ABSTRACT

OBJECTIVE@#To study the epidemiologic characteristics of human herpes virus (HHV) activated infection in the diseases of blood system and patients received allo-HSCT by statistically analyzing the screening results of 8 human herpes viruses (HHVs) of 4164 patients in Hebei Yanda LU Dao-Pei Hospital from 2012 to 2017.@*METHODS@#PCR was used to screen 8 HHVs.@*RESULTS@#Two thousand and fifty-two patients (49.28%) were HHV-positive among 4164 patients screened. Among these patients screened, the infection spectra of 8 human HHVs in hematological diseases as well as patients received allogeneic hematopoietic stem cell transplantation of totally 2994 patients were summarized as follows: the positive rate of EBV (29.49%) was the highest, that of HCMV (23.15%), HHV-6 was 18.77% and HHV-7 was 17.64%, while the remaining 4 HHVs all≤2.1%. The rate of co-infection of various HHVs was significantly higher than that of single infection of HHV among all these disease groups except familial hemophagocytic lymphohistiocytosis, for which single EBV infection was the most common. The differences of positive rates among these 8 human HHVs in hematological diseases as well as patients received allogeneic hematopoietic stem cell transplantation were statistically significant by Chi-square test of R*C tables (χ=54.99, P<0.05). For each HHV, the differences of positive rates among the above-mentioned disease groups were also statistically significant except HHV-8 (P<0.05).@*CONCLUSION@#The patients with various blood diseases have different activated infection spectra of HHVs. EBV, HCMV, HHV-6 and HHV-7 are most common in HHVs infection. Different HHVs infections correlate with different hematologion diseases.

ABSTRACT

Objective To understand the disease burden of patients with advanced schistosomiasis in Jiangsu Province. Methods Totally 450 patients with advanced schistosomiasis from southern, northern and central Jiangsu were chosen by the stratified sampling method, and surveyed by the self-designed economic burden questionnaire in 2015. The economic burden of the patients was analyzed by the descriptive analysis method, and its determinants were explored by the multiple linear regression analysis. Results A total of 450 subjects were surveyed and 434 valid questionnaires were recovered with the recovery rate of 96.44%. The per capita economic burden of advanced schistosomiasis patients was 10 217 Yuan in Jiangsu Province in 2015, including 7 221 Yuan in direct economic burden and 2 996 Yuan in indirect economic burden. The average lost labor force time was 140 days for the patients, and was 23 days for the family. The multiple linear regression analysis showed that the marital status, hospitalization health service utilization, and self-reported health score impacted on the disease economic burden. Conclusion The disease burden of patients with advanced schistosomiasis in Jiangsu Province is heavy.

ABSTRACT

Objective To assess the effectiveness of the clinical pathway for the treatment of advanced schistosomiasis he-patic fibrosis. Methods The duration of hospital stay,gross hospitalization expense,individual-paid expense,interior diame-ter of portal vein,levels of four serum hepatic fibrosis-related parameters(PIIIP,CIV,HA,and LN),and activities of ALT, AST andγ-GT were assessed and compared between the advanced schistosomiasis patients receiving the clinical pathway and ones receiving non-clinical pathway. Results There were 142 advanced schistosomiasis patients with hepatic fibrosis receiving the clinical pathway of anti-hepatic fibrosis. Compared with the patients receiving non-clinical pathway ,the gross hospitalization expenses reduced by 11.2%(t=6.310,P<0.05),and the individual-paid expenses reduced by 16.1%(t=4.326,P<0.05). The mean HA level was twice higher than the normal range,with a positive rising from 70.4%to 83.1%,and the abnormal rates of CIV andγ-GT were 64.1%and 28.9%respectively. Conclusions The clinical pathway can drastically reduce the treatment expenses in advanced schistosomiasis patients with hepatic fibrosis. However,the patients have a trend towards the persistent disease progression. Therefore,the researches of more effective therapeutic methods for advanced schistosomiasis hepatic fibro-sis are urgently needed.

ABSTRACT

Numerous studies revealed that radiation can promote the immunogenic cell death, enhance the immune response of tu-mor-antigen-specific T cells, and induce systematic antitumor immunity, which may result in abscopal effect. Abscopal effect refers to the regression of non-irradiated metastatic lesions distant from the primary tumor site that is directly subjected to irradiation. Despite that abscopal effect has been clinically reported and that many preclinical studies have confirmed its biological basis, abscopal effect remains a subclinical phenomenon, in which objective efficacy is difficult to obtain. Currently, the anti-PD-1/PD-L1 therapy in clinical applications shows a clear and definite curative effect, thereby making local radiotherapy combined with immunotherapy a new treat-ment pattern. However, the clinical practice of this new mode should be based on appropriate dose and fraction and the most suitable temporal combination to achieve the synergic antitumor immune response.

ABSTRACT

Objective To study the expression profiles of circulating microRNAs(miRNAs)in newly-developed advanced schistosomiasis patients. Methods Ten healthy controls(Group 1),ten patients with schistosomiasis history(without advanced schistosomiasis,Group 2)and ten patients with newly-developed advanced schistosomiasis(Group 3)were randomly selected from Jiangsu Province. The expression profiles of 570 human related circulatingmiRNAs were measured and analyzed with the Agilent Human microRNAs microarray Rel 12.0. Results Compared to the healthy controls,four miRNAs were up-regulated, while 16 miRNAs were down-regulated in the patients with newly-developed advanced schistosomiasis. Moreover ,six miRNAs were up-regulated,while 21 miRNAs were down-regulated in the patients in Group 2. Among of them,the expression level of miR-383 was significantly decreased by 4.23 and 11.82 folds in Group 2 and Group 3,respectively. Conclusions There are significant differences among the healthy controls,patients with schistosomiasis history(without advanced schistosomiasis)and the patients with newly-developed advanced schistosomiasisin the expression profiles of circulatingmiRNAs. Moreover ,circulat-ing miR-383 might be involved in the development of newly-developed advanced schistosomiasis.

ABSTRACT

Objective To evaluate the effects of ischemic postconditioning on nuclear factor kappa B (NF-κB) and intercellular adhesion molecule-1 (ICAM-1) expression during lung ischemia-reperfusion (I/R) injury in a dog model of cardiopulmonary bypass (CPB), and further investigate the pulmonary protection induced by ischemic postconditioning and the underlying mechanism.Methods Twelve adult mongrel dogs of both sexes, weighing 12-15 kg, were randomly divided into either CPB group or ischemic postconditioning group (IPO group) using a random number table, with 6 dogs in each group.CPB was established after the chest was opened in dogs anesthetized with pentobarbital sodium.Ischemic postconditioning was induced by 2 cycles of 5 min reperfusion followed by 5 min ischemia immediately after occlusion of the left pulmonary artery was released in group IPO.Before CPB (T1), before occlusion of the artery was released (T2) , and at 2 h after termination of CPB, lung specimens were obtained for examination of pathological changes which were scored (with light microscope) and for determination of the expression of NFκB and ICAM-1 (using Western blot) and wet/dry lung weight ratio (W/D ratio) in left lung tissues.Blood samples were collected from femoral arteries at T1 and T3 for blood gas analysis, and oxygenation index (OI), respiration index (RI) and dynamic lung compliance (Cdyn) were calculated.Results OI and Cdyn were significantly decreased, and RI was increased at T3 , and W/D ratio, pathological scores, and expression of NF-κB and ICAM-1 were increased at T2,3 than at T1 in the two groups.W/D ratio, pathological scores, and expression of NF-κB and ICAM-1 were significantly higher at T3 than at T2 in the two groups.Compared with group CPB, OI and Cdyn were significantly increased, and R1, W/D ratio, pathological scores, and expression of NF-κB and ICAM-1 were decreased at T3 in group IPO.Conclusion Ischemic postconditioning up-regulates the expression of ICAM-1 through inhibiting NF-κB activity, thus reducing lung I/R injury induced by CPB and improving the lung function in dogs.

ABSTRACT

Objective To evaluate the clinical efficacy of sodium tanshinone IIA sulfonate(STS)in the treatment of ad-vanced schistosomiasis. Methods Fifty cases with advanced schistosomiasis admitted to the Touzao Township Hospital of Dong-tai City during the period from November 2012 to November 2013 were treated with STS for 10 days,and the internal diameter of the portal vein,levels of ALT,AST,γ-GT,PIIIP,CIV,HA and LN were measured and compared before and after the adminis-tration of STS. Results The mean levels of all serological parameters except HA were within the normal range before STS treat-ment,while the highest positive rate was detected inγ-GT(26.0%)and HA(54.0%). Following the STS treatment,the mean lev-els of all parameters and the positive rates reduced,with the greatest reduction observed inγ-GT(36.7%)and HA(37.8%);how-ever,the mean HA level was still higher than the normal range. The mean internal diameter of the portal vein reduced from(10.5± 1.7)mm before the STS treatment to(9.8±1.3)mm after the STS administration,with a significant diffrtence(P<0.05). Conclu-sion STS appears effective in the treatment of advanced schistosomiasis.

ABSTRACT

<p><b>BACKGROUND</b>It has found that ischemic postconditioning (IPO) might decrease pulmonary ischemia/reperfusion (I/R) injury, which is one of the main reasons of lung injury caused by cardiopulmonary bypass (CPB). It was found that aquaporins (AQPs) play a role in the maintenance of fluid homeostasis. But it is still unclear whether IPO influences the expression of aquaporin-1 (AQP1). This study was designed to investigate whether IPO can reduce CPB-related lung injury and affect the expression of AQP1 of lungs.</p><p><b>METHODS</b>Twelve healthy dogs were divided into control group (C group) and ischemia postconditioning group (IPO group). CPB procedures were implemented. Ten minutes later, the left pulmonary artery was separated and blocked. Postconditioning consisted of two cycles of 5-minute pulmonary artery reperfusion/5-minute reocclusion starting at the beginning of reperfusion. The 2×4 cm tissues of both sides of pulmonary apex, superior, middle and inferior lobe were taken before CPB (T1), before occlusion and reopening of left pulmonary artery (T2, T3), and 2 hours after CPB (T4). Samples were used to evaluate lung injury degrees and to detect the expression of AQP1. At T1 and T4, blood was collected from femoral artery to calculate pulmonary function.</p><p><b>RESULTS</b>At T4, each pulmonary function showed significant deterioration compared with T1. Lung injury could be found at the onset of CPB. However, the expression of AQP1 decreased and wet to dry weight ratio (W/D) increased after T2. In the left lung of C group, the worst pulmonary function and structures were detected. The slightest changes were discovered in the right lung of C group. A close relationship between W/D and lung injury score was found. The lung injury score was negatively related with the expression of AQP1. It was found that the expression of AQP1 was negatively connected with W/D.</p><p><b>CONCLUSIONS</b>In dog CPB models, lung injury induced by CPB was related with down regulated expression of AQP1. AQP1 is believed to be involved in the mechanisms of lung ischemia/reperfusion (I/R) injury caused by CPB. IPO increases the expression of AQP1, provides a protective effect on lung suffering from CPB, and alleviates CPB-related lung injury.</p>

ABSTRACT

Eight patients with suspected cases of C .jejuni were etiologically diagnosed and analyzed in this study to pro-vide scientific basis for the confirmation of the cases of human campylobacteriosis in Guizhou Province ,China .Blood or feces of 8 suspected patients were employed to isolate bacteria strains .Conventional and multi-PCR techniques were applied to identify suspicious bacteria strains .The C .jejuni strains were analyzed by using Pulsed Field Gel Electrophoresis (PFGE) .Suspicious strains of C .jejuni were isolated from all the 8 suspected patients of campylobacteriosis and anticipated genes fragment were detected with multi-PCR .With the digestion of restriction enzyme SmaI ,the 8 C .jejuni strains were divided into 7 PFGE pat-terns with 7-10 DNA bands .Cluster analysis showed that the gross similarity of 8 strains of C . jejuni was more than 50% . The similarity of PFGE patterns between strain GZ201004 and GZ201005 from diarrhea patients was as high as 100% ,while the similarity of strain GZ201201 and GZ201007 was 66 .7% .Moreover ,C . jejuni were detected from all the suspected pa-tients of campylobacteriosis .PFGE results indicated that strains GZ201004 and GZ201005 were from the same source ,while all the 8 isolates showed PFGE polymorphism .

ABSTRACT

BACKGROUND:Many studies have demonstrated that FirebirdTM and Taxus Express2TM can effectively reduce intrastent restenosis.However,there are few data about long-term efficacy of two stents,and reports about middle and long-term follow up are rare.OBJECTIVE:To observe the safety and biocompatibility of FirebirdTM and Taxus Express2TM in percutaneous coronary intervention(PCI) for coronary artery disease.DESIGN,TIME AND SETTING:Non-randomized concurrent control clinical observation was performed at Department of Cardiology,Hongqi Hospital of Mudanjiang Medical College from April 2005 to April 2008.PARTICIPANTS:233 patients(268 lesions) undergoing PCI were divided into FirebirdTM(n =82),Taxus Express2TM group(n =80) and bare metal stent group(n =71).METHODS:Coronary arteriography was performed through femoral artery or radial artery.Vascular inner diameter was determined using quantitative computer analysis.The patients underwent FirebirdTM,Taxus Express2TM and bare metal stenting,respectively.The patients were reexamined and followed-up using telephone every 2-4 weeks after discharging and examined using coronary arteriongraphy after 9-12 months.The follow-up lasted for 24 months.MAIN OUTCOME MEASURES:Characteristics of arteriongraphy and stenting condition of all patients;biocompatibility of stent to host;major adverse cardiac events during hospitalization and follow-up,including death,angina pectoris attacks or heart failure;coronary artery diameter decreased ≥ 50% was regarded as restenosis.RESULTS:101 stents were implanted in Firebird group,98 in Taxus Express2 group and 85 in bare metal stent group.There was no stent defluxion,dislocation,or breakage.No noticeable platelet decrease,hemolysis or white blood cell increase was found.There were no significant differences among three groups in terms of Characteristics of arteriongraphy and stenting condition.The incidence of major adverse cardiac events and intravascular restenosis in Firebird and Taxus Express2 groups was fewer than bare metal stent group(P 0.05).CONCLUSION:No specific biocompatibility responses in treatment of coronary artery diseases using FirebirdTM and Taxus Express2TM.The two drug-eluting stents are superior over bare metal stent in reducing restenosis.The safety and efficacy of two drug-eluting stents are similar.
