ABSTRACT

Among 95 infants aged less than one year who underwent intracardiac repair for congenital heart disease at Kurume University Hospital between August 1990 and June 1995, a patients (3.9%) received primary elective open sternal (PEOS) and delayed sternal closure (DSC) after operation. 1) The mean interval for DSC was 4.3±0.9 (2-8) days, and DSC interval significantly correlated with the extracorporeal circulation (ECC) time. 2) Before DSC, patients became hemodynamically stable and requirements for inotropes and FiO<sub>2</sub> for mechanical ventilation decreased. 3) It was important to carefully manage fluid balance before DSC, and the balance after operation should be 0. 4) There was only one patient with mediastinal infection. Of the three patients who died in hospital the cause of death was pulmonary infection due to prolonged mechanical ventilation. 5) Both PEOS and DSC required careful postoperative management, but, the treatment seemed to improve postoperative results in cases in which postoperative hemodynamic status was unstable due to prolonged ECC.

ABSTRACT

Postoperative cardiac function, changes of LV wall motion and exercise tolerance test were studied in 49 patients who received coronary artery bypass grafting (CABG) using antegrade intermittent warm blood cardioplegia (IWBC) and had postoperative left ventriculography (LVG). The mean aortic cross clamp (ACC) time was 68.2±22.8 minutes, and the coronary perfusion time during ACC was 12.6±6.9 minutes (18.2±7.1% of total ACC time). Spontaneous return of normal sinus rhythm was seen in 45 patients (92%). Perioperative myocardial infarction occurred in only one patient and one patient required inotropic support>5μg/kg/min after operation. The mean cardiac index measured by Swan-Ganz catheter improved postoperatively, and the postoperative LVG revealed improvement of the LV wall motion in comparison with that of preoperative LVG, and no local asynergy due to inadequate distribution of IWBC was seen in the early postoperative period. The mean postoperative exercise tolerance performed in 24 patients was 6.6 METS (metabolic equivalents), and no patient revealed myocardial ischemia at exercise. In conclusion, the IWBC is considered to be an easy and safe technique for distal anastomoses of CABG with no influence upon the postoperative cardiac function and LV wall motion.

ABSTRACT

We present the outcome of surgical management for thoracoabdominal aneurysms in 27 patients during the past 22 years. Ischemia of visceral organs was successfully prevented by axillo-femoral temporary bypass using a 10mm PTFE graft with an 8mm branch for main visceral vessels and partial extracorporeal circulation perfusing visceral organs. No other significant problems were encountered. Paraplegia occurred in 5 patients (18.5%). Three of them had received reconstruction of the intercostal arteries. Patients treated by spinal fluid drainage developed no paraplegia. The Crawford inclusion and Piehler bypass techniques were useful in reconstructing the main visceral vessels. Two patients died of ruptured proximally anastomosed thoracic aorta after a thromboexclusion technique. Early death occurred in three patients and late death in one.

ABSTRACT

We experienced 4 cases of graft-enteric fistulas in which 2 cases were died due to intestinal hemorrhage and the other 2 cases are survived by the surgical treatment of infected graft excision with extra-anatomical bypass or anatomical bypass grafting with omental wrap. The most important thing is to prevent this late complications at the first operation by wrapping of graft with the surrounding tissues or omentum.

ABSTRACT

Coronary artery bypass grafting (CABG) was performed in 16 patients with impaired left ventricular function due to ischemic heart disease (IHD) and the surgical procedures and cardiac functions before and after operation were studied. Preoperative angiogram showed three vessel disease in all patients. The ejection fraction was less than 40% in all and the mean cardiac index (CI) was 1.97<i>l</i>/min/m<sup>2</sup>. At operation arterial graft was used in 10 patients (Group-AG) and no arterial graft but saphenous vein graft was used in 6 patients (Group-SVG). The average total cardiopulmonary bypass time, aortic cross clamping time and the number of revascularized vessels in both groups showed no significant differences. However, intraaortic balloon pumping was necessitated in one of Group-SVG and the requirement of postoperative catecholamine was in higher ratio in Group-SVG than in Group-AG. Postoperative CI improved to 3.1±0.4<i>l</i>/min/m<sup>2</sup> and 3.3±0.3 <i>l</i>/min/m<sup>2</sup> in Group-AG and Group-SVG, respectively. The postoperative New York Heart Association Functional Class improved to Class I or II in all patients and there were no significant differences of the improvement between the groups. Conclusively, it seems that the arterial grafts can be used safely and extensively in CABG for impaired left ventricular function due to IHD.

ABSTRACT

A 53 year-old male with mitral valve aneurysm was presented. This patient, who had no episodes of rheumatic fever, was admitted with complaints of general fatigue, dyspnea and continuing high fever. Echocardiographic examination showed an abnormal echo behind the anterior leaflet of mitral valve, protruding into the left atrium during systole. Angiogram showed the same abnormal change of mitral valve and mitral regurgitation (MR) and aortic regurgitation (AR). We diagnosed as mitral valve aneurysm with MR and AR due to infective endocarditis. At operation, it was revealed that the aortic valve was destroyed, resulting in severe AR, and the anterior leaflet of mitral valve was a large aneurysm itself. Both valves were replaced with St. Jude Medical valve prosthesis. Postoperative course was good and with no complications. In Japan, 21 cases of mitral valve aneurysm were reported. We discussed the clinical course and the operative procedure for mitral valve aneurysm in this report.

ABSTRACT

Cor triatriatum is one of the rare congenital cardiac malformations and once the diagnosis is correctly established, this is amenable to surgical correction. We reported a case of 25-year-old male of cor triatriatum, who had symptomes of easy fatiguability. The diagnosis of cor triatriatum was suspected preoperatively by two-dimensional echocardiogram at first, detecting abnormal diaphragm in the left atrium, and it was confirmed by color Doppler echocardiogram and transesophageal two-dimensional echocardiogram. Cardiac catheterization revealed high pulmonary capillary wedge pressure and the abnormal diaphragm in the left atrium was showed by the pulmonary arteriography. On the operation, the abnormal diaphragm was excised by the trans-septal approach, which had a small fenestration of 8mm in diameter at posterolateral site. Some considerations for clinical diagnosis and surgical treatment are discussed.

ABSTRACT

A case of 41-year-old man with large candidal vegetation on the tricuspid valve was reported. He was presented with high fever and newly developed heart murmur. Four months before admission, he had suffered from head trauma which required intravenous hyperalimentation and injection of multiple antibiotics through catheter indwelling the superior vena cava. On admission, echocardiogram showed large vegetation on the tricuspid valve, although blood cultures were sterile. At operation, tricuspid valve was replaced with St. Jude Medical prosthesis because large vegetation developed from the anterior tricuspid leaflet was confirmed. <i>Candida albicans</i> was detected by microscopic examination of the vegetation. The total of 1500mg of Amphotericine-B were administered intravenously after operation. His postoperative course was uneventful. We discussed about the availability of echocardiogram for diagnosis and the effectiveness of a combination of chemotherapy and valve replacement for treament of fungal <i>endocarditis</i>.

ABSTRACT

Thirty six patients with post-infarction complications underwent operation, and the postoperative and late follow-up results were analyzed. There were post-infarction ventricular septal perforation (VSP) in 9 patients and left ventricular aneurysm formation in 27 patients. The operative indications were poor physical work capacity in 13, cardiogenic shock or severe congestive heart failure in 10, left ventricular thrombus in 7, severe ventricular arrhythmia in 6, and repeated angina in 6. Left ventricular aneurysmectomy was performed in 14 patients and VSP closure was in 8. Coronary arteries were simultaneously bypassed in 14 patients. Three patients were died of sudden postoperative arrhythmia 10 days, 55 days and four years after operation. All survivors except two patients with preoperative massive cerebral infarction or prolonged heart failure were in New York Heart Association Class I or II in their late postoperative periods. However, five patients in whom the significant coronary lesion had not been bypassed or the bypassed grafts had occluded complained of mild angina after operation. Postoperative arrhythmia was one of major factors in the late results and simultaneous coronary artery bypass grafting was important to improve the symptoms in the late postoperative periods.

ABSTRACT

A rare case of secundum type atrial septal defect associated with cleft of the anterior mitral leaflet in 63-year-old man is reported. The electrocardiogram showed right bandle branch block and indeterminate QRS axis. Preoperative cardiac catheterization revealed left to right shunt at the atrial level and moderate mitral regurgitation due to a cleft in the anterior mitral leaflet. Marked tricuspid regurgitation due to a dilated annulus concomitantly existed. The valve deficiencies were repaired and the atrial septal defect was closed at operation. Forty cases of secundum type atrial septal defect associated with cleft mitral valve were discussed about their electrocardiograms and operative indications.
