0.0006 X3 + 0.0592 X2 - 0.8909 X + 26.753(13% < X < 76.5%), here X is the contents(W/W) of saturated fatty acid residue (C16 + C18) in the oils, Y is the yield of monoglycerides at 40 degrees C. The optimum isothermal reaction conditions for a system which IEP is 40 degrees C are: 40 degrees C, 3%-4.5% (W/W) water in glycerol, dosage of lipase is 500 u/g oil when the mole ratio of glycerol to oil is 2.5:1. The highest yield of monoglycerides is 81.4% in 48 h by means of programming temperature reaction.

ABSTRACT

Enzymatic esterification by reacting caparic acid with glycerol in solvent-free system was studied. Lipases from Pseudomonas fluoresces(PFL), Mucor miehei(MML) and Candida antarictica(CAL) possessed good catalytic activity. The optimal reaction conditions to convert capric acid with CAL are: 60 degrees C, 20-100 u of CAL per gram capric acid, 12% (W/W) of initial water content in glycerol. CAL does not express its 1,3-specificity in final product. Mechanical fraying denatured CAL partly. 96.4% of catalytic activity of CAL recovered after 5 batches of reaction. Extraction with sodium carbonate solution can decrease acid value of product from 9.8 mg KOH/g to 0.68 mg KOH/g. Applying the enzymatic esterification in open system, under vacuum or dehydrating with molecular sieves all dehydrate effectively. Molar ratio of reactants does not influence the total conversion of capric acid but influences the yield of monoglyceride. With certain protocols, the reaction period could be shortened dramatically; conversion of capric acid reached 96.9% in 5 h.
