ABSTRACT

Purpose@#The proper implantable collamer lens (ICL) size affects ICL stability. This study compared device efficacy using white-to-white diameter (WTW) measurements with Orbscan II and IOL Master 700. @*Methods@#We retrospectively studied 90 eyes (45 patients) who underwent toric ICL implantation from January 2019 to February 2020 and were followed for 1 year. The correlation between WTW and anterior chamber depth (ACD) for each measuring device was analyzed. @*Results@#The mean WTW measured by IOL Master 700 and Orbscan II was 12.2 ± 0.3 and 11.6 ± 0.3 mm, respectively, while the mean ACD was 3.28 ± 0.16 and 3.20 ± 0.15 mm. The WTW and ACD measured with IOL Master 700 averaged 0.57 ± 0.12 and 0.08 ± 0.04 mm larger than with Orbscan II. The differences were significant and the regression analysis had high correlations (R2 = 0.875 and R2 = 0.913, respectively; both p < 0.001). @*Conclusions@#WTW measured by the IOL Master 700 can be used as a reference either alone or together with the Orbscan II value to determine ICL size.

ABSTRACT

PURPOSE: To determine the effect of intraluminal brachytherapy on stent patency and survival after metallic stent placement in patients with primary bile duct carcinoma. MATERIALS AND METHODS: Twenty-seven patients with primary bile duct carcinoma underwent metallic stent placement; in 16 of the 27 intraluminal brachytherapy with an iridium-192 source (dose, 25 Gy) was the performed. Obstruction was due to either hilar (n=14) or non-hilar involvement (n=13). For statistical comparison of patients who underwent/did not undergo intraluminal brachytherapy, stent patency and survival were calculated using the Kaplan-Meier method and an independent t test. RESULTS: The mean durations of stent patency and survival were 9.1 and 10.0 months respectively in patients who underwent intraluminal brachytherapy, and 4.2 and 5.0 months in those who did not undergo this procedure (p<0.05). The mean durations of stent patency and survival among the 22 patients who died were 7.6 (range, 0.8 -16.1) and 8.3 (range, 0.8-17.3) months, respectively, in the eleven patients who underwent intraluminal brachytherapy, and 4.2 (range, 0.9-8.0) and 5.0 (range, 0.9-8.4) months in those whom the procedure was not performed (p<0.05). CONCLUSION: Intraluminal brachytherapy after stent placement extended both stent patency and survival in patients with primary bile duct carcinoma.

ABSTRACT

Concurrent sphincteric incontinence and organic impotence are not uncommon. Simultaneous implantation of the artificial sphincter and penile prosthesis is an effective proposal technically and functionally in these situations. A 44-year-old man presented with total incontinence and impotence, which developed after pelvic bone fracture. Urodynamic study showed sphincter incompetence. Pharmacologic & penile duplex sonography, nocturnal penile tumescence test confirmed arteriogenic impotence. Simultaneous implantation of an artificial sphincter(AMS 800) and a multicomponent inflatable penile prosthesis(AMS 700CXM) was done. For 5 months of follow-up the penile implant was functional, without incontinence. and there are no evidences of infection.

ABSTRACT

A clinical observation was done on 24 cases of hypospadias during the past 5 years from January 1975 to December 1979. 1. The hypospadia had high incidence rate among the anomalies of G-U tract (22.5%). 2. Of 24 hypospadias, 16 cases(66,7%) were penoscrotal, 5 cases (20.8%) were perineal and 3 cases (12.5%) were penile. 3. Cryptorchism was the most common associated anomaly and it occupied 33.3% of 15 associated anomalies. 4. Of 20 urethroplasty, 10 cases (71.1%) were successed by buried skin tube method and over-all success rate was 60%.

ABSTRACT

Primary transitional cell carcinoma of the bladder was treated with prophylactic and therapeutic Intravesical thiotepa in 7 cases. In 5 cases of stage A tumor, postoperative follow up was averaged 10.8 months and 4 cases were tumor free (80%). In 2 cases of infiltrating tumor, the instillation was not effective. Thiotepa can be given safely in the 7 days post-operative period and no leukopenia and bladder irritation symptoms were occurred.

ABSTRACT

A clinical and statistical observation was made on out-patients and in-patients admitted to the Department of Urology, Seoul National University Hospital for the past 24 years from 1954 to 1977. The observations were summarized as follows. 1. During the period, the total number of out-patients was 63,438 and that of in-patients, 6,028. 2. In the out-patients, lower urinary tract infections such as urethritis and prostatitis were the most common diseases regardless of the time period. 3. In the in-patient, the frequency of the disease has been changed with the lapse of time in the order of the occurrence. The interesting changes of the disease order related to the time period were noted as follows. 1954-1960 : genitourinary tuberculosis, urolithiasis, tumor, injury and congenital anomaly. 1961-1970 : urolitiasis, tumor, genitourinary tuberculosis, injury, infection and congenital anomaly. 1971-1977 : tumor. urolithiasis, genitourinary tuberculosis, congenital anomaly, injury and infection. 4. In 1977, 10 major diseases were tumor, ureteral stone, infertility, renal tuberculosis, B. P. H. hypospadias, varicocele, renal stone, scrotal injury and renal tumor 5. Major operations were performed on 4,122 cases during the period. Nephrectomy and ureterolithotomy were the most common operations. Recently, total cystectomy with ileal loop diversion, transurethral procedure and vasovasostomy, which require more skillful techniques, are increasing in number.

ABSTRACT

Seminal fructose in conjunction with plasma testosterone, follicle-stimulating hormone FSH),and luteinizing hormone (LH) was measured on the forty Korean adult males. These subjects were divided into four study groups, such as : Group I: Twenty cases of normospermic males. Group II: Ten cases of obstructive azoospermic males with normal or obstructive change in testis, Group III case at non-obstructive azoospermic males with hypospermatogenesis or spermatogenic arrest at various levels in testis. Group IV: Four cases of non-obstructive azoospermic males with severe seminiferous tubular atrophy and/or hyalinization, or germ cell aplasia with Sertoli cell only in testis. The results are analysed based on the following factors. 1. The average values are: seminal fructose is 301.5 mg/dl(range,131-485): plasma testosterone, 730.4mg/dl(range,420-1160): FSH, 121 mIU/ml(range,6.5-20.0) and LH,9.2 mIU/ml(range,5.3-15.9) in group I. Seminal fructose is 315.2 mg/dl(range,160-386 ); plasma testosterone, 640.0 ng/dl (range,392-840); FSH,3.2 mIU/ml(range,1.0-7.6); LH,4.2 mIU/ml(range 0.5-3.8) in group II. Seminal fructose is 257.2mg/dl(range,120-400): Plasma testosterone, 618.8 ng/dl(range,416-824): FSH,6.6 mIU/ml (range,1.0-13.0); LH,7.3 mIU/ml (range,408-664); FSH,29.9 mIU/ml(range,19.0-50.0); LH,12.1 mIU/ml(range,7.6-16.0) in group IV. 2. Seminal fructose, Plasma testosterone, FSH and LH values are remained in physiologic ranges in all subjects of group II, even Leydig cell hyperplasia is moderate in 4 cases. 3. Seminal fructose, Plasma testosterone, FSH and LH values are also remained in physiologic ranges in all subjects of group III, even moderate Leydig cell hyperplasia is found in 1 case. 4. Low Plasma testosterone(p0.1) values are consistent in group IV, even though severe Leydig cell hyperplasia exists in all cases except one. 5. No statistical correlations are noted between plasma testosterone in the range of 392-1160 mg/dl and seminal fructose in the range of 120-485mg/dl.(r=0.190,p>0.1) But seminal fructose is not significantly low when plasma testosterone is not remarkably decreased. 6. The seminal fructose in the range of 120-485 mg/dl is not under the influence of plasma FSH(range,1.0-20.0mIU/ml). 7. Low plasma testosterone in spite of severe Leydig cell hyperplasia in group IV suggests complexity of Leydig cell responsiveness to LH.

ABSTRACT

A clinical observation was made on twenty-one cases of giant hydronephrosis experienced in recent 10 years. And the following results were obtained. 1. The most common cause of giant hydronephrosis was congenital uretero-pelvic juncture stricture (11 cases). 2. The ratio of male to female was 16:5. 3. The main signs and symptoms were flank mass (15 cases) and flank pain (10 cases). 4. The volume of content was over 3,000cc in seven cases. 5. Nephrectomy or nephroureterectomy was performed in all cases except one.
