ABSTRACT

Resumen: Los tumores cardíacos pueden ser primarios o, más frecuentemente secundarios o metastásicos. Entre los tumores primarios es más frecuente el mixoma, cuya ubicación más común es en la aurícula izquierda. Las manifestaciones clínicas son diversas, producidas principalmente por obstrucción mecánica, embolizaciones, y manifestaciones constitucionales. Se comunica el caso de un paciente de 32 años, con cuadro clínico de insuficiencia cardíaca, hipertensión pulmonar severa y tromboembolismo pulmonar bilateral. Se hizo el diagnóstico de mixoma auricular izquierdo. Se resecó el tumor y se manejó la hipertensión pulmonar desde el ingreso al hospital con inhibidores de la fosfodiesterasa asociado a anticoagulación. Se discute el tema dando énfasis a aspectos fisiopatológicos involucrados tanto en la hipertensión pulmonar como en la presencia de tromboembolia pulmonar.


Abstract: Cardiac tumors may be primary or, more frequently secondary or associated to metastasis. Atril myxoma es the most frequent primary tumor, usually located in the left atrium. Clinical manifestations include those due to mitral valve occlusión, emboli and general non spedific symptoms and signs. Herein we report the clinical case of a 32 year old patient with severe pulmonary hypertension and bilateral pulmonary embolism. The tumor was extirpated, and he received phosphoro-diesterase inhiborts and anticoagulants. Subsequent clinical course was satisfactory. A brief discussion of this condicion is included.

ABSTRACT

Objetivo: Dar a conocer una rara presentación de una patología frecuente. Caso clínico: Presentamos el caso de un paciente de 27 años de edad con clínica de apendicitis aguda con antecedentes de haberse realizado 9 años atrás una apendicectomía, encontrándose un apéndice inflamado, en posición retrocecal, ascendente y subseroso. En la segunda apendicectomía se encontró un nuevo apéndice sano en la unión de las 3 tenias. El estudio histológico posterior confirmó que ambas piezas quirúrgicas eran apéndices, uno inflamado y el otro normal.


Aim: To report a rare presentation of a frequent pathology. Case report: We report a patient of 27 years old with symptoms of acute appendicitis and medical records of an appendectomyy 9 years before where surgical findings were an inflamed appendix retrocecal ascendant and subserosa. In our procedure, we found a new one at the junction of the three tapeworms. Histological examination later confirmed that both surgical specimens were appendices, one swollen and the other normal.

ABSTRACT

Cystic degeneration of the popliteal artery is an uncommon cause of intermittent claudication. We report a 52 years old male consulting for intermittent claudication referred to the calf. The angiography showed a 93% stenosis in segment of 45 mm length of the left popliteal artery with a smooth surface, suggesting an extrinsic compression. A CT angiography of the lower limb suggested the presence of a cystic degeneration. The patient was operated and the middle third of the popliteal artery was excised. The pathological study of the surgical piece was informed as artery media cystic degeneration.
