ABSTRACT

Background: Little has been known about the Hypothalamic Pituitary Adrenal (HPA) axis response to pre-competition stress (PCS) and its response to relaxation therapies such as of Mindfulness Meditation Therapy (MMT) on sports population. In shooting sports good physical as well as psychological condition is highly demanded. Researchers have been performed on the psychophysiological responses of MMT on normal and diseased persons, but little has been done on sports population especially in shooters. Objective: The purpose of current study was to estimate the contribution of MMT on Salivary Cortisol (SC), a reliable physiological marker of HPA- axis response in reducing PCS, and its effect on shooting performance (PS). Methods: 96 male elite Shooters, with mean age of 29.5±4.3years were examined as in experimental and control (48 in each). Total duration of the study was five weeks, four weeks of experimental and one week study to determine the follow-up effect. Pre, post and follow-up data of quantitative phenotypic markers of HPA-Axis activity by analysis of SC and PS were analyzed. Results: Compare to control, experimental group has shown significant result, post-intervention (p<0.001) and in follow-up (p<0.001) in SC and in PS. Resulted in reduction of PCS level and increase in PS, whereas the control group has been shown non-significant result (p<0.05).Conclusions: Results indicated that relaxation therapies such as MMT may decrease PCS and will enhance PS. It is concluded that in four weeks of MMT has an effect on HPA-Axis by decreasing the level of SC as a reliable physiological marker of PCS.

ABSTRACT

A case of eumycetoma of foot in an 8-year old male child was clinically diagnosed as chronic osteomyelitis and was microbiologically confirmed as eumycetoma. The case is being reported for its uncommon clinical presentation and etiological agent, Exophiala jeanselmei. The patient recovered completely after treatment with ketoconazole.

ABSTRACT

BACKGROUND AND OBJECTIVE: Carbohydrate-electrolyte drink has a significant role on energy balance during exercise. The present study was designed to investigate the effect of oral carbohydrate-electrolyte supplementation on sports performance and cardiovascular status of the national level male athletes during exercise and recovery. METHODS: A total of 10 male athletes (age range: 20-25 yr) were selected. The experiment was performed in laboratory (25 degrees C and 60% relative humidity) in two phases; phase 1 - no supplementation, and phase 2 - a 5 g per cent carbohydrate-electrolyte drink was given orally during exercise and a 12.5 g per cent carbohydrate-electrolyte drink during recovery. Subjects performed an exercise test at 70 per cent of VO(2)max. Performance time, heart rate during exercise and recovery were noted, blood samples were collected during exercise and recovery for the analysis of glucose and lactate levels in both the phases. RESULTS: Significant improvements were noted in total endurance time, heart rate responses and blood lactate during exercise at 70 per cent VO(2)max after the supplementation of 5 g per cent carbohydrate-electrolyte drink. However, no significant changes were noted in blood glucose and peak lactate level irrespective of supplementation of carbohydrate-electrolyte drink. Significant improvement in cardiovascular responses, blood glucose and lactate removal were noted during recovery following a 12.5 g per cent carbohydrate-electrolyte drink. INTERPRETATION AND CONCLUSION: Carbohydrate-electrolyte drink can increase endurance performance as well as enhance lactate removal and thereby delaying the onset of fatigue.

ABSTRACT

OBJECTIVE: The benign outcome of patients with a normal exercise thallium-201 myocardial perfusion study has been well established. The aim of the study was to examine the prognostic implications of normal exercise tomographic thallium images in medically treated patients with angiographic evidence of coronary artery disease (CAD). METHODS: There were 68 patients in Group A with significant angiographic CAD (54 male and 14 female) aged 52 +/- 12 years, 42 had one, 18 had two and eight had three vessel CAD (50% diameter stenosis 1, Group B had 70 patients who had normal coronary arteries at cardiac catheterization. RESULTS: Exercise test was submaximal in 40 (50.82%) patients, ST segment depression during exercise occurred in 28 patients (41.1%) and angina during exercise in 24 patients (35.2%). Majority of patients (76.47%) were on anti-anginal therapy. During a mean follow up of 30 months only two patients had cardiac events in group A patient. One died of cardiac causes and one had non fatal myocardial infarction (event rate 0.96%/year). None of these 2 patients had positive ST response during exercise. In group B only one patient had non-fatal myocardial infarction. CONCLUSION: Medically treated CAD patients including those with multivessel CAD have a benign prognosis in the presence of normal exercise thallium images. These results have long term important implication in management of such patients and cost of health delivery.

ABSTRACT

Congenital spinal lipomas as a form of dysraphism are frequently reported in literature. A few studies have highlighted their histologic features and found them different from lipomas in other parts of the body. In a review of eight cases of spinal lipomas, we found heterotopic tissue in seven. Neural tissue, muscle, bone and cartilage were frequently encountered. The presence of meningothelial cells in one case was an unusual finding. These findings support the concept of their hamartomatous origin. Malignancy, occurring later in life in one of the heterotopic elements is known, though uncommon, and is one of the reasons, besides neurologic deficit, for long term follow-up.

ABSTRACT

To evaluate the relationship of ambulatory blood pressure (ABP) recording and blood pressure response to exercise, 58 essential hypertensive patients, not taking any drugs, had symptom-limited treadmill stress test (TST) within 48-96 hours of ABP, TST time, blood pressure increase, decrease, mode of increase and decrease, were independent of ABP systolic (SBP) and diastolic blood pressure (DBP) over 24 hours, day time and night time (p = ns). SBP decrease immediately after exercise were independent of ABP data. TST achieved heart rate was related to both 24 hours SBP (r = -0.64, p = 0.00005) and DBP (r = -0.55, p = 0.00001) in both day (r = -0.64, p = 0.00001 and r = -0.54, p = 0.002) and night (r = -0.52, p = 0.0001 and r = -0.46, p = 0.003) time periods. Therefore patients with achieved heart rate < 100% (n = 18) had higher 24 hour SBP (148 vs 132 +/- 2 mm Hg, p = 0.0006) and DBP (92.4 +/- 6.4 vs 84 +/- 6.2 mm Hg, P = 0.006) day and night. It is concluded that there is no overlap of diagnostic information using blood pressure. Values in TST or ABP although achieved heart rate in exercise is inversely related to severity of hypertension.

ABSTRACT

Twenty five asphyxiated newborns (seventeen term and eight preterm) with mean gestational age of 37 weeks (range 28-48 weeks) and mean birth weight of 2.4 kg (range 0.75 kg to 3.5 kg), respectively, constituted the cases in present study. Normal CT scan was found in five term (29.4%) and two preterm babies (25%). CT abnormalities noted in term babies included hemorrhage (subarachnoid 5.8%, intracerebral 11.6%), hypodensity (mild 23.2%, moderate 11.6% severe 5.8%); hypodensity with hemorrhage 5.8% and cerebral atrophy 5.8%. In the preterm babies abnormalities included intraventricular hemorrhage in 25%, isolated hypodensity in 37.5% and hypodensity with hemorrhage in 12.5% cases. Where as mild hypodensity on CT scan in the absence of hemorrhage or other gross abnormality was indicative of a favourable outcome, moderate to severe hypodensity was indicative of ischemic brain injury and an unfavourable outcome. The presence of intraventricular hemorrhage irrespective of the size of bleed was associated with mortality in all in the present study.
