ABSTRACT

PURPOSE: The study aimed to compare characteristics associated with smoking and smoking cessation of those who had succeeded in smoking cessation. METHODS: Data were collected from January to June, 2008. The subjects were 9,819 smokers who were registered at the smoking cessation clinic of public health centers in Incheon. Four characteristics (demographic, health promotion, smoking, smoking cessation) were compared between 6-week (short-term) and 6-month (long-term) success groups. RESULTS: There was a significant difference between the 6-week and 6-month success groups for smoking cessation in demographic characteristics (gender, age, job, social security), health promotion (BMI, alcohol dependency, BP), smoking (first smoking age, smoking duration, expiration CO concentration, nicotine dependency), and smoking cessation (attempt to quit smoking, reason for smoking cessation, information source for registration). CONCLUSION: The group of short-term smoking cessation success was younger than the other. Also, short-term success group was of lower socioeconomic class than the other. The 6-month success group had a larger number of attempts to quit smoking. Therefore, smoking cessation policy should be focused more on younger people and those in lower socioeconomic status. These groups should be given advice on smoking cessation motives and more frequent counseling for smoking cessation.

ABSTRACT

PURPOSE: This study was performed in order to examine the effects of an smoking cessation counselling program for smoking cessation success. METHOD: Among a total of 468 persons who had ceased from smoking for 6-months and had visited the smoking cessation clinic of a public health center from January 2nd to December 31th in 2006, 61 in all who had a negative reaction in the urine nicotine check were selected for this study. Collected data were expiratory CO concentration, BMI, blood pressure, liver function, and lipid metabolism. These data were analyzed by descriptive statistics, repeated measured ANOVA and paired t-test with the SPSS/PC(Version 12.0) program. RESULT: There were significant changes in expiratory CO concentration, SBP, DBP, AST, ALT, and TG, but not in BMI, gamma-GTP, TC, HDL-C, and LDL-C. CONCLUSION: This study showed that smoking cessation through a smoking cessation counselling program has partially positive effects for smoking cessation success. The results of this study show that the smoking cessation counselling program at the smoking cessation clinic of a public health center should be continued for smoking cessation success.

ABSTRACT

PURPOSE: The purpose of this study was to compare the effects of an exercise program on physical fitness, obesity indices, and blood lipids in cases of non-obese and obese women. METHOD: Data was collected from May, 2006 to November, 2006 in a public health center. All Subjects(37 women) participated in an exercise program that consisted of Latin dance, muscular strength training, and dumbbell exercises. Thirty-seven women were divided into two groups(16 non-obese women and 21 obese women) by %body fat. After 8 weeks, the effects of treatment were compared between pre-test and post-test in each group. RESULTS: Physical fitness(abdominal muscle strength, muscle endurance, flexibility, agility, balance) was significantly different between the pre-test and post-test in the non-obese and obese group. Obesity indices(body weight, BMI) was significantly different in obese women after the 8-week exercise program. There was no decrease of blood lipids in either group. CONCLUSION: These findings indicate that an exercise program could be an effective nursing intervention to increase physical fitness in non-obese and obese women and to decrease obesity indices(body weight, BMI) in obese women.

ABSTRACT

PURPOSE: This study was to identify which factors are likely to influence the effectiveness of smoking cessation on adults who smoke in Metropolitan Incheon. METHOD: Data from 9,083 smokers, who visited a smoking cessation clinic of a public health center from Jan. to Oct. 2005, were provided by the Korean Health Research Society. Among 9,083 smokers, 1,495 people were selected for follow up care at 6 months in order to analyze the differences between two groups one is a successful group and the other is a failure group. RESULTS: The successful group included 639 people and the failure group 856 people. In the demographic profiles such as sex, age and motive registration, there was a significant difference between the two groups. In the view of smoking pattern and factors such as the expiratory CO level, the age of starting to smoke, the duration of smoking, alcohol, and dependence on alcohol use and nicotine, there were significant differences between the two groups. The smoking cessation method, results of uni variate analysis, the total number of visits to the smoking cessation clinics, and the use of nicotine gum or a patch(stage 1, stage 2) were significantly different in the two groups. CONCLUSION: The results of multi variate analysis have shown that the factors associated with the success for smoking cessation is the total number of visits to the smoking cessation clinic, and the dependence on alcohol.

ABSTRACT

PURPOSE: This study is designed to test the effects of a full body massage on Apgar score, postpartum mother's childbirth experience, and mother-infant attachment for primipara during labor. METHOD: Data were collected using a quasi-experiment method (nonequivalent control group, pre-post test design) from November 1, 2001 to July 31, 2002. The subjects for this experiment consist of 28 women in the experimental group and 29 in the control group, out of 57 primipara hospitalized at the U OB & GYN hospital in Inchon. A 20 minute full body massage was given to the experimental group on each of three delivery phases (latent, active, and transition), and a conventional delivery care to the control group. The experimental tools for the study are Apgar score scale developed by Apgar(1958), postpartum mother's childbirth experience scale developed by Marut and Mercer(1979), and mother-infant attachment scale developed by Cropley et al(1981). The data collected were analyzed using t-test, chi2 test, and ANCOVA on the SPSS program. RESULT: The results of the experiment are as follows: One minute(t=6.756, p=.000; F=9.181, p=.004), and five minute(t=6.129, p=.000; F=7.918, p=.007) Apgar score are increased significantly. Postpartum mother's childbirth experience is significantly increased(t=11.691, p=.000; F=11.741, p=.001). Verbal touch score(t=5.256, p=.000; F=7.169, p=.010), skin touch score(t=11.332, p=.000; F=27.073, p=.000), and total score(t=12.105, p=.000; F=30.104, p=.000) of postpartum mother's childbirth experience are significantly increased, but not for eye touch score(t=3.236, p=.002; F=.800, p=.375). CONCLUSION: This study shows that a full body massage has a significant positive effect on Apgar score, postpartum mother's childbirth experience. and mother-infant attachment. Therefore, it is suggested that the full body massage might be used clinically to help primipara during labor.

ABSTRACT

PURPOSE: This study was to test the effects of a full body massage on labor pain and delivery stress reaction for primipara during labor. METHOD: This study employed a quasi- experimental method(nonequivalent control group, pre-post test design). The subjects of this study were 57 primipara hospitalized at the U OB & GYN hospital in Inchon from November 1. 2001 to July 31, 2002. 28 women were assigned to the experimental group and 29 to the control group. The experimental group was given 20 minutes full body massage for each of three delivery phases. The control group was given conventional delivery care. The levels of labor pain were measured by the Visual Analogue Scale and the expression scores of the labor pain and the levels of delivery stress reaction were measured by pulse rate, respiration rate, and blood pressure, and State-Anxiety Scale. Data were analyzed using the repeated measures analysis of covariance(ANCOVA), t-test, X(2)- test, simple ANCOVA with SPSS program. RESULT: There was no interaction effect between time and group(F=.370, p=.693), but significant time effects were found for subjective labor pain(F=3.840, p=.028). There was no interaction effect between time and group(F=.112, p=.894), but significant group effects were found for objective labor pain(F=12.299, p=.001). There was no interaction effect between time and group for PR(F=.172, p=.843), RR(F=.626, p=.539), SBP(F=.089, p=.915), DBP(F=.748, p=.479), but significant group effects were found for SBP(F=7.547, p=.008). The level of status anxiety of the experimental group was significantly lower than the control group(F=11.787, p=.001). CONCLUSION: This study showed that the full body massage has partially positive effect on labor pain and delivery stress reaction. Therefore this study suggested that the full body massage might be used clinically to help primipara during labor.

ABSTRACT

This study was designed to test the effects of a full body massage on uterine contraction, length of labor, type of delivery, and drug intervention for primipara during labor. Data were collected using a quasi-experiment method (nonequivalent control group, pre-post test design) from November 1, 2001 to July 31, 2002. The subjects of this experiment consisted of 28 women in the experimental group and 29 in the control group, out of 57 primipara hospitalized at the U OB & GYN hospital in Inchon. The experimental group was given a 20 minute full body massage for each of the three delivery phases (latent, active, and transition). The control group was given conventional delivery care. Three (3) parameters were analyzed in this experiment. (1) The interval, duration, and strength of uterine contraction, using an electric tocodynamometer (2) The elapsed time for stage 1 and stage 2 labor. (3) The types of deliveries and drug interventions, using postpartum medical records The data collected were analyzed using the repeated measures analysis of variance (ANOVA), t-test, and X(2) test of the SPSS program. The results of the experiment are as follows: 1) Uterine contraction interval was signifi- cantly reduced (F=3.210, p=.050). Duration of uterine contraction showed significant increase only during the transition phase (t=-2.319, p=.023). Strength of uterine contraction showed no significant difference. 2) Total length of labor was significantly shortened (t=-5.245, p=.000). The length of 1st stage labor was signifi- cantly shortened (t=-5.164, p=.000), with latent phase showing (t=-4.709, p=.000), active phase (t=-2.973, p=.005), and transition phase (t=-2.031, p=.047). The length of 2nd stage labor showed no significant difference. 3) The number of natural deliveries were significantly increased (X(2)=13.127, p=.004). 4) The number of drug interventions were significantly fewer (X(2)= 4.493, p=.034). In conclusion, this study shows that a full body massage has a significantly positive effect on uterine contraction interval, length of labor, type of delivery, and drug intervention. Therefore, this study suggests that a full body massage be used clinically to help primipara during labor.

ABSTRACT

PURPOSE: Knowing the accurate gestational age(GA) is critical in nursing care of premature infants. A descriptive study was performed to examine the reliability and clinical applicability of the New Ballard examination(NBE) in premature infants. METHOD: A NBE was performed to measure GA by assessing the neuromuscular and physical maturity in the course of physical examination of a convenient sample of 74 premature infants. RESULT: 1. The highest item of NBE score was posture (mean=2.82) and the lowest item was breast (mean=1.84). 2. There was a highly correlation between both the GA by LMP(GA-LMP) and GA by NBE (GA-NBE)(r=.844, p=.000). 3. There was a greater positive relationship in neuromuscular maturity than physical maturity in the GA-NBE of the premature infants(r=.786 vs r=.933). 4.There was a positive correlation between neuromuscular, physical, total maturity, GA-LMP and GA-NBE in the birth weight, length, 1 , 5 minute apgar score. 5.There was no significant difference neuromuscular, physical, total maturity in NBE by delivery history. CONCLUSION: The study supports the reliability a clinical relevance of NBE in assessment of the accurate GA in premature infants.

ABSTRACT

The aims of this study were to identify the mothers educational needs when they have children who have had open heart surgery in congenital heart disease, to identify the children's characteristics with this problem, and also to provide the basic information on the development of the educational program for the mothers. The subjects of the study were 101 mothers of children and their children hospitalized with congenital heart disease in pediatric wards of G. University Hospital in Inchon and S. Hospital in Puchon from June to November 2000. The study used a Likert-type questionnaire with 39 questions which was based on a previous questionnaire developed by Lee Mi-ryun(1989) for adult open heart surgery patients. The reliability of the questionnaire was Cronbach' alpha= .9375. The data were collected directly from mothers of the patients in wards, The t-test, ANOVA was adopted for the data analysis. The key results of the study are as follows: 1) The average age of the children was 32.90 months. The majority of the children are male and weighted 3.0-3.5kg at birth. Most of the children had VSD(29.7%). and 10.9% of children had a family history of heart disease. 2) An average score of 4.62 out of a maximum of 5,00 was recorded for the educational needs of the children's mother. The highest score of 4,69 was for home care after discharge followed by a score of 4.67 for diagnosis and prognosis on congenital heart disease. And pre-post operation care scored 4.51. The mean scores of single-question items of educational need were. in order. 4.81 for immunization after operation, 4.80 for recovery process, and 4.77 for prognosis of the disease. The score of 4.18 for measurement of intake and output was the lowest. 3) The analysis on the children's mothers educational needs by social-demographic characteristics such as sex. age of patient and mother. economical status. educational status. number of child. hospitalization times. and hospitalization cause showed no significant difference statistically. but there was a significant difference between mothers with jobs and without jobs.

ABSTRACT

The purpose of this study was to find whether the educational program contributed to the increase of caring confidence and rearing attitude of the newborn mothers. The subjects for this study were 40 newborn mothers-20 control group and 20 experimental group-who delivered a normal neonate and were gotten general informations from the nurses at the K medical center in Seoul. Especially, the experimental group was educated for 30-40 minutes in maternal educational program through lectures, demonstrations, discussion with textbooks and visual materials by a researcher at 2-3 days after delivery. The data were collected two times-at 2-3 days after delivery and at 4 weeks after discharge-from October, 1998 to January, 1999 by investigators interviewing with a structured questionnaire. Pharis(1978)'s self confidence scale was used after some modification to measure caring confidence and Cohler's maternal attitude scale to check mother's rearing attitude. Analysis of data was done by using of frequency, %, chi-square-test, Independent sample t-test. The results of this study were as follows : 1. The Confidence of the experimental group was significant higher than the control group(t=2.519, p=.016) and the rearing attitude changed more positively(t=5.545, p=.000). 2. In the confidence scale, the statements having a significant change were holding a baby, changing diapers, feeding, and caring umblicus. 3. In the rearing attitude scale, the statements having a significant change were 12 of 20 statements.

For the experimental group tactile and kinesthetic stimulation was applied 2 times a day for 10 days(10:00~11:00 hours in the morning and 17:00~18:00 in the afternoon). As a weight weighing instrument, electronic indicator scale(Cas Co, Korea) was used. To determine urine cortisol concentration level in stress hormone, radio immuno assay method was used. And high performance liquid chlomatogarphy was used to determine urine norepinephrine, concentration level. To determine behavior status, tools developed by anderson et al(1990) and remodeled by Kim Hee-Sook(1996) were used. Collected data were analyzed with the SAS program using x2-test, student t-test, repeated measures ANOVA and paired t-test. The result were as follow. 1. As for the daily weight gain, the experimental group showed first change in weight and this group also showed higher weight in the average weight than the control group. Statistically, however, there was no significant factor between the two groups. 2. The cortisol concentration in urine showed decrease in the experimental group norepinephrine concentration in urine showed increase in both experimental and control groups. No statistical significance was shown between the two groups. 3. In the aspect of behavior status, the experimental group showed statistical significance by showing inactive in the state of alert and conversion to a positive state than the control group. In conclusion, the sensory stimulation in this study showed a positive aspect through there was no statistical significance n the weight gain and urine stress hormone concentration. In the behavior status, there was statistical significance in the frequency of staying inactive in the state of alert and conversion to a positive state.

ABSTRACT

This study was designed to compare the educational needs of the mothers of LBW infants and normal neonate for development of the educational program. The subjects for this study were 37 mothers of LBW and 60 mothers of normal neonate at 3 general hospitals in Seoul and Inchon. The data were collected during the period from July to November, 1996. The Educational Need for Infant Care was measured by questionnaire that has developed by researchers. The data analyzed by descriptive statistics, non-paired t-test, ANOVA, ANCOVA using SPSS PC+. The results were as fellow ; 1) The educational needs of the mothers of LBW infant were higher than the mothers of normal neonate(F=14.50, P=.000). 2) There were significant associations between the educational needs of the mothers of LBW infant and delivery history(nullipara, or multipara.) and caregiver of postpartum(t=-2.08, P=.045 ; F=3.94, P=.0282). 3) There were significant associations between the educational needs of the mothers of normal neonate and numbers of children and caregiver of postpartum(F=5.53, P=.0064 ; F=3.22, P=.0480). 4) The educational need by content was signs, symptomes and management of disorders(i.g.cyanosis, seizure, fever, vomiting, etc.) were higher than general care of baby(i.g. feeding, diaper change etc.) at two groups. In conclusion, when the nurses teaching the method of infant care to mother, there should be in consideration of delivery history and caregiver of postpartum. Also, its educational contents must be included of signs, symptomes and management of disorders.
