ABSTRACT

Drug rash with eosinophilia and systemic symptoms syndrome (DRESS) is a rare but serious hypersensitivity drug reaction most frequently associated with antiepileptics. We report a case of carbamazepine-induced DRESS syndrome in a 61-year-old man who was recently initiated on carbamazepine (6 weeks back) and presented with a history of acute febrile illness of 10 days duration. General examination showed multiple erythematous coalescent papules and rash over the body with relative sparing of the face with lymphadenopathy. Laboratory results revealed eosinophilia, atypical lymphocytosis, transaminitis, and negative serology for hepatitis. Registry of severe cutaneous adverse reactions (RegiSCAR) scoring system case is categorized as a definite case with a score of 7. Carbamazepine was discontinued and with the initiation of intravenous steroids; the transaminitis improved, fever and rashes resolved.

ABSTRACT

Prevention is always better than cure especially in diseases such as Type 2 DM which is fast gaining the status of a potential epidemic in India with more than 62 million diabetic individuals currently diagnosed with the disease. The disease Madhumeha can be correlated with Type 2 DM. The disease is characterized by metabolic abnormalities and long term complications involving the eyes, kidneys, nerves and blood vessels. Madhumeha being an Anushangi vyadhi will make the person suffer for life time. Complications are further more difficult to treat. Hence it is always recommended in Ayurvedic classics to prevent the manifestation of diseases as much as possible and also to prevent the Upadravas if Madhumeha is already manifested. A good and proper diet in disease is worth a hundred medicines and no amount of medication can do well to a patient who does not follow a strict regimen of diet. Pathya ahara is the first and foremost step while considering the prevention of Madhumeha. Another factor which has important role in the disease manifestation is improper Vihara which can be considered for increased urbanisation, high prevalence of obesity, sedentary lifestyles and stress. Healthy life style has a key role in preventing Madhumeha and also to ease the life with Madhumeha by delaying the complications. Hence the present study is aimed at collecting and compiling various preventive measures which are explained by our Ayurvedic Acharyas to prevent Madhumeha and its complications.

ABSTRACT

This study was performed on 45 children aged 2.5 to 12 years [mean 4.9 years] with history of foreign bodies in the nose to determine the causes behind the insertion of foreign bodies and the investigations needed. Full physical and E.N.T examination was performed before and after removal of foreign bodies. X-rays of nasal cavity and sinuses, audiological and tympanometric testing were performed. Our results showed that insertion of foreign in the nose is associated with E.N.T diseases like: Adenoid hypertrophy which was present in 21 patients [46.6 %], maxillary sinusitis in 4 patients [9 %], chronic rhinitis in 3 patients [6.6 %], acute rhinitis in 5 patients [11.1%], deviated nasal septum in 2 patients [4.4 %], 3 patients [6.6 %] were mentally retarded, and finally no findings in 7 patients [15.7%].Furthermore, we suggest that radiological, audiological ,and tympanometric evaluation should be performed in all children with nasal foreign bodies
