ABSTRACT

Clinical observation was done on 300 cases of chronic prostatitis who visited Urologic Department. Korea University Hospital during the period of 24 months from January 1, 1976 to December 31, 1977. The results are summarized as follows. 1. The age distribution was between I9 and 65, while 55 percent of total cases belonged to the age group of 20 to 29. 2. The decreased libido was complained in 3l cases (10.3%) of chronic prostatitis, among them, 22 cases be1onged to married group. while 9 cases to unmarried group. Among the married group, 17 patients including S patients who had had vasectomy previously were between the age of SO and 49.3. Asymptomatic cases were found in 4S patients (14.3%), while many others had vague symptoms such as urethral discomfort. urethral discharge. pain on urination and perineal discomfort. 4. The past history of nonspecific urethritis were found in the l58 cases (52.7%). The duration of the infection was between l month to 6 months in 84 cases (53.1%), and between 13 months to 24 months in 49 cases (3l%). 5. Urinalysis were within normal limits in 229 cases (76.3%). Microscopic examination of prostatic fluid and urine shred wag done in all cases and in 249 cases (83%) revealed the presence of infection.

ABSTRACT

Most foreign bodies in the urinary bladder are self-introduced through the urethra as a means of masturbation. Our five cases of self-introduced foreign bodies in the bladder are summarized below. Case 1 : 28-year-old male. A plastic tube of 32cm. in length was introduced into the bladder during masturbation 8 days prior to 3dmission. A Lowsley foreign body forceps was used to remove it. Case 2 : 16-year-old male. Patient stated that a coiled wire was introduced into the bladder for the purpose of masturbation 2 years ago. A stone was formed over the wire as a nucleus and this stone was removed through the suprapubic cystotomy. Case 3 : 15-year-old male. A metal tip of the screw-driver was slipped through the urethra into the bladder during masturbation . Next day it was removed with the grasping cystoscopic forceps. Case 4 : l9-year-old male. Patient stated that a wire of 15cm. in length was introduced into the bladder for the purpose of masturbation. 5 years later, a stone was found on cystoscopy and this stone was removed surgically. Case 5 27-year-old female. Patient stated that an earpick cleaner was introduced into the bladder during masturbation. A No. 5Fr. flexible stem foreign body cystoscopic forceps was introduced into the bladder through the McCarthy panendoscope and this earpick cleaner was removed easily.

ABSTRACT

A new technique of plastic reconstruction of penile skin using scrotal skin pedicle graft was devised This technique was applied in 6 patients who were admitted to the Department of Urology, Korea University Hospital during the period of 5 years and 6 months from January 1974 to June 1979 and the results were excellent in all cases. The primary causes of penile skin defect in these patients were injection of paraffin into the subcutaneous tissues of the penile shaft(4 Pts.) and tissue necrosis resulted from infiltration of local anesthetics for performing circumcision (2 Pts.). The procedure was as follows: 1st stage; With the patient in supine position, under general orspinal anesthesia, the removal of paraffin or fibrotic tissues and necrotic tissues were carried out After the length of penile skin defect was measured and the same length was applied in making three parallel transverse incisions over the scrotal skin and the skin between these incisions was undermined to from two tunnels underneath. The penis was drawn downward through the proximal tunnel to cover the denuded area over the dorsal side of the penis and the glans was exposed. Both the proximal and distal borders of the proximal tunnel were approximated to healthy skin borders of dorsal aspect of the penis. The scrotal skin flap forming distal tunnel was then reversed inside out inwardly and upwardly and approximated the proximal and distal borders with healthy skin borders of distal and proximal borders of ventral aspect of the penis so that the ventral denuded area of the penis could be covered. Then the denuded scrotal skin was closed by approximation of remained scrotal skin edges. Sutures. were carried out in interrupted manner with 4-O Nylon except the ventral side of proximal portion of the penis and closure of the scrotal skin, where 3-0 catgut were used. 2nd stage; After healing had taken place the penis was freed by simple dissection of two lateral pediclcs from the scrotum under local anesthesia. Then simple closure was performed on every dissected borders. The advantages of this technique are as follows: 1. Indwelling of urethral catheter or urinary diversion such as suprapubic cystostomy isnot necesssry in first stage operand and the patient can void urine without difficulty. 2. No discomfort or pain is complained on erection of the penis because the penis is lifted away from the scrotum by the reversed scrotal skin flap. 3. The second stage of this operation is so simple that the procedure is performed under local anesthesia. 4. There isono danger of urethral injury in second stage operation

ABSTRACT

A 44-year-old female patient was admitted to Korea University Hospital with a history of frequency, pain and headache after urination for 2 years. The past history was not remarkable and she never had trouble with hypertension, Urinalysis revealed microscopic hematuria and urinary VMA level was 9.33mg it 24 hour urine Cystoscopy showed the presence of ovoid tumor mass measuring approximately 4cm in greatest diameter. A biopsy specimen, obtained by TUR showed histological evidence of pheochromocytoma. She underwent segmenal resection of bladder at the 14th day of admission and the postoperative course was uneventful.

ABSTRACT

In 12 cases of bladder tumor, 30cc of 2% methylene blue was introduced into the bladder, one hour previous to the transurethral biopsy, T.U.R. or segmental resection of the bladder, for the purpose of the staining of tumor tissue, and the following results were obtained. 1. This method was especially useful for the difference of tumor tissue in the cases of cystitis with tumor. 2. Most of the tumor tissue which was not covered with bladder mucosa stained in the deep blue so that one can easily identify tumor tissues from normal mucosa. 3. On operation, tumor masses could be resected easily because of obvious demarkation between the tumor mass and normal mucosa of the bladder. 4. In conclusion, this method was helpful to identify tumor tissues and useful for the removal of tumor remnants.
